Objective To investigate the therapeutic effect of Ziwuliuzhu acupuncture in a rat model of insomnia and its regulatory effect on the glutamic acid(Glu)/γ-aminobutyric acid(GABA)-glutamine(Gln)metabolic loop. Methods Forty male SD rats were randomly assigned to control group, model group,Najia group and Nazi group(n=10).In the latter 3 groups, rat models of insomnia were established by intraperitoneal injections of p-chlorophenylalanine and verified using a sodium pentobarbital-induced sleep test. After modeling, the rats in Najia and Nazi groups received acupuncture for 7 days at specifically chosen sets of acupoints based on the Ziwuliuzhu rationale in traditional Chinese medicine.Pathological changes in the hypothalamic tissue of the rats were examined with HE staining, and the levels of Glu and GABA in the hypothalamus were determined with high-performance liquid chromatography(HPLC)-mass spectrometry(MS)/MS.Immunohistochemistry was used to detect the expressions of GABAA receptors(GABAARs)in the hypothalamus, and the expression levels of glutamate decarboxylase(GAD65/67)and glutamine synthetase(GS)were determined with Western blotting. Results Compared with the model group, the rats in Najia and Nazi groups exhibited decreased Glu levels and GABAA receptor expression and increased GABA levels with a decreased Glu/GABA ratio in the hypothalamus.Ziwuliuzhu acupuncture significantly increased the protein expressions of GAD65 and GAD67 and lowered the expression of GS in the hypothalamus in the rat models of insomnia. Conclusion Ziwuliuzhu acupuncture produces sedative and hypnotic effects in rat models of insomnia possibly by regulating Glu and GABA-Gln metabolism to restore the excitatory/inhibitory balance between Glu and GABA.

While tumor immunotherapy has improved the prognosis of many patients, immunotherapy drug resistance develops in most tu-mors.Studies of new targets and therapies are needed to overcome this challenge.Growing evidence indicates that Siglecs function in im-mune checkpoints.Siglecs belong to a family of salivary acid-binding immunoglobulin-like lectins. The majority of Siglecs are immunosup-pressive transmembrane proteins that bind to sialic acid-containing glycans on glycoproteins and glycolipids.Sialoglycan-Siglec interactions contribute to tumor immune escape and are strongly correlated with poor prognosis in tumor patients. This review will discuss Siglec func-tion in immune checkpoints, and how the interaction between sialic acid and Siglecs regulates the tumor microenvironment to exert thera-peutic effects.

To investigate the prevalence and associated factors of fall risk among Chinese older adults, and to examine the roles of urban-rural differences, regional disparities, physical health status, and psychosocial factors in falls among this population, thereby providing evidence for tailored fall prevention strategies. Using data from the 2024 National Routine Physical Fitness Surveillance, a multi-stage stratified sampling method was employed to recruit community-dwelling older adults aged 60-79 years across China. High fall-risk individuals were identified using the Chinese version of the self-rated fall risk questionnaire, while demographic, physical health, and psychological indicators were collected via questionnaires and objective measurements. A generalized linear mixed model (GLMM) with province as a random effect was used to analyze fall risk factors. Among 7000 eligible participants (male: 44.2%, female: 55.8%), the sample comprised 2124 (60-64 years), 2014 (65-69 years), 1660 (70-74 years), and 1202 (75-79 years) individuals, with 58.4% from rural and 41.6% from urban areas. A total of 733(10.5%) were identified as high fall-risk, with higher prevalence among females (10.9%), urban residents (11.5%), and the oldest age group (75-79 years: 12.4%). GLMM random-intercept logistic regression revealed that advanced age ( The prevalence of high fall risk among Chinese community-dwelling older adults aged 60-79 years is 10.5%. Fall risk demonstrates significant associations with multiple factors including muscle strength, movement patterns, sleep quality, and social support. Strategies enhancing grip strength, promoting regular exercise and high-intensity leisure activities, improving sleep quality, fostering spousal support, and boosting life satisfaction may substantially reduce fall risk in this population.

Chinese Journal of Endocrine Surgery 2023;17(3):370-372
Lung cancer is one of the malignant tumors with the highest morbidity and mortality. Epidermal growth factor receptor is expressed in the majority of non-small cell lung cancer tumor cells, making it possible to give these patients more precise and targeted therapies. The mechanisms of abnormal EGFR regulation are closely related to the efficacy of targeted therapy and the occurrence of drug resistance. Therefore, this review will review the expression mechanism of EGFR in NSCLC and the acquired drug resistance of epidermal growth factor receptor tyrosine kinase inhibitor after treatment of NSCLC.

Chinese Journal of Trauma 2023;39(12):1057-1069
Pulmonary blast injury has become the main type of trauma in modern warfare, characterized by externally mild injuries but internally severe injuries, rapid disease progression, and a high rate of early death. The injury is complicated in clinical practice, often with multiple and compound injuries. Currently, there is a lack of effective protective materials, accurate injury detection instrument and portable monitoring and transportation equipment, standardized clinical treatment guidelines in various medical centers, and evidence-based guidelines at home and abroad, resulting in a high mortality in clinlcal practice. Therefore, the Trauma Branch of Chinese Medical Association and the Editorial Committee of Chinese Journal of Trauma organized military and civilian experts in related fields such as thoracic surgery and traumatic surgery to jointly develop the Clinical treatment guideline for pulmonary blast injury ( version 2023) by combining evidence for effectiveness and clinical first-line treatment experience. This guideline provided 16 recommended opinions surrounding definition, characteristics, pre-hospital diagnosis and treatment, and in-hospital treatment of pulmonary blast injury, hoping to provide a basis for the clinical treatment in hospitals at different levels.
