1.Investigation of latent tuberculosis infection among the elderly in rural areas of Changxing County, Zhejiang Province

ObjectiveTo investigate the current status of latent tuberculosis infection (LTBI) among the elderly population in rural areas of Changxing County, Zhejiang Province, and to provide an evidence for the development of LTBI prevention and control measures. MethodsBetween January and May 2024, elderly individuals participating in urban and rural residents’ health checkups were screened for Mycobacterium tuberculosis infection using a domestically produced interferon-γ release assay (IGRA) kit. Individuals tested positive by IGRA but without active tuberculosis were classified as LTBI cases. The prevalence of LTBI among the participants was subsequently analyzed. ResultsAmong the 6 765 subjects, 637 tested positive by IGRA, including one identified active tuberculosis patient, resulting in a LTBI prevalence rate of 9.40%. There was a statistically significant difference in positivity rates across different IGRA methodologies (χ2=35.530, P<0.001). Higher LTBI rate was observed in males, individuals with a history of diabetes mellitus, and those with a history of pulmonary tuberculosis, exhibiting statistically significant differences (χ2=32.401, P<0.001; χ2=5.789, P=0.020; χ2=39.248, P<0.001, respectively.) No statistically significant difference in LTBI rate was found across different age groups (χ2=0.238, P=0.971). ConclusionThe prevalence of LTBI among the elderly rural residents in Changxing County is relatively low. Male, individuals with a history of diabetes mellitus, and those with a history of pulmonary tuberculosis have an increased risk of LTBI, warranting targeted risk monitoring and timely interventions.

According to the work goals and tasks determined by edition outline of the Chinese Pharmacopoeia 2025 Edition, the Chinese Pharmacopoeia 2025 Edition has been completed. Among them, 52 new pharmaceutical excipients monographs have been added, an increase of 15.5% compared with the 2020 Edition, and the total number has reached 387. This article focuses on the general framework and the main characteristics of the standards of pharmaceutical excipients in the Chinese Pharmacopoeia 2025 Edition, which can contribute to accurately understand and utilize the standards in Chinese Pharmacopoeia.

BackgroundAttention deficit hyperactivity disorder (ADHD) is a neurodevelopmental disorder characterized by age-inappropriate inattention, excessive activities incongruous with setting, and emotional impulsivity. Subthreshold ADHD (sADHD) is clinically defined as the presence of ADHD symptoms that do not meet the full diagnostic criteria for ADHD. Children with sADHD exhibit deficits in executive function, demonstrate more conduct, learning, and anxiety-related problems compared to typically developing children, and show even poorer working memory performance than children diagnosed with ADHD. Currently, there is limited epidemiological research on sADHD in China, with few studies simultaneously investigating the prevalence of both ADHD and sADHD in children. ObjectiveTo investigate the prevalence of ADHD and sADHD among children aged 6–13 years in Chongqing, analyzing their distribution characteristics within this population, with the aim of providing references for developing preventive measures against both ADHD and sADHD. MethodsFrom October to November 2023, a total of 3 398 students in grades 1–6 from six primary schools in Jiangbei District, Chongqing were selected using a stratified cluster random sampling method. The occurrence of ADHD and sADHD was evaluated by using the short version (18-item version) of the Swanson, Nolan, and Pelham IV rating scales (SNAP-IV) and the Chinese vision of Schedule for Affective Disorder and Schizophrenia for School-aged Children-Present and Lifetime Version (K-SADS-PL). ResultsThe ADHD detection rate among children in Chongqing was 1.90% (95% CI: 0.014–0.024). Boys showed a significantly higher ADHD detection rate than girls (χ2=7.733, P=0.005). No statistically significant differences were found in ADHD detection rates across different grades or age groups (χ2=7.347, 12.362, P>0.05). The sADHD detection rate was 6.32% (95% CI: 0.054–0.072). Similarly, boys exhibited significantly higher sADHD detection rates than girls (χ2=21.005, P<0.01). Significant differences emerged across different grades (χ2=20.559, P=0.001), while no statistically significant difference was observed in age groups (χ2=12.070, P=0.060). ConclusionThe ADHD detection rates were comparable across all grade levels and age groups from 6–13 years old. Second-grade children demonstrated notably higher sADHD rates compared to other grades, while boys demonstrated higher prevalence rates than girls for both ADHD and sADHD. 5.Whole-genome sequencing analysis of Staphylococcus aureus in a food poisoning outbreak in Suzhou, Jiangsu Province

ObjectiveTo conduct an epidemiological investigation, isolate and identify the pathogenic bacteria, and perform whole-genome sequencing (WGS) analysis of a food poisoning outbreak in Suzhou High-tech Zone, so as to provide references for the prevention and management of similar incidents. MethodsAn epidemiological investigation was carried out on affected individuals and restaurant staff. Multi-pathogen screening was performed on rectal swab samples from affected cases and restaurant staff, as well as on environmental swab samples from food preparation areas and retained food samples. WGS was performed on the pathogens isolated from the samples. ResultsA total of 15 cases were identified through hospital visit record reviews and on-site inquiries. Of the 15 case samples, 14 tested positive for Staphylococcus aureus (SA). In addition, 3 out of 15 staff samples and 6 out of 34 retained food samples tested positive for SA. WGS results revealed that the 6 food-derived isolates and 13 case-derived isolates shared the same sequence type (ST), Staphylococcal Protein A (SPA) type, and enterotoxin gene profile, with whole-genome single nucleotide polymorphism (wgSNP) differences of ≤5. Two novel SA sequence types: ST9159 and ST9161, were also identified in this study. ConclusionThis food poisoning outbreak is caused by contamination food with SA harbouring the seb and sec enterotoxin genes. The primary causes of contamination and cross-contamination included improper food handling practices, the use of the same utensils for raw and cooked foods, and extended storage of certain dishes at room temperature. Strengthening sanitary supervision in food handling facilities is crucial for reducing the occurrence of similar incidents.
6.Negative pressure wound therapy assisted reconstruction of soft tissue defects of limbs with free anterolateral thigh perforator flap

Objective:To investigate the effect and clinical efficacy of assisted negative pressure wound therapy (NPWT) dressing on reduction of the size of free anterolateral thigh perforator flap (ALTPF) in reconstruction of soft tissue defects of limbs, and to observe the clinical efficacy of NPWT in free ALTPF surgery.Methods:From June 2022 to January 2024, 19 patient (13 males and 6 females) with soft tissue defects in limbs received surgical treatment using NPWT assisted transfer of free ALTPF in the Department of Repair and Reconstruction Surgery, Foshan Hospital of Traditional Chinese Medicine, the Eighth School of Clinical Medicine, Guangzhou University of Traditional Chinese Medicine. Of the 19 defects, 8 were in calf, 7 in foot and around ankle, 3 in forearm and 1 around elbow. The soft tissue defects ranged from 11.2 cm×9.5 cm to 24.5 cm×10.5 cm, and with comminuted fracture in different degrees as well as the exposure of bone, tendon or nerve. During surgery, the ALTPFs were harvested with a reduced width from the donor sites at 10.5 cm×7.0 cm to 24.0 cm×7.5 cm in size. After an ALTPF had been transferred to the recipient site, the remaining defect that was not covered by the ALTPF was then covered by a NPWT dressing. After the flap was stabilised, NPWT dressing was removed and the defect was closed by directly suture. Surgical data were taken and recorded during surgery. The healing of wound and survival of flaps were observed after surgery. Scheduled postoperative follow-ups were conducted through outpatient clinic or via WeChat reviews to monitor the overall appearance of recipient and donor sites, colour of flap, complications and functional recovery, and other relevant information. Functional recovery of upper limb was evaluated according to the Evaluation Trial Standards of Upper Limb Partial Functional of Hand Surgery of Chinese Medical Association, and the functional recovery of lower extremity was evaluated according to the Lower Extremity Functional Scale (LEFS).Results:The flaps were all fully viable, with soft texture and good appearance, the width of the flaps was designed 2.0-3.5 cm smaller than that of the defect, and the size of the flaps was 32.9-77.3 cm 2 smaller than that of the defects. All donor sites were closed by suture in the primary surgery, and left with linear scars. Postoperative follow-up lasted for 4 to 18 months. Of the 4 patients with upper limb injuries, 3 achieved the limb function in excellent and 1 in good; While of the 15 patients with lower limb injuries, 12 achieved the limb function in excellent, 2 in good and 1 in fair. Conclusion:Combination of NPWT dressing and free ALTPF is in accordance with the theory of flap economics and the concept of minimal damage to donor site. It avoids the waste of soft tissues, and provides a new idea for traumatic surgery.

8.Data-Driven Construction of Operation Evaluation System for Large Medical Equipment in Children's Hospital

Objective Through data collection and analysis,the method of evaluating the operation quality of large medical equipment in children's hospital is explored and suggestions on the use and configuration of large medical equipment is put forward.Methods Collect the equipment operation data through the Internet of Things,and combine the hospital HIS,RIS,HRP and other information system data to establish the operation evaluation system of large medical equipment of children's hospital.CRITIC method is used to quantitatively evaluate single type of equipment and single equipment.Results Hospital big data platform realizes the longitudinal analysis of the operation data of a single large equipment and forms a visual chart,which is displayed on the PC and mobile terminals.Municipal platform can conduct horizontal analysis on the equipment operation data to realize the comprehensive quantitative evaluation of the operation level of large equipment of children's hospital and put forward suggestions for use and configuration.Conclusion A large equipment operation evaluation system for children's hospital is established through data collection and analysis,and the fine management level of large medical equipment is improved.
9.Effect of high-frequency rTMS therapy on brain function remodeling and function of core muscle group in patients with stroke

Objective:To explore the effect of high-frequency repetitive transcranial magnetic stimulation(rTMS)on brain functional remodeling and function of core muscle group in patients with stroke.Methods:A total of 104 patients with stroke who admitted to Ruijin Hospital Affiliated to Shanghai Jiaotong University School of Medicine from January 2022 to June 2023 were prospectively selected,and they were randomly divided into a control group(52 cases,received conventional rehabilitation treatment)and a study group(52 cases,received conventional rehabilitation+high-frequency rTMS treatment).The differences in clinical efficacy,brain functional remodeling(LI value),and postural assessment scale for stroke(PASS)of the function of core muscle group and manual muscle test(MMT)scores of core muscle group between two groups were analyzed.Results:The total effective rate of clinical treatment in study group was 92.31%,which was significantly higher than that in the control group(73.08%),and the difference was significant(x2=6.718,P<0.05).There were no significant difference in the scores of PASS and MMT between the two groups before treatment(P>0.05).The scores of PASS and MMT of two groups after treatment were significantly higher than those before treatment(t=21.017,13.359,P<0.05),respectively.After treatment,the PASS and MMT scores of study group were respectively(31.15±1.78)points and(11.88±2.03)points,which were significantly higher than those[(28.33±1.82)points and(9.41±2.27)points]of control group(t=7.988,5.849,P<0.05),respectively.After treatment,the brain activation areas of study group mainly distributed in the precentral gyrus,precuneus,somatosensory area,cerebellum and ipsilateral supplementary motor area,which appeared a trend of centralization.The distribution of brain activation areas of control group after treatment still showed the characteristics of bilateral extensive activation,which did not appear significant changes than those before treatment.There was no significant difference in LI value between the two groups before treatment(P>0.05).After treatment,the LI values of two groups were significantly improved,and the LI value of study group was significantly higher than that of control group(t=17.199,P<0.05).Pearson correlation analysis was performed on the LI values and the scores of PASS and MMT when the hemiplegic side of the patients of two groups conducted chunking motion.The results showed that the LI values appeared significantly positive correlation with PASS score and MMT score(r=0.863,0.847,P<0.05),respectively.Both two groups did not occur serious abnormal reactions during the treatment.Two patients of study group reported they occurred mild dizziness and headache,which were relieved by themselves after rest.Conclusion:High-frequency rTMS can effectively improve the treatment effect for patients with stroke,and promote the recovery of the motor function area of brain and the function of core muscle group of patients.
10.Effects of MT-1207 on blood glucose,blood lipids and atherosclerosis in mice

Objective To study the effect of MT-1207 on blood glucose,blood lipids and atherosclerosis in mice.Methods The apolipoprotein E knockout(ApoE-/-)mice were fed with normal feed,drug feed containing losartan and drug feed containing MT-1207 at a dosage of 30 mg/kg.The body weight,blood glucose and blood lipids were detected,and the plaque area of atherosclerotic was evaluated.8-week-old male C57 mice were fed a high fat diet and given intragastric administration of MT-1207 and losartan at a dose of 30 mg/kg per day.The body weight,blood glucose and lipids levels were also examined to further evaluate the effects of MT-1207 on blood glucose and lipids levels.Results ApoE-/-mice treated with MT-1207 and losartan gained weight faster.There was no significant improvement in blood glucose and lipid levels,and no significant change in atherosclerotic plaque area.MT-1207 and losartan had no significant improvement effect on blood glucose and blood lipids of C57 mice.Conclusion MT-1207 and losartan couldn't improve the levels of blood glucose,blood lipids and atherosclerosis,and couldn't aggravate atherosclerosis.
