1.Adverse reaction surveillance analysis of domestic human papillomavirus vaccines with different production processes

This study aims to compare the vaccination rates and incidence of adverse reaction rates following administration of two domestically produced human papillomavirus (HPV) vaccines in individuals aged 9-30 years,investigate the impact of distinct manufacturing processes and vaccination schedules on adverse reaction rates. From November 2023 to June 2024, the Immunization Planning Department of Liuzhou Center for Disease Control and Prevention conducted a single-center, randomized, open-label, parallel-group trial using community-based recruitment of eligible participants aged 9 to 30 years. Participants were randomly assigned to receive either of two domestically produced HPV vaccines (Walrinvax or Cecolin). As specified in the vaccine package inserts, subjects were stratified into a two-dose regimen group (aged 9-14 years) and a three-dose regimen group (aged 15-30 years). Vaccination rates were recorded, and adverse reactions within 0-30 days post-vaccination were monitored. The results showed that a total of 400 participants were enrolled. Both the full vaccination rate and the timely completion rate were significantly higher in the two-dose regimen group compared to the three-dose regimen group (Fisher′s exact test, P<0.01; χ2=7.06, P<0.01). A total of 985 doses were administered. The overall adverse reaction rate was 18.78% (185/985), with local and systemic reactions occurring at 8.02% (79/985) and 10.76% (106/985), respectively. The most frequent adverse reactions were injection site pain (4.97%, 49/985) and fever (4.47%, 44/985). No grade 4 or special-interest adverse events were reported.The incidence of adverse reactions for the two domestic HPV vaccines with different production processes (at 0/6 months) was 13.96% (55/394) and 17.46% (69/395) respectively, with no statistically significant difference (χ2=1.83, P>0.05).The adverse reaction rate was significantly lower in the 9-14 years group (9.77%) compared the 15-30 years group (24.91%)(χ 2=35.67, P<0.01). In conclusion, both domestic HPV vaccines demonstrated a favorable safety profile in the 9-30 years age group, with mostly mild adverse reactions. Compared to the three-dose schedule (15-30 years group), the two-dose HPV vaccination schedule (9-14 years group) significantly reduced the incidence of adverse reactions and improved vaccination compliance.

Loss-of-function variants in CSDE1 have been strongly linked to neuropsychiatric disorders, yet the precise role of CSDE1 in neurogenesis remains elusive. In this study, we demonstrate that knockout of Csde1 during cortical development in mice results in impaired neural progenitor proliferation, leading to abnormal cortical lamination and embryonic lethality. Transcriptomic analysis revealed that Csde1 upregulates the transcription of genes involved in the cell cycle network. Applying a dual thymidine-labelling approach, we further revealed prolonged cell cycle durations of neuronal progenitors in Csde1-knockout mice, with a notable extension of the G1 phase. Intersection with CLIP-seq data demonstrated that Csde1 binds to the 3' untranslated region (UTR) of mRNA transcripts encoding cell cycle genes. Particularly, we uncovered that Csde1 directly binds to the 3' UTR of mRNA transcripts encoding Cdk6, a pivotal gene in regulating the transition from the G1 to S phases of the cell cycle, thereby maintaining its stability. Collectively, this study elucidates Csde1 as a novel regulator of Cdk6, sheds new light on its critical roles in orchestrating brain development, and underscores how mutations in Csde1 may contribute to the pathogenesis of neuropsychiatric disorders.

4.Study on the prevalence and influencing factors of frailty in older adults with non-ST-segment elevation acute coronary syndrome

Objective:To investigate the prevalence and influencing factors of frailty among older adults diagnosed with non-ST-segment elevation acute coronary syndrome(NSTE-ACS).Methods:We conducted a cross-sectional study involving patients aged 65 years and older with NSTE-ACS, who were admitted to the Cardiology Center and the Department of Geriatrics at Beijing Friendship Hospital, Capital Medical University, between January 2020 and November 2021.Patients were categorized into non-frail, pre-frail, and frail groups based on the FRAIL scale.We collected clinical data, including general health conditions, comorbidities, laboratory results, treatments, and comprehensive geriatric assessments.Logistic regression analysis was employed to identify the influencing factors associated with frailty and pre-frailty in older adults with NSTE-ACS.Results:A total of 528 patients with NSTE-ACS were included in the study, comprising 308 males(58.3%)and 220 females(41.7%). The age range of participants was from 65 to 90 years, with a median age of 72(68, 76)years.The prevalence of frailty among older adults with NSTE-ACS was 11.4%(60/528), while pre-frailty was observed in 51.9%(274/528), and non-frailty in 36.7%(194/528). Compared to the non-frail and pre-frail groups, patients in the frail group were older, had a higher proportion of females, exhibited a greater prevalence of chronic diseases, and presented with elevated inflammatory markers.Additionally, frail patients demonstrated poorer nutritional status and reduced functional ability(all P<0.005). Risk factors for frailty in older adults with NSTE-ACS included older age( OR=1.110, 95% CI: 1.032-1.194, P=0.005), diabetes( OR=2.489, 95% CI: 1.091-5.679, P=0.030), cerebrovascular disease ( OR=4.151, 95% CI: 1.660-10.384, P=0.002), chronic kidney disease ( OR=42.874, 95% CI: 3.957-464.513, P=0.002), and elevated white blood cell levels( OR=1.424, 95% CI: 1.125-1.802, P=0.003). Conversely, being male( OR=0.252, 95% CI: 0.105-0.604, P=0.002)was identified as a protective factor against frailty in this patient population.For pre-frail older adults with NSTE-ACS, identified risk factors included diabetes( OR=1.882, 95% CI: 1.199-2.955, P=0.006), cerebrovascular disease( OR=1.938, 95% CI: 1.176-3.195, P=0.009), and chronic kidney disease ( OR=12.137, 95% CI: 1.536-95.934, P=0.018). Similarly, being male( OR=0.601, 95% CI: 0.376-0.961, P=0.033)was also a protective factor for pre-frailty in older adults with NSTE-ACS. Conclusions:The prevalence of frailty and pre-frailty among older adults with NSTE-ACS is notably high.Common risk factors for frailty and pre-frailty in this population include female gender, diabetes, cerebrovascular disease, and chronic kidney disease.
5.Research progress on gut microbiome metabolites and depressive disorders

Depressive disorders are common mental illnesses characterized by significant and persistent low mood, with features such as high prevalence, high disability rate, and high suicide rate. The microbiota-gut-brain axis may be one of the potential mechanisms underlying depressive disorders. Gut microbiota metabolites, as important mediators of MGBA signaling, play roles in depressive disorders through multiple pathways. These include short-chain fatty acids, which can regulate the transmission of the vagus nerve, inflammatory responses, and 5-hydroxytryptamine synthesis; secondary bile acids, which can activate farnesoid X receptor and Takeda G protein-compled receptor 5; and choline, which can regulate DNA methylation and trimethylamine N-oxide production. This article reviews the literature on the potential mechanisms of action of gut microbiota metabolites, such as short-chain fatty acids, secondary bile acids, and choline, in depressive disorders. The literature was retrieved from CNKI, PubMed, and Web of Science databases from 2010 to 2025. It aims to provide a theoretical basis for the prevention and treatment of depressive disorders.

8.Construction of an influencing factors model of supply quality of medical equipment services based on grounded theory in geriatric department

Objective:To construct influencing factors model of supply quality of medical equipment service based on grounded theory,so as to realize scientific,refined and standardized management of geriatric medical equipment. Methods:Based on the grounded theory of open coding,axial coding and selective coding,and taking the specific application of geriatric medical equipment as the core category,the influencing factor model of service supply quality was constructed to carry out total quality management of equipment. A total of 100 geriatric medical equipment in clinical use in Beijing Friendship Hospital were selected,and the equipment were managed from January to December 2021 by using conventional management methods,and the equipment were managed from January to December 2022 by using the grounded theoretical model. The self-designed recognition questionnaire was used to investigate the recognition scores of 4 engineers,6 doctors,10 nursing staff members,and 40 patients who used equipment for management. The defect rate of equipment management,use efficiency,management indicators,and multi-dimensional efficiency scores of the equipment were compared between the two management models. Results:The average scores of engineers,nurses,doctors,and patients who participated in the use management of grounded theoretical model were (92.25±2.14),(93.14±3.69),(92.56±3.14),and (93.58±4.36),respectively,which were higher than those of the conventional management model and the differences were statistically significant (t=18.897,13.729,12.843,9.826,P<0.05). After using the grounded theory model for management,there were 5,6,4,and 5 sets of equipment occurred packaging errors,missing equipment,damaged equipment,and insufficient supply,respectively. The defect rates of grounded theoretical model were 5%,6%,4%,and 5%,respectively,which were lower than those of the conventional management model. The differences were statistically significant (x2=6.438,12.706,11.054,10.286,P<0.05). The scores of equipment use efficiency,resource allocation,information basis,technical support,and management performance indicators of the grounded theory model were (92.68±5.36),(93.65±3.65),(94.26±4.01),(93.65±3.69),and (92.54±4.39),respectively. All were higher than those of the conventional management model,and the differences were statistically significant (t=8.729,16.856,12.113,15.593,10.358,P<0.05). The average scores for equipment use efficiency,maintenance efficiency,and repair efficiency under the management of grounded theory model were (93.65±3.26),(95.32±4.65),and (94.68±3.54),respectively,which were higher than those under the management of conventional management model,and the differences were statistically significant (t=19.915,12.362,10.686,P<0.05). Conclusion:The application of the factors model of supply quality of medical equipment service based on grounded theory can ensure the stability and safety of the equipment,and improve the quality of medical services and enhance the use efficiency of the equipment.
9.Comprehensive management strategy of interstitial lung disease induced by trastuzumab deruxtecan

Trastuzumab deruxtecan(T-DXd)has demonstrated significant efficacy in clinical trials for human epidermal growth factor receptor 2(HER2)-expressing breast cancer,gastric cancer,lung cancer and other solid tumors.Its overall safety profile is manageable and tolerable,including the clinically concerning interstitial lung disease(ILD).The etiology of ILD is varied,among which drug-induced ILD is an exclusionary diagnosis.The incidence of ILD caused by different antitumor drugs varies with different symptoms,and the pathogenesis remains unclear.T-DXd-induced ILD is mostly Grades 1-2,and implementing a standardized clinical management protocol can reduce the incidence of severe ILD events,improve patient prognosis,and help maximize the clinical benefits of T-DXd.This article summarized the epidemiology,etiology,risk factors,and potential mechanisms of drug-induced ILD,with a focus on the incidence,time to onset,and outcomes of T-DXd-induced ILD after standardized clinical management.It aimed to help readers understand the importance of standardized clinical management before and during T-DXd treatment.Regarding specific clinical management strategies,the article reviewed comprehensive management approaches for T-DXd-induced ILD based on clinical trial protocols and real-world experiences from both domestic and international perspectives,covering patient screening,patient education,ILD monitoring,diagnosis,and treatment.Before initiating T-DXd treatment,patient screening helps identify those at high risk for ILD,and T-DXd should be used cautiously in these high-risk patients.Effective patient education can enhance patient initiative,encouraging them to promptly report suspected symptoms,which contributes to early identification of ILD.During T-DXd treatment,it is important to regularly monitor symptoms and signs related to ILD,implement regular imaging monitoring and leverage multidisciplinary team collaboration to diagnose ILD as early as possible,thereby minimizing the risk of severe ILD.If symptoms or imaging suggest ILD,T-DXd treatment must be immediately interrupted,and relevant examinations should be completed to rule out other possible causes while considering corticosteroid treatment.Upon ILD diagnosis,subsequent T-DXd dose adjustments,corticosteroid therapy,and supportive treatments should be guided by severity.The article also explored whether patients with T-DXd-induced ILD can be re-treated,concluding that Grade 1 ILD patients might be eligible for re-treatment under specific conditions.In conclusion,the article reviewed the epidemiology,characteristics,clinical trial-recommended management strategies,and real-world management measures of T-DXd-induced ILD,integrating clinical expert experiences to summarize and discuss comprehensive management strategies for it.This aimed to enhance clinicians'understanding of T-DXd-induced ILD and provide valuable insights for early identification,timely diagnosis,and proper management of it.
10.Research on the association between chronic pain and multidimensional frailty in the elderly inpatients

Objective:This study aims to investigate the prevalence of chronic pain among elderly inpatients and its association with physical and cognitive frailty.Methods:A total of 540 elderly patients from the Department of Geriatric Medicine at Beijing Friendship Hospital were included in the study between January 2019 and December 2023.Based on the Numerical Rating Scale(NRS)scores, patients were categorized into two groups: those experiencing pain and those without pain.Univariate analysis was conducted to compare general information, physical frailty, and cognitive frailty between the two groups; however, no significant relationship was identified between cognitive decline and chronic pain.Additionally, Binary Logistic regression was employed to identify the independent factors influencing chronic pain.Results:The prevalence of chronic pain among elderly hospitalized patients was found to be 19.4%.Univariate analysis indicated that factors such as educational background, anxiety and depression, activities of daily living(ADL), polypharmacy, and physical frailty were associated with chronic pain.Furthermore, binary logistic regression identified educational background, polypharmacy, and physical frailty as independent influencing factors of chronic pain.Conclusions:The prevalence of chronic pain in elderly hospitalized patients is notably high, highlighting the need for more prospective studies to explore the relationship between chronic pain and multi-dimensional frailty, including social frailty.Addressing chronic pain in the elderly necessitates multidisciplinary interventions that consider the potential impact of frailty on pain.
