1.Three-dimensional binding treatment for avulsion fractures of inferior pole of patella utilizing suture anchor.

OBJECTIVE:
To explore the feasibility and effectiveness of suture anchor double-pulley technique combined with suture three-dimensional binding via bone tunnel technique for avulsion fractures of the inferior pole of the patella.
METHODS:
A clinical data of 38 patients with avulsion fractures of the inferior pole of the patella, who met the selective criteria and were admitted between September 2021 and April 2023, was retrospectively analyzed. The fractures were treated with suture anchor double-pulley technique combined with suture three-dimensional binding via bone tunnel technique in 18 cases (group A) and steel wire tension-band fixation in 20 cases (group B). There was no significant difference in terms of age, gender, cause of fracture, side of fracture, and disease duration between the two groups ( P>0.05). The length of incision, operation time, occurrence of complications, the range of motion of knee joint, and Böstman score of knee joint at last follow-up were recorded. The fracture healing was evaluated through X-ray films and the time of fracture healing was recorded.
RESULTS:
All incisions healed by first intention. The length of incision was significantly shorter in group A than in group B ( P<0.05). There was no significant difference in the operation time between the two groups ( P>0.05). All patients were followed up 12-24 months (mean, 16.1 months). X-ray films showed that all fractures healed and there was no significant difference in the healing time between the two groups ( P>0.05). At last follow-up, the range of motion and Böstman score of the knee joint in group A were significantly better than those in group B ( P<0.05). During follow-up, 1 patient (5.6%) in group A had one anchor mild prolapse and 3 patients (15.0%) occured internal fixation irritation in group B. But there was no significant difference in the incidence of complications between the two groups ( P>0.05).
CONCLUSION
For the avulsion fractures of the inferior pole of the patella, the suture anchor double-pulley technique combined with suture three-dimensional binding via bone tunnel technique has advantages of reliable fixation, small incision, avoidance of secondary operation to remove internal fixator, and fewer complications, with definite effectiveness.

2.A intervention trial of effects of mindfulness-based stress reduction on chronic pain in naval personnel

Objective:To explore the effects of mindfulness-based stress reduction(MBSR)on pain relief,anxiety and depression and quality of life in naval personnel with chronic pain.Methods:A total of 72 naval person-nel with chronic pain were randomly divided into MBSR group and routine intervention group.The routine interven-tion group received routine care,while the MBSR group received MBSR in addition to routine care.The Short-Form McGill Pain Questionnaire(SF-MPQ),Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS),and 36-item Short Form Health Survey Scale(SF-36)were used at baseline and after 8 weeks of intervention.Results:The differences in the scores of 3 subjects of SF-MPQ,SAS,SDS and SF-36 in MBSR group at baseline and 8 weeks after intervention were higher than those in routine intervention group(Ps<0.05).Conclusion:It suggests that mindfulnecs-based stress reduction could alleviate the degree of pain,anxiety and depression of patients with chronic pain in naval personnel and improve their quality of life.
3.Mental health disparities in people living with human immunodeficiency virus: A cross-sectional study on physician-patient concordance and treatment regimens

Background::Human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS) can profoundly affect the mental health of the people living with HIV (PLWH), with higher rates of anxiety, depression, and sleep disturbances. The disparities in neuropsychological problems evaluated by physicians and self-assessed by patients are still unknown.Methods::A total of 5000 PLWH and 500 physicians from 167 hospitals were enrolled in this cross-sectional study from September 2022 to February 2023. 4-Item Patient Health Questionnaire (PHQ-4) was used for the evaluation of depressive issues and anxiety issues by PLWH. Each physician assessed 10 PLWH under their care for the presence of depressive or anxiety issues. The primary outcomes of this study are the concordance rates on the depressive issues and anxiety issues evaluation between physicians and PLWH. The Cohen’s kappa test was used to assess the agreement between physicians and PLWH.Results::The concordance rate for the evaluation of depressive issues is 73.84% (95% confidence interval [CI]: 72.60-75.04%), and it is significantly different from the expected rate of 80% ( P <0.001). Similarly, the concordance rate for the evaluation of anxiety issues is 71.74% (95% CI: 70.47-72.97%), which is significantly different from the expected rate of 80% as per the null hypothesis ( P <0.001). The overestimation rate by physicians on depressive issues is 12.20% (95% CI: 11.32-13.14%), and for anxiety issues is 12.76% (95% CI: 11.86-13.71%). The mismatch rate for depressive issues is 26.16% (95% CI: 24.96-27.40%), and for anxiety issues is 28.26% (95% CI: 27.02-29.53%). The underestimation rate by physicians on depressive issues is 13.96% (95% CI: 13.03-14.95%), and for anxiety issues is 15.50% (95% CI: 14.52-16.53%). For the treatment regiments, PLWH sustained on innovative treatment regimen (IR) related to a lower prevalence of depressive issues (odds ratio [OR] = 0.71, 95% CI: 0.59-0.87, P = 0.003) and a lower prevalence of anxiety issues (OR = 0.63, 95% CI: 0.52-0.76, P <0.001). PLWH switch from conventional treatment regimen (CR) to IR also related to a lower prevalence of depressive issues (OR = 0.79, 95% CI: 0.64-0.98) and a lower prevalence of anxiety issues (OR = 0.81, 95% CI: 0.67-0.99). Conclusion::Nearly one in three PLWH had their condition misjudged by their physicians. The findings underscore the need for improved communication and standardized assessment protocols in the care of PLWH, especially during the acute phase of HIV infection.
4.Latent classes and associated factors of suicidal ideation among high school students

Objective:
To understand current situation epidemiology and associated factors of suicidal ideation among high school students in Yixing, so as to provide basis for targeted intervention.
Methods:
From March to May 2019, a questionnaire survey was conducted on 12 799 students from 3 junior high schools and 4 senior high schools in Yixing City, Jiangsu Province, using a stratified cluster random sampling method. Latent profile analysis was used to classify suicidal ideation among high school students, the chi square test was used to compare the differences in suicidal ideation among different characteristics of students, multiple Logistic regression was used to analyze influencing factors, a risk predictive nomogram model was constructed and then verified.
Results:
Three latent classes of suicidal ideation among high school students was divided into three categories were observed: none or mild, moderate, and severe. Among them, 3 034 (23.7%) had moderate suicidal ideation and 753 (5.9%) had severe suicidal ideation. The Logistic regression results showed that gender was female, academic performance was lower midrange, smoking, drinking, popularity with classmates(less popular and unpopularity), family member relationships(general/occasional contradictions/contradictions), trust in others(more trusted/less trusted/less trusted at all), past or current relationships, physical bullying, relationship bullying, verbal bullying, and sexual bullying were the influencing factors for severe suicidal ideation among students ( OR =3.27; 2.18 ;1.63;1.72;2.66, 6.05;3.00,3.29, 6.38;1.71, 6.04, 12.48; 2.50; 1.59; 2.16; 1.45; 1.63, P <0.05). The nomogram prediction model had good discrimination.
Conclusions
Suicide ideation is influenced by multiple factors. Family and peer situations, as well as being bullied, are all related to the degree the severity of suicidal ideation. Efforts can be made to improve students family and interpersonal relationships, control bullying, then reduce their suicidal ideation which might help prevent suicide ideation among students.
5.Metabolomics analysis of serum and urine in patients with traumatic spinal cord injury

BACKGROUND:Traumatic spinal cord injury primarily relies on scale assessment and imaging examinations in clinical practice.However,there are limitations in predicting the prognosis of the injury.Therefore,the use of metabolomics technology for biomarker screening is significant for estimating the extent of damage,injury and recovery,as well as developing new therapies. OBJECTIVE:To characterize the metabolic features of patients with traumatic spinal cord injury using metabolomics technology and explore potential biomarkers and disrupted metabolic pathways. METHODS:Serum and urine samples were collected from 20 patients with traumatic spinal cord injury(observation group)and 10 healthy subjects(control group).Metabolites were analyzed and multivariate statistical analysis was then performed for data processing to screen differential metabolites.Metabolic pathway enrichment was performed using MetaboAnalyst software.Logistic regression was applied to construct a biomarker combination model,and its relationship with the American Spinal Injury Association grading was analyzed. RESULTS AND CONCLUSION:Significant differences in 160 and 73 metabolites were detected in the serum and urine samples of the two groups,respectively.Pathway enrichment analysis showed evident disturbances in lipid metabolism after traumatic spinal cord injury,including sphingolipid,arachidonic acid,α-linolenic acid,and arachidonic acid metabolism,as well as glycerophospholipid and inositol phosphate biosynthesis.The combination of two identified biomarkers,telmisartan and quercetin glycoside,showed a correlation with the American Spinal Injury Association grading in both serum and urine levels.Thus,metabolomics technology provides assistance in further understanding the pathological mechanisms of traumatic spinal cord injury and screening therapeutic targets.The identified metabolic biomarker combination may serve as a reference for assessing the severity of traumatic spinal cord injury.
6.Efficacy of repetitive transcranial magnetic stimulation in the treatment of negative symptoms in patients with schizophrenia

Objective:To investigate the efficacy of repetitive transcranial magnetic stimulation (rTMS) in the treatment of negative symptoms in patients with schizophrenia and its effect on brain-derived neurotrophic factor (BDNF).Methods:A total of 130 patients with negative symptoms of schizophrenia who received treatment at The Third Hospital of Quzhou from March 2021 to March 2023 were included in this randomized controlled study. They were divided into a control group and a study group ( n = 65 per group). Both groups of patients were treated with antipsychotic drugs. Based on this, patients in the study group were treated with high-frequency rTMS, while those in the control group were treated with pseudo-rTMS. After 8 weeks of treatment, Positive and Negative Syndrome Scale (PANSS), Scale for Assessment of Negative Symptoms (SANS), and Personal and Social Performance Scale (PSP) scores were evaluated in each group before and after treatment. Serum BDNF levels were compared between the two groups before and after treatment. Adverse reactions were observed during the treatment. Results:After 8 weeks of treatment, the PANSS negative subscale score and SANS score in the study group were (16.45 ± 3.98) points and (35.41 ± 6.29) points, respectively, which were significantly lower than (20.08 ± 4.16) points and (41.76 ± 7.36) points in the control group ( t = -7.46, -6.85, both P < 0.05). PSP score in the study group was (66.85 ± 8.93) points, which was significantly higher than (58.79 ± 8.28) points in the control group ( t = 5.62, P < 0.001). There were no significant differences in PANSS positive subscale score, general psychopathology scale score or total score between the two groups (all P > 0.05). After 8 weeks of treatment, the serum BDNF level in the study group was (12.05 ± 2.13) μg/L, which was significantly higher than (8.86 ± 1.94) μg/L in the control group ( t = 9.73, P < 0.001). There was no significant difference in the incidence of adverse reactions during the treatment period between the two groups ( P > 0.05). Serum BDNF level was negatively correlated with PANSS and SANS scores ( r = -0.81, -0.85, both P < 0.001), while it was positively correlated with PSP score ( r = 0.82, P < 0.001). Conclusion:High-frequency rTMS can effectively alleviate the negative symptoms of schizophrenia, increase the secretion of BDNF, and be highly safe.
7.Protective effect of melatonin on formaldehyde exposure-induced acute lung injury in rats through modulation of the Nrf2 pathway

Objective :
To investigate the protective effect of melatonin (MT) on formaldehyde (FA) inhalation-in- duced acute lung injury (ALI) in rats and its mechanism through the regulation of nuclear factor E2-related factor 2 (Nrf2) signaling pathway.
Methods :
Fifty female Wistar rats were randomly divided into Control group ,FA group,FA + MT 5 mg / kg group,FA + MT 10 mg / kg group and FA + MT 20 mg / kg group,with 10 rats in each group.Except for the Control group,all other groups inhaled 3 mg / m3 FA daily for 21 d consecutively to construct the tainted model,and then treated with different MT doses for 14 d.The tainting was continued during the MT treatment.Hematoxylin-eosin (HE) staining was used to observe the histopathological changes in lung tissue,lung water content and lung coefficient were weighed and measured,glutathione ( GSH) ,superoxide dismutase (SOD) and 8-hydroxydeoxyguanosine ( 8-OHdG) levels were measured by absorbance photometric method ,and enzyme linked immunosorbent assay(ELISA) was used to measure the levels of tumor necrosis factor-alpha (TNF-α) ,in- terleukin (IL) -6,and IL-1 β concentrations,Western blot to detect the protein expression levels of Nrf2,heme ox- ygenase-1 (HO-1) ,nuclear factor-κB ( NF-κB) ,and phosphorylated nuclear factor-κB ( p-NF-κB) in lung tis- sues,and quantitative polymerase chain reaction(qPCR) to detect the Nrf2,HO-1,and Kelch-like ECH-associated protein 1 (Keap1) mRNA expression levels.
Results :
Compared with the control group,lung injury was obvious in rats in the FA group ; lung tissue GSH and SOD levels were reduced ,and 8-OHdG levels were elevated ( P < 0. 05) ; alveolar lavage fluid TNF-α , IL-6,and IL-1 β levels were elevated (P<0. 05) ; Nrf 2 and HO-1 protein expression levels were reduced in the lung tissue (P<0. 05) ,and p-NF-κB protein expression levels were was ele- vated (P<0. 05) ; the relative mRNA expression of Nrf2 and HO-1 in lung tissue was decreased,and the relative mRNA expression of Keap1 was elevated (P<0. 05) .Compared with the FA group,the lung injury of rats in the MT group was improved ; the levels of GSH and SOD in the lung tissue were increased (P<0. 05) ,and the level of 8-OHdG was decreased (P<0. 05) ; the levels of TNF-α , IL-6,and IL-1 β in the alveolar lavage fluid were de- creased (P<0. 05) ; and the expression levels of the Nrf2 and HO-1 proteins in the lung tissue were increased (P <0. 05) .p-NF-κB protein expression level was decreased (P <0. 05) ; the relative mRNA expression levels of Nrf2 and HO-1 in lung tissues were increased (P<0. 05) ,and the relative mRNA expression level of Keap1 was decreased (P<0. 05) in lung tissues,and all of them were in a dose-dependent manner.
Conclusion
MT can al- leviate oxidative stress and inflammatory responses and mitigate FA exposure-induced acute lung injury by regula- ting the Nrf2 / Keap1 / HO-1 signaling pathway.
8.Applications and advances of desensitizing therapeutic agents in pre-sensitized renal transplantation

Kidney transplantation (KT) is one primary treatment of end-stage renal disease (ESRD). As reported by different centers, 10-year survival rate of transplanted kidneys fluctuates from 75% to 80%. Capable of reducing all-cause mortality in renal failure, KT can significantly improve survival rate and quality-of-life of ESRD patients. The risk of post-transplant rejection is much higher in pre-sensitised recipients than in non-sensitised ones due to the pre-existing anti-human leukocyte antigen antibodies. However, with a rapid development of various desensitisation techniques, transplantation rate and postoperative human/kidney survival rate of recipients have been greatly enhanced. And presensitisation is no longer a contraindication to KT. This review focused upon the latest advances in desensitisation therapeutic agents for pre-sensitised KT.
9.Lysosomal membrane protein Sidt2 knockout induces apoptosis of human hepatocytes in vitro independent of the autophagy-lysosomal pathway.

OBJECTIVE:
To explore the regulatory mechanism of human hepatocyte apoptosis induced by lysosomal membrane protein Sidt2 knockout.
METHODS:
The Sidt2 knockout (Sidt2-/-) cell model was constructed in human hepatocyte HL7702 cells using Crispr-Cas9 technology.The protein levels of Sidt2 and key autophagy proteins LC3-II/I and P62 in the cell model were detected using Western blotting, and the formation of autophagosomes was observed with MDC staining.EdU incorporation assay and flow cytometry were performed to observe the effect of Sidt2 knockout on cell proliferation and apoptosis.The effect of chloroquine at the saturating concentration on autophagic flux, proliferation and apoptosis of Sidt2 knockout cells were observed.
RESULTS:
Sidt2-/- HL7702 cells were successfully constructed.Sidt2 knockout significantly inhibited the proliferation and increased apoptosis of the cells, causing also increased protein expressions of LC3-II/I and P62(P < 0.05) and increased number of autophagosomes.Autophagy of the cells reached a saturated state following treatment with 50 μmol/L chloroquine, and at this concentration, chloroquine significantly increased the expressions of LC3B and P62 in Sidt2-/- HL7702 cells.
CONCLUSION
Sidt2 gene knockout causes dysregulation of the autophagy pathway and induces apoptosis of HL7702 cells, and the latter effect is not mediated by inhibiting the autophagy-lysosomal pathway.

10.Investigation and analysis of knowledge status and educational needs of caregivers for children with epilepsy

Objective:To determine the level of epilepsy knowledge of caregivers for children with epilepsy and analyze its influencing factors, and investigate caregivers' educational needs and their acceptance for remote education, in order to provide reference for clinical telenursing education.Methods:From March to September 2022, 221 caregivers of epileptic children in the outpatient department and ward of neurology department of Xuzhou Children's Hospital were recruited by convenient sampling method for cross-sectional investigation. The status of caregivers' knowledge and educational needs were investigated by the general information questionnaire, epilepsy knowledge questionnaire, epilepsy knowledge needs questionnaire and telenursing acceptance questionnaire, and the influencing factors of knowledge level were analyzed by multiple linear regression.Results:The average score of epilepsy knowledge of caregivers was (15.68 ± 6.43) points. The course of disease, taking medicine on time, education background and monthly income of caregivers were the influencing factors of caregivers' knowledge level, and the difference was statistically significant ( P <0.05). 94.12% (208/221)- 96.38% (213/221) of the caregivers had high educational needs, and they had the highest demand for safety guidance during seizures. Caregivers' acceptance of remote education was moderate, ranging from 34.39% (76/221) to 71.95% (159/221). Conclusions:Caregivers' epilepsy knowledge needs to be improved. Medical institutions should formulate education plans according to the different characteristics of caregivers. Caregivers have a high demand for nursing knowledge, and medical staff should increase health education. Before giving health education based on remote nursing platform, we should fully understand the attitude of caregivers to the platform, so that they can master disease knowledge, strengthen their disease management ability, and improve the quality of life of children.
