Gout is the most prevalent inflammatory arthritis worldwide and represents a growing global health burden. It arises from hyperuricemia resulting from disordered purine metabolism, leading to monosodium urate (MSU) crystal deposition and recurrent inflammatory arthritis. Although hyperuricemia is the principal risk factor, genetic susceptibility, impaired urate transport, and emerging factors such as the gut microbiota also contribute to disease development. Acute flares are mediated by MSU crystal-induced activation of the NLRP3 inflammasome and subsequent interleukin-1β production, whereas chronic inflammation results in tophus formation and structural joint damage. Gout is closely associated with metabolic syndrome, chronic kidney disease, and cardiovascular disease, which contribute to persistently elevated mortality. Although advances in imaging and pharmacologic therapy have improved diagnosis and management, the real-world use of urate-lowering therapy remains suboptimal. Further research and sustained efforts by healthcare professionals are needed to improve long-term disease control.
2.Combined Treatment of Negative Pressure Wound Therapy and Temporary Tension Suture for Intractable Hindfoot Ulcer with Chronic Osteomyelitis: A Case Report

The heel is the second most common area for pressure injuries, which can lead to serious and threatening extremity infections. The late stages of deep hindfoot ulcers, where osteomyelitis commonly coexists, can result in an extended therapeutic window. Standard treatments for hindfoot ulcers complicated by osteomyelitis encompass debridement and flap surgery. In severe cases, below-knee amputation is also considered. Recent studies have reported the efficacy of negative pressure wound therapy (NPWT) in addressing refractory foot ulcers. The authors developed a temporary tension suture (TTS) in combination with NPWT to shorten the treatment duration of refractory foot ulcers, providing additional appropriate tension for wound coverage. The authors report for the first time a case of a 73-year-old female patient who had been experiencing an intractable hindfoot ulcer with chronic osteomyelitis for 18 months. NPWT and TTSs were applied concurrently with favorable outcomes.

6.Comparison of two microfocused ultrasound treatment protocols for eyebrow lifting in a Korean female cohort in Republic of Korea: a split-face clinical study

Background:
Microfocused ultrasound (MFU) has emerged as a noninvasive treatment modality for facial aging, with proven efficacy in eyebrow lifting and wrinkle reduction. However, there are only limited split-face studies that quantitatively compare treatment protocols with different focal depths. This prospective, singlecenter, randomized, split-face, open-label study aimed to compare the clinical outcomes of two MFU protocols (2-depth vs. 3-depth) for eyebrow elevation and improvement of glabellar wrinkles.
Methods:
Twenty-two female participants were enrolled and randomly assigned to two groups (n = 11 per group). One group received the 3-depth protocol (2.0 mm, 2.5 mm, 3.0 mm) on the left forehead and the 2-depth protocol (2.0 mm, 3.0 mm) on the right, while in the other group the protocols were reversed. Clinical outcomes were assessed at baseline and at 4, 8, 12, and 16 weeks post-treatment. Evaluations included average eyebrow height (AEH), maximum eyebrow height (MEH), glabellar line scale (GLS), and participant satisfaction.
Results:
Both AEH and MEH showed consistently greater improvements with the 3-depth protocol compared to the 2-depth protocol. Although the differences did not reach statistical significance, a trend toward enhanced lifting effects with the 3-depth protocol was observed at all time points. GLS demonstrated the greatest reduction, with 90.9% of participants achieving a ≥1-point improvement by week 12.
Conclusion
Both MFU protocols were effective for eyebrow elevation and glabellar wrinkle reduction.However, the 3-depth protocol yielded greater improvements, suggesting a potential clinical benefit of multidepth treatment in this female cohort.
7.Current understanding of modulated electro-hyperthermia in cancer treatment

Traditional hyperthermia involves increasing the temperature at the tumor site to above 39 ℃, inducing death in cancer cells. Although hyperthermia is an effective cancer treatment, its clinical application has decreased due to potential complications, including damage to surrounding normal tissue. In recent years, modulated electro-hyperthermia (mEHT) has emerged as an effective and safe treatment modality. mEHT selectively heats tumor cells to 42–43 °C, while reducing the average temperature in the treatment area, including the surrounding normal tissue, compared to conventional methods. Additionally, mEHT may be used in combination with systemic chemotherapy and radiation therapy in tumor treatment, providing a synergistic effect to increase efficacy. As chemotherapy and radiation therapy technologies advance, the application of combined mEHT may improve clinical outcomes. In this study, we review and discuss reports on the clinical outcomes of mEHT combined with chemotherapy and/or radiation therapy, which are established anticancer treatments.
8.Interlaboratory Comparison Study (Ring Test) of Next-Generation Sequencing–Based NTRK Fusion Detection in South Korea

Purpose:
Tropomyosin receptor kinase (TRK) inhibitors are approved for the treatment of neurotrophic receptor tyrosine kinase (NTRK) fusion-positive tumors. The detection of NTRK fusion using a validated method is required before therapeutic application. An interlaboratory comparison study of next-generation sequencing (NGS)–based NTRK gene fusion detection with validated clinical samples was conducted at six major hospitals in South Korea.
Materials and Methods:
A total of 18 samples, including a positive standard reference and eight positive and nine negative clinical samples, were validated using the VENTANA pan-TRK (EPR17341) and TruSight Oncology 500 assays. These samples were then tested using four different NGS panels currently being used at the six participating institutions.
Results:
NTRK fusions were not detected in any of the nine negative clinical samples, demonstrating 100% specificity in all six participating institutions. All assays showed 100% analytical sensitivity to identify the NTRK fusion status in formalin-fixed paraffin-embedded (FFPE) samples, although with variable clinical sensitivity. False-negative results were due to low tumor purity, poor RNA quality, and DNA-based sequencing panel. The RNA-based targeted NGS assay showed an overall high success rate of identifying NTRK fusion status in FFPE samples.
Conclusion
This study is the first to test the proficiency of NGS-based NTRK detection in South Korea with the largest participating institutions. RNA-based NGS assays to detect NTRK fusions can accurately characterize fusion transcripts if sufficient RNA of adequate quality is available. The comparative performance data will support the implementation of targeted NGS-based sequencing assays for NTRK fusion detection in routine diagnostics.
9.Risk factors for early-onset lung cancer in Korea: analysis of a nationally representative population-based cohort

OBJECTIVES:
We examined the associations of socioeconomic factors, health behaviors, and comorbidities with early-onset lung cancer.
METHODS:
The study included 6,794,287 individuals aged 20-39 years who participated in a Korean national health check-up program from 2009 to 2012. During the follow-up period, 4,684 participants developed lung cancer. Multivariable Cox regression analysis was used to estimate the independent associations of potential risk factors with incident lung cancer.
RESULTS:
Older age (multivariable hazard ratio [mHR], 1.13; 95% confidence interval [CI], 1.12 to 1.14) and female sex (mHR, 1.62; 95% CI, 1.49 to 1.75) were associated with increased lung cancer risk. Current smoking was also associated with elevated risk (<10 pack-years: mHR, 1.12; 95% CI, 1.01 to 1.24; ≥10 pack-years: mHR, 1.30; 95% CI, 1.18 to 1.45), but past smoking was not. Although mild alcohol consumption (<10 g/day) was associated with lower lung cancer risk (mHR, 0.92; 95% CI, 0.86 to 0.99), heavier alcohol consumption (≥10 g/day) was not. Higher income (highest vs. lowest quartile: mHR, 0.86; 95% CI, 0.78 to 0.94), physical activity for at least 1,500 metabolic equivalent of task-min/wk (vs. non-exercisers: mHR, 0.83; 95% CI, 0.69 to 0.99) and obesity (vs. normal weight: mHR, 0.89; 95% CI, 0.83 to 0.96) were associated with lower lung cancer risk, whereas metabolic syndrome was associated with increased risk (mHR, 1.13; 95% CI, 1.03 to 1.24).
CONCLUSIONS
In young adults, age, female sex, smoking, and metabolic syndrome were risk factors for early-onset lung cancer, while high income, physical activity, and obesity displayed protective effects.
