Chinese Medical Ethics 2026;39(1):121-128
Community- and home-based elderly care services, as an important service model designed to meet the living and medical needs of the elderly, hold significant importance in improving their quality of life and alleviating the pressure on the traditional healthcare systems through their policies and implementation effects. This paper analyzed the necessity of community- and home-based medical and elderly care services in China, identified primary issues in the implementation of current policies, and explored pathways for system optimization, to build a more efficient and fair service system. It found that the content of the community- and home-based integrated medical and elderly care services lacked breadth and depth, the service quality and effectiveness were poorly assessed, resource investment and allocation efficiency were uneven, and the supervision and policy support were insufficient. Therefore, it was suggested to construct a demand-driven policy framework, optimize resource allocation to shift towards balanced regional coverage, establish a diversified home-based integrated medical and elderly care service system based on demand, and create a sustainable supervision and evaluation mechanism for the implementation effect of integrated medical and elderly care, with a view to providing theoretical support and practical guidance for the reform and optimization of community- and home-based elderly care services policies in China.

ObjectiveTo analyze the policy texts regarding the inclusion of 44 assisted reproductive medical service projects into basic medical insurance in China and to provide recommendations for the formulation and improvement of subsequent policies. MethodsMethods such as textual analysis, qualitative research, and descriptive statistics were employed. Based on the theory of policy instruments, a two-dimensional analytical framework integrating policy instruments with policy objectives was constructed to systematically examine the current status and existing issues in the inclusion of assisted reproductive medical service projects in basic medical insurance. ResultsDemand-based and environment-based policy instruments were more frequently utilized, accounting for 42.11% and 37.89%, respectively. Supply-based policy instruments accounted for only 20.00%, indicating limited use. In terms of policy objectives, standardized service management constituted the largest proportion (27.65%), whereas integrating service resources accounted for the smallest share (10.00%), showing an imbalanced and insufficient utilization of policy instruments. ConclusionTo better advance and implement the inclusion of assisted reproductive medical service projects in basic medical insurance and foster the construction of a fertility-friendly society, it is essential to strengthen the use of supply-based policy instruments, promote internal structural balance, expand the medical insurance catalog of assisted reproductive technologies, increase reimbursement rates, intensify publicity efforts, formulate scientific planning, establish rigorous qualification evaluations, enhance the synergy of policy instruments, and optimize the structure of policy instruments.

By consulting relevant literature of ancient herbal books and processing specifications, this paper made a systematic research and analysis of Ostreae Concha, including the name, producing area, harvesting, quality, historical evolution of processing, relevant processing specifications, modern processing technology, and changes in chemical composition and pharmacological effects before and after processing, in order to provide documentary evidence for the research on processing technology and the establishment of quality standards. According to the textual research, it is known that Ostreae Concha has a long history of being used in medicine, and there have been many aliases and local names in each historical period. Shennong's Classic of the Materia Medica(Shennong Bencaojing) began to use Muli as the correct name, which has continued to use to today, and there were also aliases such as Muge, Zuogu Muli and Haoke. Ostreae Concha has a wide range of localities and irregular harvesting periods. The ancients believed that its left shell was of superior quality, but this has not been seen in modern. And there were many kinds of processing methods of Ostreae Concha, such as grinding, roasting, calcining, frying, simmering, quenching and so on, and the calcining was still in use. The different editions of Chinese Pharmacopoeia from 1963 to 2020 contain only calcined Ostreae Concha, and the local processing specifications mainly include three kinds of processed products(calcined products, salt-soaked products and vinegar-soaked products). Modern processing research mainly focuses on process optimization, changes in chemical composition and pharmacological effects, and the research methods are relatively single. Overall, there are currently issues such as inconsistent processing standards, unclear process parameters and imperfect quality standards, which are not conducive to the quality control and standardized clinical use of Ostreae Concha. Therefore, it is necessary to further investigate the pharmacological substance basis of Ostreae Concha and its processed products in order to elucidate the processing mechanism, standardize the processing technology and improve the quality standard.

Chinese Medical Ethics 2025;38(6):679-686
Shared Decision-Making (SDM) is a crucial concept in modern medicine, emphasizing the joint participation of doctors and patients in the medical decision-making process. However, the authoritative position of doctors and the passive role of patients in traditional medical models often overlook the personal wishes and needs of patients, leading to tense doctor-patient relationships and medical disputes. By listening to and understanding their stories, narrative medicine helps doctors gain a more comprehensive understanding of patients’ situations and balances medical advice with patient needs in the decision-making process. Through systematic literature analysis and theoretical exploration, this paper investigated the application effects and mechanisms of narrative medicine in different medical contexts, as well as analyzed its specific role in the process of SDM. The aim is to explore the value of narrative medicine in bridging differences in SDM, revealing its role in promoting doctor-patient communication, enhancing decision-making participation, and improving medical outcomes. Researches had found that narrative medicine enhanced doctors’ “narrative ability”, promoted emotional communication and trust between doctors and patients, reduced conflicts and misunderstandings in decision-making, and improved patients’ sense of participation and trust, thus playing an important role in SDM. Therefore, by enhancing doctor-patient communication and understanding, promoting SDM and treatment selection between doctors and patients, personalized care and treatment optimization, advocating for doctor-patient co-construction, improving consultation efficiency, restoring the patient’s subject position, and other methods, it can bridge doctor-patient differences, promote communication and enhance decision-making participation, and improve medical outcomes and patient satisfaction.

BACKGROUND:Studies have shown that patients with premature ovarian failure have changes in the structure of intestinal flora and that imbalance of intestinal microbiota may be one of the important mechanisms in the development of premature ovarian failure. OBJECTIVE:To investigate the effect of Liuwei Dihuang Wan on oxidative stress and intestinal microbiota in premature ovarian failure mice induced by cyclophosphamide. METHODS:Forty-five female ICR mice were randomized into three groups:blank group(normal mice),model group(premature ovarian failure mice),and Liuwei Dihuang Wan group.A mouse model of premature ovarian failure was prepared by one-time intraperitoneal injection of cyclophosphamide(120 mg/kg)in the latter two groups. After successful modeling, the Liuwei Dihuang Wan group was intragastrically administered for 28 continuous days, and the other two groups were intragastrically administered with the same amount of normal saline for 28 days.Mouse body mass was recorded weekly and ovarian index was calculated. The development of mouse follicles was observed using hematoxylin-eosin staining.ELISA method was used to detect serum levels of anti-Mullerian hormone, estradiol, follicle stimulating hormone, superoxide dismutase, glutathione peroxidase, and malondialdehyde.Meanwhile, the gut microbiome of all mice was detected through 16S rDNA sequencing. RESULTS AND CONCLUSION:The mice in the model group had loose hair, decreased vigor and grip strength, almost no increase in body mass, and decreased ovarian index.Whereas, the mouse body mass and ovarian index were increased after treatment with Liuwei Dihuang Wan(P<0.05).The estrous cycle of mice in the model group was disorganized;Liuwei Dihuang Wan could restore the estrous cycle and reduce the number of atretic follicles in mice with premature ovarian failure. The serum levels of follicle stimulating hormone and malondialdehyde in the model group significantly increased(P<0.01),while the levels of estradiol,anti-Mullerian hormone, superoxide dismutase, and glutathione peroxidase significantly decreased(P<0.01).Liuwei Dihuang Wan could significantly decrease the serum levels of follicle stimulating hormone and malondialdehyde(P<0.01),and increase the levels of estradiol,anti-Mullerian hormone, superoxide dismutase, and glutathione peroxidase.According to the 16S rDNA sequencing results,Liuwei Dihuang Wan could regulate the abundance and diversity of intestinal microbiota, and increase the relative abundance of beneficial bacteria.KEGG pathway analysis showed that the intestinal microbiota and metabolic pathways, biosynthesis of secondary metabolites, microbial metabolism in different environments, and biosynthesis of amino acids were regulated by Liuwei Dihuang Wan.To conclude, the changes in the structure of intestinal microbiome may be one of the potential mechanisms of Liuwei Dihuang Wan in treating premature ovarian failure.Liuwei Dihuang Wan can regulate the structure of intestinal microbiome, increase the number of beneficial bacteria, reduce the number of harmful bacteria, and thus improve the balance of intestinal microbiota. This regulatory effect helps to reduce oxidative stress levels and further inhibit ovarian oxidative stress in mice with premature ovarian failure.

Objective:To examine the therapeutic effects and molecular mechanisms of nuclear receptor NR4A1 in alleviating postoperative ileus (POI) in a rat model.Methods:Twenty-four Sprague-Dawley rats were randomly(random number) divided into four groups ( n=6/group): sham-operated control, POI model, POI model + NR4A1 stimultior (Cytosporone B, 13 mg/kg), and POI model + NR4A1 antagonist (DIM-C-pPhCO2Me, 2 mg/kg). After 24 hours, intestinal tissues and serum were collected for analysis. We assessed: (1) histopathological changes, (2) intestinal motility via propulsion rate, (3) NR4A1 expression by immunohistochemistry, (4) epithelial apoptosis via TUNEL assay, (5) inflammatory cytokines (IL-6, IL-4) by ELISA, (6) tight junction protein (occludin) by Western blot, and (7) p38MAPK/NF-κB pathway activation through combined western blot and immunofluorescence analyses. Results:Compared with sham controls, POI model rats showed (all P<0.05): significantly reduced NR4A1 expression, severe mucosal damage, increased inflammatory infiltration, elevated epithelial apoptosis, decreased occludin expression, impaired intestinal motility, upregulated pro-inflammatory cytokines (IL-6, IL-4), and activated p38MAPK/NF-κB signaling. NR4A1 activation with Cytosporone B significantly reversed these pathological changes (all P<0.05), while NR4A1 inhibition exacerbated them. Conclusions:NR4A1 activation attenuates POI by suppressing p38MAPK/NF-κB-mediated inflammation and preserving intestinal barrier function, suggesting its potential as a therapeutic target for postoperative ileus.

Objective:To investigate the causes and management strategies of anesthetic complications during percutaneous spinal endoscopic surgery under local anesthesia. Methods:A total of 16 800 patients (8 625 males and 8 175 females) who underwent percutaneous spinal endoscopic surgery under local anesthesia (including intravenous basic anesthesia) in Tianjin Hospital, Shandong Public Health Clinical Center and Hebei General Hospital from February 2012 to February 2023 were retrospectively analyzed. The average age was 45.3±21.6 years (range, 12-84 years). There were 220 cases of posterior cervical keyhole endoscopic surgery, 50 cases of thoracic transforaminal endoscopic surgery, 70 cases of thoracic posterior interlaminar endoscopic surgery, 11 670 cases of lumbar transforaminal endoscopic surgery, and 4 790 cases of lumbar posterior interlaminar endoscopic surgery. The occurrence time, clinical manifestations, management of intraoperative anesthesia complications were recorded, as well as surgical segments, puncture sites, complication symptoms, signs, outcome and prognosis. Results:All patients received percutaneous water-mediated uniaxial spinal endoscopic surgery under local anesthesia. There were 9 patients experienced anesthesia complications, including 6 cases of epidural diffusion of anesthetics and 3 cases of anesthetics mistakenly entering the subarachnoid space. There were 4 males and 5 females, aged 48.4±18.2 years (range, 28-84 years). There were 1 case of T 12L 1 disc herniation, 1 case of C 5-6 disc herniation, 3 cases of L 4-5 disc herniation and 4 cases of L 5S 1 disc herniation. Surgical segments and procedures: 1 case of C 5-6 posterior Keyhole endoscopic surgery, 1 case of T 12L 1 transforaminal endoscopic surgery, 2 cases of L 4-5 transforaminal endoscopic surgery, 1 case of L 4-5 interlaminar endoscopic surgery, and 4 cases of L 5S 1 interlaminar endoscopic surgery. Anesthesia complications all appeared 5-10 min after injection of local anesthetics, with symptoms of decreased oxygen saturation, decreased blood pressure, altered consciousness, and sensory and motor dysfunction of limbs. 6 patients with epidural diffusion of anesthetics recovered completely after symptomatic treatment in 5 cases, and 1 case was left with foot drop. Three patients with anesthetics mistakenly entering the subarachnoid space were immediately converted to the supine position, of which one recovered by mask oxygenation; 1 patient improved after emergency tracheal intubation, rehydration, and application of vasoconstrictive medications; and 1 patient developed multiple complications such as multiorgan failure, rhabdomyolysis, and sepsis after tracheal intubation, and recovered at 3 months after surgery with symptomatic treatment. Conclusions:Epidural diffusion and entering into subarachnoid space of anesthetics are serious complications of local anesthesia in percutaneous spinal endoscopic surgery. In addition to sensory and motor dysfunction of the limbs, the functions of the respiratory and circulatory systems can also be affected. It is necessary to be alert to the occurrence of anesthesia-related complications during operation and early identification and treatment.
