1.Clinical Utility of Modified Asian FINDRISC and Random Capillary Blood Glucose for the Detection of Dysglycemia in the Adult Population

Introduction:
Type 2 diabetes mellitus is a major healthcare problem
worldwide, yet high percentage of patients are undiagnosed. There is a dire need for effective screening strategies.
We examine the use of modified Asian FINDRISC
(M-FINDRISC) and POCT random capillary blood glucose
(rCBG) for the detection of dysglycemia
Methodology:
This cross-sectional, observational study included 151
adults aged ≥30 years without a prior history of diabetes
in healthcare and community settings. Hemoglobin A1c
of ≥6.3% was used for the diagnosis of diabetes, and 5.7–
6.2% for prediabetes. Areas under the receiver operating
curve (ROC-AUC) for M-FINDRISC were analyzed.
The sensitivity and specificity of the combined use of
M-FINDRISC and rCBG for the detection of dysglycemia
were evaluated.
Results:
The mean age of the participants was 44.6 ± 11.0 years.
They had high body mass index (27.8 ± 4.9 kg/m²). Most
were sedentary (only 25.2% reported physical activities
of ≥30 min/day). A total of 52.3% had first-degree
relatives with diabetes, but less than 12% were taking
antihypertensive medication. Out of 151 participants, 3.3%
(n = 5) were diagnosed with diabetes mellitus and 37.7%
(n = 57) with prediabetes, giving an overall prevalence of
41.1% for dysglycemia. The mean age of participants with
dysglycemia was significantly older (47.9 vs. 42.3 years,
p-value 0.002). Other parameters were not statistically
different for the two groups. Mean M-FINDRISC score was
9.7 ± 3.9 in the normal and 10.8 ± 4.0 in the dysglycemic
group, while rCBG was 6.4 ± 1.1 mmol/L and 7.1 ± 3.1
mmol/L, respectively. ROC-AUC of M-FINDRISC for
dysglycemia was 0.561. A combined screening strategy
(M-FINDRISC score ≥7.5 and CBG ≥7.8 mmol/L) yielded
high specificity (89.9%) but low sensitivity (17.7%) for
dysglycemia.
Conclusion
While the combined screening strategy utilizing
M-FINDRISC and POCT random CBG improves specificity, M-FINDRISC showed limited predictive value for
dysglycemia in this specific cohort, making it a less ideal
tool for screening.
2.A Case Series of Carcinoid Syndrome: A Single-Centre Experience

Introduction:
Carcinoid syndrome is a hormonal complication accompanying neuroendocrine tumors (NETs) and is defined
by chronic diarrhea and/or flushing in the presence of
systemically elevated levels of serotonin or its metabolite
5-hydroxyindolacetic acid. The condition is often diagnosed
late because of nonspecific symptoms, and many patients
present with metastatic disease. This was a retrospective
review of patients with carcinoid syndrome who were followed up at the Endocrine Clinic, Putrajaya Hospital,
up to date.
Case:
Three women aged between 30 and 40 years were diagnosed
with carcinoid syndrome secondary to metastatic NETs.
Two cases were diagnosed before and during their
pregnancies, resulting in one successful pregnancy while
another required termination of pregnancy. All patients
presented with symptomatic disease, predominantly
flushing and chronic diarrhea and abdominal distension,
leading to the diagnosis within the same year to as long
as a decade after symptomatology. All were found to have
extensive liver and nodal metastases at diagnosis. Liver
biopsy showed tumor grade 1 in two of the cases, while the
case with tumor grade 2 exhibited an aggressive disease
course with carcinoid heart disease and bone metastases.
None were amenable to surgery. Somatostatin analogues
(SSAs) were the mainstay of treatment, administered every
2–4 weeks and titrated according to clinical response.
Cytotoxic chemotherapy and bone-targeted agent were
given for the case with rapid disease progression and
skeletal-related event. Somatostatin analogue radiolabeled
peptide therapy (PRRT) was reserved for symptom control
in progressive disease refractory to first-line SSAs, after
assessment with PET-Ga68 DOTATATE imaging.
Conclusion
This case series highlights delayed diagnosis and advanced
presentation due to its nonspecific symptoms. The occurrence in young women, including during pregnancy,
underscores the complexity of management and the need
for individualized multidisciplinary care, with somatostatin
analogues, systemic therapy, and PRRT playing key roles
in symptoms control and disease progression.
3.The Hidden Hazard After Total Parathyroidectomy: A Case Report

Introduction:
Palpation thyroiditis is an underrecognized condition
that occurs after parathyroidectomy. Symptoms range
from mild to overt thyrotoxicosis that may result in lifethreatening cardiac arrhythmias and acute coronary
syndrome. Diagnosis is made by having suppressed
thyroid-stimulating hormone (TSH) and elevated free
thyroxine 4 (fT4) or free thyroxine 3 (fT3) and low
radionuclide uptake on thyroid scan.
Case:
We report a case of a 72-year-old female with underlying
end-stage renal disease on regular hemodialysis who
underwent total parathyroidectomy for tertiary hyperparathyroidism. Her thyroid function test (TFT) preoperatively was normal, and cardiac assessment showed
normal echocardiogram findings and sinus rhythm on
electrocardiogram. Her operation was uncomplicated,
involving neck exploration and all 4-gland removal.
Postoperatively, she had new-onset atrial fibrillation
(AF) with rapid ventricular response at day 5, requiring
electrical and chemical cardioversion. She had recurrent
tachyarrhythmias on day 6. Her repeated TFT showed
suppressed TSH, 0.036 mIU/L (NR 0.38–5.33) and raised
fT4, 35.4 pmol/L (NR 7.9–14.4), fT3–fT4 ratio 0.26 (<0.3),
consistent with thyrotoxicosis. She was treated as thyroid
storm (Burch-Wartofsky Point Scale 45), after ruling
out other causes. She was started on glucocorticoids,
propylthiouracil, and Lugol’s iodine. Despite biochemical
improvement, she continued to develop recurrent AF
up till Day 12 post-op, requiring multimodal treatment,
including beta-blocker and antiarrhythmic agents. She
was later discharged well on low-dose carbimazole. Her
ultrasound neck showed a normal thyroid gland, and her
thyroid antibodies were negative.
Conclusion
A high level of suspicion is needed to detect thyroiditis
post-parathyroidectomy due to its potential adverse impact
on surgical outcomes. Treatment is mainly symptomatic,
and the disease is self-limiting. Risk factors identified
include secondary/tertiary hyperparathyroidism and
bilateral neck exploration that involves manipulation of
the thyroid gland.

Adrenocortical carcinoma (ACC) is a rare malignant tumour originating from the adrenal cortex. Half of the cases are functional, with ACTH independent autonomous cortisol production being the most common. It is rare for ACC to present with markedly elevated metanephrine levels which is a typical characteristic of pheochromocytoma. We report a case of a large functioning adrenal tumour with overlapping biochemistry features of ACC and pheochromocytoma. Biopsy confirmed the histopathological diagnosis of metastatic ACC.

The present pandemic prevention and control of 2019 novel coronavirus diseases(COVID-19) is still severe in China and abroad, which is however witnessing a dimensional application and success of the information technology.For example, the remote consultation system of epidemic prevention and control had played a key role in Henan province in its fight against COVID-19. The architecture of the system was composed of software and hardware architecture, data exchange technology, security system design, and data collection specifications. By the end of September 2021, the audio and video systems of 147 designated hospitals for patients of COVID-19 had been constructed, and 98 of which had achieved clinical data sharing and interaction. The remote consultation system effectively guaranteed the real-time sharing of case data, saved diagnosis costs and treatment time, laying a solid foundation for the pandemic prevention and control of COVID-19.

OBJECTIVETo study the mechanism and clinical effect of using the manipulatin of move, adduction, rotation method for the treatment of shoulder dislocation.
METHODSfrom January 2010 to March 2012,120 patients with shoulder dislocation admitted were randomly divided into treatment group and control group, 60 cases in each group. In the treatment group, there were 31 males and 29 females; In control group, there were 30 males and 30 females. In treatment group, 60 patients create their own "on the move, adduction, rotation manipulative treatment,the other 60 cases in control group used traditional restoration methods in wearing the traction treatment. The efficacy of two methods of treatment of shoulder dislocation were compared on reset process once successfully rate, the patient's level of pain (VAS).
RESULTSIn treatment group, restoration once successfully were in 59 cases, improvement of the pain (VAS) values was 3.76 +/- 1.05, the results were excellent in 57 cases, good in 2, poor in 1,without concurrent fractures and nerve injury, without re-dislocation after 3 months. In control group, restoration once successfully were in 50 cases, improvement of pain (VAS) values was 5.67 +/- 1.15, the results were excellent in 45 cases, good in 7, poor in 8. The reset once successfully rate,improvement of VAS values and clinical effect in treatment group were better than that of control group.
CONCLUSIONOn the move, adduction, rotation method has advantages of more wearing traction with uniform force, work together to focus, saving time and effort, easy and convenient to avoid iatrogenic injury.

9.Clinical application of atlantoaxial pedicle screw internal fixation for treatment of atlantoaxial dislocation.

OBJECTIVETo investigate the clinical effect of atlantoaxial pedicle screw internal fixation for treatment of atlantoaxial dislocation.
METHODSSixteen patients with atlantoaxial dislocation were treated from Dec. 2005 to June 2007, included 10 males and 6 females, aged from 38 to 45 years old (means 40.5 years). Among them 12 patients combined with nerve injury, according to ASIA grade: there were 3 cases in grade B, 5 cases in grade C, 4 cases in grade D. All patients received preoperative CT, radiograph and skull traction. Intraoperative posterior approach general spine pedicle screw-rod orthopaedics fixation system used and iliac bone block were implanted in space of posterior atlantal arch and axial vertebral plate. The outcome and complications were observed in the near future.
RESULTSThere was no vascular or neural injury found. The patients were followed up for 12 to 24 months (means 18 months). All head pain, acid storm symptoms were improved after operation. According to the Odom's clinical efficacy evaluation standard, 12 cases were excellent, 4 were good. Eleven cases of 12 with nerve injury recovered significantly. By ASIA classification: 1 cases was in grade B, 2 cases were in grade C, 5 cases were in grade D, 4 cases were in grade E. No looseness or breakage of screw occurred. Bony fusion was achieved in all cases.
CONCLUSIONPosterior approach atlantoaxial pedicle screw internal fixation have the advantages of direct screw placement, short-segment fusion, intraoperative reduction, fixation reliable, high fusion rate, and it can restablish the upper cervical vertebrae stability and help to recover the spinal cord and nerve function.

10.Sequential drugs treatment for central pain following spinal cord injury.

OBJECTIVETo investigate methods and therapeutic effects of sequential drugs treatment for central pain following spinal cord injury.
METHODSA total of 28 patients suffered from central pain following spinal cord injury were treated with sequential drugs from 1994 to 2008, including 23 males and 5 females, ranging in age from 25 to 59 years (mean 42 years). According to the patients' response to drugs, the therapy grade was adjusted step by step until the pain was relieved. Basing on VAS scores before and after drugs treatment, analgesic effect was evaluated. The first grade drugs: COX-2 inhibitors. The second grade drugs: Tricyclic antidepressant drugs (Amitriptyline) + COX-2 inhibitors + Carbamazepine. The third grade drugs: Tricyclic antidepressant drugs (Amitriptyline) + Gabapentin + Neurotropin/COX-2 inhibitors.
RESULTSThe pain of all of 28 patients was relieved to different extent. The VAS scores decreased by 23.3 +/- 1.2 in the first grade drugs treatment group. The VAS scores decreased by 54.5 +/- 3.8 in the second grade drugs treatment group. The VAS scores decreased by 65.8 +/- 5.1 in the third grade drugs treatment group (P<0.05).
CONCLUSIONThe sequential drugs treatment for central pain following spinal cord injury has a good analgesia effect and little adverse reaction.
