1.Research on The Genealogical Inference Efficiency of High-density SNPs

ObjectiveThis study aims to explore the potential of different orders of magnitude single-nucleotide polymorphism (SNP) locus combinations for predicting distant kinship relationships. A high-density SNP locus set was constructed, and a comprehensive assessment of its inference capability was conducted. MethodsFirstly, we selected three commercial chip panels, CGA (Chinese genotyping array, Illumina), GSA (Global screening array, Illumina), Affy (23MF_V2 high-density SNP array, Affymetrix) and merged them after quality control, forming a high-density SNP locus panel(1 180 k). Secondly, we selected 161 samples and collected their peripheral blood samples by using whole-genome sequencing technology. Within this sample population, the levels of kinship relationships fully covered the range from level 1 to level 9, and the number of kinship pairs at each level was consistently maintained at over 50 pairs. From 161 samples data of whole-genome sequencing, the 1 180 k locus set was extracted, which is referred to as the high-density SNP locus set in the following text. The kinship inference was conducted using the identity-by-descent (IBD) algorithm with the selected optimal parameters. To comprehensively evaluate the performance of the high-density SNP locus set in kinship inference, we compared it with the three commercial chip panels, the intersection of these three chip loci, and the control sets constructed by randomly reducing the number of the high-density SNP locus set. Based on the changes in the IBD lengths, as well as the dynamic trends in prediction accuracy, we conducted a scientific assessment of the kinship inference capability of the high-density SNP locus set. ResultsAfter screening, a set of 1 184 334 autosomal SNPs was obtained. During the process of screening the optimal IBD length threshold, the result revealed that 0 cM, 1 cM, and 2 cM all demonstrated good applicability. However, to avoid the issue of a large amount of redundant information caused by setting a too low IBD length threshold, this study ultimately selected 2 cM as the optimal threshold. Compared with the average results of three chip panels, the high-density SNP locus set increased the total IBD length and the average IBD length across levels 1-9; the accuracy of the confidence interval for level 8 was 70.97%, which represented a 3.50% improvement; the average confidence interval accuracy for levels 1-8 was 91.39%, representing a 1.00% increase; and the false negative rates at levels 8 and 9 were reduced by 2.42% and 6.76%, respectively. The system efficacy of the high-density SNP locus set for kinship inference of first to eighth degree relationships reached 98.91%. Through random reduction of the high-density SNP locus set results, it is found that increasing the number of SNPs with the panel, the detection efficiency of IBD length showed a significant upward trend. At the same time, the overall trend in the accuracy of kinship relationship prediction as well as the confidence interval accuracy also indicated that both metrics steadily increased with the addition of more loci. ConclusionThe results show that the high-density SNPs panel significantly enhances the efficacy of distant kinship inference, accurately covering kinship degrees, with the average confidence interval accuracy for first to eighth degree relationships stably above 90%. The study finds that increasing the number of SNPs panel can improve the ability to predict distant kinship.
2.Effects of subanesthetic dose of esketamine on postoperative anxiety and recovery in patients undergoing laparo-scopic cholecystectomy

OBJECTIVE To investigate the effects of subanesthetic dose of esketamine on postoperative anxiety and recovery in patients undergoing laparoscopic cholecystectomy. METHODS A total of 200 patients scheduled for laparoscopic cholecystectomy at Suzhou Ninth Hospital Affiliated to Soochow University from January 2023 to December 2024 were randomly assigned to control group (n=100) and observation group (n=100). One minute before the initiation of anesthesia, patients in the control group received intravenous injections of Propofol emulsion injection, Sufentanil citrate injection, and Succinylcholine chloride injection. On this basis, patients in the observation group received an intravenous injection of Esketamine hydrochloride injection. The anxiety status of patients in both groups was compared, along with their general intraoperative conditions (including sufentanil dosage, duration of pneumoperitoneum, operative time, anesthesia time, and extubation time), postoperative recovery, incidence of adverse reactions, and the need for dezocine rescue analgesia. Heart rate and mean arterial pressure, entropy index (state entropy and response entropy), inflammatory marker levels [interleukin-6 (IL-6) and C-reactive protein (CRP)], numerical rating scale (NRS) for pain intensity were compared between the two groups at different time points. RESULTS No significant differences were found between the two groups in pneumoperitoneum duration, operative time, anesthesia time,extubation time, incidence of postoperative dry mouth, entropy index or length of stay in the post-anesthesia care unit (P>0.05). Compared with the control group, the observation group showed significantly lower postoperative STAI-S scores, reduced intraoperative sufentanil consumption, decreased incidence of postoperative nausea, vomiting, and shivering, the need for dezocine rescue analgesia, as well as lower plasma IL-6 and CRP levels at 24 h after surgery, and NRS (P<0.05). The heart rate and mean arterial pressure of patients in the observation group at the start of surgery, end of surgery, and during extubation were all significantly higher than those in the control group (P<0.05). CONCLUSIONS Subanesthetic dose of esketamine can effectively alleviate postoperative anxiety, reduce intraoperative opioid consumption, suppress postoperative inflammatory response, relieve postoperative pain, and promote recovery in patients undergoing laparoscopic cholecystectomy.
3.Sodium lactate modulates TLR4/NF-κB signaling pathway for treatment of right heart failure

Aim To investigate the effects of sodium lactate(NALA)on right heart failure induced by monocrotaline(MCT)-induced pulmonary arterial hy-pertension in rats and to reveal the underlying mecha-nisms.Methods Forty male Sprague-Dawley(SD)rats were randomly allocated into four groups,with ten rats in each group,namely,MCT group,NALA group,and NALA+MCT group;the MCT and NALA+MCT groups were administered a single intraperito-neal injection of MCT at 60 mg·kg-1 to induce pul-monary hypertension,and one week later,the NALA and NALA+MCT groups received intraperitoneal in-jections of NALA at 0.1 g·kg-1(once a day,for 5 weeks),while the CON and MCT groups received e-qual volumes of physiological saline(once a day,for 5 weeks);right heart function was assessed using echo-cardiography,right ventricular and pulmonary artery remodeling were evaluated via histopathological sec-tions,and the expression levels of ANP,BNP,and in-flammatory factors were measured by ELISA,along with assessments of oxidative stress levels,Western blot detection of the expression levels of proteins in the TLR4/NF-κB signaling pathway.Results Compared to the CON group,the MCT group exhibited increased RVSP and RVHI,decreased right heart function,in-creased collagen fiber deposition,and elevated oxida-tive stress and inflammatory factor expression,and the expression levels of proteins in the TLR4/NF-κB signa-ling pathway increased(P<0.05);compared to the MCT group,the NALA+MCT group showed reduced RVSP and RVHI,improved right heart function,atten-uated pulmonary vascular remodeling,decreased ex-pression of ANP,BNP,inflammatory factors,and H2O2,along with increased antioxidant enzyme expres-sion,and the expression levels of proteins in the TLR4/NF-κB signaling pathway decreased(P<0.05).Conclusion NALA can inhibit right ventric-ular remodeling in rats with pulmonary hypertension,and the underlying mechanism may involve the allevia-tion of inflammatory responses and oxidative stress through the inhibition of the TLR4/NF-κB signaling pathway.
4.Mediating effects of coping style on exercise fear and exercise rehabilitation compliance in UAP patients after PCI

Objective To investigate the mediating effects of coping style after percutaneous coronary intervention(PCI)in patients with unstable angina pectoris(UAP)on exercise fear and exercise rehabilitation compliance.Methods A total of 152 UAP patients in the First People's Hospital of Jiujiang from February 2020 to December 2023 were selected by convenient sampling method,and questionnaires were conducted by using coping style questionnaire scale,exercise fear scale and exercise rehabilitation compliance scale to analyze the mediating effect of coping style on exercise fear and exercise rehabilitation compliance.Results Exercise fear was negatively correlated with exercise rehabilitation compliance(P<0.05).Exercise fear was negatively correlated with face coping,and positively correlated with avoidance and submission(P<0.05).The compliance of exercise rehabilitation was positively correlated with face coping,and negatively correlated with avoidance and submission coping(P<0.05).The results of the mediation model showed that the total effect of exercise fear on the compliance of exercise rehabilitation was-1.659,in which the direct effect of exercise fear on the compliance of exercise rehabilitation was-1.435,the direct effect of coping style on the compliance of exercise rehabilitation was 0.276,and the mediating effect of coping style was-0.224,all P<0.05.Conclusion The coping styles of UAP patients after PCI partially mediate effects between exercise fear and exercise rehabilitation compliance.It is recommended that medical staff pay more attention to patients'exercise fear and negative coping styles,and take timely intervention measures to reduce adverse effects,thereby improving patients'exercise rehabilitation compliance.

6.Construction and value analysis of a risk assessment model based on PHFS for pressure-related injuries in medical equipment

Objective:To construct a risk assessment model based on probabilistic hesitant fuzzy set(PHFS)for pressure-related injuries in medical equipment,and analyze its application value in control and management for risk of pressure-related injuries in medical equipment in the department of emergency.Methods:The frequency of events caused by pressure-related injuries of equipment was counted as statistical method.Combined with the risk assessment factors of pressure-related injuries,the technique for order preference by similarity to ideal solution(TOPSIS)based on PHFS was used to evaluate the risk level of pressure-related injuries of equipment,and formulate corresponding measures for risk management of pressure-related injuries.A total of 120 used medical equipment at the Department of Emergency of the First Affiliated Hospital of Xi'an Jiaotong University from January 2024 to December 2024 were selected.Using the random number table method,60 equipment were managed with the conventional risk management mode for pressure-related injuries(conventional management mode),and the other 60 equipment were managed with risk assessment model based on PHFS(model management mode)for pressure-related injuries in medical equipment.The incidence of pressure-related injuries of equipment,scores of operational quality of equipment,incidence of pressure-related injuries at different staging were compared between two kinds of management modes.A self-made questionnaire was used to investigate the satisfactions of emergency doctors,nurses,patients,and medical engineers in the department of equipment,who used and managed equipment.Results:The scores of operational standardization,pressure controllability,and diagnostic comfort in the model management mode were respectively(93.36±4.56),(90.54±3.69)and(93.65±4.69)points,which were significantly higher than those in the conventional management mode,while the score of material sensitivity was(30.23±3.26)points,which was significantly lower than that in the conventional management mode(t=15.941,11.896,11.511,17.200,P<0.05).The incidences of occurring pressure-related injuries in the referred logs of diagnosis and treatment of respiratory equipment,monitoring equipment,infusion equipment,and oxygen supply equipment in the model management mode were significantly lower than those in the conventional management mode(x2=9.924,9.493,9.796,8.075,P<0.05).The incidences of stage 1,2,3,and 4 of pressure-related injuries of equipment in the model management mode were significantly lower than those in the conventional management mode(x2=7.209,6.985,8.048,7.473,P<0.05).The satisfactions of emergency doctors,nurses,patients,and medical engineers in the department of equipment for the clinical use of equipment in the model management mode were significantly higher than those in the conventional management mode(t=12.499,13.200,14.410,9.226,P<0.05).Conclusion:The risk assessment model based on PHFS for pressure-related injury of medical equipment can reduce the incidences of pressure-related injuries of medical equipment in the department of emergency,and improve the safety of using medical equipment,and enhance the service quality of clinical equipment,and increase the satisfaction for equipment in clinical applications.
7.Effects of Jiedu Yizhi Formula on neuroinflammation in a mouse model of Alzheimer's disease via PI3K/Akt/mTOR signaling pathway

AIM To investigate the mechanism of Jiedu Yizhi Formula on cognitive function and neuroinflammation in a mouse model of Alzheimer's disease(AD).METHODS 50 APP/PS1 double transgenic mice were randomly divided into the model group,the donepezil group,and the low-dose,moderate-dose,and high-dose Jiedu Yizhi Formula group(1.78,3.56 and 7.12 g/kg),with 10 mice in each group,in contrast to the 10 WT mice of the blank group.Following anesthesia administration and 8-week oral gavage regimen with respective drugs,all mice underwent final tissue sample collection.The mice had their learning and memory ability assessed by Morris water maze and nesting behavior scores;their pathology of brain tissue and Aβ expression observed using HE,Nissl and IHC staining;their polarization of microglia and the expression of inflammatory factors in hippocampal tissue detected by IF and ELISA;their hippocampal expression of PI3K/Akt/mTOR signaling pathway detected by RT-qPCR and Western blot.RESULTS Compared with the blank group,the model group had lower scores in total swimming distance,frequency in crossing the platform,residence time in the target quadrant,and nesting behavior scores(P<0.05,P<0.01);prolonged evasion latency(P<0.01);more disorganized arrangement of pyramidal cells,solidification and deep staining,unclear demarcation,irregular cell shapes,reduction of Nyctinidia,and increased Aβ deposition in the brain tissue(P<0.01);elevated expression of hippocampal microglia M1-type markers CD16/32 and lba-1(P<0.01);decreased levels of M2-type marker CD206(P<0.05);elevated levels of TNF-α and IL-1β(P<0.01);decreased expressions of IL-13 and IL-4(P<0.01);and decreased levels of PI3K,Akt and mTOR mRNA,and reduced p-PI3K,p-Akt and p-mTOR protein expressions(P<0.01).Compared to the model group,the donepezil group and the Jiedu Yizhi Formula groups showed statistically significant improvements in the aforementioned indexes(P<0.05,P<0.01),with the magnitude of improvement being higher in the high-dose Jiedu Yizhi Formula group.CONCLUSION Jiedu Yizhi Formula suppresses microglia Ml-type polarization while enhancing M2-type polarization via activation the PI3K/Akt/mTOR signaling pathway,which subsequently reduces inflammatory cytokine secretion.This mechanism attenuates Aβ deposition in brain tissues and ameliorates cognitive dysfunction in AD mouse models.

Central nervous system(CNS)degenerative disorders refer to a spectrum of pathological alterations triggered by struc-tural damage to cerebral neural tissues,clinically manifested as diverse neurological dysfunction syndromes,including multiple sclerosis(MS),neurodegenerative diseases(NDs),and ische-mic stroke.The hallmark pathological features of these disorders involve irreversible neuronal damage and decompensation of functional neural networks,ultimately leading to progressive neurological deficits.Notably,with the accelerating global popu-lation aging,the incidence of these diseases has surged signifi-cantly.According to WHO statistics,they now rank among the top three global causes of disability and mortality.Current re-search has confirmed that the pathogenesis of CNS degenerative disorders exhibits high heterogeneity,encompassing multifaceted pathophysiological processes such as genetic predisposition,oxi-dative stress,protein misfolding,and metabolic dysregulation.This intricate pathogenic network not only complicates clinical differential diagnosis but also poses substantial challenges to the development of precision therapeutic strategies.Importantly,re-cent studies have revealed that mitochondrial homeostasis disrup-tion-induced inflammatory cascades(termed mitochondrial in-flammation)play a pivotal regulatory role in neurodegenerative progression.Key molecular mechanisms include impaired mito-phagy,aberrant mitochondrial DNA(mtDNA)release and NL-RP3 inflammasome activation.This review systematically deci-phers the molecular regulatory network of mitochondrial inflam-mation,with a focus on its biological effects in critical pathologi-cal events such as blood-brain barrier disruption,microglial hy-peractivation and neuronal apoptosis.The overarching aim is to provide a theoretical foundation for developing innovative thera-peutic strategies targeting mitochondrial homeostasis restoration.
9.Aerobic Exercise-Induced Hippocampal Exosomal miR-126a-5p in Ameliorating Diabetic Cognitive Dysfunction

Diabetes-related cognitive impairment(DCI)is a major complication of type 2 diabetes melli-tus(T2DM).Although exercise is essential in alleviating DCI,the underlying mechanisms remain un-clear.The aim of this study is to investigate the role and mechanism of exosomal miR-126a-5p induced by exercise in ameliorating DCI.Twenty-four 16-week-old male db/db mice were randomly divided into dia-betes group(n=12;DM)and exercise intervention group(n=12;DE).The control group consisted of male m/m mice of the same age group(n=12;CON).The DE group underwent 8 weeks of moderate in-tensity treadmill training(10 m/min,5 days a week).In the MWM experiment,compared to the CON group,the DM group exhibited prolonged escape latency(P<0.01),reduced swimming speed and target quadrant time(P<0.001),and decreased expression of miR-126a-5p and EX-miR-126a-5p in hipp-ocampal tissue(P<0.001).After exercise intervention,the DE group showed improved performance with decreased escape latency(P<0.05),increased swimming speed and target quadrant time(P<0.05),and elevated levels of exosomal miR-126a-5p(P<0.001).Morphological staining revealed a de-crease in the expression and proportion of NeuN in hippocampal neurons and an increase in the expression and proportion of glial cells in the CA1 and CA3 regions of DM group mice compared to CON group mice(P<0.05),while DE group mice showed increased fluorescence intensity and proportion of neurons(P<0.05).Western blotting analysis revealed that the DM group also showed significant upregulation of amy-loid β(Aβ),high mobility group box 1(Hmgb1),and NF-κB in the hippocampus(P<0.05),which were reduced after exercise(P<0.05).Moreover,exosomal miR-126a-5p overexpression greatly de-creased the levels of Hmgb1,NF-κB,and amyloid precursor protein(APP)in HT22 cells and TNF-α,IL-1β in supernatant exposed to HG(P<0.05),while inhibition of miR-126a-5p led to increased levels of these proteins(P<0.05).In conclusion,eight weeks of treadmill exercise improved cognitive function in db/db mice,likely through the EXs-miR-126/HMGB1/NF-κB pathway to reduce inflammation in hip-pocampal tissue.
10.Effect of Porphyromonas gingivalis on Th17/Treg imbalance and vascular endothelial function in ApoE-/-mice

Objective To investigate the effect of Porphyromonas gingivalis(P.gingivalis)on Th17/Treg imbalance and vascular endothelial function in ApoE-/-mice.Methods Eight-week-old ApoE-/-mice were inoculated orally with P.gingivalis every other day and received high-fat diet for 8 weeks.The alveolar bone loss was assessed by Micro-CT and the oral microflora of mice was analyzed by 16S rRNA sequencing technology.HE staining and oil red O staining were used to detect plaque formation in ApoE-/-mice.The total cholesterol(TC),triglyceride(TG),low-density lipoprotein cholesterol(LDL-C)and high-density lipoprotein cholesterol(HDL-C)levels of mice were detected by biochemical detection technology.Real-time quantitative PCR(qRT-PCR)and immunohistochemistry(IHC)were used to detect the indicators of endothelial injury in the model aortic plaques,including vascular cell adhesion molecule(VCAM),intercellular adhesion molecule(ICAM),matrix metallopeptidase(MMP)-2 and MMP-9.The relative proportions of Th17 and Treg in aortic plaque and spleen were detected by IHC,qRT-PCR and flow cytometry,including interleukin-17(IL-17),retinoic acid related orphan receptor γt(RORγt)and forkhead box P3(Foxp3).Results In ApoE-/-mice infected with P.gingivalis,the severity of alveolar bone loss was increased(P<0.01);the diversity of oral microbiota was decreased(P<0.05),and the composition of oral microbiota was changed.The proportion of atherosclerotic plaque area was increased(P<0.01),paralleled by elevated serum levels of TG,TC and LDL-C(P<0.05),alongside a significant de-crease in HDL-C(P<0.01).Aortic plaques exhibited upregulated protein expression of VCAM and ICAM as well as heightened mRNA levels of ICAM,VCAM,MMP-2 and MMP-9(P<0.05).The levels of IL-17 related protein and RORγt transcriptional activity in aortic plaque were increased(P<0.01),while the levels of Foxp3 related protein and gene were decreased(P<0.01).The ratio of Th17/Treg cells in spleen was imbalanced(P<0.05).Conclusion Periodontitis caused by the imbalance of the oral flora of ApoE-/-mice induced by P.gingivalis can cause Th17/Treg imbalance in aortic plaque and exacerbate the endothelial dysfunction during ather-osclerosis.
