1.Association between thyroid function levels and phenotypes associated with sarcopenia

BACKGROUND:Several observational studies have found a close relationship between thyroid function levels and sarcopenia,but the causal relationship between thyroid function levels and the onset of sarcopenia is not yet clear. OBJECTIVE:To investigate the causal relationship between thyroid function levels and sarcopenia using a two sample Mendelian randomization method. METHODS:A two sample Mendelian randomization analysis was conducted using genome-wide association study data on thyrotropin,free triiodothyronine,free tetraiodothyronine,subclinical hyperthyroidism,subclinical hypothyroidism,and four related phenotypes of sarcopenia-lefthand grip strength,right hand grip strength,limb lean mass,and gait speed.The inverse-variance weighted method,weighted median method,simple mode method,weighted median estimator method,and MR Egger regression method were used as analysis methods,while heterogeneity test,pleiotropy test,MR-PRESSO,leave-one-out method,funnel plot and other methods were used for sensitivity analysis. RESULTS AND CONCLUSION:Elevated levels of thyroid-stimulating hormone increased left-(β=0.02,SE=0.01,P=0.01)and right-handed grip strength(β=0.02,SE=0.01,P=0.01),an increase in free triiodothyronine decreased left-(β=-0.06,SE=0.02,P=9.5×10-5)and right-handed grip strength(β=-0.07,SE=0.02,P=9.3×10-5),and subclinical hyperthyroidism decreased gait speed(β=-4.4×10-3,SE=1.7×10-3,P=0.01).The sensitivity analysis results were basically consistent with the main analysis results.To conclude,an increase in thyroid-stimulating hormone is a protective factor for sarcopenia,and elevation of free triiodothyronine and subclinical hyperthyroidism may increase the risk of sarcopenia.
2.Effect of nourishing yin and tonifying yang method on inflammatory response and bone metabolism in patients with type 2 diabetes mellitus complicated with osteoporosis

Objective:To investigate the effect of nourishing yin and tonifying yang method on inflammatory response and bone metabolism in patients with T2DM complicated with osteoporosis (OP).Methods:A randomized controlled trial. From January 2022 to December 2023,80 patients with T2DM and OP in our hospital were selected as the observation objects, and they were divided into two groups by random number table method, with 40 cases in each group.On the basis of conventional treatment, the control group chewed vitamin D calcium chewable tablets, and the observation group added nourishing yin and tonifying yang Chinese medicine.Both groups were treated for 6 months. TCM syndrome scores were performed before and after treatment ;the levels of fasting blood glucose (FPG), 2 hPG and HbA1c were detected by intelligent blood glucose monitor;the levels of serum neutrophils and lymphocytes were detected by automatic blood analyzer, and the neutrophil/lymphocyte ratio (NLR) was calculated;the levels of serum IL-1β and TNF-α were detected by automatic chemiluminescence analyzer, the levels of type I procollagen amino terminal propeptide (PINP), type I collagen carboxy terminal peptide β special sequence (β-CTX) and 25-hydroxy vitamin D3 [25-(OH)D3] were detected by ELISA;Bone mineral density (BMD) was detected by bone mineral density detector. The adverse reactions during treatment were observed and recorded, and the clinical efficacy was evaluated.Results:The total effective rate was 92.5 %(37/40) in the observation group and 75.0 % (30/40) in the control group,the difference between the two groups was statistically significant ( χ2=4.50, P=0.034).After treatment, the scores of soreness and weakness of waist and knees, soreness and pain of waist and back, clear and long urine, pale tongue and white coating in the observation group were lower than those in the control group ( t=3.11, 3.75, 3.51, 3.74, P<0.01);the levels of serum FPG, 2 hPG and HbA1c in the observation group were lower than those in the control group ( t=3.11,3.20,3.39, P<0.01).The levels of serum NLR (2.63 ± 0.68 vs. 3.24 ± 0.79, t=3.70), IL-1β [(81.65 ± 8.30) ng/L vs. (89.03 ± 8.98) ng/L, t=3.82] and TNF-α [(35.14 ± 5.11) μg/L vs. (39.96 ± 5.38) μg/L, t=4.11] in the observation group were lower than those in the control group ( P<0.01). After treatment, the levels of PINP [(29.83 ± 3.92) ng/L vs. (34.02 ± 4.03) ng/L, t=4.71] and β-CTX [(21.30 ± 3.95 ) ng/L vs. (25.32 ± 4.18) ng/L, t=4.42] in the observation group were lower than those in the control group ( P<0.01), the levels of 25-(OH)D3 [(42.86 ± 5.12) μg/L vs. (38.08 ± 4.55) μg/L, t=4.41] and BMD [(0.90 ± 0.18) g/cm 3vs. (0.78 ± 0.16) g/cm 3, t=3.15] were higher than those in the control group ( P<0.01).During the treatment, the incidence of adverse reactions was 12.5% (5/40) in the observation group and 10.0 % (4/40) in the control group,there was no significant difference between the two groups ( χ2=0.13, P=0.723). Conclusion:The method of nourishing yin and tonifying yang can effectively improve the TCM syndromes of T2DM patients with OP, reduce the levels of blood glucose and inflammatory factors, improve bone metabolism, improve clinical efficacy and have good treatment safety.

Objective:To explore the effective way to improve the protection efficiency of intellectual property rights of medical institutions in the field of biomedicine by building an efficient patent pre-examination management system, so as to promote the transformation of achievements.Methods:Through literature review, case analysis and combined with the actual examination situation of the intellectual property protection center, the study analyzed and summarized the characteristics of long R&D cycle, rapid technology iteration, multidisciplinary intersection in the field of biomedicine, and the necessity of hospitals to carry out patent pre-examination work.Results:The pre-examination awareness of scientific researchers in medical institutions was weak, the drafting of application documents was not standardized, and the patent achievements were out of touch with the market.Conclusions:To accelerate intellectual property conversion of innovation achievements, innovation management strategies should be implemented: standardized processes, pre-evaluation, professional team support, process control and talent cultivation.

5.Correlation and influencing factors of sarcopenia with cardiac structure and function in elderly inpatients

Objective:To investigate the correlation and influencing factors between sarcopenia and cardiac structure and function in elderly inpatients.Methods:A total of 945 elderly patients, aged 60 years and older, were recruited from the Geriatrics Department at Xuanwu Hospital.Sarcopenia was assessed, while cardiac structure and function were evaluated through echocardiography and NT-proBNP measurements.Results:Patients were classified into two groups: a sarcopenia group(n=185, 19.6%)and a non-sarcopenia group(n=760, 80.4%).Compared to the non-sarcopenia group, the sarcopenia group exhibited a reduction in left atrial diameter(LAD), left ventricular end-diastolic diameter(LVEDD), left ventricular mass(LVM), stroke volume(SV), and cardiac output(CO)(all P<0.05).In contrast, NT-proBNP levels were significantly elevated in the sarcopenia group( P<0.001).Multivariate logistic regression analysis identified older age( OR=1.080, P<0.001)and elevated NT-proBNP( OR=1.001, P<0.001)as independent risk factors for sarcopenia.Conversely, higher LAD( OR=0.858, P<0.001)and LVM( OR=0.988, P=0.002)were recognized as independent protective factors.Additionally, NT-proBNP demonstrated a negative correlation with handgrip strength( r=-0.241, P<0.001), gait speed( r=-0.215, P<0.001), SPPB score( r=-0.270, P<0.001), and appendicular skeletal muscle mass(ASM)( r=-0.208, P<0.001), while exhibiting a positive correlation with the 5-time chair stand test( r=0.169, P<0.001).LVM was positively correlated with handgrip strength( r=0.213, P<0.001)and ASM( r=0.456, P<0.001).Furthermore, LAD was positively correlated with handgrip strength( r=0.071, P=0.029)and ASM( r=0.305, P<0.001). Conclusions:Notably, significant differences exist in cardiac structure and function between elderly patients with sarcopenia and those without.Furthermore, indicators related to sarcopenia are correlated with alterations in cardiac structure and function.
6.Preliminary study to evaluate early musculoskeletal injury by using shear wave elastography

Objective To evaluate feasibility of early skeletal muscle injury by using shear wave elastography.Methods Sixty patients with unilateral knee osteoarthritis treated in Department of Orthopedics of People's Hospital Affiliated to Fujian University of Traditional Chinese Medicine from May 2020 to January 2021 were selected as the research subjects.Conventional scanning and shear wave elastography techniques were applied to investigate affected and healthy knees.Measurement included cartilage morphology,thickness and damage,joint cavity effusion,synovial membrane thickness,ligament morphology,meniscus protrusion degree,and detection of shear wave values in attachment area of quadriceps tendon on patella.Thickness and shear wave values of tendons in different observation areas on affected side and healthy side were compared.Affected knee joint was assigned a value based on the ultrasound characteristics,and Spearman correlation was used to analyze correlation between ultrasound score and shear wave value.Results Among 60 patients,fifty-six cases(56/60,93.3%)of cartilage injury in knee joint,30 cases(30/60,50.0%)of synovial hyperplasia in knee joint,35 cases(35/60,53.3%)of medial meniscus extrusion in knee joint,57 cases(57/60,95.0%)of effusion in knee joint cavity,21 cases(21/60,35%)of osteophyte formation in knee joint,and zero case of popliteal cyst in knee joint.There were 34 cases with ultrasound score less than three points,26 cases with score three to seven points,and zero case with score more than seven points.Shear wave value of the tendon in affected lateral area was significantly greater than that in affected medial area(P<0.01).Shear wave value of tendon in healthy medial area was significantly higher than that in affected medial area(P<0.01).Shear wave value of quadriceps tendon decreases with increase of ultrasound score,showed a high correlation(r=0.955,P<0.01).Conclusion Shear wave elastography is a non-invasive and relatively easy-to-operate method for evaluating muscle injury,which is highly correlated with severity of knee osteoarthritis,can help detect early strain points,providing possibilities for grading and diagnosis of knee osteoarthritis.
7.Long-term auditory monitoring in children with Alport syndrome based on different degrees of renal injury.

Objective:To investigate long-term auditory changes and characteristics of Alport syndrome(AS) patients with different degrees of renal injury. Methods:Retrospectively analyzing clinical data of patients diagnosed AS from January 2007 to September 2022, including renal pathology, genetic detection and hearing examination. A long-term follow-up focusing on hearing and renal function was conducted. Results:This study included 70 AS patients, of which 33(25 males, 8 females, aged 3.4-27.8 years) were followed up, resulting in a loss rate of 52.9%.The follow-up period ranged from 1.1to 15.8 years, with 16 patients followed-up for over 10 years. During the follow-up, 10 patients presenting with hearing abnormalities at the time of diagnosis of AS had progressive hearing loss, and 3 patients with new hearing abnormalities were followed up, which appeared at 5-6 years of disease course. All of which were sensorineural deafness. While only 3 patients with hearing abnormalities among 13 patients received hearing aid intervention. Of these patients,7 developed end-stage renal disease(ESRD), predominantly males (6/7). The rate of long-term hearing loss was significantly different between ESRD group and non-ESRD group(P=0.013). There was no correlation between the progression of renal disease and long-term hearing level(P>0.05). kidney biopsies from 28 patients revealed varying degrees of podocyte lesion and uneven thickness of basement membrane. The severity of podocyte lesion was correlated with the rate of long-term hearing loss(P=0.048), and there was no correlation with the severity of hearing loss(P>0.05). Among 11 cases, theCOL4A5mutationwas most common (8 out of 11), but there was no significant correlation between the mutation type and hearing phenotype(P>0.05). Conclusion:AS patients exhibit progressive hearing loss with significant heterogeneity over the long-term.. THearing loss is more likely to occur 5-6 years into the disease course. Hearing abnormalities are closely related to renal disease status, kidney tissue pathology, and gene mutations, emphasizing the need for vigilant long-term hearing follow-up and early intervention.

9.Development and application evaluation of a mobile terminal-based atlas applet for Oral Histopathology

Objective:To explore the effectiveness of applet-assisted teaching in the pedagogy of stomatology and gain suggestions for the improvement of the applet.Methods:Using digital technology, we built a professional atlas applet for Oral Histopathology that covered a wide range of contents including illustrations, histological slides, schematic diagrams, and hand-drawn pictures of knowledge points. Based on the WeChat platform, the applet had the functions of bilingual (Chinese/English) annotation and real-time interactive communication between teachers and students. Thirty dental students were given quizzes before and after using the atlas applet. The scores were analyzed using the t-test with SPSS 22.0 software. At the same time, 45 students filled a questionnaire after using the applet for one month, which involved comments on the use of the digital atlas, the degree of satisfaction and suggestions for the applet during learning, and suggestions for its co-use by teachers and students in the future. Results:After using the atlas applet to assist with learning, the test scores of the students were improved significantly in all domains. There were significant differences between the test and control groups in the score of completion questions [(17.00±2.61) vs. (15.03±1.85), P<0.05], the score of identification of images [(27.93±5.08) vs. (25.13±3.31), P<0.05], and the total score [(78.77±8.59) vs. (72.90±6.08), P<0.05]. The questionnaire feedback showed that the students were highly satisfied with the atlas applet-assisted learning, the function of teacher-student interaction, and the overall performance of the applet. Conclusion:An atlas is one of the crucial teaching tools of Oral Histopathology for its intuitionistic and realistic presentation of tissue structures, which plays an important role in the theoretical learning process for dental students. The combination of the atlas and electronic media based on portable mobile devices is beneficial for students' understanding of knowledge.
