1.Independent and Combined Impacts of Sleep Duration and Physical Activity on the Incidence of Cardiovascular Disease in Chinese Population

Objectives:To explore the independent and combined effects of sleep duration and physical activity(PA)on the incidence of cardiovascular disease(CVD).Methods:The study subjects were derived from three sub-cohorts of the China-PAR project.Information on sleep and PA was collected from the surveys conducted in 2007-2008,and the incidence and mortality data of CVD were obtained during the follow-up survey from 2012 to 2015.Sleep duration was categorized into≤6 h/d,>6-8 h/d,and>8 h/d,and the PA level was classified according to the tertiles of PA volume(MET-h/d).The Cox proportional hazards model stratified by cohort source was applied to estimate the associations of PA and sleep duration with CVD incidence,coronary heart disease(CHD)incidence,and stroke incidence.Results:A total of 93 933 participants were included.During an average follow-up of(5.82±0.98)years,3 862 CVD events were documented.Compared with the low PA group,the hazard ratios(HRs)and 95%confidence intervals(CIs)of the CVD incidence,CHD incidence,and stroke incidence for the high PA group were 0.73(0.65-0.82),0.62(0.49-0.77)and 0.88(0.76-1.01),respectively.A U-shaped relation between sleep duration and incidence of CVD and stroke was observed.Compared with those who slept for>6-8 h/d,the risk of CVD(HR[95%CI]:1.10[1.00-1.21],P=0.04)and stroke(HR[95%CI]:1.33[1.18-1.49],P<0.01)was higher among participants slept>8 h/d.The risk of CVD,CHD and stroke tended to be higher in the≤6 h/d sleep group.Compared with the high PA combined with>6-8 h/d,the risk of stroke was highest in the high PA combined with>8h/d sleep group(HR[95%CI]:1.74[1.37-2.22],P<0.05),while the risks of CVD(HR[95%CI]:1.69[1.39-2.04],P<0.05)and CHD(HR[95%CI]:1.99[1.41-2.81],P<0.05)were highest in the low PA combined with≤6 h/d sleep group.Conclusions:Adequate physical activity and appropriate sleep duration are associated with lower risk of morbidity from CVD,CHD and stroke.

3.Clinical features analysis of 65 cases of multiple myeloma with extramedullary disease

Objective:To study the clinical features, treatment and prognosis of multiple myeloma (MM)complicated with extramedullary disease (EMD).Methods:A total of 65 patiens admitted to the Characteristic Medical Center and Tianjing First Central Hospital from January 2014 to May 2020 were analyzed retrospectively as the study subjects. The age ranged from 22 to 79 years, with a median age of 56(22,79) years, and the ratio of female to male was 27/38. They were respectively treated by chemotherapy, radiotherapy or autologous hematopoietic stem cell transplantation (AHSCT). Fluorescence in situ hybridization (FISH) was used in all 65 patients to detect t(4;14), t(14;16), 1q21, del(17p). The measurement data of non-normal distribution were expressed in M (Q1,Q3), and the survival curve was constructed by Kaplan-Meier method.Result:The increase of β2- microglobulin and lactate dehydrogenase, multiple osteolytic lesions were found in all patients. The most common rate of chromosomal abnormalities were10.8% for t(4;14), 16.9% for t(14;16), 18.5% for 1q21, 21.5% for del(17p). Both 1q21 and del(17p) were co-existed in 7.7% of patients. The effective rate of 65 patients treated with V-DRPACE regimen was 89.2%(58/65). Thirty-two patients who were in partial remission were treated with radiotherapy. The complete remission rate of radiotherapy was 18.8%(6/32). Twenty patients eligible for transplantation underwent AHSCT and 13 patients achieved complete remission. At the end of follow-up, the median overall survival(OS) of 65 patients was 44.8 months, and progression-free survival (PFS) was11 months.Conclusion:MM with EMD usually involve multiple sites, prone to multiple osteolytic lesions, increased β2- microglobulin and lactate dehydrogenase. The rate of cytogenetic abnormalities in such patients is significantly increased. The V-DRPACE regimen is effective in treating MM patients with extramedullary lesions and can benefit from AHSCT.
4.Treatment of dermatomyositis secondary to refractory acute myelogenous leukemia with azacitidine :a case report and literature review

The patient was a 60-year-old female. Acute myelogenous leukemia (AML) was diagnosed. Azacytidine acts as a DNA transferase inhibitor can treat acute myelogenous leukemia. After treatment with azacitidine, the patient developed multiple red papules on the facial skin, which were pathologically confirmed as dermatomyositis. Dermatomyositis may be induced by azacitidine in the treatment of myelodysplastic syndrome (MDS) and acute myelogenous leukemia, but the dermatomyositis will disappear after hormonal and immunosuppressive therapy.
5.Fruit and vegetable intake and the risk of arterial hypertension in China: A prospective cohort study

Background::Population-based epidemiological evidence regarding the association between fruit and vegetable intake and the incidence of hypertension is inconsistent. This prospective cohort study aimed to investigate the association between fruit and vegetable intake and the risk of new-onset hypertension.Methods::Based on the project of Prediction for Atherosclerotic Cardiovascular Disease Risk in China (China-PAR), 58,981 Chinese adults without hypertension at baseline were included. Information on fruit and vegetable intake was collected using a food-frequency questionnaire. Cox proportional hazards models were performed to estimate the hazard ratios (HRs) and 95% confidence intervals (CIs) of incident hypertension.Results::During 640,795 person-years of follow-up, 21,008 new cases of hypertension were recorded. Compared with participants in the lowest quintile (Q1) of total fruit and vegetable (TFV) intake, the HRs (95% CIs) of incident hypertension were 0.90 (0.86-0.95), 0.85 (0.81-0.90), 0.82 (0.78-0.86), and 0.83 (0.78-0.88) for the Q2 to Q5 group ( ptrend < 0.001), respectively. In further analyses categorizing participants according to the recommended intake level (500 g/day), we found that increasing the intake of TFV, even though it was still insufficient for the recommendation, also had a protective effect against the incident hypertension. When considering the intake of fruit or vegetable separately, we found similar trends as the TFV intake. Conclusion::These results suggest that a higher intake of fruit and vegetable is beneficial for preventing hypertension in Chinese adults.

Objective:
To investigate the relationship between physical activity (PA) and the risk of incident hypertension among population in rural areas of China.
Methods:
The Community Intervention of Metabolic Syndrome in China & Chinese Family Health Study (CIMIC) was conducted in 2007-2008. Data on PA, smoking, drinking, blood pressure and other variables were obtained at baseline. Then the follow-up study of incident hypertension was performed during 2012-2015. A total of 41 457 participants aged ≥18 years and free from hypertension at baseline were included in the final analyses. PA was calculated as metabolic equivalent (MET) for each participant. Cox proportional hazard models were used to explore the relationship of PA with incident hypertension according to the quartiles of PA.
Results:

Objective:
To explore the lipid accumulation product (LAP) formula for Beijing adults and to investigate the relationship between lipid accumulation product and hypertension, as well as diabetes.
Methods:
A cross-sectional study with a representative sample of 19 606 residents in Beijing aged 18-79 years was conducted in 2011. The sex-specific hypothetical minimum waist circumference (WC) was calculated in order to obtain the more applicable LAP formula. Multivariate logistic regression was used to analyze the associations of LAP, combination of LAP and body mass index (BMI) with hypertension and diabetes.
Results:
The LAP formula for Beijing adults was established as follows: LAP (male)= (WC-61.3) × TG, and LAP (female)= (WC-55.6) × TG. There was an obvious trend of increased risk of hypertension and diabetes with an increment in the tertiles of the LAP both in men and women. The

Objective To observe the clinical efficacy of Weixuening granule combined with prednisone for treatment of patients with primary immune thrombocytopenia (ITP) and its impact on platelet-associated antibodies(PAIgG, PAIgM, PAIgA). Methods Sixty patients with ITP were randomly divided into a observation group (30 cases, Weixuening granule combined with prednisone) and a control group (30 cases, prednisone therapy alone), 3 months constituting a therapeutic course. The clinical effects, platelet count (PLT) and PAIgG, PAIgM, PAIgA of two groups were observed after two courses of treatment. The time of withdrawing prednisone, relapse rate in 1 year and adverse reactions were also monitored. Results The total effective rate of the observation group was higher than that of the control group [90.00%(27/30) vs. 73.33%(22/30), P<0.05]. The time of withdrawing prednisone was significantly shorter in the observation group than that in the control group(days:39.8±7.8 vs. 55.3±6.5, P<0.05). The relapse rate in 1 year in the observation group was obviously lower than that of the control group [23.3%(7/30) vs. 50.0%(15/30), P < 0.05]. The PLT levels were markedly increased after treatment in the two groups, the levels of platelet-associated antibodies PAIgM and PAIgA after treatment in the two groups were lower than those before treatment, and the degree of improvement in observation group was superior to that of the control group [PLT (×109/L):113.25±15.47 vs. 84.79±20.37, PAIgM:(28.91±10.42)%vs. (30.75±9.37)%, PAIgA:(19.32±11.54)%vs. (21.15±15.16)%]. The level of PAIgG in the observation group was decreased obviously after treatment, while in the control group, it did not have significant change before and after treatment, and in the comparison of the level of PAIgG after treatment between the two groups, there was statistical significant difference [(15.29±9.02)%vs (32.41±9.34)%, P<0.05]. There were adverse reactions in the two groups, including centrality obesity, polytrichosis, acne, and hyperglycemia;the incidence of the above side effects in observation group was lower than that of the control group [73.3%(22/30) vs. 86.7%(26/30)], but the difference was not statistically significant (P>0.05). Conclusions The combined use of Weixuening granule and prednisone for treatment of patients with primary ITP is more effective than administration of prednisone alone;the adverse reactions are less in combined therapy group. Possibly, the better therapeutic effect of combined therapy is related to the decrease of platelet-associated antibodies and increase of the platelet generation.
