1.Epidemiological survey of osteoporosis in Beijing over the past decade: a single-center analysis of dual-energy X-ray absorptiometry scans from 30 599 individuals.

OBJECTIVES:
To analyze bone mass distribution and the factors affecting bone mass in a general Chinese Han cohort undergoing physical examinations at our center.
METHODS:
We retrospectively collected the data of bone mineral density (BMD) measurements from 30 599 healthy Han Chinese adults (age≥20 years) who underwent dual-energy X-ray absorptiometry scans at our hospital from July, 2013 to July, 2023. Basic parameters including height, body weight, and gender were recorded, and descriptive statistics and correlation analyses were performed using R software.
RESULTS:
In this cohort, the male individuals had a mean peak BMD of 1.00±0.12 g/cm2 in the lumbar vertebrae, 0.94±0.14 g/cm2 in the femoral neck, and 0.99±0.13 g/cm2 in the total hip, significantly higher than the values in the female individuals [0.99±0.12 g/cm2 in the lumbar vertebrae (P=0.022), 0.79±0.11 g/cm2 in the femoral neck (P<0.001), and 0.88±0.11 g/cm2 in the total hip (P<0.001)]. In the overall cohort, the BMD values of the lumbar spine and femur decreased with age after reaching their peak levels. There was a positive correlation between BMD value and body mass index (BMI) in both male and female individuals. The 2013-2014 period recorded the lowest BMD values in the lumbar, hip, and femoral neck, which tended to increase steadily in the following years (2015-2023).
CONCLUSIONS
Our data suggest that the BMD values vary among different populations, and future multi-center studies using more accurate BMD detection technology are warranted to capture the variation patterns of BMD with demographic characteristics of specific populations.

2.Construction and value of personalized service platform based on IoTs technique of medical equipment for rehabilitation information

Objective:To construct a personalized service platform based on Internet of Things technique of medical equipment for rehabilitation information,so as to analyze the application effect of that in the management for rehabilitation information of patients.Methods:Based on the Internet of Things technique of medical equipment,a personalized service platform was constructed to conduct intelligent management for rehabilitation information of patients through sensor technique,wireless communication technique and big data analysis technique.A total of 112 patients who received treatment in the department of rehabilitation medicine of Dushan Hospital,Nanyang City Hospital of Traditional Chinese Medicine from February to December 2023 were selected,and they were divided into control group which adopted conventional platform to manage rehabilitation information,and observation group which adopted personalized service platform based on IoTs technique of medical equipment for rehabilitation information to manage rehabilitation information,with 56 cases in each group.A self-made questionnaire was used to investigate the satisfaction of patients of two groups for the service platform of rehabilitation information,and the acceptance scores of maintainers for platform,relevant operators,doctors and nurses for different service platform.The service efficiency and success of rehabilitation treatment of different service platforms were compared.Results:53 patients satisfied for the service platform of rehabilitation information(94.64%)in the observation group,which was higher than that in the control group,and the difference was statistically significant(x2=13.187,P<0.05).The scores of convenience,flexibility and continuity of patients of observation group were respectively(94.74±3.36),(94.57±4.12)and(93.65±4.02)for the platform,which were higher than those of control group,and the differences were statistically significant(t=14.279,13.604,13.553,P<0.05).The acceptance scores of maintainers for platform,relevant operators,doctors and nurses were respectively(91.69±2.11),(94.52±3.65),(93.87±4.58)and(94.74±3.36)for the personalized service platform for rehabilitation information of the observation group,which were higher than those in the control group,and the differences were statistically significant(t=15.280,13.236,12.373,14.279,P<0.05).The waiting time,appointment time and treatment cycle of patients in observation group were respectively(4.65±2.01)min,(3.21±1.03)d and(13.02±1.02)months,which were shorter than those of control group,and the differences were statistically significant(t=9.768,11.401,12.300,P<0.05).Conclusion:The personalized service platform based on the IoTs technique of medical equipment for rehabilitation information can take doctors to timely adjust rehabilitation programs by combining the dynamic changes of patients'information,and formulate targeted rehabilitation guidance,and provide effective rehabilitation treatment for patients,and reduce waiting time of patient,and shorten the rehabilitation cycle.
3.Efficacy of catheter balloon dilation and high-intensity Shaker training on treating post-brainstem stroke cricopharyngeal achalasia and the impact on post-stroke aspiration pneumonia

Objective:To observe the therapeutic effects of catheter balloon dilation and high-intensity Shaker training on cri-copharyngeal achalasia after brainstem stroke and the effect on post-stroke aspiration pneumonia.Method:Fifty patients with swallowing disorders after brainstem stroke admitted to Yangfangdian Hospital and Wangjing Hospital between July 2022 and December 2022 were selected for this non-blinded clinical trial.The pa-tients were divided into the control group(n=25)and the experimental group(n=25)according to the order of ad-mission and receipt of balloon dilation.The control group was treated with conventional swallowing therapy and increased the intensity of Shaker training,while the experimental group was treated with additional transoral bal-loon dilatation.The changes of swallowing function and incidence of aspiration pneumonia were compared using videofluoroscopic swallowing study(VFSS),the standard swallowing function assessment scale(SSA),Rosenbek infiltration/misaspiration scale,feeding function and the incidence of aspiration pneumonia.The self-assessment de-pression scale(SDS)and anxiety self-assessment anxiety scale(SAS)were used to assess the changes of pa-tients'psychological status.Result:There was no statistically significant differences in swallowing function and psychological status between the two groups before treatment(P>0.05).After 8 weeks of treatment,both groups showed improvement in SSA,VFSS and Rosenbek scores(P<0.05),and significant reductions in SAS and SDS,with significant improvement in feeding function.The improvement in swallowing and psychological status of both groups was better than pre-treatment,with significantly higher in the experimental group than the control group(P<0.05).Conclusion:Both the rehabilitation training method of swallowing with increasing the intensity of Shaker training and balloon dilation are effective in the recovery of swallowing disorder,but the combined treatment is more ef-fective.Both group showed improvements in Rosenbek score and reduced incidence of aspiration after treatment,indicating that the rehabilitation training with increasing intensity of Shaker training and balloon dilation can effec-tively reduce the incidence of aspiration pneumonia,and the combined treatment was more effective.

6.Effect of early postoperative comprehensive rehabilitation on children and youth with supracondylar fracture of humerus complicated with ulnar nerve injury

ObjectiveTo observe the therapeutic effect of early postoperative comprehensive rehabilitation on elbow joint dysfunction and ulnar nerve injury in children and adolecents with supracondylar fracture of humerus complicated with ulnar nerve injury. MethodsA total of 49 children with supracondylar fracture of humerus complicated with ulnar nerve injury after operation were selected from January, 2016 to December, 2021 in Wangjing Hospital, which were randomly divided into control group (n = 24) and treatment group (n = 25). The control group accepted wax therapy and acupuncture, and the treatment group accepted medicine fumigation, joint mobilization and electromyographic biofeedback, for twelve weeks. They were assessed with The Hospital for Special Surgery Elbow score (HSS) and Medical Research Neurotrauma Society Report (MCRR) before and after treatment. ResultsAfter treatent, the HSS scores increased in both groups (|t| > 8.345, P < 0.001). The HSS score was significantly higher in the treatment group than in the control group (t = 4.536, P < 0.001). The d-value of HSS scores before and after treatment was significantly higher in the treatment group than in the control group (t = 3.717, P < 0.05). The rate of excellent recovery of ulnar nerve function was significantly higher in the treatment group than in the control group (χ2 = 5.975, P < 0.05). ConclusionEarly postoperative comprehensive rehabilitation could romote the recovery of elbow function and ulnar nerve injury in children and youth with supracondylar fracture of humerus complicated with ulnar nerve injury.
7.Effect of suspension combined with acupuncture on chronic non-specific neck pain

ObjectiveTo explore the effect of suspension combined with acupuncture on chronic non-specific neck pain (CNNP). MethodsA total of 81 patients with unilateral CNNP from the rehabilitation outpatient clinic of Wangjing Hospital from March, 2022 to March, 2023 were divided into suspension group (n = 27), acupuncture group ((n = 27) and combined group (n = 27) randomely. The suspension group underwent conventional suspension, the acupuncture group received conventional acupuncture treatment, and the combined group underwent suspension and acupuncture, for six weeks. The Visual Analogue Scale (VAS), Neck Disability Index (NDI), and cross-sectional area (CSA) of longus colli and multifidus in the neck using ultrasound imaging were compared before and after treatment. ResultsAfter treatment, the VAS and NDI score, and CSA of longus colli and multifidus improved in three groups (|t| > 4.473, P < 0.001), and the combined group was better than the other two groups (P < 0.05). ConclusionSuspension and acupuncture can relief the pain, improve function, and increase CSA of longus colli and multifidus in patients with unilateral CNNP, while the combination of the two methods is better.
8.Analysis of plantar pressure for functional ankle instability

ObjectiveTo observe the distribution characteristics of plantar pressure of patients with functional ankle instability (FAI). MethodsFrom January to July, 2021, the Freestep plantar pressure analysis system was used to measure plantar pressure of 20 patients with FAI in Department of Rehabilitation, Beijing Tongren Hospital. The data beteen the healthy side and the affected side were compared. ResultsWhen standing with both feet and eyes open, the weight ratio of the forefoot load was less (t = 2.251, P = 0.036), and the weight ratio of the hindfoot load was more (t = 2.251, P = 0.036) in the affected side than in the healthy side. When standing with the affected foot and eyes open, the average longitudinal distance (Y) was more in the affected side than in the healthy side (t = 3.271, P < 0.01); when standing with the affected foot and eyes closed, the length of the ball, the area of the ellipse, the average speed, and the average Y were more in the affected side than in the healthy side (t > 3.140, P < 0.01). When walking, the force area and load of the medial heel were more (t > 2.260, P < 0.05), the maximum pressure and average pressure were significantly less (t > 5.005, P < 0.001), and the line increment of the pressure center and the ratio of foot gait line length/footprint length were significantly more (t > 20.567, P < 0.001) in the affected side than in the healthy side. The contribution of loads of both forefoot and hindfoot was significantly different from the health side (t = 2.153, P < 0.001). ConclusionFor patients with FAI, when standing with both feet, the center of gravity of the affected side is more unevenly distributed; when standing with the affected foot, the posture control stability is obviously weaker, especially when the eyes are closed; when walking, the dynamic stability of the affected side is worse.
9.Effect of power electrical stimulation in combination with task-oriented training on lower extremity motor function in stroke patients with foot drop

Objective Toinvestigatetheeffectofpowerelectricalstimulation(PES)incombination withtask-specifictrainingonlowerextremitymotorfunctioninstrokepatientswithfootdrop.Methods Fifty patients with poststroke foot drop were enrolled retrospectively. They were divided into either an experimental group or a control group (n=25 in each group). The control group received routine rehabilitation treatment. On this basis,the experimental group was given PES in combination with task-oriented training. Both groups of patients were trained 2 times a day,once for 40 minutes,5 days a week for 6 weeks. The ankle active range of motion,plantar flexor muscle tension,and tibialis anterior muscle strength were determined before and after treatment. The balance and lower extremity motor function of the patients were assessed by using Berg balance scale score,Fugl-Meyer lower extremity score,modified Barthel index,and Holden walk grading. At thesametime,thewalkingspeedandsteplengthbeforeandaftertreatmentwerecompared.Results Six weeks after treatment,the ankle activity,plantar flexors tension,and tibialis anterior muscle strength scores in patients of the experimental group were improved compared with before treatment,and each indicator was significant better than the control group (t=6. 261,-6. 163,and 2. 968,respectively;all P<0. 05). Berg balance scale,walking speed and step length scores were also improved as compared with before treatment, and each indicator was better than the control group (t=10. 733,9. 074,and 9. 013,respectively;all P<0.01). The lower limb motion scores,modified Barthel indexes,and Holden walk grading scores were improved compared with before treatment,and each indicator was significantly better than the control group (t=3.261,7.573,and4.010,respectively;allP<0.05).Conclusion Usingpowerelectricalstimulation in combination with task-oriented training may effectively improve the lower extremity motor function in stroke patients with foot drop.
