1.Preparation of osteoporotic femoral condylar bone defect model in rabbits and its critical value

BACKGROUND:In most species,a bone defect that is longer than 1.5 or 2 times its diameter can be considered a critical bone defect,and when the bone defect volume reaches the critical value,it cannot heal on its own.Currently,there is no uniform standard for the size of critical-sized defects in the osteoporotic femoral condyle of rabbits.OBJECTIVE:To establish a rabbit model with different sizes of bone defects in the osteoporotic femoral condyle and to determine the critical-sized defects of osteoporotic femoral condyle in rabbits.METHODS:Thirty-six 3-month-old female New Zealand white rabbits were randomly divided into ovariectomy group(n=30)and sham operation group(n=6).Rabbits in the ovariectomy group underwent bilateral ovariectomy to establish an osteoporosis model,and then femoral condyle bone defect models of different diameters(diameters were 4,5,6,and 7 mm,and depths were 8 mm)were further established;rabbits in the sham operation group did not undergo ovariectomy.At 8 and 12 weeks after modeling,3 rats were randomly selected from each group for CT scanning and three-dimensional reconstruction to evaluate the healing of bone defects.Afterwards,samples were taken for gross observation and hematoxylin-eosin staining to observe the growth of new bone in the femoral condyle bone defect area.RESULTS AND CONCLUSION:(1)All rabbits survived and moved well after modeling of osteoporosis and femoral condyle bone defect.(2)At 12 weeks after osteoporosis modeling,dual-energy X-ray absorptiometry results showed that the bone mineral density of lumbar vertebrae in ovariectomy group was significantly lower than that in sham operation group(P<0.05).Hematoxylin-eosin staining showed that the bone trabeculae in the ovariectomy group became thinner and sparse.The proportion of bone tissue area in the ovariectomy group was significantly lower than that in the sham operation group(P=0.00).Micro-CT results showed that the bone tissue parameters of the femoral condyle in the ovariectomy group were significantly different from those in the sham operation group(P<0.05),and the ovariectomy group showed obvious characteristics of osteoporosis.(3)CT showed that the bone defect in the 4 mm and 5 mm diameter groups was basically completely repaired at 12 weeks after surgery.There was more new bone tissue in the 6 mm diameter group,but the central part of the bone defect was not completely repaired.A small amount of new bone tissue grew in the 7 mm diameter group,and the bone defect was obvious.(4)Gross observation at 12 weeks after surgery showed that the femoral condyle bone defect in the 4 mm and 5 mm diameter groups was completely repaired.Obvious depression was seen in the bone defect area of the 6 mm and 7 mm diameter groups,and the bone defect was not completely repaired.(5)Histological observation at 12 weeks after surgery showed that the bone defect area of the 4 mm and 5 mm diameter groups was completely filled with new bone,and the trabecular structure was irregular;while there were new trabeculae in the periphery of the 6 mm and 7 mm diameter groups,and the bone defect in the central area was still obvious.(6)The results showed that during the 12-week experimental observation period of osteoporotic femoral condyle defects in rabbits,under the condition of the same defect depth of 8 mm,femoral condyle defects with a diameter ≥ 6 mm could not heal on their own,while femoral condyle defects with a diameter<6 mm were completely repaired.A diameter of 6 mm and a depth of 8 mm can be used as the critical bone defect value of osteoporotic femoral condyle in rabbits.
2.Effect of acupuncture on postoperative delirium in diabetic patients undergoing surgery under general anesthesia

Objective:To evaluate the effect of acupuncture on postoperative delirium (POD) in diabetic patients undergoing surgery under general anesthesia.Methods:In this randomized controlled trial, 92 diabetic patients of either sex, aged 30-80 yr, with a body mass index of 18-28 kg/m 2, of American Society of Anesthesiologists Physical Status classification Ⅱ or Ⅲ, scheduled for elective surgery under general anesthesia, were divided into 2 groups ( n=46 each) using a table of random numbers: control group (group C) and acupuncture group (group A). Group A received acupuncture at the Baihui (GV20), Shenting (GV24) and Sishencong (EX-HN1) acupoints before anesthesia. The needles were retained for 30 min, with manual stimulation applied every 10 min for 10 s each time. After 4 stimulations, routine anesthesia was carried out. Group C received routine anesthesia only. Regional cerebral oxygen saturation was recorded on admission to the operating room (T 0), after anesthesia induction (T 1), at the start of surgery (T 2), at the end of surgery (T 3), and immediately after tracheal extubation (T 4). The POD developed within 3 days after surgery was assessed. The occurrence of needle-related adverse effects such as fainting, subcutaneous bleeding, and local paresthesia was recorded. Results:Compared with group C, the incidence of POD was significantly reduced, and the regional cerebral oxygen saturation was increased at T 1, 4 in group A ( P<0.05). Conclusions:Acupuncture can decrease the development of POD in diabetic patients undergoing surgery under general anesthesia, which is related to an increase in regional cerebral oxygen saturation.
3.Early clinical efficacy of Yiqi Tongluo Decoction on ischemic cerebrovascular disease after interventional therapy

Objective:
To observe the clinical efficacy and safety of Yiqi Tongluo Decoction in the intervention of early traditional Chinese medicine (TCM) syndromes after ischemic cerebrovascular disease (ICVD) intervention.
Methods:
From October 2020 to July 2023, a randomized, double-blind, placebo-controlled study was conducted to include 60 patients with qi deficiency, blood stasis, and phlegm obstruction syndrome after ICVD interventional therapy. They were assigned to the Yiqi Tongluo Decoction treatment group (30 cases) and the TCM placebo routine treatment control group (30 cases) according to the randomized block design. Both groups received routine standardized treatment of Western medicine, including dual antiplatelet, lipid regulation, and control of risk factors for cerebrovascular disease. The treatment group was treated with Yiqi Tongluo Decoction based on the control group. The course of treatment was 60 days and follow-up was carried out 2 and 6 months after the operation. The improvement of qi deficiency syndrome, blood stasis syndrome, phlegm syndrome score and TCM syndrome score, modified Rankin score (mRS), Barthel index (BI) score, Fatty acid-binding protein 4 (FABP4) level, incidence of transient ischemic attack (TIA) and ischemic stroke (IS) and incidence of adverse reactions, Head and neck CT angiography (CTA) or digital subtraction angiography (DSA) examination were collected. The clinical efficacy of the patients 2 months after the operation was taken as the main outcome index to preliminarily evaluate the early and long-term efficacy of Yiqi Tongluo Decoction after the ICVD intervention. The early and long-term clinical efficacy and safety of Western medicine standardized treatment combined with TCM Yiqi Tongluo Decoction on patients with qi deficiency, blood stasis and phlegm obstruction syndrome after ICVD intervention were evaluated. The safety of Yiqi Tongluo Decoction in the treatment of patients after ICVD intervention with white blood cell (WBC), C-reactive protein (CRP), fibrinogen (FIB), plasminogen time (PT), recurrence of cerebral ischaemia and restenosis in patients at 2 and 6 months after treatment were evaluated.
Results:
Compared to the control group, the TCM syndrome scores for qi deficiency, blood stasis and phlegm syndrome in the treatment group reduced significantly, the clinical efficacy improved significantly, the mRS score and FABP4 were reduced, and the BI score was increased. Adverse events such as cerebral ischaemia were fewer in the treatment group than in the control group, but the difference was not statistically significant; levels of CRP, WBC and PT were reduced, and levels of FIB were reduced at 6 months post-treatment, all P<0.01, and images were intuitively compared. The treatment group was superior to the control group.
Conclusion
Yiqi Tongluo Decoction combined with Western medicine standard treatment can improve the early clinical efficacy of ICVD patients with qi deficiency, blood stasis and phlegm obstruction syndrome after interventional surgery, improve neurological impairment and daily living ability, reduce the state of qi deficiency syndrome, blood stasis syndrome and phlegm syndrome after interventional surgery, and improve the clinical efficacy of TCM. At the same time, it can reduce the level of FABP4, the target of atherosclerosis and restenosis after interventional surgery, reduce the level of inflammation after interventional surgery in patients with ICVD, regulate coagulation function, and reduce the incidence of long-term recurrence of cerebral ischemia after interventional surgery, with good safety.
4.Syndrome Differentiation and Treatment of Alopecia Areata Based on the Turbid Toxin Theory from the Perspectives of the Liver,Spleen,and Kidney

Alopecia areata (AA) is a common hair loss disorder, and the core pathogenesis is the internal gene-ration of turbid toxin caused by qi movement disorder in the liver, spleen, and kidney. Turbid toxin serves as both a pivotal etiological trigger and a pathological driver of disease exacerbation. Clinically, AA can be classified into four principal patterns, including liver constraint with spleen deficiency, internal accumulation of damp-heat, liver-kidney depletion, and qi-blood depletion. Therapeutic strategies prioritize clearing and resolving turbid toxin while regulating the qi movement of the liver, spleen, and kidney. Accordingly, different formulas were applied. Self-formulated Shugan Jianpi Huazhuo Formula (疏肝健脾化浊方) is suggested to drain dampness, resolve turbidity, and unblock qi movement. Self-formulated Sanjiao Fenxiao Jiedu Formula (三焦分消解毒方) can be used to clear heat, drain dampness and resolve toxin. Self-formulated Zishen Yanggan Toudu Decoction (滋肾养肝透毒汤) can clear and vent latent toxins, while Self-formulated Guiqi Shengfa Didu Formula (归芪生发涤毒方) is employed to tonify qi and blood, purge toxins, and regenerate vitality. By differentiating and treating AA based on the functional patterns of the liver, spleen, and kidney, this approach expands the application scope of the turbid toxin theory and provides valuable insights for treatment of AA.

Rodent models play a crucial role in research on human periodontal diseases,providing key evidence for investigation into the pathological mechanisms of periodontal bone defects.Relevant research in the field involves gene expression,inflammatory regulation mechanisms,host-microbial interactions,as well as disease resolution and healing processes.Research methodology in the field falls under 2 categories-periodontal inflammation models and surgical defect models.The former simulates periodontal defects by inducing periodontal diseases,while the latter constructs clinically simulated periodontal defects through surgical removal of periodontal tissue.However,the currently available animal models of periodontitis face challenges in simultaneously capturing the disease complexity,tracking dynamic repair processes,and meeting translational needs.Herein,we reviewed and summarized the methods and characteristics of periodontal disease modeling in recent years.We proposed the establishment of a multimodal assessment framework integrating technologies such as spatial transcriptomics,single-cell sequencing,and in vivo fluorescence imaging,which may serve as a critical pathway for overcoming existing research challenges.
7.Electroencephalograph combined with miR-146a-5p to predict cognitive dysfunction in Alzheimer's disease patients

Objective To explore the predictive value of electroencephalograph(EEG)quantitative parameters combined with serum microRNAs(miR)-146a-5p in cognitive dysfunction in patients with Alzheimer's disease(AD).Methods A total of 96 AD patients diagnosed in Shaoxing Seventh People's Hospital from March 2022 to May 2024 were selected to assess whether the patients had cognitive impairment according to the Montreal cognitive scale(MoCA)score,and 40 patients with cognitive impairment were set as occurrence group(MoCA score<26points).A total of 56 patients with normal cognitive function were set as non-developing group(MoCA score ≥ 26points).Quantitative electroencephalography(EEG)parameters and serum miR-146a-5p were compared between the two groups.The risk factors of cognitive dysfunction in AD patients were analyzed by Logistic regression,and receiver operating characteristic curve was used to predict efficacy of EEG quantitative parameters and serum miR-146a-5p on cognitive dysfunction in AD patients.Results The δ frequency band,θ frequency band,and serum miR-146a-5p levels were lower in occurrence group than those in non occurrence group,while β frequency band and α frequency band were higher in occurrence group than in those non occurrence group(P<0.05).δ frequency band,θ frequency band,miR-146a-5p,age,and years of education were risk factors for cognitive dysfunction in AD patients(P<0.05).The area under the curve(AUC)of predicting cognitive impairment in AD patients using the combination of δ frequency band,θ frequency band,βfrequency band,α frequency band,and miR-146a-5p was 0.908.The sensitivity of predicting cognitive impairment in AD patients using the combination of delta frequency band,δ frequency band,β frequency band,α frequency band,and miR-146a-5p was higher than that of individual detection(P<0.05).Conclusion The decrease in δ frequency band,θ frequency band,serum miR-146a-5p,and the increase in β frequency band and α frequency are closely related to the occurrence of cognitive dysfunction in AD patients.Combined detection of EEG of electroencephalogram and serum miR-146a-5p can improve the predictive efficiency of cognitive dysfunction.

10.Advances in the application of physiologically-based pharmacokinetic model in EGFR-TKI precision therapy

Epidermal growth factor receptor-tyrosine kinase inhibitor(EGFR-TKI)represent a class of small-molecule targeted therapeutics for oncology treatment,and serve as first-line therapy for advanced non-small cell lung cancer(NSCLC)with EGFR-sensitive mutations,with representative agents including gefitinib,dacomitinib,and osimertinib.In clinical practice,dose adjustment of EGFR-TKI may be required for cancer patients under special circumstances such as drug combinations or hepatic/renal impairment.Physiologically-based pharmacokinetic(PBPK)model,capable of predicting pharmacokinetic(PK)processes in humans,has emerged as a vital tool for clinical dose optimization.This article sorts the modeling methodologies,workflows,and commonly used software tools for PBPK model,and summarizes the current applications of PBPK model in EGFR-TKI precision therapy as of June 30,2024.Findings demonstrate that PBPK modeling methods commonly employ the"bottom-up"approach and the middle-out approach.The process typically involves four steps:parameter collection,compartment selection,model validation,and model application.Commonly used software for modeling includes Simcyp,GastroPlus,and open-source software such as PK-Sim.PBPK model can be utilized for predicting drug-drug interactions of EGFR-TKI co-administered with metabolic enzyme inducers or inhibitors,acid-suppressive drugs,or traditional Chinese and Western medicines.It can also adjust dosages in conjunction with genomics,predict PK processes in special populations(such as patients with liver or kidney dysfunction,pediatric patients),evaluate the efficacy and safety of drugs,and extrapolate PK predictions from animal models to humans.
