1.Clinical application and outcomes of natural cycle and modified natural cycle IVF for individualized assisted reproduction among patients with DOR

Objective:To investigate the outcomes of natural cycle (NC) and modified natural cycle (MNC) assisted reproductive technology (ART) in patients with diminished ovarian reserve (DOR), and to provide a scientific basis for individualized treatment strategies for DOR patients.Methods:A retrospective cohort analysis was performed on the clinical data of DOR patients who underwent ART at the Center for Reproductive Medicine of the Department of Obstetrics and Gynecology, Peking University Third Hospital from January 1, 2015 to December 31, 2023. Patients were divided into the NC group ( n=801) and the MNC group ( n=385) based on their treatment protocol. The primary outcomes were cycle cancellation rate and oocyte retrieval rate. Secondary outcomes included clinical pregnancy rate and live birth rate per fresh embryo transfer cycle and frozen-thawed embryo transfer cycle, cumulative pregnancy rate and cumulative live birth rate per started cycle and per transfer cycle, as well as laboratory parameters such as the number of retrieved oocytes, the number of two pronuclei (2PN) fertilized oocytes, the number of transferable embryos, and transferable embryo formation rate. Further, multivariate logistic regression was used to analyze the impact of the treatment protocol on pregnancy and live birth outcomes. Results:There were no statistically significant differences between the NC and MNC groups in terms of general characteristics such as age, body mass index, and baseline hormone levels (all P>0.05). The cycle cancellation rate was significantly higher in the NC group [19.10% (153/801)] than in the MNC group [10.65% (41/385), P<0.001], and the oocyte retrieval rate was significantly lower in the NC group [66.31% (431/650)] than in the MNC group [74.86% (259/346), P=0.005]. The number of retrieved oocytes [1 (0,1)], the number of 2PN fertilized oocytes [1 (0,1)], and the number of transferable embryos [0 (0, 1)] were also significantly lower in the NC group than in the MNC group [1 (1, 2), P<0.001; 1 (1, 1), P<0.001; 0 (0, 1), P<0.001]. However, there were no statistically significant differences in 2PN fertilization rate and transferable embryo formation rate between the NC and MNC groups (all P>0.05). In both fresh embryo transfer cycles and frozen-thawed embryo transfer cycles, there were no statistically significant differences in clinical pregnancy rate and live birth rate between the NC and MNC groups (all P>0.05). The cumulative pregnancy rate per started cycle and transfer cycle, the cumulative live birth rate per started cycle and per transfer cycle were also not significantly different between the NC and MNC groups (all P>0.05). Multivariate logistic analysis showed no significant association between NC and clinical pregnancy or live birth compared with MNC. Conclusion:While MNC to some extent reduced the cycle cancellation rate and improved oocyte retrieval rates compared with NC, it did not ultimately improve pregnancy outcomes in DOR patients.
2.Up-regulation of macrophage inwardly rectifying potassium channel Kir2.1 contributes to macrophage activation and cardiac inflammatory injury

AIM:To investigate the roles of up-regulated inwardly rectifier potassium channel 2.1(Kir2.1)in macrophage activation and cardiac inflammatory injury,in order to clarify the mechanism of Kir2.1 regulation in inflam-matory injury and cardiac repair.METHODS:The RAW264.7 macrophages were activated by lipopolysacharide(LPS)and treated with Kir2.1 agonist zacopride or lentivirus-Kir2.1 overexpression(Kir2.1-OE).Macrophages were randomly divided into control,LPS,LPS+zacopride(or LPS+Kir2.1-OE),and LPS+zacopride+BaCl2 groups.The effects of Kir2.1-OE and AG490[Janus kinase 2(JAK2)inhibitor]on the JAK2/signal transducer and activator of transcription 3(STAT3)signaling pathway in macrophages were also investigated.The expression of CD86,interleukin-6(IL-6)and Kir2.1 in M1 macrophages was detected by RT-qPCR or immunofluorescence staining.The expression of JAK2/STAT3 molecules was detected by Western blot.The RAW264.7 macrophages were incubated with LPS,LPS+zacopride or LPS+zacopride+BaCl2 for 12 h,and then co-cultured with H9C2(2-1)cardiomyocytes for 48 h.The expression of Kir2.1,IL-4,IL-6,IL-1β,B-cell lymphoma-2(Bcl-2),Bcl-2-associated X protein(Bax),caspase-3,cleaved caspase-3,calcium/calmodulin-dependent protein kinase II(CaMKII)and p-CaMKII in cardiomyocytes was detected by Western blot.We fur-ther compared the effects of zacopride and KN-93,a known CaMKII inhibitor,on cardiac CaMKII.After being incubated with LPS for 12 h and changed the medium,RAW264.7 macrophages were co-cultured with H9C2(2-1)cardiomyocytes which was pretreated with KN-93.The cardiomyocytes were divided into control,LPS,and LPS+KN-93 groups.The ex-pression of CaMKII and p-CaMKII were detected by Western blot.RESULTS:Zacopride inhibited LPS-induced M1-type polarization of macrophages in a Kir2.1-dependent manner as showed by a significant decrease in CD86(M1-type marker)and IL-6(P<0.05).Zacopride or Kir2.1-OE inhibited LPS-induced activation of JAK2/STAT3 inflammatory signaling pathway in macrophages,with effects similar to the JAK2 inhibitor AG490.The H9C2(2-1)cardiomyocytes were co-cul-tured with M1-polarized macrophages(P<0.05).Zacopride inhibited M1 macrophage-induced inflammatory injury in car-diomyocytes,which was manifested as decreased expression of IL-1β and IL-6,increased expression of IL-4,and de-creased apoptosis.Zacopride also inhibited activation of CaMKII in a Kir2.1-dependent manner in H9C2(2-1)cells co-cultured with macrophages(P<0.05).CONCLUSION:Up-regulation of Kir2.1 may inhibit LPS-induced M1-type polar-ization of macrophages via inhibiting JAK2/STAT3 signaling pathway.Up-regulation of macrophage Kir2.1 may play a pro-tective role in cardiac repair after myocardial infarction by negative regulation of CaMKII signaling.

Objective To retrieve,evaluate and summarize the best evidence summary for prevention and nursing care of pregnancy-induced diastasis recti abdominis and to provide references for clinical practice.Methods We systematically searched computer decision support systems,guideline websites,association official websites and databases for evidence on prevention and nursing care of pregnancy-induced diastasis recti abdominis,including guidelines,expert consensuses,clinical decision-making,systematic reviews and randomized controlled trials.The retrieval period was from the inception of databases to January 2024.There were 4 researchers who evaluated the quality of the included literature independently,extracted and summarized the evidence according to the subject.Results A total of 18 articles were included,including 2 clinical decisions,4 guidelines,5 expert consensuses,3 systematic reviews,and 4 randomized controlled trials.24 pieces of best evidence were summarized from 5 aspects,including the antenatal prevention,team building,nursing assessment,intervention strategies,and effecti-veness evaluation.Conclusion This study summarizes the best evidence summary for prevention and nursing care of pregnancy-induced diastasis recti abdominis,which can provide a reference basis for nurses to manage diastasis recti abdominis and improve the life quality of maternal woman.
6.Effect of Shegan Mahuangtang and Its Pungent and Bitter Chinese Herbs on Airway Inflammation and Expression of TRPV1/TAS2R14 in Lung Tissue of Rat Model of Cold Asthma

ObjectiveTo investigate the effects of Shegan Mahuangtang and its pungent and bitter Chinese herbs on the expression of transient receptor potential vanilloid-1 (TRPV1) and bitter taste receptor 14 (TAS2R14) in the lung tissue of the rat model of cold asthma. MethodSeventy SD rats were randomized into 7 groups: normal, model, Shegan Mahuangtang, pungent Chinese herbs, bitter Chinese herbs (6.43 g·kg-1), dexamethasone (0.5 g·kg-1), and Guilong Kechuanning (10 g·kg-1). The rat model of cold asthma was established by intraperitoneal injection and subcutaneous injection of 10% ovalbumin (OVA) and aluminium hydroxide in the limbs, combined with 2% OVA atomization and cold (2-4 ℃) stimulation. The rats were treated with corresponding drugs by gavage and atomization, and the normal and model groups were treated with the same amount of normal saline for 3 weeks. After the last excitation, airway inflammation and cell proliferation were observed by hematoxylin-eosin (HE), periodic acid-Schiff (PAS), and Masson staining of the lung tissue. The levels of interleukin-5 (IL-5), tumor necrosis factor-α (TNF-α), thymic stromal lymphopoietin (TSLP), and transforming growth factor-β1 (TGF-β1) in the serum were measured by enzyme-linked immunosorbent assay (ELISA). The expression of TRPV1 and TAS2R14 was detected by immunofluorescence. The expression of TRPV1, TAS2R14, phospholipase Cβ2 (PLCβ2), B-cell lymphoma-2 (Bcl-2), and α-smooth muscle actin (α-SMA) in the lung tissue was determined by Western blot. ResultCompared with the normal group, the model group showed decreased water intake, food intake, and body weight, increased airway inflammatory cell infiltration, goblet cell proliferation, tissue fibrosis and collagen deposition, elevated levels of IL-5, TNF-α, TSLP, and TGF-β1 in the serum (P<0.01), upregulated expression of TRPV1, PLCβ2, and α-SMA, and downregulated expression of TAS2R14 and Bcl-2 (P<0.05, P<0.01). Compared with model group, Shecgan Mahuangtang, pungent Chinese herbs, and bitter Chinese herbs increased the water intake, food intake, and body weight, reduced the inflammatory cell infiltration and goblet cell proliferation, alleviated tissue fibrosis and collagen deposition, lowered the levels of IL-5, TNF-α, TSLP, and TGF-β1 in the serum (P<0.01), downregulated the expression of TRPV1, PLCβ2, and α-SMA, and upregulated the expression of TAS2R14 and Bcl-2 (P<0.05, P<0.01). ConclusionShegan Mahuangtang and its pungent and bitter Chinese herbs can reduce OVA-induced airway inflammation, downregulate the expression of TRPV1, PLCβ2, and α-SMA, and upregulate the expression of TAS2R14 and Bcl-2 in asthmatic rats. Moreover, bitter Chinese herbs outperformed pungent Chinese herbs, and the combination of them enhanced the therapeutic effect. It is suggested that Shegan Mahuangtang and its pungent and bitter Chinese herbs may ameliorate the OVA-induced airway inflammation by inhibiting TRPV1 and activating TAS2R14.

This paper reviews the status quo of recent years’ research on autophagy and damage of chondrocytes in Kashin-Beck disease (KBD) and osteoarthritis (OA). PI3K/AKT signaling pathway and mTOR regulate the autophagy activity of chondrocytes. PI3K/AKT signaling pathway can promote the proliferation of chondrocytes and cause their damage by inhibiting their autophagy activity. This might play an important role in the occurrence and progression of bone and joint diseases such as KBD and OA, and provide scientific basis for revealing the pathogenesis and treatment plan of them.
9.Pharmacodynamic Material Basis of Hippophae Folium in Treatment of Hyperlipidemia

ObjectiveTo analyze the pharmacodynamic material basis of Hippophae Folium in the treatment of hyperlipidemia by network pharmacology and experimental verification. MethodThe hyperlipidemic HepG2 cell model induced by oleic and palmitic acid (molar ratio 2∶1) was established. The optimal concentration of Hippophae Folium containing serum was determined by cell counting kit (CCK)-8 method. The cells were intervened by the medicated serum, and oil red O staining was used to determine the success of the model. The contents of total cholesterol (TC) and triglyceride (TG) were determined by enzyme-linked immunosorbent assay (ELISA). The possible mechanism of action was analyzed by network pharmacology, and molecular docking was performed to detect the binding ability of the potential targets. ResultCCK-8 assay showed that 10% medicated serum was the optimal concentration for cell growth. Oil red O staining proved that the hyperlipidemic cell model induced by oleic and palmitic acid has been built. After treatment with medicated serum, the contents of TG and TC decreased, indicating that Hippophae Folium had a good therapeutic effect on hyperlipidemia. Network pharmacology revealed that the core targets of Hippophae Folium in the treatment of hyperlipidemia were albumin (ALB), peroxisome proliferative activated receptor γ (PPARγ) and matrix metalloprotein(MMP)-9, involving 755 biological processes, 73 molecular functions and 3 cellular components. By Kyoto Encyclopedia of Genes and Genomes(KEGG) analysis, it was found PPAR, hypoxia inducible factor(HIF)-1, AMP-activated protein kinase(AMPK) and other signal pathways were involved in the treatment of hyperlipidemia by Hippophae Folium. ConclusionHippophae Folium containing serum (10%) could reduce lipid accumulation and intracellular TG and TC levels in hyperlipidemic cell model, and its mechanism of action might be realized by activating PPAR signal pathway.
