1.Research progress in human immunodeficiency virus entry inhibitors

Human immunodeficiency virus(HIV)entry inhibitors are important in preventing HIV infection by specifically targeting viral envelope proteins(gp120/gp41)or host receptors(CD4/CCR5/CXCR4)to inhibit HIV entry into host cells. This article systematically reviews the research progress of HIV entry inhibitors in recent years,and some of the novel inhibitors such as Fostemsavir,Ibalizumab,and bifunctional fusion inhibitors(2P23-iMab)have shown better effects in inhibiting multidrug-resistant strains. Although great progress has been made in the research and development of HIV entry inhibitors,there are still a series of challenges including drug resistance,drug limitation,target toxicity,which limit their widespread clinical application. Future research directions should focus on multi-target combination strategies,broad-spectrum neutralizing antibody development,and long-term design. In summary,the analyses on the research progress,challenges and future development directions of HIV entry inhibitors will provide a reference for the further research and development and optimization of HIV entry inhibitors,and then promote the development of AIDS prevention and control.
2.Nutritional Support Analysis of A Cancer Patient with Postoperative Septic Shock Combined Acute Kidney Injury and Undertaking Continuous Renal Replacement Therapy

Objective To explore the role of clinical pharmacists in pharmaceutical care of patients with severe infection receiving nutritional support therapy during continuous renal replacement therapy(CRRT),and to provide information for clinical rational use of drugs.Methods Clinical pharmacists participated in the treatment of a patient with severe postoperative septic shock and undertaking CRRT,we analyzed the pathophysiological characteristics of the patient at each stage,and cooperated with clinicians to formulate individualized nutritional support regimen.The patient was closely monitored during the whole treatment and the nutritional support prescription was adjusted accordingly.Results Clinical pharmacists participated in formulating,adjusting of the nutrition treatment regimen and monitoring of its implementation.The patients gradually recovered after comprehensive treatment and monitoring.Conclusion Clinical pharmacists can provide individualized nutritional support regimen and effective monitoring given patient's nutritional needs changes during the different disease stages,which guaranteed the rational medicines usage in clinical practice.

5.Study on the use of antimicrobial agents in hospitalized trauma patients based on medical big data

Objective To investigate the relationship between the trauma severity and the usage of antibacterial drugs and to provide reference for standard protocol of proper antibiotic use in wound care.Methods ICD-10 and AIS were used to set up the relationship and to analyze the use of antibiotics in patients with different trauma score.Results 25 035 trauma patients were enrolled in this study.Those patients were divided into five groups according to the AIS score with least severe as group 1 to most severe as group 5.The patient percentage in group 1 to 5 was 21.92%,67.73%,8.86%,0.97% and 0.52% respectively.The five most frequently used antibiotic classes are second generation cephalosporins,third generation cephalosporins,first generation cephalosporins,fluoroquinolones and penicillin/beta lactamase inhibitor combination, accounted for 29.69%,22.57%,20.33%,4.66% and 4.47% of total DDDs of antibacterial drugs.Individually, the top 10 antibiotics are cefuroxime (12.21%), cefazolin (8.31%), ceftriaxone (7.74%), cefathiamidine (7.34%), cefotiam (4.87%), ceftazidime (3.68%), amoxicillin/clavulanic acid (3.63%), levofloxacin (3.59%), cefoxitin (3.56%), flucloxacillin (3.52%);gentamicin (2.27%), ornidazole (2.00%) and cefoperazone/tazobactam (1.44%) were used most in their categories respectively.The variety and quantity of antibacterial drugs used for different trauma patients were different.Conclusion The trauma score based on ICD-AIS can reflect the severity of trauma.The use of antibiotics in patients with different trauma score can provide reference for the clinical applications of antibiotics in wound care.
6.Comparative analysis of the anterior decompression and fusion with internal fixation and simple decompression in the treatment of cervical vertebral multilevel degeneration

Objective To analyze the clinical value of anterior decompression and fusion with internal fixation and simple decompression in the treatment of cervical vertebral multilevel degeneration.Methods Selected 40 patients with cervical degenerative changes in our hospital from 2012 to 2014,and 20 patients of them underwent anterior decompression and fusion with internal fixation while the other 20 cases took simple decompression.All the patients were well followed up,and the clinical therapeutic effect of the two groups were compared and analyzed.Results After operation,the VAS score and JOA score showed that the scores of the patients underwent anterior decompression and fusion with internal fixation were significandy improved compared with the simple decompression group,and the difference was statistically significant(P < 0.05).Compared with the simple decompression group,the cervical curvature and the curvature of the surgerysection in the anterior decompression and fusion with internal fixation group were significantly improved,especially in the first year after surgery (P < 0.05).The cervical spine joint activity of the two group were improved in the first year after surgery,and the difference was significant (P < 0.05),but there was no significant difference in the degree of cervical joint activity at the last follow-up(P > 0.05).The anterior decompression and fusion with internal fixation is better than simple decompression in the degree of cervical joint activity with statistical significance (P < 0.05).Conclusion The clinical effect of anterior decompression and fusion with internal fixation in treatment of multiple segment degeneration of cervical spine is better than that of simple decompression,it can meet the clinical requirements of biomechanics.
