1.Improvement of quality standards for Zhuang medicine Yingbupu (Aralia armata)

OBJECTIVE To improve the quality standard of Zhuang medicine Yingbupu (Aralia armata). METHODS A total of 23 batches of Yingbupu (A. armata) were studied. Their macroscopic characteristics and powder microscopic features were observed. TLC was employed for the qualitative identification of oleanolic acid and araloside A. Items such as water content, total ash, acid-insoluble ash, and ethanol-soluble extract were determined according to the methods specified in the 2020 edition of the Chinese Pharmacopoeia (part Ⅳ). UPLC fingerprint was established for 23 batches of samples by using Similarity Evaluation System for Chromatographic Fingerprints of Traditional Chinese Medicine (2012 edition), and the contents of oleanolic acid and araloside A were determined. RESULTS The powder microscopic characteristics of the medicinal material were distinctive. Oleanolic acid and araloside A were detected by TLC in all 23 batches. Among the 23 batches of samples, the content ranges of moisture, total ash, acid-insoluble ash, and ethanol-soluble extract were 6.9% to 10.4%, 1.8% to 6.8%, 0.1% to 1.9%, and 2.8% to 8.4%, respectively. Based on the UPLC fingerprint, a total of 15 common peaks were obtained, and 9 of these common peaks were identified. The content ranges of oleanolic acid and araloside A in the 23 batches of samples were 0.86% to 2.69% and 0.16% to 1.10%, respectively. CONCLUSIONS This study has added items such as moisture and total ash content fingerprint, TLC identification. A preliminary quality standard has been established for the medicinal material of Yingbupu (A. armata), stipulating that the moisture content should not exceed 11.0%, the total ash content should not exceed 5.0%, the acid-insoluble ash content should not exceed 2.5%, the ethanol-soluble extract(No. content should not be less than 4.0%, and the contents of zyyzdxk-2023165) oleanolic acid and araloside A should not be less than 1.00% and 0.45%( calculated by a dried basis), respectively.
2.Development of Non-Invasive Bi-Level Breathing Therapy System.

At present, there is no effective drug treatment for obstructive sleep apnea hypopnea syndrome (OSAHS). It is usually treated by mechanical ventilation through a ventilator. In this paper, a non-invasive bi-level breathing therapy system suitable for home scenarios is developed. The system supports single-level and bi-level positive airway pressure therapies, and introduces the function of inspiratory synchronous trigger based on flow monitoring to enhance the synchrony of patient-ventilator synchronization. The test results show that the performance indicators of the system meet expectations. Each ventilation mode can operate normally and can meet the requirements for the use of home non-invasive ventilators.

3.Analysis of Brain-Computer Interface Technology in the Medical Field and the Regulation of the US FDA.

Brain-computer interface (BCI) technology is an innovative and cutting-edge medical advancement that enables direct interaction between the brain and external devices, facilitating the reconstruction of daily functions for patients or serving as a method for neuro-regulation therapy. Although this technology offers a broad range of clinical applications, there are problems as potential risks, individual variations, and the need for long-term monitoring of its effects during utilization. Consequently, the comprehensive evaluation of its safety and effectiveness poses a considerable challenge for regulatory agencies. This study provides a concise introduction to the development history and various types of BCI technology, followed by a summary of the regulatory situation for different types of BCI medical devices in the United States. Furthermore, the regulatory requirements imposed by the US FDA on this product category are analyzed. Finally, the article concludes by presenting a summary and future perspective on the current development of BCI technology, with the aim of offering beneficial insights and guidance for the regulation of BCI medical devices.

4.Mixed urinary incontinence:will urgency and urge urinary incontinence symptoms resolve after RMUS?

Objective To explore the efficacy of retropubic midurethral synthetic sling(RMUS)in relieving urgency and urge urinary incontinence(UUI)symptoms in patients with mixed urinary incontinence(MUI).Methods Clinical data of 44 female MUI patients treated with RMUS during Jan.2018 and Dec.2020 in Shanghai Renji Hospital were retrospectively analyzed.All patients had positive results in stress test and Marshall-Marchetti test before operation,and 27 of them completed ICIQ-FLUTS-LF questionnaire.Urodynamic(UDS)tests suggested that 9 patients(20.5%)presented detrusor overactivity(DO).During RMUS procedure,the tension of the sling was adjusted based on the anatomical landmarks.The postoperative efficacy and improvement of urinary incontinence were analyzed.Results The patients aged(58.59±9.08)years,with a body mass index of 24.71±2.77.Among the 40 patients who completed telephone interview 2 years after surgery,the subjective cure rate was 85.0%(34/40).Among the 27 patients with records of questionnaires before and after surgery,there were significant differences in the incidence of urine leakage[100%(27/27)vs.18.5%(5/27)],stress urinary incontinence(SUI)[100%(27/27)vs.18.5%(5/27)]and UUI[70.4%(19/27)vs.29.6%(8/27)](P<0.05).However,no statistical differences were found regarding nocturia voiding episode(≥1 times),urgency,dysuria,hesitancy,strain to void,intermittent stream and enuresis(P>0.05).Based on preoperative UDS test,there were significant differences regarding the subjective cure rate in patients with or without preoperative DO[55.6%(5/9)vs.93.5%(29/31)],incidence of SUI[66.7%(4/6)vs.4.8%(1/21)],and UUI[66.7%(4/6)vs.19.0%(4/21)](P<0.05),while there was no statistical difference regarding urgency[66.7%(4/6)vs.33.3%(7/21)](P>0.05).Conclusion RMUS is effective in treating MUI patients with positive stress test and Marshall-Marchetti test results,which can relieve SUI and UUI symptoms,but has no effects on urgency symptoms.DO on preoperative urodynamics results in poorer subjective outcomes.
5.Clinical application of lower eyelid blepharoplasty with orbital fat releasing and repositioning through supra-suborbicularis oculi fat

Objective:To evaluate the clinical effect of orbital fat releasing and repositioning through supra-suborbicularis oculi fat (SOOF) in lower eyelid blepharoplasty.Methods:The authors retrospectively analyzed the clinical data of patients who received lower eyelid blepharoplasty in the Department of Plastic Surgery, Beijing Tsinghua Changgung Hospital from January 2017 to January 2023. Lower eyelid blepharoplasty with orbital fat releasing and repositioning through supra-SOOF was performed. The tear trough ligament and orbicularis retaining ligament were released between the orbicularis oculi muscle and SOOF via skin approach. Orbital septal fat was released to this space, lateral fat flap was fixed to the SOOF capsule, medial fat flap was fixed to the superficial fascia of the levator labii superioris muscle, orbital septum was repositioned, and lateral canthus was suspended. After the operation, lower eyelid morphology and complications were evaluated.Results:A total of 58 patients were enrolled, including 8 males and 50 females with the age of (49.8 ± 10.4) years (32 to 70 years). All patients had obvious lower eyelid bags on both sides, combining the tear trough deformity and (or) palabromalar groove. Fifty-seven patients were followed up for (29.1 ± 16.9) months (6-67 months). All patients were satisfied with the operation. The appearance of lower eyelid was greatly improved, with elimination of fat prominence and tear trough deformity. Mild lower eyelid ectropion was observed two cases 1 week after the operation, and were relieved by moderate massage 2 weeks postoperatively. In one case, mild lower eyelid retraction was observed 1 month postoperatively, with no complaint of discomfort or further treatments. Bulbar conjunctival hemorrhage occurred in 1 case and recovered without further interventions.Conclusion:Lower eyelid blepharoplasty with orbital fat releasing and reposting through supra-SOOF can effectively release the tear trough ligament and the orbicularis retaining ligament, with stronger fat fixation and less surgical difficulty. After the operation, eyelid contour was flat and tight with fewer complications and higher patient satisfaction.

7.Development of Wireless Wearable Sleep Monitoring System Based on EEG Signal

A wireless wearable sleep monitoring system based on EEG signals is developed.The collected EEG signals are wirelessly sent to the PC or mobile phone Bluetooth APP for real-time display.The system is small in size,low in power consumption,and light in weight.It can be worn on the patient's forehead and is comfortable.It can be applied to home sleep monitoring scenarios and has good application value.The key performance indicators of the system are compared with the industry-related medical device measurement standards,and the measurement results are better than the special standards.
8.Development of Non-Invasive Continuous Blood Pressure Measurement System Based on Multiple Feature Parameters

To address the issue of the difficulty in implementing non-invasive continuous blood pressure measurement technology in China,this study developed a high-performance synchronous electrocardiogram(ECG)and photoplethysmography(PPG)signal acquisition system.A PC-based human-computer interaction software platform was constructed,and continuous blood pressure measurement-related algorithms were integrated.Multiple feature parameters such as pulse wave transit time based on synchronous ECG and PPG signals were extracted,enabling non-invasive continuous blood pressure measurement.To verify the measurement accuracy of the system,tests and comparative verifications were carried out.The results demonstrated that the system could meet the requirements of relevant standards and have good application value.
9.Effect of parent-child alienation on epression among surface ship officers and soldiers:mediating role of resilience

Objective To investigate the effect of parent-child alienation on depression in surface ship officers and soldiers based on the theory of"diathesis-stress",and the mediating role of resilience between parent-child alienation and depression in them.Methods A cross-sectional study was conducted on 599 officers and soldiers from a surface ship unit.The participants were surveyed with inventory of alienation toward parents,connor-davidson resilience scale and patient health questionnaire-9 to obtain and analyze their demographic-military characteristics of their depression scores.The participants with depression scores ≥5 were recruited as the subjects,and Spearman correlation analysis was used to explore the correlation among parent-child alienation,resilience and depression.On the basis of hierarchical regression analysis,AMOS software was used to establish a structural equation modelling of intermediary effects.Results The depression score was 1(0,4)in the participants,and the depression scores of those with service length ≥11 years were comparatively higher than those with shorter length.Our results indicated that parent-child alienation was positive correlated with depression(r=0.451,P<0.001),while resilience was negatively correlated with depression and parent-child alienation(r=-0.412,-0.407,P<0.001).Regression analysis revealed that parent-child alienation had a direct positive predictive value for depression(β=0.574,P<0.001),and resilience showed a negative predictive value for depression(β=-0.211,P<0.01).Model analysis displayed that resilience had a significant mediating role in the effect of parent-child alienation on depression among these surface ship officers and soldiers,with an effect value of 0.088,and accounting for 15.86%of the total effect.Conclusion Parent-child alienation has a significant influence on depression among surface ship officers and soldiers,with resilience playing a partial mediating role.
10.Relationship between stress and insomnia in plateau soldiers:mediating role of parent-child alienation and moderating role of expressive suppression

Objective To investigate the influence of stress on insomnia among plateau soldiers,and explore the mediating role of parent-child alienation and the moderating role of emotion regulation strategies in this relationship.Methods A cross-sectional study design was adopted in this study.Our Self-designed General Information Questionnaire,Plateau Soldiers Stressor Scale,Insomnia Severity Index,Inventory of Alienation towards Parents,and Emotion Regulation Scale were employed to survey 2 065 plateau soldiers at an average altitude of 4 200 m.With aid of SPSS 26.0 statistics,descriptive analysis,correlation analysis and regression analysis were applied to analyze stress status,parent-child alienation,insomnia and emotion regulation in the participants,and then a structural equation model was established.Results ① The scores of stress,parent-child alienation and insomnia were significantly higher in the participants who were older,well educated(P<0.01)and were or had been in a marital relationship(married,divorced,or widowed)(P<0.001)than those without above conditions.② Significant pairwise positive correlations were observed among stress,parent-child alienation,insomnia,cognitive reappraisal,and expressive suppression(r=0.08~0.61,P<0.001).③ Stress and expressive suppression could positively predict parent-child alienation(β=0.14~0.43,P<0.001),and stress,parent-child alienation and expressive suppression could positively predict insomnia(β=0.04~0.25,P<0.01).Cognitive reappraisal could not significantly predict parent-child alienation or insomnia.④ Parent-child alienation towards parents partially mediated the relationship between stress and insomnia,with a mediation effect accounting for 7.29%of the total effect.⑤ Expressive suppression moderated the impact of stress on insomnia(β=-0.01,P<0.001),and the higher expressive suppression level mitigated the adverse effect of stress on insomnia when compared with the lower expressive suppression level.Conclusion For plateau soldiers,stress significantly predicts insomnia,with parent-child alienation playing a mediating role.Furthermore,a higher level of expressive suppression can effectively buffer the influence of stress on insomnia.
