1.Dynamics of HIV reservoir and α4β7 cells in patients with different immune reconstitution outcomes during long-term antiretroviral therapy

Objective:To investigate immunological non-responders (INRs) and immunological responders (IRs) during long-term antiretroviral therapy (ART), and study the dynamics of HIV reservoir and α4β7 cells in INRs and IRs and their correlation.Methods:Twenty-six patients with chronic HIV infection who received ART for 5 years were included. They were divided into INRs (CD4 + T cell counts≤350 cells/μl, n=9) and IRs (CD4 + T cell counts≥500 cells/μl, n=17) based on immune reconstitution outcomes. The percentages and numbers of α4β7 cells in both groups at baseline, ART 1, 3, and 5 years were detected by flow cytometry, and the levels of HIV DNA and cell-associated HIV RNA were quantified by real-time fluorescent quantitative PCR during the same periods. HIV viral decay, α4β7 cells dynamics, and their correlations with T cells were compared at baseline, ART 1, 3 and 5 years between the two groups. Results:Over 5 years of ART, INRs exhibited higher HIV reservoir levels compared to IRs, but the decline trend was not slow. The counts of α4β7 cell were lower and the growth trend was slow in INRs ( P<0.05). α4β7 cell counts were strongly positively correlated with CD4 + T cell counts at all timepoints (Year 1: r=0.887; Year 3: r=0.878; Year 5: r=0.887; P all <0.001), while showing significantly negative correlations with activated CD38 + HLA-DR + CD4 + T cells (Year 1: r=-0.619, P=0.001), CD38 + HLA-DR + CD8 + T cells (Year 1: r=-0.517; Year 5: r=-0.532; P all <0.01), and PD-1 + CD4 + T cells (Year 1: r=-0.476, Year 5: r=-0.390, P all <0.05). Conclusions:During long-term ART, INRs maintained higher HIV reservoir and lower α4β7 cell counts compared with IRs, and decreased α4β7 cells may be associated with disease progression.
2.Progress in the mechanism and clinical application of platelet-rich plasma in the treatment of intrauterine adhesions

Intrauterine adhesions are adhesions of the uterine wall caused by damage to the basal layer of the endometrium, which will affect women's menstruation and fertility. At present, the effect of surgery and drug treatment is not ideal, and it is easy to relapse. Platelet-rich plasma (PRP) is a kind of plasma with high concentration of platelets obtained by centrifuging autologous blood, containing a variety of bioactive factors (platelet-derived growth factor, transforming growth factor-β, vascular endothelial growth factor, insulin-like growth factor 1, etc.), which can significantly promote cell proliferation and differentiation, angiogenesis, relieve inflammation and prevent infection, reduce fibrosis and be used as biological scaffold in the treatment of uterine adhesion. It can significantly improve endometrial morphology, reduce fibrosis and improve fertility. In view of the advantages of PRP in the treatment of intrauterine adhesions, this paper will discuss the characteristics, mechanism and application of PRP in the treatment of intrauterine adhesions, so as to further understand the important role of PRP in the treatment of intrauterine adhesions and lay a foundation for its clinical application.

5.Efficacy analysis of Deanxit in the treatment of irritable bowel syndrome and functional dyspepsia overlap syndrome

Objective To investigate the efficacy of antidepressant Deanxit in the treatment of irritable bowel syndrome (IBS) and functional dyspepsia (FD) overlap syndrome.Methods One hundred and twentyeight patients with IBS and FD overlap syndrome admitted into the Jiangwan Hospital of Hongkou District,Shanghai for medical treatment from March,2014 to December,2017 were randomly divided into the control group and the observation group,each consisting of 64 patients.The control group received pinaverium bromide and omeprazole,while the observation group was treated with Deanxit.The differences in the efficacy,the Hamilton anxiety rating scale(HAMA) and the Hamilton depression rating scale (HAMD) were compared and analyzed between the patients of the 2 groups.Results Total effective rate of the observation group was 96.7%,which was significantly higher than that of the control group,and statistical significance could be seen when comparisons were made between the 2 groups (P < 0.01).After intervention,both the HAMD and HAMA scores in the observation group decreased notably and were significantly lower than those in the control group,with statistical significance (P < 0.01).Conclusion Deanxit could achieve sure results in the treatment of IBS and FD overlap syndrome,effectively improve clinical symptoms and relieve anxiety or depression.Therefore,it is worth clinical promotion.

7.Immunogenicity of DNA vaccines encoding regulatory/accessory proteins derived from three different prevalent strains in China

0bjective To determine the immunogenicities of DNA vaccines expressing tat-rev-integrase(c-half)-vif-neffusion genes(TRIVN) derived from prevalent B', B'/C and AE recombinant subtypes of HIV-1 in China. Methods Two DNA vaccines were constructed by inserting the codon optimized tat-revintegrase(c-half)-vif-nef fusion genes derived from B' and B'/C subtype of HIV-1 into mammalian expression vector pSVI. 0. DNA vaccine containing tat-rev-integrase (c-half)-vif-nef fusion gene derived from HIV-1AE2f has been constructed previously. In vitro expression efficiencies of three DNA vaccines were determined by Western blot and their immunogenicities were compared by immunizing female BALB/c mice. IFN-γ ELISPOT assay was used to read out the specific T cell immunity. Results The constructed DNA vaccines were validated by restriction enzyme digestion and DNA sequencing. Western blot assay showed three constructed DNA vaccines could be expressed at a comparable level in vitro. After vaccination, AE-TRIVN mounted T cell immune responses at (948.0 ± 330.0) SFCs/106 splenocytes, followed by the mixed DNA vaccine[ (500.0 ± 155.0) SFCs/106 splenocytes ], RL-TRIVN r[ ( 195. 1 ± 44.0) SFCs/106 splenocytes ]and CN-TRIVN [ (89.5 ± 17.0) SFCs/106 splenocytes]. Interestingly, we observed that single DNA vaccination induced specific T cell responses predominantly targeting Integrase (C-half) and Vif, whereas the mixed DNA could significantly improve T cell responses against Nef. Conclusion AE-TRIVN was the most immunogenic among the three DNA vaccines and the mixed DNA vaccination could change the immunogenic hierarchy of T cell epitopes across the fusion genes vaccine.
8.The efficacy and safety of HAA regimen as induction chemotherapy in 150 newly diagnosed acute myeloid leukemia

Objective To explore the efficacy and safety of HAA regimen (homoharringtonine,cytarabine and aclarubicin) in the treatment of 150 newly diagnosed adult acute myeloid leukemia (AML).Methods All patients entered the study from May 1999 to June 2008 were treated with HAA regimen. Coxsurvival analysis was used to estimate the survival rate and differences between M1/M2 and M4/M5 were compared with 2-sided log-rank test. Results Out of the 150 patients, 121 (81%) achieved complete remission (CR). After the first course, CR rate was 68%. The CR rates of 97%, 84% and 38% were achieved in patients with favorable, intermediate and unfavorable cytogenetics, respectively. For the patients with CR, the median follow-up time was 16.5 ( 1.5-100.5 ) months, and the estimated 3-year survival rate was 45%. The estimated 3-year relapse free survival rate was 52% for the 121 patients with CR.Conclusions HAA regimen may be an efficacious and safe regimen with a good toleration in the induction therapy for newly diagnosed AML, and a high CR rate could be achieved with only one or two courses.
