1.Analysis of Related Factors Influencing One-year Recurrence of Polymyalgia Rheumatica

To identify factors associated with the recurrence of polymyalgia rheumatica(PMR) within one year. This study included 64 patients diagnosed with PMR at Peking Union Medical College Hospital between January 2019 and June 2024. The baseline characteristics of patients with and without recurrence were compared, and logistic regression analysis was performed to identify risk factors for recurrence. The mean age at onset was 65.1±7.9 years, with a male-to-female ratio of 1:3.3. The average duration from onset to diagnosis was 4.5±3.7 months. At baseline, the average erythrocyte sedimentation rate(ESR) was 67.0±29.2 mm/h, with 11 patients(17.2%) having an ESR > 100 mm/h, and the average C-reactive protein(CRP) level was 57.9±51.3 mg/L. Corticosteroids were used as the initial treatment in 95.3% of patients, with an average dose of 21.6±11.6 mg/day. During the 12-month follow-up, 35.9% of patients experienced recurrence, with the median time to first recurrence being 8.2±3.3 months.The cumulative recurrence rates at 3, 6, 9, and 12 months were 6.3%, 14.1%, 25%, and 35.9%, respectively. Comparisons between patients with and without recurrence revealed significant differences in age(68.7±6.5 The one-year recurrence rate of PMR is 35.9%. Older age and lower serum albumin levels are associated with recurrence, and age may be associated with disease recurrence within one year.
2.Human amniotic mesenchymal stem cells overexpressing neuregulin-1 promote skin wound healing in mice

BACKGROUND:Neuregulin 1 has been shown to be characterized in cell proliferation,differentiation,and vascular growth.Human amniotic mesenchymal stem cells are important seed cells in the field of tissue engineering,and have been shown to be involved in tissue repair and regeneration. OBJECTIVE:To construct human amniotic mesenchymal stem cells overexpressing neuregulin 1 and investigate their proliferation and migration abilities,as well as their effects on wound healing. METHODS:(1)Human amniotic mesenchymal stem cells were in vitro isolated and cultured and identified.(2)A lentivirus overexpressing neuregulin 1 was constructed.Human amniotic mesenchymal stem cells were divided into empty group,neuregulin 1 group,and control group,and transfected with empty lentivirus and lentivirus overexpressing neuregulin 1,or not transfected,respectively.(3)Edu assay was used to detect the proliferation ability of the cells of each group,and Transwell assay was used to detect the migration ability of the cells.(4)The C57 BL/6 mouse trauma models were constructed and randomly divided into control group,empty group,neuregulin 1 group,with 8 mice in each group.Human amniotic mesenchymal stem cells transfected with empty lentivirus or lentivirus overexpressing neuregulin-1 were uniformly injected with 1 mL at multiple local wound sites.The control group was injected with an equal amount of saline.(5)The healing of the trauma was observed at 1,7,and 14 days after model establishment.Histological changes of the healing of the trauma were observed by hematoxylin-eosin staining.The expression of CD31 on the trauma was observed by immunohistochemistry. RESULTS AND CONCLUSION:(1)Human amniotic mesenchymal stem cells overexpressing neuregulin-1 were successfully constructed.The mRNA and protein expression of intracellular neuregulin 1 was significantly up-regulated compared with the empty group(P<0.05).(2)The overexpression of neuregulin 1 promoted the migratory ability(P<0.01)and proliferative ability of human amniotic mesenchymal stem cells(P<0.05).(3)Human amniotic mesenchymal stem cells overexpressing neuregulin 1 promoted wound healing in mice(P<0.05)and wound angiogenesis(P<0.05).The results showed that overexpression of neuregulin 1 resulted in an increase in the proliferative and migratory capacities of human amniotic mesenchymal stem cells,significantly promoting wound healing and angiogenesis.
3.Criteria and prognostic models for patients with hepatocellular carcinoma undergoing liver transplantation

Hepatocellular carcinoma (HCC) is a leading cause of cancer-associated death globally. Liver transplantation (LT) has emerged as a key treatment for patients with HCC, and the Milan criteria have been adopted as the cornerstone of the selection policy. To allow more patients to benefit from LT, a number of expanded criteria have been proposed, many of which use radiologic morphological characteristics with larger and more tumors as surrogates to predict outcomes. Other groups developed indices incorporating biological variables and dynamic markers of response to locoregional treatment. These expanded selection criteria achieved satisfactory results with limited liver supplies. In addition, a number of prognostic models have been developed using clinicopathological characteristics, imaging radiomics features, genetic data, and advanced techniques such as artificial intelligence. These models could improve prognostic estimation, establish surveillance strategies, and bolster long-term outcomes in patients with HCC. In this study, we reviewed the latest findings and achievements regarding the selection criteria and post-transplant prognostic models for LT in patients with HCC.
4.Clinical Observation of Fire Needle Combined with ALA-PDT in the Treatment of Moderate to Severe Acne Vulgaris

Objective To observe the clinical efficacy of fire needle combined with 5-aminolevulinic acid photodynamic therapy(ALA-PDT)in the treatment of moderate to severe acne vulgaris.Methods A total of 112 patients diagnosed with acne vulgaris in the dermatology outpatient department of Ankang Hospital of Traditional Chinese Medicine from February 2021 to February 2023 were selected as the study subjects.The patients were randomly divided into an observation group and a control group using a random number table,with 56 cases in each group.The control group was treated with ALA-PDT,while the observation group received fire needle treatment in addition to the control group's treatment,once a week for a total of 4 weeks.After treatment,the clinical efficacy of the two groups was evaluated.Changes in lesion symptom scores and the Global Acne Grading System(GAGS)scores were observed before and after treatment,as well as the skin barrier function.The serum levels of dehydroepiandrosterone(DHEA)and insulin-like growth factor-1(IGF-1)were compared before and after treatment.The recurrence rate was followed up,and the safety and incidence of adverse reactions in the two groups were evaluated.Results(1)After treatment,the lesion symptom scores and GAGS scores of both groups were significantly improved(P<0.05),and the observation group showed significantly better improvement in lesion symptom scores and GAGS scores compared to the control group,with a statistically significant difference(P<0.05).(2)After treatment,the stratum corneum hydration,transepidermal water loss(TEWL),and epidermal pH of facial skin in both groups were significantly improved(P<0.05),and the observation group showed significantly better improvement in these parameters compared to the control group,with a statistically significant difference(P<0.05).(3)After treatment,the serum levels of DHEA and IGF-1 in both groups were significantly improved(P<0.05),and the observation group showed significantly better improvement in DHEA and IGF-1 levels compared to the control group,with a statistically significant difference(P<0.05).(4)The total effective rate in the observation group was 89.29%(50/56),while it was 73.21%(41/56)in the control group.The efficacy of the observation group was superior to that of the control group,with a statistically significant difference(P<0.05).(5)At 8 and 12 weeks after treatment,the recurrence rates in the observation group were 10.00%(5/50)and 16.00%(8/50),respectively,while they were 34.14%(14/41)and 46.34%(19/41)in the control group.The recurrence rate in the observation group was lower than that in the control group,with a statistically significant difference(P<0.05).(6)The incidence of adverse reactions in the observation group was 16.07%(9/56),while it was 10.71%(6/56)in the control group.There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Fire needle combined with ALA-PDT in the treatment of moderate to severe acne vulgaris can effectively promote lesion repair,alleviate clinical symptoms,restore skin barrier function,and reduce the recurrence rate,with significant efficacy.The mechanism may be related to the regulation of serum DHEA and IGF-1 levels.
5.Exploration of post competency-oriented standardized training for naval surgical residents

By reviewing the practice and current situation of the standardized training for naval surgical residents,in combination with the practice of the standardized training for resident doctors in Shanghai,this paper sums up the existing experience and explores clinical teaching,management and patterns of standardized training for naval residents based on the post competence.The aim of the study is to strengthen naval primary medical care,enhance trainees'working competence and medical support capabilities,improve the quality and level of standardized training for naval residents,so as to provide reference for the further development of the training strategy of naval officers.
6.Current situation and influencing factors of blood pressure measurement cognition and behavior in community patients

Objective To investigate the cognition level and behavior compliance of blood pressure measurement in community residents and analyze the related influencing factors, and to provide evidence for community health management and blood pressure control. Methods A questionnaire survey was conducted to investigate 4470 community patients. Questionnaires included basic personal information,blood pressure measurement cognition, and blood pressure measurement behavior related issues. SPSS 19.0 was used to analyze the basic information, blood pressure measurement cognition, and pressure measurement behavior of the survey subjects. Logistic regression was performed to analyze relevant factors affecting blood pressure measurement cognition and behavior compliance. Results The overall cognitive compliance rate for blood pressure measurement among the visiting community patients was 31.52%. Age, education level, and chronic disease had a statistically significant impact on the cognitive knowledge (P<0.05). The overall behavior compliance rate of blood pressure measurement among the community patients was 23.69%. The cognition, age and education had a statistically significant impact on the overall behavior compliance rate of blood pressure measurement (P<0.05). Conclusion The cognitive level and standardized behavior of blood pressure measurement of community patients need to be improved. More attention should be paid to the elderly, low education level residents and community residents without chronic diseases, to promote community residents to form correct and standardized behavior of blood pressure measurement through health education.
7.Dynamic continuous emotion recognition method based on electroencephalography and eye movement signals.

Existing emotion recognition research is typically limited to static laboratory settings and has not fully handle the changes in emotional states in dynamic scenarios. To address this problem, this paper proposes a method for dynamic continuous emotion recognition based on electroencephalography (EEG) and eye movement signals. Firstly, an experimental paradigm was designed to cover six dynamic emotion transition scenarios including happy to calm, calm to happy, sad to calm, calm to sad, nervous to calm, and calm to nervous. EEG and eye movement data were collected simultaneously from 20 subjects to fill the gap in current multimodal dynamic continuous emotion datasets. In the valence-arousal two-dimensional space, emotion ratings for stimulus videos were performed every five seconds on a scale of 1 to 9, and dynamic continuous emotion labels were normalized. Subsequently, frequency band features were extracted from the preprocessed EEG and eye movement data. A cascade feature fusion approach was used to effectively combine EEG and eye movement features, generating an information-rich multimodal feature vector. This feature vector was input into four regression models including support vector regression with radial basis function kernel, decision tree, random forest, and K-nearest neighbors, to develop the dynamic continuous emotion recognition model. The results showed that the proposed method achieved the lowest mean square error for valence and arousal across the six dynamic continuous emotions. This approach can accurately recognize various emotion transitions in dynamic situations, offering higher accuracy and robustness compared to using either EEG or eye movement signals alone, making it well-suited for practical applications.

8.A method for emotion transition recognition using cross-modal feature fusion and global perception.

Current studies on electroencephalogram (EEG) emotion recognition primarily concentrate on discrete stimulus paradigms under controlled laboratory settings, which cannot adequately represent the dynamic transition characteristics of emotional states during multi-context interactions. To address this issue, this paper proposes a novel method for emotion transition recognition that leverages a cross-modal feature fusion and global perception network (CFGPN). Firstly, an experimental paradigm encompassing six types of emotion transition scenarios was designed, and EEG and eye movement data were simultaneously collected from 20 participants, each annotated with dynamic continuous emotion labels. Subsequently, deep canonical correlation analysis integrated with a cross-modal attention mechanism was employed to fuse features from EEG and eye movement signals, resulting in multimodal feature vectors enriched with highly discriminative emotional information. These vectors are then input into a parallel hybrid architecture that combines convolutional neural networks (CNNs) and Transformers. The CNN is employed to capture local time-series features, whereas the Transformer leverages its robust global perception capabilities to effectively model long-range temporal dependencies, enabling accurate dynamic emotion transition recognition. The results demonstrate that the proposed method achieves the lowest mean square error in both valence and arousal recognition tasks on the dynamic emotion transition dataset and a classic multimodal emotion dataset. It exhibits superior recognition accuracy and stability when compared with five existing unimodal and six multimodal deep learning models. The approach enhances both adaptability and robustness in recognizing emotional state transitions in real-world scenarios, showing promising potential for applications in the field of biomedical engineering.

9.HLA typing and STR loci chimerism analysis of a case of acute lymphoblastic leukemia after HLA haploidentical hematopoietic stem cell transplantation

Objective: To analyze the HLA typing and STR loci chimerism in a patient with recurrent acute lymphoblastic leukemia after HLA-haploidentical hematopoietic stem cell transplantation. Methods: HLA typing was performed on peripheral blood, buccal swabs and saliva samples after transplantation using PCR-sequence-specific oligonucleotide probes (PCR-SSOP) and next-generation sequencing (NGS). Additionally, STR analysis was conducted on these samples using a 21-locus STR assay kit to detect STR loci. Results: The HLA typing and STR locus outcomes of the patient's peripheral blood and the second saliva sample post-transplantation were in full concordance with the test results of the donor (father), whereas the HLA typing and STR locus results derived from the buccal swabs and the first saliva sample indicated chimerism between the donor and the recipient. Conclusion: In the follow-up and monitoring after transplantation, apart from focusing on peripheral blood samples, it is recommended to regularly monitor HLA typing and STR loci chimerism in patients' buccal swabs and saliva samples to comprehensively evaluate the transplantation effect and recurrence risk.
10.Thoughts on Development Path of Traditional Chinese Medicine Processing Technology from Perspective of Traditional Medicine and Techniques

As an important part of Chinese traditional culture, the processing technology of traditional Chinese medicine(TCM) carries the wisdom of TCM for thousands of years, and its process is complex and rigorous. With the popularization of modern production technology, traditional processing techiques are facing the dual pressures from technological innovation and production standardization under the perspective of intangible cultural heritage. The modernization of TCM processing technology is an inevitable trend for industrial upgrading, but it cannot be separated from the foundation of traditional skills and ignore the core concepts and cultural values it embodies. Therefore, by analyzing the core characteristics of TCM processing technology and its differences with modern production, this paper discusses the establishment of a synergistic innovation mechanism between traditional techniques and modern technologies, the promotion of joint research and development between scientific research institutes and the industry, the strengthening of standardization of processing techniques, and the enhancement of social education and industry training to improve the recognition and inheritance of processing techniques in order to achieve the goal of innovation and protection of TCM processing technologies in the context of modernization, and to promote the high-quality development of the TCM processing industry.
