1.Research on the reconstruction of doctor-patient relationships in patients with sudden deafness from the perspective of narrative medicine

Patients with sudden deafness encounter greater psychological challenges and communication barriers after experiencing sudden hearing loss, and traditional medical models often fail to adequately address their unique needs. This paper analyzed the current situation of emotional and behavioral changes in patients with sudden deafness, and the gap between their expectations and the reality of medical care. From the perspective of narrative medicine, the theory and characteristics of the reconstruction of the doctor-patient relationships in patients with sudden deafness were explored. The results showed that narrative medicine can enhance patients’ emotional resonance and understanding, improve the efficiency and quality of doctor-patient communication, promote the formulation of personalized treatment plans, and enhance treatment adherence and satisfaction. Based on these results, strategies and pathways for the reconstruction of doctor-patient relationships for patients with sudden deafness were proposed, including building empathetic bridges and tapping into mechanisms of emotional resonance within narrative medicine; optimizing communication strategies and promoting the application of narrative techniques in doctor-patient dialogues; connecting narrative pathways and advocating the exploration of stories and strategies in personalized treatments; as well as facilitating treatment adherence and making full use of the psychodynamic effects of narrative medicine. Narrative medicine, as a patient-centered medical practice, can effectively promote the reconstruction of doctor-patient relationships, enhance treatment effectiveness, and offer a more humane treatment experience for patients.
2.Association of blood selenium exposure with sex hormones among men aged 18-79 years in China

Objective:To investigate the association between blood selenium levels and sex hormones in Chinese men aged 18-79 years.Methods:Data were derived from the China National Human Biomonitoring survey conducted in 2017-2018, with a final sample size of 5 414 men. General demographic characteristics, behavioral habits, and dietary frequency were collected through questionnaires and physical examinations. Fasting blood samples were collected to measure blood lead, serum testosterone, and estradiol levels. Complex sampling linear regression models were used to analyze the associations between blood selenium levels and testosterone, estradiol, and the testosterone/estradiol ratio, adjusting for confounding factors including age, education level, marital status, smoking status, alcohol consumption, seafood intake, soy product intake, protein supplement intake, BMI, and diabetes status.Results:The mean age of the 5 414 participants was (46.85±27.91) years; 4 774 (91.65%) were of Han ethnicity and 4 505 (86.68%) were married. The median ( Q1, Q3) blood selenium concentration in men was 97.80 (80.64, 116.99) μg/L. After adjusting for confounding factors, the complex sampling linear regression model revealed negative associations between blood selenium levels and both testosterone levels and the testosterone/estradiol ratio, with a significant linear trend ( Ptrend<0.05). Compared with the Q1 group, the β (95% CI) values for testosterone in the Q2, Q3, and Q4 groups were -0.02 (-0.06 to 0.02), -0.03 (-0.08 to 0.01), and -0.06 (-0.09 to -0.02), respectively. Similarly, the β (95% CI) values for the testosterone/estradiol ratio in the Q2, Q3, and Q4 groups were -0.01 (-0.03 to 0.02), -0.01 (-0.04 to 0.04), and -0.03 (-0.06 to -0.01), respectively. Subgroup analysis indicated stronger associations between blood selenium levels and testosterone/estradiol levels in non-smoking and obese men (BMI≥28 kg/m2). Conclusion:Blood selenium levels are negatively associated with testosterone levels and the testosterone/estradiol ratio in Chinese adult males.
3.Association of cadmium internal exposure levels with blood lipid in adults aged 18 to 79 years in China

Objective:To explore the association of blood and urinary cadmium levels with lipid profile levels and dyslipidemia in Chinese adults aged 18 to 79 years.Methods:Based on the China National Human Biomonitoring (CNHBM) program, a cross-sectional survey was conducted from 2017 to 2018 using a multi-stage stratified random sampling method, including a total of 10 713 adults aged 18 to 79 years. Data was obtained through questionnaires, physical examinations, biological sample collection, and laboratory testing. Multiple linear mixed effect model (MLMM) and generalized linear mixed effect model (GLMM) were used to analyze the association of blood and creatinine-corrected urinary cadmium levels with lipid profile levels as well as dyslipidemia among adults.Results:The age of 10 713 participants was (47.23±0.24) years, with 5 372 males accounting for 61.3% of the national population. The weighted mean±standard error (SE) of total cholesterol (TC), triglycerides (TG), low-density lipoprotein cholesterol (LDL-C), and high-density lipoprotein cholesterol (HDL-C) was (5.21±0.03), (1.86±0.03), (2.96±0.03), and (1.43±0.01) mmol/L, respectively. The prevalence rate of hypercholesterolemia, hypertriglyceridemia, mixed hyperlipidemia, low HDL-C, and high LDL-C was 16.0%, 21.6%, 6.6%, 13.5%, and 10.0%, respectively. MLMM showed that, after adjusting for relevant confounders, log-transformed blood cadmium levels were positively associated with increased levels of TC, TG and LDL-C ( P<0.05). When blood cadmium levels were categorized into quartiles, compared to the lowest exposure group ( Q1), participants in the highest blood cadmium exposure group ( Q4) had increases of 0.19 (95% CI: 0.06, 0.32) mmol/L in TC and 0.25 (95% CI: 0.08, 0.43) mmol/L in TG. GLMM indicated that, after adjusting for confounders, higher blood cadmium exposure levels were associated with increased risks of hypercholesterolemia, hypertriglyceridemia, mixed hyperlipidemia, and high LDL-C ( P<0.05). Further analysis by quartiles showed that, compared to the blood cadmium Q1 exposure group, the OR value (95% CI) for the Q4 group was 1.53 (1.12, 2.08) for hypercholesterolemia, 1.54 (1.09, 2.17) for hypertriglyceridemia, 2.24 (1.47, 3.40) for mixed hyperlipidemia, and 1.49 (1.07, 2.09) for high LDL-C. Conclusion:The cadmium internal exposure levels are associated with blood lipid profile levels as well as the incidence of dyslipidemia in Chinese adults aged 18 to 79.
4.Clinical study on the novel adjustable titanium mesh fixation tent screw in alveolar ridge augmentation

Objective This experiment aims to evaluate the clinical efficacy of a novel adjustable titanium mesh fixation tent screw by comparing its bone augmentation effects with those of conventional titanium screws combined with prefabricated titanium mesh in ca-ses where simultaneous implant placement is not feasible,thereby providing a new bone augmentation strategy.Methods This pro-spective study included 14 patients(20 bone augmentation sites)with severe alveolar bone defects treated at the Implant Center of the Department of Prosthodontics,Changsha Stomatological Hospital,from January 2023 to August 2023.The sites were randomly divided into two groups:the experimental group(novel tent screw group)received bone augmentation using novel tent screws combined with prefabricated titanium mesh,while the control group(conventional titanium screw group)underwent bone augmentation with conven-tional titanium screws combined with prefabricated titanium mesh.Cone beam computed tomography(CBCT)was performed preopera-tively(T0),immediately postoperatively(T1),and 6 months postoperatively(T2)to measure alveolar bone width at 2,6,and 10 mm below the crest and alveolar bone height at the intended implant sites.Vertical and horizontal bone volume changes,bone graft re-sorption,bone augmentation success rates,and postoperative complication rates were analyzed.Statistical analysis was conducted using SPSS 25.0.Results At 6 months postoperatively,vertical bone augmentation in the experimental group(3.26±2.02)mm was signif-icantly greater than in the control group(1.80±0.64)mm(P<0.05).Horizontal bone augmentation at 2 mm below the crest in the experimental group(4.45±2.46)mm was significantly higher than in the control group(2.45±1.02)mm(P<0.05),but no signif-icant differences were observed at 6 and 10 mm below the crest(P>0.05).Both groups exhibited low vertical and horizontal bone re-sorption with no significant differences.The bone augmentation success rate was 100%in both groups.The complication rate was 27.3% in the experimental group and 25% in the control group,with no statistically significant difference(P>0.05).Conclusion For severe alveolar bone defects where simultaneous implantation is not feasible,the novel tent screw combined with prefabricated titanium mesh achieves superior vertical and horizontal bone augmentation.Compared to conventional titanium screws,the novel tent screw demonstrates sig-nificantly better vertical bone augmentation and horizontal augmentation at 2 mm below the crest,effectively restoring alveolar ridge contours.
5.Discussion on the biological connotations of the pathogenesis of "earth congestion and wood depletion" in anxiety based on "intestinal flora-bile acid metabolism"

In recent years, the relationship between intestinal flora dysbiosis and abnormal bile acid metabolism and anxiety has received widespread attention. This article discussed the biological mechanism of the pathogenesis of anxiety from the perspective of intestinal flora and bile acid metabolism, in order to provide new ideas and theoretical basis for the TCM prevention and treatment of anxiety. According to TCM, spleen and stomach belong to earth, liver and gallbladder belong to wood, when spleen qi is healthy, the normal distribution of water and grain essence can be achieved, so that the liver can be nourished, qi and blood is sufficient, and its excretory function is normal, and the bile is sufficient. Once the spleen is not healthy, the distribution of water and grain essence is good, affecting the metabolism of fluids, resulting in phlegm and dampness, the qi is not smooth, can affect the liver's excretory function; liver and wood depression for a long time, transgressing the spleen and earth, qi and blood lack of biochemical sources, the formation of soil congestion and wood depression of the pathological phenomenon will appear. From the viewpoint of modern medicine and molecular biology, changes in the structure of intestinal flora affect the organism through neurological, endocrine, immune and metabolic pathways, which is consistent with the pathogenesis of "congestion of the earth"; disorders in the metabolism of bile acids can lead to changes in neurotransmitters and synaptic structure in the brain, causing anxiety symptoms, which is consistent with the characteristics of "wood depression". The bile acid metabolism disorder can cause neurotransmitter and synaptic structure changes in the brain, causing anxiety symptoms, which is consistent with the characteristics of "wood depression". It is important to regulate the intestinal flora and bile acid metabolism pathway to ease the liver and strengthen the spleen to improve anxiety.
6.The effects of pulsed electric field combined gemcitabine therapy on the proliferation and stemness of HCCC-9810 cholangiocarcinoma stem cells

Objective:To investigate the effects of pulsed electric field (PEF) combined with gemcitabine (GEM) on the viability and stemness of HCCC-9810 cholangiocarcinoma stem cells.Methods:HCCC-9810 cholangiocarcinoma stem cells were established in serum-free, cytokine-rich medium and divided into four groups: the control group, GEM group, PEF group, and the pulsed electric field combined with gemcitabine (PEF+ GEM) group. Cell proliferation was detected using the Cell Counting Kit-8 (CCK8) assay. Cell viability and apoptosis rate were measured by flow cytometry. Cell invasion ability was assessed using the Transwell assay. The expression of stemness marker proteins CD133 and Octamer-binding transcription factor 4 (OCT4), as well as the expression of β-catenin, was detected by Western blotting.Results:Regarding cell viability, the GEM, PEF, and PEF+ GEM groups showed significantly lower cell viability and higher apoptosis rate than the control group at 24 h, 48 h, and 72 h (all P<0.05). At 48 h and 72 h, the PEF+ GEM group showed significantly lower cell viability (7.2%±0.3% and 5.9%±0.8%, respectively) than the GEM group (50.7%±0.6% and 31.0%±1.2%, respectively) and the PEF group (12.2%±0.2% and 12.8%±0.2%, respectively) (all P<0.05). Regarding stemness inhibition, the PEF+ GEM groups showed significantly lower expression levels of CD133 and OCT4 at 24 h, 48 h, and 72 h compared with the control group (all P<0.05). Notably, at 48 h, the PEF+ GEM group showed a significantly lower expression level of the OCT4 (0.61±0.02) than the GEM group (0.87±0.08) and the PEF group (1.00±0.10) ( P<0.01). Furthermore, at 24 h and 48 h, the GEM, PEF, and PEF+ GEM groups showed significantly lower expression levels of β-catenin compared with the control group (all P<0.05). Conclusion:Pulsed electric field combined with gemcitabine therapy demonstrated more effective anti-proliferation and cancer stemness inhibition effects on HCCC-9810 cholangiocarcinoma stem cells compared with either monotherapy.
