Non-alcoholic fatty liver disease (NAFLD) is one of the most prevalent forms of liver diseases globally. Its progression can lead to cirrhosis and end-stage liver disease, and there is currently a lack of effective pharmacological treatments. Adenosine monophosphate-activated protein kinase (AMPK), as a regulatory hub for maintaining cellular energy homeostasis, can coordinate key cellular processes such as adipogenesis, glucose metabolism, and mitochondrial functions. Its activation exerts metabolic regulatory effects through pathways including inhibiting lipogenesis, enhancing mitochondrial β-oxidation, regulating inflammation and oxidative stress, and promoting autophagy. Accordingly, AMPK emerges as a potential target for the prevention and treatment of NAFLD. Traditional Chinese Medicine (TCM), with low toxicity, high accessibility, and multi-component, multi-target synergistic effects, has demonstrated unique value in NAFLD treatment, particularly showing notable advantages in regulating the AMPK signaling pathway. Sichuan is known as the treasure house of TCM, and the active components of its authentic medicinal materials such as Coptidis Rhizoma not only reflect regional characteristics in AMPK signaling regulation but also form a multi-level metabolic regulatory network through crosstalk with pathways such as sirtuin 1 (SIRT1) and peroxisome proliferator-activated receptor α (PPARα). They can achieve specific regulation by directly activating AMPK and modulating upstream and downstream targets, exerting prominent effects in ameliorating hepatic steatosis and inflammation. This study systematically reviews the research findings on TCM for the prevention and treatment of NAFLD over the past five years, elaborating the mechanisms by which TCM treats NAFLD through regulating the AMPK signaling pathway. It aims to provide new perspectives and references for clinical diagnosis and treatment, basic research, and drug development.

Metabolic dysfunction-associated fatty liver disease (MAFLD) is a chronic liver condition closely associated with dysregulated lipid metabolism, with the central pathological event of mitochondrial dysfunction in hepatocytes. The mitochondrial calcium uniporter (MCU) complex is a key molecular apparatus regulating mitochondrial calcium (mtCa2+) homeostasis, and mtCa2+ overload due to its dysfunction plays a critical role in the progression of MAFLD. This article systematically elaborates on the pathogenesis of MAFLD, with a particular focus on how the central signaling axis of “MCU complex dysfunction-mtCa2+ overload” drives the disease progression of MAFLD by disrupting energy metabolism, activating the adenosine monophosphate-activated protein kinase signaling pathway, triggering the NOD-like receptor protein 3 inflammasome, and inducing cell death. It also points out that traditional Chinese medicine can exert an interventional effect on multiple downstream pathways through various targets and has shown unique advantages and a significant potential in alleviating MAFLD by maintaining mitochondrial homeostasis and targeting the MCU complex, and therefore, it is expected to provide new strategies for the prevention and treatment of MAFLD.

ObjectiveTo screen for and validate key differentially expressed proteins in serum of patients with metabolic dysfunction‑associated fatty liver disease (MAFLD), and to provide a basis for identifying diagnostic biomarkers and developing intervention strategies for this disease. MethodsA total of 20 patients who were diagnosed with MAFLD in the third affiliated hospital of Chengdu University of Traditional Chinese Medicine from December 2024 to January 2025 were enrolled as MAFLD group, and 20 healthy controls were enrolled as HC group. Demographic data and serum biochemical parameters were collected from both groups. Data-independent acquisition (DIA) quantitative proteomics was used to measure the relative expression levels of serum proteins, and related methods were used to obtain significantly differentially expressed proteins, including the gene ontology functional enrichment analysis, the Kyoto Encyclopedia of Genes and Genomes pathway enrichment analysis, protein-protein interaction (PPI) network, Hub node interaction, and weighted gene co-expression network analysis (WGCNA). Rats were given a high-fat diet to establish a model of MAFLD, and randomly assigned to the control group and model group. Serum biochemical parameters and inflammatory factors were measured, liver histopathological changes were observed, and the protein expression levels of MER proto-oncogene tyrosine kinase (MERTK) and matrix metalloproteinase-9 (MMP-9) in liver tissue were verified. The independent-samples t test was used for comparison of normally distributed continuous data between groups, while the Mann-Whitney U test was used for comparison of non-normally distributed continuous data or data with heterogeneity of variance between groups; the chi-square test or the Fisher’s exact test was used for comparison of categorical data between groups. ResultsCompared with the HC group, the MAFLD group had significant increases in the serum levels of aspartate aminotransferase (t=-7.348, P<0.05), alanine aminotransferase (Z=5.088, P<0.05), alkaline phosphatase (t=-3.943, P<0.05), triglyceride (Z=2.070, P<0.05), and high-density lipoprotein cholesterol (t=3.867, P<0.05). A total of 3 559 proteins were identified by DIA serum proteomics, among which 160 differentially expressed proteins were mainly enriched in pathways including metabolic disorders, immune inflammatory response, and extracellular matrix remodeling. The PPI network and the Hub node analysis showed that serpin family E member 1, MMP-9, and intercellular cell adhesion molecule-1 were core regulatory proteins. WGCNA showed that the MEyellow module was significantly positively correlated with MAFLD phenotype (cor=0.55, P<0.001), which was functionally enriched in the efferocytosis pathway. MERTK (P<0.001) and MMP-9 (P<0.001) showed the most significant differential expression in this module. A rat model of MAFLD was successfully established using a high-fat diet, and compared with the control group, the model group had enlarged, yellowish, and greasy liver and disordered structure of hepatic lobules, hepatocyte fatty degeneration, and inflammatory cell infiltration based on HE staining, as well as massive lipid droplet deposition based on oil red O staining, while no obvious fibrous cords were observed in Masson staining. Compared with the control group, the model group had significant increases in the serum levels of aspartate aminotransferase (t=-5.148, P<0.05), alanine aminotransferase (t=-5.663, P<0.05), total bilirubin (t=-13.061, P<0.05), triglyceride (Z=-2.722, P<0.05), total cholesterol (t=3.588, P<0.05), and tumor necrosis factor-α (t=-16.741, P<0.05), as well as significant increases in the protein expression levels of MERTK (t=-11.448, P<0.001) and MMP-9 (t=-10.875, P<0.001) in liver tissue. ConclusionMMP-9 and MERTK are abnormally expressed in MAFLD and may be used as potential diagnostic biomarkers for this disease.

Ion mobility spectroscopy(IMS)has many advantages such as fast detection speed, high sensitivity, and portability, and thus plays an important role in on-site detection of chemical agents, explosives, drugs, environmental pollutants, and other pollutants. As a key component of IMS,the ion number density and temporal width of the injected ion packets directly determine the IMS detection sensitivity and resolution. In yhis study, the dual-parallel-grid structure of Tyndall-Powell gate(TPG),which could effectively isolate the electric fields within the ionization region, gate region, and drift region, was utilized to develop a dual effect TPG gating method for simultaneously enhancing the mobility discrimination reduction and ion packet temporal width compression, thereby improving the IMS performance.A TPG-IMS platform was thus built up and the effects of parameters such as gate opening pulsed width, gate penetration voltage, and ion injection voltage on the sensitivity and resolving power of IMS were systematically investigated using the dual-effect TPG gating method. The results demonstrated that, when detecting diethyl phosphate(DEP)and diethyl methylphosphate(DEMP)mixture using the dual effect TPG gating method, the peak currents of(DEP)2H+and(DEP·DEMP)H+ions with low K0 values were increased by 18 and 45 times respectively, while maintaining a high resolution of about 90.The limits of detection for DEP and DEMP were decreased from 4 ppb(10-9)to 235 ppt(10-12)and from 5 ppb to 156 ppt, respectively. This gating method only regulated the potential of the TPG grid adjacent to the drift region, without changing the structure of the ion mobility tube, making it convenient to apply on existing commercial instruments.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.

【Objective】 Dyslipidemia has shown to be associated with cardiovascular, metabolic and renal diseases. This study aimed to investigate the association between residual cholesterol and the risk of subclinical renal damage (SRD). 【Methods】 A total of 2 342 participants were recruited from the previously established Hanzhong Adolescent Hypertension Study cohort. According to estimated glomerular filtration rate(eGFR) and urinary albumin-to-creatine ratio(uACR), the subjects were divided into SRD group and non-SRD group. The associations of residual cholesterol with eGFR, uACR, and the risk of SRD were analyzed by multiple linear and Logistic regression analyses. 【Results】 Residual cholesterol was positively correlated with uACR(r=0.081, P<0.001) but negatively correlated with eGFR (r=-0.091, P<0.001). Multiple linear regression analysis revealed that residual cholesterol was an influencing factor of uACR (β=0.075, P<0.001) and eGFR (β=-0.027, P<0.001) after adjustment for gender, age, smoke, alcohol, exercise, BMI, hypertension, diabetes and serum uric acid. In addition, Logistic regression analysis revealed that residual cholesterol was significantly associated with the risk of SRD independently of potential confounders [OR(95% CI)=1.387 (1.113-1.728), P<0.001]. Further subgroup analysis showed that residual cholesterol was significantly associated with the risk of SRD in women but not in men. 【Conclusion】 Residual cholesterol is a contributing factor in the risk of subclinical renal damage with gender-specific association.

【Objective】 4-like protein with down-regulated expression and development in neural precursor cells (NEDD4L) plays an important role in blood pressure (BP) regulation and sodium homeostasis by regulating epithelial sodium channel protein. In this study, we aimed to explore the relationship of NEDD4L gene polymorphisms with BP responses to sodium and potassium intake. 【Methods】 In 2004, 514 subjects from 124 families in Meixian County, Shaanxi Province, were recruited to establish a salt-sensitive hypertension study cohort. All the subjects received a 3-day baseline survey, a 7-day low-salt diet, a 7-day high-salt diet, and finally a 7-day high-salt and potassium supplementation. Their BP was measured and peripheral blood samples were collected at different intervention periods. The 14 gene polymorphisms of NEDD4L gene were genotyped and analyzed by MassARRAY platform. 【Results】 BP decreased on a low-salt diet, and significantly increased on a high-salt diet, and decreased again after potassium supplementation. NEDD4L SNPs rs74408486 were significantly associated with systolic BP, diastolic BP and mean arterial pressure responses to the low-salt diet. SNPs rs292449 and rs2288775 were significantly associated with pulse pressure response to the high-salt diet. In addition, SNPs rs563283 and rs292449 were significantly associated with diastolic BP, mean arterial pressure, and pulse pressure responses to high-salt and potassium supplementation diet. 【Conclusion】 NEDD4L gene polymorphisms were significantly associated with BP responses to sodium and potassium intake, suggesting that NEDD4L gene may be involved in the development of salt sensitivity and potassium sensitivity.
