China Pharmacy 2026;37(10):1307-1312
OBJECTIVE To compare the efficacy and safety of vedolizumab (VDZ) versus infliximab (IFX) in biologic-naive(Bio-naive) patients with moderate-to-severe active ulcerative colitis (UC), and to analyze the factors influencing efficacy. METHODS Clinical data were retrospectively collected from Bio-naive patients with moderate-to-severe active UC who received treatment in the Department of Gastroenterology at Shanxi Provincial People’s Hospital from June 2023 to June 2024. Based on the type of biologic agent administered, the patients were divided into the IFX group (41 cases) and the VDZ group (30 cases). Patients in the two groups received IFX (5 mg/kg) or VDZ (300 mg) for induction and maintenance of remission therapy. The two groups were compared regarding modified Mayo score, serological indicators (hemoglobin, albumin, platelet count, erythrocyte sedimentation rate, C-reactive protein), combined medication, efficacy-related indexes (clinical response rate/remission rate, and endoscopic response rate/remission rate), and the occurrence of adverse drug reactions (ADR). Based on Logistic regression model, univariate and multivariate analyses were conducted to identify potential factors influencing clinical remission at week 14 and endoscopic remission at week 38. RESULTS There were no statistically significant differences in clinical response rate/remission rate, or endoscopic response rate/remission rate between the two groups at weeks 14 and 38 ( P >0.05). However, at week 14 of treatment, the proportion of patients using concomitant corticosteroids in VDZ group was 26.67%, significantly higher than the 7.50% in IFX group ( P <0.05). There was no statistical significance in the overall incidence of ADR between the two groups ( P >0.05); all ADRs in the IFX group were grade 3 and led to treatment discontinuation (6 cases), whereas ADR in the VDZ group was grade 2 and did not interrupt therapy (1 case). Univariate and multivariate regression analyses revealed that disease type (relapsing) was significantly associated with clinical remission at week 14 of treatment, and a history of smoking was significantly associated with endoscopic remission at week 38 of treatment (the odds ratios were 0.08 for both, with 95% confidence intervals of 0.01-0.77 and 0.01-0.91 respectively, P <0.05). CONCLUSIONS For Bio-naive patients with moderate-to-severe active UC, VDZ and IFX demonstrate comparable efficacy in inducing and maintaining clinical remission and promoting mucosal healing, as well as overall safety. Although IFX can achieve faster control of inflammation in the early stage of the disease, it causes more severe ADR. Disease type (relapsing) and smoking history are identified as independent negative predictors for short-term clinical remission and long-term endoscopic remission, respectively.

The disease-syndrome combination model of spleen deficiency with dampness pattern in ulcerative colitis(SDDP-UC) is an important experimental carrier for traditional Chinese medicine (TCM) research on the prevention and treatment of ulcerative colitis (UC), and the quality of model construction and evaluation directly influences the scientific rigor and translational value of related research conclusions. However, this field still lacks methodological synthesis and a standardized consensus. Based on a comprehensive review of existing literature, this paper summarized isomorphic cues between the spleen deficiency with dampness pattern and UC across four dimensions, including energy metabolism, immune homeostasis, mucosal barrier, and intestinal microecology. The cues were mainly involved in impaired mitochondrial energy supply and glucose metabolic reprogramming, a lowered pro-inflammatory threshold of innate immunity with insufficient adaptive immune regulation, disruption of epithelial barrier gating accompanied by compromised repair capacity, and attenuation of the luminal hypoxia barrier with accumulation of toxic metabolites. A mutually reinforcing process between local "form damage" and systemic "Qi depletion" was further interpreted from a holistic perspective. Regarding modeling strategies, existing studies predominantly use rats as the carrier, apply combined interventions such as improper diet, external damp exposure, and fatigue-related dysregulation to establish the spleen deficiency with dampness pattern background, and subsequently superimpose chemical stimulation to induce UC-like colonic damage, with a total modeling period generally spanning three to four weeks. In terms of the evaluation system, a multidimensional framework integrating syndrome assessment, histopathology, mechanistic indices, and pharmacodynamic counter-verification was outlined. On this basis, current methodological bottlenecks of models were systematically identified, including syndrome drift risk and compounded stress dilemma in temporal sequencing, syndrome confounding from etiological simulation, cross-sectional evaluation bias related to modeling duration, inadequate disease-syndrome linkage and control design within the evaluation system, and limited controls with overly single-track decision logic in formula-based syndrome verification. To address the above issues, a construction and evaluation strategy emphasizing streamlining of core etiological factors, multi-node dynamic monitoring, integration of core disease-syndrome indicator clusters, and establishment of a formula-based syndrome verification system was proposed, providing a reference for the standardized construction and scientific evaluation of the SDDP-UC model.
3.Analysis of thermal environment and students thermal comfort in primary and secondary school classrooms in winter
Chinese Journal of School Health 2026;47(2):168-172
Objective:
To evaluate the current situation of thermal environment in primary and secondary school classrooms during winter, and to analyze students thermal comfort needs, so as to provide a basis for improving classroom thermal environment.
Methods:
From December 16 to 26, 2024, a stratified cluster random sampling method was used to select 90 classrooms from 15 primary and secondary schools in centralized/air conditioned heating areas(Liaoning Province, Tianjin City, Shanghai City) and naturally ventilated areas(Anhui Province and Jiangxi Province)for on site environmental measurement. A questionnaire survey was conducted among 743 students. The differences between groups using the χ 2 test were compared. Based on actual measurement data, a predicted mean vote prepared percentage of dissatisfied (PMV-PPD) model for centralized/air conditioned classrooms and an adaptive model for naturally ventilated classrooms were established, and the thermal neutral temperature and comfort interval were calculated.
Results:
The average outdoor temperature during on site measurement was 4.00(0.20,7.00)℃. In classrooms with centralized or air conditioned heating systems, the measured average temperature was (19.33±2.59)℃, with a thermal comfort range of 20.35-25.35 ℃ and a thermal neutral temperature of 22.85 ℃. And 13.92% of students reported feeling cold, while 80.80% felt comfortable. In classrooms with natural ventilation, the measured average temperature was (12.26±1.83)℃, with a thermal neutral temperature of 19.67 ℃ and a thermal comfort range of 16.17-23.17 ℃. About 48.33% of students reported feeling cold, and 49.81 % felt comfortable. The results of univariate analysis showed that there were statistically significant differences in shoe thickness, temperature sensation, relative humidity sensation and wind speed sensation between centralized/air conditioned heating areas ( χ 2= 7.01 , 31.47, 13.57, 13.80,all P <0.05). There were also statistically significant differences in school stage for primary and secondary school students, body mass index, classroom location for seat, temperature sensation, relative humidity sensation and wind speed sensation between naturally ventilated areas ( χ 2=42.13, 11.13, 11.04, 60.39, 29.27, 38.46,all P <0.05).
Conclusions
There are differences in thermal environment and students subjective thermal comfort in primary and secondary schools under different ventilation modes in winter. The temperature standards for heated classrooms should be revised, and differentiated environmental regulation strategies should be adopted based on different ventilation methods to improve students health and comfort levels.

ObjectiveTo establish the mouse models of psoriasis-like lesions induced by continuous cold exposure or intermittent cold exposure combined with imiquimod (IMQ), and to evaluate the interventional effects of Kaixuan Jiedu core prescription (KXJD) on the two models. MethodsMale C57BL/6J mice were selected and classified into two experimental batches. The first batch of 36 mice was randomized into a room temperature group, a continuous cold exposure (10 ℃/24 h) group, and an intermittent cold exposure (10 ℃/6 h) group. Each group was further divided into a normal subgroup and a model subgroup (topical application of IMQ to induce skin lesions), with 6 mice in each subgroup, for modeling and evaluation. The second batch of 54 mice, with 6 in each group, were subjected to the same temperature grouping with an additional KXJD (30.42 g·kg-1, continuous gavage for 5 days) group. Comprehensive evaluation of model characteristics and KXJD efficacy was conducted through Psoriasis Area and Severity Index (PASI) scoring, skin temperature measurement by infrared thermography, histopathological observation by hematoxylin-eosin (HE) staining, detection of vascular endothelial growth factor (VEGF) and platelet endothelial cell adhesion molecule 1 (CD31) by immunohistochemistry, detection of Claudin-1 and Occludin by immunofluorescence assay, determination of serum levels of tumor necrosis factor-α (TNF-α) and interleukin (IL)-10 by enzyme-linked immunosorbent assay (ELISA), and quantification of mRNA levels of IL-17A, IL-23, IL-6, and chemokine ligand 20 (CCL20) in skin lesions by quantitative Real-time polymerase chain reaction (Real-time PCR). ResultsModel mice in all temperature groups exhibited typical psoriasis-like skin lesions. Compared with the normal groups, the model groups showed increased PASI scores, decreased skin temperatures (P<0.05), obvious epidermal thickening, parakeratosis, and dermal inflammatory cell infiltration, as well as elevated mRNA levels of IL-17A, IL-23, IL-6, and CCL20 (P<0.05). Cold exposure further aggravated psoriasis. The total PASI score of the intermittent cold exposure model group was higher than that of the room temperature model group (P<0.05). The serum IL-10 did not show a compensatory elevation, and the blood vessels presented a characteristic of elevated CD31 expression (P<0.05) without a synchronous increase in VEGF. The continuous cold exposure model group exhibited more significant dermal capillary tortuosity and dilation, with the highest mRNA levels of IL-17A, IL-23, IL-6, and CCL20 among all groups. Compared with the respective model groups, KXJD intervention alleviated skin lesions, reduced epidermal thickness and inflammatory cell infiltration, and increased skin temperature, with the temperature increase being particularly significant in the intermittent cold exposure+KXJD group (P<0.05). Furthermore, KXJD down-regulated the expression of VEGF and CD31, restored the expression of Claudin-1 and Occludin, decreased the mRNA levels of IL-17A and IL-23 (P<0.05), and reduced the serum TNF-α level. ConclusionThis study successfully established compound psoriasis-like mouse models induced by cold exposure combined with IMQ. It confirms that cold aggravates the severity of psoriasis by exacerbating the closure of Xuanfu (sweat pores), microcirculation disorders, and immune imbalance. Moreover, different cold exposure patterns have distinct mechanism differences. Continuous cold exposure focuses on enhancing the inflammatory response via the IL-23/IL-17 axis and angiogenesis, simulating chronic aggravation under a long-term cold environment. Intermittent cold exposure tends to impair immune regulation and induce microvascular endothelial stress, corresponding to acute exacerbations caused by sudden temperature drops. KXJD can effectively alleviate psoriasis-like skin lesions under cold conditions by unblocking Xuanfu, regulating vasomotor function, and correcting abnormal immune-inflammatory responses.

ObjectiveTo investigate the ameliorative effect and potential mechanisms of Kaixuan Jiedu core prescription (KXJD) on imiquimod (IMQ)-combined restraint stress-induced psoriasis-like skin lesions in mice. MethodsA total of 36 C57BL/6J mice were randomly assigned to six groups (n=6 per group): Normal group, restraint stress blank group, model group, restraint stress model group, KXJD group (30.42 g·kg-1), and methotrexate group (0.001 g·kg-1). Psoriasis-like lesions were induced by topical IMQ application combined with restraint stress. After 5 days of intervention, the severity of skin lesions was evaluated using the Psoriasis Area and Severity Index (PASI). The following parameters were assessed: Hypothalamic-pituitary-adrenal (HPA) axis-related hormones, including adrenocorticotropic hormone (ACTH), corticotropin-releasing hormone (CRH), and corticosterone (CORT), central neuronal markers, including neuronal nuclei antigen (NeuN), microtubule-associated protein 2 (MAP2), and beta Ⅲ-tubulin (β-Ⅲ tubulin), microglial activation markers, including ionized calcium-binding adapter molecule-1 (Iba-1) and cluster of differentiation 11b (CD11b), and skin nerve fiber markers, including protein gene product 9.5 (PGP9.5) and serotonin (5-HT), as well as the expression levels of inflammatory cytokines tumor necrosis factor-alpha (TNF-α), interleukin-6 (IL-6), and interleukin-1 beta (IL-1β). ResultsCompared with the normal group, both the model group and the restraint stress model group exhibited more severe psoriasis-like skin lesions, with significantly elevated serum levels of ACTH, CRH, and CORT (P<0.01). The expression of glucocorticoid receptor (GR) and corticotropin-releasing hormone receptor 1 (CRHR1) in the skin was significantly downregulated. The expression of NeuN, MAP2, β-Ⅲ tubulin, Iba-1, and CD11b in the hypothalamus and hippocampus was significantly upregulated, suggesting abnormal changes in neuronal number or structure and aberrant activation of glial cells (P<0.05, P<0.01). In addition, the expression of PGP9.5 and 5-HT in the skin, as well as the mRNA levels of TNF-α, IL-6, and IL-1β were all markedly elevated (P<0.05, P<0.01). Compared with the model group, the restraint stress model group showed further exacerbation in most parameters. Compared with the restraint stress model group, the KXJD group exhibited significantly reduced PASI scores (P<0.01), significantly decreased serum CRH, ACTH, and CORT levels (P<0.05, P<0.01), upregulated skin GR and CRHR1 expression, inhibited abnormal neuronal changes and microglial activation in the hypothalamus and hippocampus, attenuated skin nerve fiber hyperplasia and amine signaling molecule release, and significantly reduced pro-inflammatory cytokine mRNA expression (P<0.05, P<0.01). ConclusionKaixuan Jiedu Core prescription can effectively ameliorate IMQ combined with restraint stress-induced psoriasis-like skin lesions in mice by modulating HPA axis function, inhibiting central neuropathological changes, and attenuating skin nerve fiber hyperplasia, amine signaling molecule (5‑HT) release and local inflammatory responses.

Objective:To investigate the efficacy of drug-eluting beads transcatheter arterial chemoembolization (DEB-TACE)combined with Apatinib Mesylate and Camrelizumab in the treatment of older patients with hepatocellular carcinoma and portal vein tumor thrombus(PVTT).Methods:A total of 80 older patients with hepatocellular carcinoma and PVTT, admitted to the Second People's Hospital of Pinghu City from January 2020 to December 2023, were selected as the study subjects.Among them, 40 patients receiving DEB-TACE combined with Apatinib Mesylate and Camrelizumab constituted the experimental group, while the other 40 patients receiving Apatinib Mesylate combined with Camrelizumab formed the control group. The response evaluation criteria in solid tumors(RECIST)were employed to assess the clinical efficacy of both groups.Overall survival(OS)and progression-free survival(PFS)were compared, and univariate and multivariate Cox regression analyses were used to analyze the influencing factors of OS in patients. Results:The objective response rate(ORR)and disease control rate(DCR)of the experimental group were significantly higher than those of the control group[60.0%(24/40) vs.32.5%(13/40), 80.0%(32/40) vs.50.0%(20/40), χ2=6.085, 0.013, P=0.013, 0.004]. The median OS and PFS of the experimental group were significantly greater than those of the control group(18.3 months vs.12.2 months, Log-rank χ2=6.470, P=0.011; 8.2 months vs.5.7 months, Log-rank χ2=11.106, P=0.001). Multivariate Cox proportional regression analysis indicated that ALBI grade( HR=1.356, 95% CI: 1.159-1.588, P=0.006), extrhepatic metastasis( HR=1.133, 95% CI: 1.015-1.265, P=0.009), and DEB-TACE( HR=1.921, 95% CI: 1.293-2.854, P=0.001)were independent prognostic factors in older patients with hepatocellular carcinoma and PVTT. Conclusions:The combination of DEB-TACE, Apatinib Mesylate, and Camrelizumab in the treatment of older patients with hepatocellular carcinoma and PVTT can significantly enhance clinical efficacy and extend patient survival.

BACKGROUND:In most species,a bone defect that is longer than 1.5 or 2 times its diameter can be considered a critical bone defect, and when the bone defect volume reaches the critical value,it cannot heal on its own.Currently, there is no uniform standard for the size of critical-sized defects in the osteoporotic femoral condyle of rabbits.OBJECTIVE:To establish a rabbit model with different sizes of bone defects in the osteoporotic femoral condyle and to determine the critical-sized defects of osteoporotic femoral condyle in rabbits.METHODS:Thirty-six 3-month-old female New Zealand white rabbits were randomly divided into ovariectomy group(n=30)and sham operation group(n=6).Rabbits in the ovariectomy group underwent bilateral ovariectomy to establish an osteoporosis model, and then femoral condyle bone defect models of different diameters(diameters were 4,5,6,and 7 mm,and depths were 8 mm)were further established;rabbits in the sham operation group did not undergo ovariectomy. At 8 and 12 weeks after modeling,3 rats were randomly selected from each group for CT scanning and three-dimensional reconstruction to evaluate the healing of bone defects.Afterwards, samples were taken for gross observation and hematoxylin-eosin staining to observe the growth of new bone in the femoral condyle bone defect area.RESULTS AND CONCLUSION:(1)All rabbits survived and moved well after modeling of osteoporosis and femoral condyle bone defect.(2)At 12 weeks after osteoporosis modeling,dual-energy X-ray absorptiometry results showed that the bone mineral density of lumbar vertebrae in ovariectomy group was significantly lower than that in sham operation group(P<0.05).Hematoxylin-eosin staining showed that the bone trabeculae in the ovariectomy group became thinner and sparse. The proportion of bone tissue area in the ovariectomy group was significantly lower than that in the sham operation group(P=0.00).Micro-CT results showed that the bone tissue parameters of the femoral condyle in the ovariectomy group were significantly different from those in the sham operation group(P<0.05),and the ovariectomy group showed obvious characteristics of osteoporosis.(3)CT showed that the bone defect in the 4 mm and 5 mm diameter groups was basically completely repaired at 12 weeks after surgery. There was more new bone tissue in the 6 mm diameter group, but the central part of the bone defect was not completely repaired.A small amount of new bone tissue grew in the 7 mm diameter group, and the bone defect was obvious.(4)Gross observation at 12 weeks after surgery showed that the femoral condyle bone defect in the 4 mm and 5 mm diameter groups was completely repaired.Obvious depression was seen in the bone defect area of the 6 mm and 7 mm diameter groups, and the bone defect was not completely repaired.(5)Histological observation at 12 weeks after surgery showed that the bone defect area of the 4 mm and 5 mm diameter groups was completely filled with new bone, and the trabecular structure was irregular;while there were new trabeculae in the periphery of the 6 mm and 7 mm diameter groups, and the bone defect in the central area was still obvious.(6)The results showed that during the 12-week experimental observation period of osteoporotic femoral condyle defects in rabbits, under the condition of the same defect depth of 8 mm,femoral condyle defects with a diameter ≥ 6 mm could not heal on their own, while femoral condyle defects with a diameter<6 mm were completely repaired.A diameter of 6 mm and a depth of 8 mm can be used as the critical bone defect value of osteoporotic femoral condyle in rabbits.

Neurodegenerative diseases are a class of nervous system disorders characterized by delayed onset and selective neuronal dysfunction, which are closely associated with aging.Given the challenges associated with early detection and the irreversible nature of the condition, the pursuit of specific biomarkers has emerged as a critical component of disease prevention strategies.Optical coherence tomography angiography (OCTA) generates images which exhibit near-histological resolution without the requirement for contrast agent injection, demonstrating superior sensitivity compared to fundus photography. OCTA has the ability to identify and evaluate neurodegenerative diseases via the quantitative analysis of retinal parameters. The OCTA of the retina and choroid has been verified as a highly sensitive instrument for detecting microvascular abnormalities in both ocular and systemic retinal disorders. This review focuses on the application of OCTA in various neurodegenerative diseases, such as Alzheimer′s disease, Parkinson disease, Huntington′s disease, amyotrophic lateral sclerosis and Wolfram syndrome, with the aim of providing a reference for the early diagnosis and prevention of neurodegenerative diseases.

In recent decades, the incidence of human papillomavirus(HPV)-associated oropharyngeal squamous cell carcinoma(OPSCC)has shown a marked increase.Significant changes have also occurred in the OPSCC diagnosis and treatment paradigm.Deter-mining HPV status prior to treatment is now essential, and radiotherapy/chemotherapy,immunotherapy,and minimally invasive surgical techniques have progressively emerged as key modalities for managing OPSCC.However, alongside these paradigm shifts,a comprehen-sive technical consensus guiding the entire diagnostic and therapeutic process for OPSCC patients is currently lacking.Given China's large population base and the rising incidence of OPSCC,an expert panel convened to develop a clinical technical consensus on OPSCC diagno-sis and management tailored to China's specific context. This consensus aims to further enhance and standardize understanding of OPSCC management techniques among relevant healthcare professionals.
