1.Development of a multimodal deep learning-based risk prediction model integrating clinical and radiomic features for short-term acute kidney injury following partial nephrectomy

Objective:To develop and validate a deep learning-based multimodal model integrating clinical and radiomic features for predicting acute kidney injury(AKI)risk after partial nephrectomy.Methods:A retrospective analysis was conducted on 416 patients who underwent partial nephrectomy at Zhongshan Hospital,Fudan University from January 2023 to January 2025. The cohort included 100 AKI patients[defined by a ≥ 25% reduction in postoperative evaluated glomerular filtration rate(eGFR)within 48 hours sustained for >24 hours]and 316 non-AKI patients(1∶3 ratio,randomly matched with 16 additional cases for redundancy). Clinical and radiomic features were extracted from preoperative contrast-enhanced CT scans using PyRadiomics. Demographics included 259 males and 158 females,with a median age of 57(49,65)years,body mass index of(24.1 ± 3.3)kg/m2,preoperative eGFR of(88.5 ± 18.3)ml/(min·1.73 m2),postoperative eGFR(48-hour)of(76.0 ± 21.9)ml/(min·1.73 m2),Zhongshan Score(ZSscore)of 7.34 ± 2.01,and R.E.N.A.L. score of 7.50 ± 1.71. All tumors were T 1a stage. Patients were divided into training(n = 312)and test(n = 104)sets(3∶1 ratio). A clinical model was constructed via multivariate logistic regression,while radiomic and combined(clinical + radiomic)models utilized an artificial neural network(ANN)with 1 input layer,5 hidden layers,1 output layer,and 10 5 training epochs. Model performance was evaluated by using receiver operating characteristic(ROC)curves and area under the curve(AUC),and was compared to the Martini model. Feature contributions were interpreted via SHapley Additive exPlanations(SHAP). Results:In the test set,the results of multivariate logistic regression showed that patient’s weight,preoperative eGFR,R.E.N.A.L. score,surgical approach,and operation time were risk factors for AKI( P < 0.05). The AUC of the clinical feature prediction model constructed based on the above factors was 0.852(95% CI 0.775?0.929). In the test set,the AUC of the Martini model was 0.725(95% CI 0.565?0.791). The radiomic model,trained on 1 315 imaging features,achieved an AUC of 0.898(95% CI 0.804?0.993)with 94.2%(98/104)accuracy. The combined clinical and radiomic model,integrating 1 315 radiomic features and clinical features,demonstrated superior performance with an AUC of 0.946(95% CI 0.887?1.000)and 96.2%(100/104)accuracy,outperforming both the clinical model( P = 0.03)and the Martini model( P < 0.01). SHAP analysis identified the top five predictors in the combined model:ZSscore(SHAP value:0.78),long-run low gray-level emphasis(SHAP value:0.61),run-length non-uniformity(SHAP value:0.58),size-zone non-uniformity(SHAP value:0.46),and gray-level co-occurrence matrix joint energy(SHAP value:0.36). Conclusions:The deep learning-based multimodal model integrating clinical and radiomic features accurately predicts AKI risk after partial nephrectomy,offering a novel strategy for preoperative risk stratification and personalized intervention.
2.Impact of real-time computer endoscopy-assisted system on the detection rate of colorectal lesions

Objective To evaluate the advantages of a real-time computer endoscopy-assisted system(EndoAngel)for colorectal lesions detection in colonoscopy.Methods 2 000 patients who underwent EndoAngel assisted colonoscopy and conventional colonoscopy were selected for the study in a single-center,self-controlled study.According to different examination methods,the patients were divided into artificial intelligence(AI)group and traditional colonoscopy group,each with 1 000 cases.The results were statistically analyzed and compared with the polyp detection rate and adenoma detection rate of the two groups using pathological diagnosis as the gold standard.Further subgroup analysis will be conducted based on the seniority of the operating physicians.Results AI group's polyp detection rate was higher(39.3%)than conventional colonoscopy group polyp detection rate(29.0%),with statistically significant difference(x2=23.59,P=0.000).Of these,the detection rates of hyperplastic polyps and adenomatous polyps were 19.1%and 25.2%,which were significantly higher than those of 12.4%and 20.8%in the conventional colonoscopy group,and the differences were statistically significant(x2=16.92,P=0.000;x2=5.46,P=0.019).Further subgroup analysis of the two groups by physician seniority,the polyp detection rate of AI low seniority group(36.6%)was higher than that of conventional colonoscopy low seniority group(20.40%),with a statistically significant difference(x2=32.20,P=0.000).Among them,the detection rates of hyperplastic polyp(17.8%)and adenomatous polyp(23.6%)in AI low seniority group were higher than those in the conventional colonoscopy low seniority group(12.8%vs 13.6%),and the differences were significant(x2=4.82,P=0.028;x2=16.51,P=0.000).There were no significant differences in adenomatous polyp detection rates between the two groups of senior physicians.Conclusion EndoAngel assisted system can improve the polyp detection rate of colonoscopy,especially for the effect of low seniority physicians is more significant.
3.Effect of Echinacea purpurea extract on digestive ability and intestinal barrier function of Bamei ternary pigs

The aim of the experiment was to study the effects of Echinacea purpurea extract on the digestive ability,immune function and intestinal barrier of the Bamei ternary pigs.Forty-eight healthy pigs of 50 days old were randomly divided into four groups,each with three replicates and four pigs in each replicate.The control group(CON)was fed a basal diet,and the experimental group was divided into 5 g/kg(T1),10 g/kg(T2)and 15 g/kg(T3)of Echinacea extract in the basal diet.The pre-test period was 5 days and the main test period was 35 days with free feeding and drinking during the test period.The results showed that:There was no significant effect on the apparent digestibility of nutrients and jejunum morphology in the test group compared to the CON group(P>0.05).Amylase and trypsin activities were significantly higher in the experimental group and lipase activities were significantly higher in T2 and T3 groups compared to CON group(P<0.05).GSH,CAT and T-AOC activities were significantly higher in the experimental group compared to CON group(P<0.05).In comparison with CON group,the levels of IL-1 and IL-6 were significantly lower in the experimental group(P<0.05),and the levels of TNF-α were significantly lower in the T2 and T3 groups(P<0.05);and the levels of IL-10 were signifi-cantly higher in the T3 group(P<0.05).The mRNA and protein expression levels of TLR4,MyD88,TRAF6,and NF-κB were significantly lower in the experimental group(P<0.05).The mRNA and protein expression levels of CLDN-1,OCLN and ZO-1 were significantly increased in the test group(P<0.05).The results between the test groups showed that the protein expression levels of CLDN-1 and OCLN were significantly increased in the T3 group compared with the T1 group(P<0.05).In conclusion,the addition of Echinacea extract to the ration can,to a certain ex-tent,improve the digestive ability,balance the immune ability,and improve the intestinal barrier function of the Bamei ternary pigs,thus promoting the intestinal health of pigs.

As the location with special public health environments,funeral parlors are of paramount importance in the prevention and control of infectious diseases as well as disinfection practices.This paper analyzes the unique hy-gienic characteristics of funeral parlors,summarizes relevant laws,regulations,standards,and literatures in funeral parlors at home and abroad,and elaborates the problems relevant to infection prevention and control in funeral par-lors from perspectives of body disposal risks,microbial contamination characteristics in funeral parlors,and the current status of staff's knowledge on infectious diseases.It aims to enhance the professional prevention and control capabilities of funeral service personnel and management personnel,ensure the health and safety of service recipients and staff,optimize service quality,provide theoretical basis and practical guidance for forming a sound infectious disease prevention and control system for funeral service institutions,and clarify research directions for the preven-tion and control of infectious diseases in funeral parlors in the future.
5.Women's experiences of participation and informed choice in shared decision-making during childbirth:a meta-synthesis of qualitative research

Objective:To systematically integrate the lived experiences of women regarding decision-making participation and informed choice during intrapartum care, providing evidence-based guidance for improving obstetric healthcare management and service quality, and enhancing women′s childbirth experiences.Methods:Computerized searches were conducted in PubMed, Web of Science, Cochrane Library, CINAHL, PsycINFO, Scopus, ProQuest, China National Knowledge Infrastructure, Wanfang, Chinese Scientific Journals Database, and China Biology Medicine disc for qualitative studies on women′s experiences of shared decision-making participation and informed choice during childbirth. The search period spanned from January 1, 1997 to August 15, 2024. Literature quality was assessed using the Joanna Briggs Institute Critical Appraisal Checklist for Qualitative Research. Meta-aggregation was employed to synthesize findings by grouping similar results into new categories and integrating outcomes based on logical relationships between categories.Results:Eleven studies were included, yielding 47 key findings synthesized into 10 categories and further integrated into four overarching themes: complex emotional experiences in intrapartum decision-making participation; Diverse coping strategies for intrapartum decision-making participation; multiple stressors faced during intrapartum decision-making participation; multifaceted support needs in intrapartum decision-making participation.Conclusions:Midwives and healthcare providers should deliver education on shared decision-making and informed choice during the third trimester. Continuous attention to maternal emotional shifts regarding decision-making participation and informed choice during labor is essential. Comprehensive decision support, effective intrapartum communication, and initiatives to promote maternal autonomy through empowerment and capacity building should be prioritized.
