Objective To explore the efficacy of DSA-guided intrathecal drug delivery system combined with Zi Wu Liu Zhu Acupoint Therapy for management of cancer pain and provide reference for its standardized clinical application.Methods and Results Recommendations were formulated based on literature review and expert group discussion,and consensus was reached following expert consultation.The consensus recommendations are comprehensive,covering the entire treatment procedures from preoperative assessment and preparation,surgical operation process,postoperative management and traditional Chinese medicine treatment to individualized treatment planning.The study results showed that the treatment plans combining traditional Chinese with Western medicine effectively alleviated cancer pain,reduced the use of opioid drugs,and significantly improved the quality of life and enhanced immune function of the patients.Postoperative follow-up suggested good treatment tolerance among the patients without serious complications.Conclusion The formulated consensus is comprehensive and can provide reference for clinicians to use DSA-guided intrathecal drug delivery system combined with Zi Wu Liu Zhu Acupoint Therapy.The combined treatment has a high clinical value with a good safety profile for management of cancer pain.
2.High-risk factors for early postoperative recurrence in patients with colorectal cancer liver metastasis

Objective:To investigate the high-risk factors for early postoperative recurrence in patients with colorectal cancer liver metastasis (CRLM).Methods:The clinical data of 156 CRLM patients who underwent hepatic metastasectomy at the Li Huili Hospital, Ningbo University from Jan 2015 to Dec 2021 was retrospectively analyzed.Results:Among 156 patients, the postoperative recurrence rate was 55.8% (87/156).There were significant differences ( P<0.05) in the primary tumor site, T stage, TBS score, preoperative concentrations of CEA, CA199 and CA125, the maximum diameter of metastatic tumors and the number of metastatic tumors between the group with recurrence in one year and the group without recurrence in one year. Multivariate analysis showed that preoperative CA125 concentration ( OR=1.021, P=0.020), CEA concentration ( OR=1.044, P=0.018), and tumor burden score ( OR=3.067, P=0.011) were high risk factors influencing early recurrence ( P<0.05). In the low TBS score group(≤4), among the 49 patients who underwent simultaneous resection, 13 suffered from recurrence within 1 year, with a recurrence rate of 26.5%. Meanwhile, among the 29 patients who underwent staged resection, 17 suffered from recurrence within 1 year, with a recurrence rate of 58.6% ( P<0.05). In the high TBS score group(>4), among the 51 patients who underwent simultaneous resection, 37 suffered from recurrence within 1 year, with a recurrence rate of 72.5%, while in the 27 patients who underwent staged resection, 20 suffered from recurrence within 1 year, with a recurrence rate of 74.1% ( P>0.05). Conclusions:Preoperative TBS score, preoperative CA125 concentration, and CEA concentration are high-risk factors for postoperative recurrence in patients with colorectal liver metastases, simultaneous resection in patients with a low TBS score can reduce the risk of recurrence within 1 year after surgery.
3.Clinical Study on the Treatment of Thin Endometrium of Kidney Deficiency and Blood Stasis Syndrome by Herb-Partitioned Moxibustion and Estradiol Valerate on the Navel and Estrogen

Objective To observe the clinical efficacy of the combination of umbilical moxibustion and estradiol valerat in patients with thin endometrium.Methods A random number table was used to randomly divide 60 patients with thin endometrium of kidney deficiency and blood stasis type who met the inclusion criteria into an observation group of 30 cases and a control group of 30 cases.The control group took oral estradiol valerate tablets(DELPHARMLille S.A.S.,specification:1 mg/tablet),2 mg/day(bid),continuously for 21 days starting from the 5th day of the menstrual cycle;On the basis of the control group,the observation group was treated with herbal moxibustion on the navel,starting from the 7th day of the menstrual cycle,once a week,with an interval of 5-7 days.No treatment was given during the menstrual cycle,and both groups were treated continuously for three menstrual cycles.Observe the changes in endometrial thickness,Gonen classification of endometrium,uterine artery pulsatility index,resistance index,and traditional Chinese medicine clinical symptom score before and after treatment in two groups of patients.Results After treatment,the endometrial thickness of the two groups of patients increased(P<0.01),while the uterine artery pulsatility index and resistance index decreased compared to before(P<0.01).The total score of traditional Chinese medicine clinical symptoms decreased compared to before treatment(P<0.01).The above indicators were better in the observation group than in the control group(P<0.05).But the endometrial classification between the two groups of patients was no significant difference(P>0.05).Conclusion The combination of umbilical moxibustion and estradiol valerate can effectively promote uterine artery blood flow perfusion in patients with kidney deficiency and blood stasis type thin endometrium,reduce uterine artery pulsatility index and resistance index,increase the thickness of endometrium,and improve the clinical symptom score of traditional Chinese medicine.
4.Ethical and legal review of single women in the application of assisted reproductive technology

Assisted reproductive technology (ART) can address the issues faced by infertile couples and help families achieve their desine to have children. Starting from both theoretical and practical basis, this paper introduced a discussion on the legitimacy of ART application for single women. The challenges encountered by single women in ART application were objectively analyzed, including untimely legislation, unclear applicable subjects, the commodification of sperm and eggs, inadequate institutional guarantees, and insufficient safeguarding of offspring’s rights and interests, etc. This paper proposed solutions to the problems encountered by single women in ART application, including adhering to the relevant principles, strengthening legislative norms, clarifying the applicable subjects, providing institutional guarantees, safeguarding the offspring’s rights and interests, and seeking a balance between ethics and law. It aimed to provide references for related wrok, assisting single women in achieving a balance between ethics and law in ART application and protecting the reproductive rights of single women to the greatest extent.

Background::Exercise, as the cornerstone of pulmonary rehabilitation, is recommended to chronic obstructive pulmonary disease (COPD) patients. The underlying molecular basis and metabolic process were not fully elucidated.Methods::Sprague-Dawley rats were classified into five groups: non-COPD/rest ( n = 8), non-COPD/exercise ( n = 7), COPD/rest ( n = 7), COPD/medium exercise ( n = 10), and COPD/intensive exercise ( n = 10). COPD animals were exposed to cigarette smoke and lipopolysaccharide instillation for 90 days, while the non-COPD control animals were exposed to room air. Non-COPD/exercise and COPD/medium exercise animals were trained on a treadmill at a decline of 5° and a speed of 15 m/min while animals in the COPD/intensive exercise group were trained at a decline of 5° and a speed of 18 m/min. After eight weeks of exercise/rest, we used ultrasonography, immunohistochemistry, transmission electron microscopy, oxidative capacity of mitochondria, airflow-assisted desorption electrospray ionization-mass spectrometry imaging (AFADESI-MSI), and transcriptomics analyses to assess rectal femoris (RF). Results::At the end of 90 days, COPD rats’ weight gain was smaller than control by 59.48 ± 15.33 g ( P = 0.0005). The oxidative muscle fibers proportion was lower ( P < 0.0001). At the end of additional eight weeks of exercise/rest, compared to COPD/rest, COPD/medium exercise group showed advantages in weight gain, femoral artery peak flow velocity (Δ58.22 mm/s, 95% CI: 13.85-102.60 mm/s, P = 0.0104), RF diameters (Δ0.16 mm, 95% CI: 0.04-0.28 mm, P = 0.0093), myofibrils diameter (Δ0.06 μm, 95% CI: 0.02-0.10 μm, P = 0.006), oxidative muscle fiber percentage (Δ4.84%, 95% CI: 0.15-9.53%, P = 0.0434), mitochondria oxidative phosphorylate capacity ( P < 0.0001). Biomolecules spatial distribution in situ and bioinformatic analyses of transcriptomics suggested COPD-related alteration in metabolites and gene expression, which can be impacted by exercise. Conclusion::COPD rat model had multi-level structure and function impairment, which can be mitigated by exercise.
9.Gender differences of plasma glial cell line-derived neurotrophic factor levels in patients with major depressive disorder

Objective To investigate gender differences of plasma glial cell line-derived neurotro-phic factor (GDNF) levels in patients with major depressive disorder (MDD). Methods MDD subjects (male 20,female 36) and healthy controls (HCs) (male 35,female 45) were divided into four groups by gender. Plasma levels of GDNF were measured and compared in different gender groups. The clinical symp-tom severity of MDD patients was evaluated by 17-item Hamilton Depression Scale (HAMD-17) and Hamil-ton Anxiety Scale (HAMA-17). Results (1)The plasma GDNF level in male patients with major depres-sive disorder (( 1. 55 ± 0. 43 ) pg/ml ) was significantly lower than that in healthy controls (( 1. 86 ± 0. 50)pg/ml,F=4. 64,P=0. 036). There was no significant difference in GDNF level between female de-pression patients((1.62±0.46)pg/ml)) and female healthy control((1. 64±0. 48)pg/ml,F=0. 18,P=0. 672). In HCs,the GDNF level of male was significantly higher than that of female((1. 86±0. 50)pg/ml, (1. 64±0. 48)pg/ml,F=2. 04,P=0. 045). There was no significant difference in GDNF level between male and female patients(P>0. 05). (2) GDNF level in male patients with major depressive disorder was nega-tively correlated with HAMA score(r=-0. 388,P=0. 034). Conclusion The expression of GDNF is affect-ed by sex factors,which may be related to the different pathogenesis of MDD.
