1.Correlation between CD4+T lymphocytes and viral load in newly identified human immunodeficiency virus infected individuals in Huangpu District,Shanghai,2023

ObjectiveTo analyze the detected results of CD4+T lymphocytes and viral load in newly identified human immunodeficiency virus (HIV) infected patients in Huangpu District of Shanghai in 2023, to explore the correlation between them, so as to provide a scientific basis for the development of targeted prevention and control measures and antiviral treatment programs. MethodsThe data of CD4 cell count, viral load and demographic characteristics of the newly infected patients living with HIV in Huangpu District, Shanghai in 2023 were collected and analyzed by using descriptive epidemiological method. ResultsThe mean CD4 cell count of the 67 newly identified HIV infected patients in Huangpu District was (301.22±235.19) cells·µL-1, with a mean viral load of (5.15±1.28) ×105 copies·mL-1.There were statistically significant differences in CD4 cell count and viral load among different age groups (P<0.05), but there were no statistically significant differences by gender and marital status (both P>0.05). The CD4 cell count and CD4/CD8 ratio both were negatively correlated with the lg value of viral load (r=-0.290, -0.378; P=0.027, 0.002). ConclusionThe CD4 cell counts of the newly identified HIV infected patients in Huangpu District in 2023 were generally low, the proportion of patients with high viral load was high, but the risk for elderly infected with HIV was high. The elderly have gradually become the key population for AIDS prevention and control in Huangpu District. It is recommended to expand HIV screening in the elderly to reduce the risk of HIV transmission and increase the rate of early detection and treatment.
2.Trends in adenoidectomy in children in Beijing tertiary hospitals from 2013 to 2022

To examine the evolution of surgical techniques and trends in overall inpatient burden for pediatric adenoidectomy in Beijing tertiary hospitals from 2013 to 2022. A retrospective observational study was conducted using the regional health information platform of Beijing. Data from children aged ≤14 years who underwent adenoidectomy between 2013 and 2022 were extracted, including total hospitalization cost, length of stay(LOS), surgical material cost, surgical fee, operative technique, perioperative antimicrobial drugs cost, coagulation factor cost, and blood transfusion cost. The Mann-Kendall trend test was used to assess temporal changes in total hospitalization expenses and the structure of cost components. The results showed that over the 10-year period from 2013 to 2022, a total of 25 989 children underwent adenoidectomy in tertiary hospitals. The proportion of children aged ≤6 years increased from 59.83% to 76.11%, showing a significant upward trend ( Z=2.15, P=0.032). Only one case required surgical hemostasis due to postoperative bleeding. During the ten-year period, the median hospitalization cost for adenoidectomy in tertiary hospitals was ¥12 425.82 (¥11 307.43, ¥14 955.42).Overall hospitalization cost demonstrated a fluctuating downward pattern, decreasing from ¥15 229.73 in 2013 to ¥13 927.52 in 2022, this declining trend was not statistically significant( Z=-0.54, P=0.592). In contrast, the surgical costs showed an upward trend over the decade increasing from ¥1 856.22 in 2013 to ¥3 726.45 in 2022, which was statistically significant ( Z=3.22, P=0.001), while the cost of surgical materials showed no significant increase ( Z=1.79, P=0.074).Concurrently, the average LOS decreased remarkably from 10.56 days in 2013 to 3.26 days in 2022 ( Z=-3.94, P<0.001). The cost of perioperative antimicrobial drugs decreased ( Z=-3.94, P<0.001), while the cost of coagulation factors and blood transfusion remained unchanged ( Z=0.54, P=0.592; Z=0.56, P=0.578). Comparison between 2013-2017 and 2018-2022 showed a significant increase in the use of coblation from 28.9% to 42.5% ( χ2=638.7, P<0.001).Furthermore, in the coblation group, total hospitalization cost decreased by 27.73%, surgical cost increased by 94.98%, surgical material cost decreased by 10.33%, LOS shortened by 56.24%, and antimicrobial drug cost increased by 43.03%. In contrast, the non-coblation group showed a 23.94% increase in total hospitalization cost, a 57.08% increase in surgical procedure cost, a 33.88% increase in material cost, and a 30.14% reduction in LOS and a 26.0% decrease in antimicrobial drugs cost. In conclusion,from 2013 to 2022, total hospitalization cost for pediatric adenoidectomy in Beijing tertiary hospitals remained stable. Compared to non-coblation techniques, coblation was associated with a shorter LOS, lower total costs, a higher proportion of surgical fees, and a decreased proportion of material costs, without a significant increase in overall healthcare costs.

4.Study on the correlation between viral load and activation and exhaustion levels of CD8 +T cells in HIV/AIDS patients

Objective:To investigate and analyze the correlation between the expression levels of CD38, HLA-DR and programmed cell death-1 (PD-1) on peripheral blood CD8 +T cells and HIV-1 RNA viral load, immune activation and exhaustion in patients with human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS). Methods:A total of 81 HIV/AIDS patients (64 without antiretroviral therapy and 17 with therapy) and 40 healthy donors in the same period were enrolled as the control group. Flow cytometry was used to analyze the CD4 +T lymphocyte count and the expression levels of activation markers CD38 and HLA-DR and apoptosis marker PD-1 on CD8 +T cells. HIV-1 RNA in the plasma of HIV-1 infected patients was quantitatively detected by real-time fluorescence quantitative polymerase chain reaction. Variance analysis was used to compare the expression levels of CD38, HLA-DR and PD-1 on CD8 +T cells between HIV/AIDS patients and healthy controls. Spearman correlation analysis was used to analyze the correlation between different T lymphocyte counts and HIV RNA viral load, and the correlation between HIV RNA viral load and peripheral blood CD8 +T cell CD38, HLA-DR and PD-1. Results:Among the 81 HIV/AIDS patients, 69 (85.19%) were males and 12 (14.81%) were females, with an age M ( Q1, Q3) of 58 (36.5, 65.0) years. There were 60 HIV/AIDS patients over 55 years old (74.07%) and 21 HIV/AIDS patients between 18 and 55 years old (25.93%). The results of variance analysis showed that compared with the healthy control group, the expression levels of CD38, HLA-DR and PD-1 on CD8 +T cells in HIV/AIDS patients increased, and the differences were statistically significant (all P<0.05). In addition, the expression of CD38, HLA-DR and PD-1 increased significantly in patients with CD4 +T cell count less than 350 cells/μl, and the differences were statistically significant (all P<0.05). Spearman correlation analysis showed that CD4 +and CD4 +/CD8 +were negatively correlated with viral load in HIV/AIDS patients ( r=-0.407 and -0.378, respectively, both P<0.05), and CD8 +was positively correlated with viral load ( r=0.356, P<0.05). When the HIV RNA level was≤10 5 CPs/ml, there was no correlation between the HIV RNA level and the expression levels of CD38, HLA-DR and PD-1 on CD8 +T cells (all P>0.05). However, when the level of HIV RNA was>10 5 CPs/ml, the level of HIV RNA was positively correlated with the expression levels of CD38, HLA-DR and PD-1 on CD8 +T cells ( r=0.412, 0.387, 0.395, respectively, all P<0.05). Conclusions:The activation levels of CD38 and HLA-DR and the expression of PD-1 on CD8 +T cells in the peripheral blood of HIV/AIDS patients are increased. When the viral load is high, the HIV RNA viral load is positively correlated with the activation and exhaustion levels of CD8 +T cells.

6.Correlation analysis between mental health and work engagement of nurses

Objective:To investigate the status of mental health and work engagement of clinical nurses, to explore the correlation between them, and to provide scientific basis for improving the mental health and increase work engagement levels of nurses.Methods:From August to September 2018, 1 140 nurses in Taihe Hospital of Shiyan City of Hubei Province · Affiliated Hospital of Hubei University of Medicine were selected as survey objects by convenient sampling method, and the mental health and work engagement of nurses were investigated by Symptom Checklist-90 (SCL-90) and Utrecht Work Engagement Scale (UWES-9), correlation analysis was used to explore the relationship between mental health and work engagement, and multiple linear regression was used to analyze the influencing factors of work engagement in clinical nurses.Results:The score of SCL-90 of clinical nurses was 123.01±38.14 and the score of UWES-9 was 3.53±1.40. Correlation analysis showed that the nurse with total scores of mental health and work engagement, vitality, dedication, and concentration were negative correlation( r values were -0.250--0.218, P<0.01). Multiple linear regression analysis showed that age, depression, and paranoia were the influencing factors of clinical nurses work engagement ( t values were 4.682, -3.211, -2.715, P<0.01). Conclusions:The mental health level of nurses is good, and their work engagement level is average. Mental health is closely related to work engagement. Nursing managers should take effective measures to improve the mental health and increase work engagement levels of nurses.
7.Reliability and validity of Chinese version of the Spirituality and Spiritual Care Rating Scale

Objective:To translate the Spirituality and Spiritual Care Rating Scale (SSCRS) into Chinese and test its reliability and validity.Methods:After obtaining authorization from the original author, we carried out translation, back translation and cross-cultural adaptation based on the Brislin principle, and formed the Chinese version of the SSCRS. From April to June 2019, we conducted the questionnaire survey among 189 nurses at a Class Ⅲ Grade A hospital in Shiyan by convenience sampling to evaluate the reliability and validity of the Chinese version of SSCRS.Results:The Chinese version of SSCRS had 17 items. Exploratory factor analysis extracted a total of 4 common factors, with a cumulative contribution rate of 62.83%. The Cronbach's α coefficient of the entire scale was 0.840, the split-half reliability was 0.842, and the test-retest reliability was 0.860. The scale-level content validity index of all items was 0.91, and the item-level content validity index of each item ranged from 0.81 to 1.00.Conclusions:The Chinese version of SSCRS has a good reliability and validity, which can be used as an effective tool to evaluate nurses' cognition of spirituality and spiritual care in China.
8.Reliability and validity test of Chinese version of the Oral Health Assessment Tool

Objective? To translate the Oral Health Assessment Too(l OHAT) into Chinese and examine the Chinese version of OHAT's reliability and validity. Methods? The scale was translated according to Brislin's two-person translation and back-translation model and was modified and adjusted through expert consultation and small sample pre-experiment to form the Chinese version of OHAT. In order to verify the reliability and validity of the scale, 200 community elderly people were selected for oral health assessment by convenience sampling method from January to February 2018. Results? There were 8 items in the Chinese version of OHAT, the critical ratio of each item had statistical significance (P< 0.01), the scale content validity index (S-CVI) was 0.875, and the item-content validity index (I-CVI) was 0.80-1.00. Three common factors were extracted from the scale, the cumulative contribution rate was 65.218%, and the factor load of each item was more than 0.4. The Cronbach's alpha coefficient of the scale was 0.710 and the retest reliability was 0.811. Conclusions? OHAT reflects the oral health status of the elderly. The Chinese version of OHAT has good reliability and validity and can be used as a tool for oral health assessment of the elderly in China.
9.The application of the growth mentor program in the standardized training of the new nurses

Objectives? To explore the effects of applying growth mentor program in the training of new nurses. Methods? By convenient sampling, the newly recruited nurses who signed official employment contract with the First Affiliated Hospital of Xi'an Jiaotong University were selected as subject. A total of 756 nurses who enrolled from 2008 to 2012 were put into the control group and 812 nurses enrolled from 2013 to 2017 were in the observation group. The control group received routine training while the observation group received the training of the growth mentor program. The two groups was compared in the resignation rate, annual assessment scores, and patients' satisfaction. Results? The differences in resignation rate between the observation group (0.37%) and the control group (1.06%) were statistical significant (P<0.01). The annual assessment scores of the observation group (88.63±2.17) and patients' satisfaction (96.78±1.76) were all higher than the control group with statistical significance (P< 0.01). Conclusions? The establishment and implementation of the new nurses' growth mentor program can significantly reduce the nurses' resignation rate, improve the nurses' professional skills and patients' satisfaction, therefore the program should be widely applied in new nurses' training in hospitals.
10.Cuprous oxide nanoparticles effect on the epithelial-mesenchymal transition of B16 cells

Objective To research the epithelial-mesenchymal transition (EMT) effects of cuprous oxide nanoparticles (CONPs) on melanoma.Methods Cuprous oxide nanoparticles were prepared hydrothermally.The B16 cells were cultured with cuprous oxide nanoparticlesat different concentrations (5,25,50 μg/ml).The changes of the morphology of the B16 cell were observed under the inverted microscope.The effects of CONPs on B16 cell migration ability were detected through the Wound healing assay and the Transwell assay.Then cell immunofluorescence and western blotting were used to test the EMT related molecular markers, including E-cadherin, N-cadherin, Cytokeratin, and Vimentin.Results The synthesized cuprous oxide nanoparticles distribute uniformly with a diameter of 40 nm.Our study indicated that CONPs inhibited the EMT of B16 cell.A conversion process was discovered in this study.In B16 cells, CONPs inhibited B16 cell migration, promoted the expression of E-cadherin, Cytokeratin and Desmoplakin, while the expression of N-cadherin and Vimentin was repressed in protein level.Conclusion Cuprous oxide nanoparticles can significantly restrain the invasion and metastasis of melanoma cells and inhabit the EMT of B16 cells.
