Chronic migraine is a common episodic brain dysfunction disorder, which often progresses from migraine and is frequently accompanied by sleep disorders, anxiety, depression or obesity. Extensive connections exist between the hypothalamus and other components of the central nervous system; through integrating pain signals, neural conduction and inflammatory pathways, the hypothalamus becomes an important hub for migraine chronicization. This review, combined with clinical imaging evidence, analyzes the pathogenic mechanisms of chronic migraine and its complications from perspective of the hypothalamus as the higher neural center, so as to provide useful references for prevention and treatment of chronic migraine.

Objective:To explore the expression level and clinical significance of low-density granulocytes (LDGs) in peripheral blood of patients with inflammatory bowel disease (IBD) .Methods:A cross-sectional survey was conducted. Clinical data of IBD patients admitted to the Department of Gastroenterology, the First Affiliated Hospital of Anhui Medical University from August 2022 to January 2023 were collected. A total of 45 healthy people were enrolled as the normal control group. Simple endoscopic score for Crohn′s disease (SES-CD) was used to evaluate the activity of Crohn′s disease (CD) and Mayo endoscopic score (MES) was used to evaluate the activity of ulcerative colitis (UC). Laboratory indicators included LDGs, white blood cell count (WBC), hemoglobin (HB), platelet count (PLT), C-reactive protein (CRP), neutrophil to lymphocyte ratio (NLR), and fecal calprotectin (FC). The difference in LDGs level between IBD patients and the normal control group was statistically analyzed and the relationship between LDGs level and clinical characteristics of IBD patients was analyzed. Receiver operating characteristic (ROC) curve was used to calculate the predictive value of LDGs for the disease activity of IBD.Results:A total of 130 IBD patients were included, including 83 CD patients and 47 UC. Compared with the normal control group, the LDGs levels of patients in the CD group and UC group were respectively higher[CD: 0.53% (0.32%, 1.41%) vs. 0.19% (0.12%, 0.29%), H= 57.71, P<0.001; UC: 0.87% (0.43%, 1.90%) vs. 0.19% (0.12%, 0.29%), H= 73.23, P<0.001]. Compared with the CD patients in remission phase, the level of LDGs in active CD patients was higher ( P<0.001). Compared with the CD patients in mild activity phase, the LDGs levels of CD patients in moderate and severe activity phases were higher (all P<0.05). In terms of different disease behaviours, the level of LDGs in stricturing CD patients was the highest [1.37% (0.91%, 3.06%), all P<0.05]. Compared with the UC patients in remission phase, the level of LDGs in active UC patients was higher ( P<0.001). Compared with the UC patients in mild activity phase, the LDGs levels of UC patients in moderate and severe activity phases were higher (all P<0.05). In the CD group, LDGs level was positively correlated with CRP, FC, NLR, PLT, simple Crohn′s disease activity index (CDAI), and SES-CD ( r= 0.374, 0.548, 0.345, 0.284, 0.764, 0.721, all P<0.05). In the UC group, LDG levels was positively correlated with CRP, FC, NLR, Sutherland disease activity index (DAI), and MES ( r= 0.325, 0.666, 0.474, 0.638, 0.740, all P<0.05). In CD patients, the cut-off value of LDGs was 0.565%, with the area under curve (AUC) of 0.873, sensitivity of 73.50%, and specificity of 99.30%. In UC patients, the cut-off value of LDGs was 0.545%, with the AUC of 0.877, sensitivity of 76.90%, and specificity of 100.00%. Conclusion:The expression of LDGs in IBD patients is significantly high, which correlates with the disease activity and may be used as a biological marker for the clinical evaluation of IBD patients.

Objective:To explore the predictive value of pre-treatment platelet-to-albumin ratio (PAR) in short-term prognosis of endoscopic treatment for cirrhosis with esophageal and gastric variceal bleeding(EGVB).Methods:By retrospective analysis method, the clinical data of 195 cirrhotic patients with EVGB from January 2019 to April 2022 treatment at Bengbu First People′s Hospital were collected and analyzed. The PAR was calculated according to platelet count and albumin. The independent risk factors that affecting 6-week rebleeding and death were analyzed by univariate and multivariate Cox regression, the receiver operating characteristic (ROC) curve was drawn to evaluate the predictive value of PAR for rebleeding and death, and Kaplan-Meier survival analysis was used to evaluate the rebleeding rate and survival rate of patients with different PAR ratios. Results:Among 195 patients, 36 patients were rebleeding and 159 patients were non-rebleeding within 6 weeks; while 15 cases died and 180 cases survived. The platelet count, PAR in the rebleeding group were lower than those in the non-rebleeding group, the direct bilirubin, triglyceride, alanine transaminase, prothrombin time and mortality in the rebleeding group were higher than those in the non-rebleeding group: 74.0(66.5, 88.8) × 10 9/L vs. 98.0(85.0, 111.0)×10 9/L, 2.48(2.18, 2.78) vs. 3.35(2.81, 4.04), 18.5(14.0, 23.8) μmol/L vs. 16.0(11.0, 20.0) μmol/L, (4.73 ± 2.52) mmol/L vs. (3.94 ± 1.65) mmol/L, 36.0(27.0, 46.0)U/L vs. 21.0(13.3, 33.0)U/L, (14.78 ± 1.63) s vs. (13.47 ± 0.87) s, 36.11%(13/36) vs. 1.26%(2/159), there were statistical differences ( P<0.05). Cox multivariate regression showed that PAR, alanine transaminase were the independent risk factors for the rebleeding ( P<0.05), PAR was the independent risk factor for the death within 6 weeks ( P<0.05). The area under the curve (AUC) of PAR for predicting 6-week rebleeding and death was 0.876, 0.776, the cut-off was 2.94, 2.71, the specificity was 69.8%, 72.2%, the sensitivity was 94.4%, 73.3%, respectively. According to the cut-off of PAR to predict rebleeding, the 6-week rebleeding rate in the PAR≤2.94 group was higher than that in the PAR>2.94 group ( χ2 = 36.88, P<0.01). According to the cut-off of PAR to predict death, the 6-week mortality rate in the PAR≤2.71 group was higher than in the PAR>2.71 group ( χ2 = 16.44, P<0.01). Conclusions:PAR can be used as a predictor for rebleeding and death within 6 weeks of EGVB in cirrhotic patients.

Objective To establish an enterovirus 71(EV71) infection model of tree shrew primary renal cells. Methods Tree shrew primary renal cells were obtained by trypsin digestion. After subculture and purification,EV71 virus was used to infect these primary cells. The culture supernatant of these EV71-infected cells was collected for virus titer detection at 1,2,4,6 and 8 days post-infection.The cells were collected for detection of EV71 VP1 protein by Western blot assay.Furthermore, the expression and location of VP1 protein in the infected cells were detected by indirect immunofluorescence assay.Vero cells were taken as positive control to evaluate the infectivity of EV71 virus to tree shrew primary renal cells. Results Morphologically, the cultured cells were proved to be majorly consisted of the primary renal cells after subculture and purification. The obtained primary cells were infected by EV71 virus. The virus titer was up to 1.3×106 TCID 50/mL during 48-96 h post-infection,proving that EV71 virus infected and proliferated in the tree shrew primary renal cells.Western blot showed that the viral VP1 protein was detected from infected primary cells at 2 to 8 d post infection.VP1 protein was also observed in the cytoplasm at 2 to 6 d post infection by indirect immunofluorescence.Compared with Vero cells, the infectivity of EV71 virus to tree shrew primary renal cells and its proliferation were confirmed. Conclusions Based on the successful establishment of cell culture of tree shrew primary renal cells, the infectivity to the obtained cells and proliferation of EV71 virus in the cells are confirmed. The model of EV71 virus-infected tree shrew primary renal cells is initially established.
