Animal model research on spleen and stomach diseases in traditional Chinese medicine (TCM) is of great significance for elucidating the nature of diseases and syndromes and for revealing the mechanisms of action of Chinese herbal medicinals. At present, studies on classical TCM syndrome models of spleen and stomach diseases mainly focus on spleen deficiency syndrome, liver constraint syndrome, and damp-heat syndrome. Model construction is mostly based on the etiological and pathophysiological characteristics of syndrome, and model evaluation primarily involves macroscopic manifestations and physicochemical indicators. This paper summarizes the current research status of animal models integrating disease and syndrome for seven common spleen and stomach diseases, including chronic gastritis and gastric precancerous lesions, gastroesophageal reflux disease, functional dyspepsia, inflammatory bowel disease, irritable bowel syndrome, functional constipation, and functional diarrhea. The modeling methods and characteristics of disease-syndrome combined animal models for each disease are analyzed. It is proposed that future research on disease-syndrome integration in spleen and stomach diseases should move toward syste-matic, precise, and integrative development, and that interdisciplinary and cross-disciplinary research approaches should be adopted to enhance the predictive value and application efficiency of disease-syndrome combined animal models.

Aligning with the ulcerative colitis (UC) dynamic characteristics of latent pathogens, that is "remaining latent and concealed, and manifesting when triggered", and following the latent pathogen theory, it is proposed that the core pathogenesis of UC recurrence is latent damp-heat pathogen concealed internally, progressing from qi to blood level over time, and leading to qi-blood disharmony and intestinal collateral damage. Accordingly, stage-based treatment is recommended. During the active stage, it is suggested to clear damp-heat, move qi, and regulate blood using modified Gegen Qinlian Decoction(葛根芩连汤). During the remission stage, the treatment should focus on reinforcing healthy qi and venting pathogen, regulate and tonify the spleen and kidney with a self-made Jianpi Qingchang Mixture(健脾清肠合剂). These approaches may provide a reference for the clinical treatment of UC.

As a new type of model organism, zebrafish is gradually gaining prominence in the field of scientific research. The unique biological characteristics and advantages of zebrafish make them play an increasingly important role in the quality evaluation of traditional Chinese medicine. Compared with other common experimental animals, zebrafish have a fast reproductive and growth speed and high embryo transparency, making them an ideal model for evaluating the quality of traditional Chinese medicine. This provides a new perspective and method for research on traditional Chinese medicine. With the growing global interest in traditional Chinese medicine, it has become crucial to find scientific and accurate methods to evaluate the quality and effectiveness of traditional Chinese medicine. The introduction of the zebrafish model has brought new breakthroughs in the quality evaluation of traditional Chinese medicine. To further promote the application of zebrafish in evaluating the quality of traditional Chinese medicine, this article systematically searched and sorted out the previous studies related to the application of zebrafish for this purpose since 2023. The commonly used disease models and indicators of zebrafish in evaluating the effectiveness of traditional Chinese medicine, as well as the mechanism of zebrafish in exploring the active ingredients of traditional Chinese medicine, were primarily reviewed. The application of zebrafish in evaluating the safety of traditional Chinese medicine and the typical examples in ensuring the quality of traditional Chinese medicine were demonstrated. The limitations encountered by zebrafish models in evaluating the quality of traditional Chinese medicine were highlighted. The resolution of these problems will help further improve the accuracy and reliability of zebrafish in evaluating the quality of traditional Chinese medicine. The article discussed the evaluation of effectiveness, safety, and quality control of zebrafish applied in traditional Chinese medicine, so as to provide a reference for establishing standards for traditional Chinese medicine and promoting its modernization in the future.

BACKGROUND:Inflammation affects the osteogenic differentiation of periodontal ligament stem cells, and the osteogenic ability and autophagy level of periodontal ligament stem cells are closely related. However, there are no relevant reports on whether inflammation affects the osteogenic ability and autophagy level of periodontal ligament stem cells at different stages of osteogenic differentiation. OBJECTIVE:To explore alkaline phosphatase expression and autophagy periodontal ligament stem cells levels in periodontitis and normal conditions. METHODS:Periodontal ligament stem cells from normal and periodontitis patients were isolated and cultured, and underwent Vimentin,pan-CK,and Stro-1 fluorescence staining. At 3,7,and 14 days of osteogenic differentiation, western blot assay was used to detect the protein expression levels of alkaline phosphatase,LC3B,Beclin1,and ATG5 in normal and inflammatory periodontal ligament stem cells. The mRNA expression levels of alkaline phosphatase, bone sialoprotein,osteocalcin,Runx2,LC3B,Beclin1,and ATG5 were detected by real-time PCR. RESULTS AND CONCLUSION:(1)Stro-1 was positive,Vimentin was positive, and pan CK was negative in periodontal ligament stem cells.(2)At 3,7,and 14 days after osteogenic differentiation, compared with normal periodontal ligament stem cells, the mineralization nodules formed by periodontal ligament stem cells from inflammatory sources were significantly reduced(P<0.01);the expression of alkaline phosphatase protein and mRNA was significantly lower(P<0.05);the mRNA expression levels of bone sialoprotein, osteocalcin, and Runx2 were significantly decreased(P<0.05).(3)At 7 and 14 days after osteogenic differentiation, compared with normal periodontal ligament stem cells, the expression levels of ATG5,LC3B,and Beclin1 proteins and mRNA of periodontal ligament stem cells were downregulated(P<0.05).These findings suggest that inflammation reduces the activity of periodontal ligament stem cells in mineralizing nodule formation and the expression of alkaline phosphatase and weakens the autophagy potential of periodontal ligament stem cells at 7 and 14 days after osteogenic differentiation.

For patients with moderate-to-severe functional mitral regurgitation (FMR) who continue to experience heart failure symptoms despite optimized medical and device therapy, transcatheter mitral valve edge-to-edge repair (TEER) is increasingly becoming a reliable treatment option. With the continuous research and development and improvement of TEER-related devices, there are currently dozens of domestically developed TEER devices undergoing clinical trials in China. In this study, we report the first case of a patient with severe FMR treated with the X-Clip® TEER system. The patient, a 60-year-old male, suffered FMR attributed to dilated cardiomyopathy. Preoperative transthoracic echocardiography showed severe mitral regurgitation (4+). He underwent percutaneous repair using the X-Clip® system, and immediate postoperative ultrasound showed mild mitral regurgitation. At the 1-month follow-up, the patient’s symptoms and New York Heart Association (NYHA) functional class improved, and a follow-up transthoracic echocardiogram showed mild mitral regurgitation (1+).

COMPERA 2.0 risk stratification has been demonstrated to be useful in patients with precapillary pulmonary hypertension (PH). However, its suitability for patients at risk for post-capillary PH or PH associated with left heart disease (PH-LHD) is unclear. To investigate the use of COMPERA 2.0 in patients with severe aortic stenosis (SAS) undergoing transcatheter aortic valve replacement (TAVR), who are at risk for post-capillary PH, a total of 327 eligible SAS patients undergoing TAVR at our institution between September 2015 and November 2020 were included in the study. Patients were classified into four strata before and after TAVR using the COMPERA 2.0 risk score. The primary endpoint was all-cause mortality. Survival analysis was performed using Kaplan-Meier curves, log-rank test, and Cox proportional hazards regression model. The study cohort had a median (interquartile range) age of 76 (70‒80) years and a pulmonary arterial systolic pressure of 33 (27‒43) mmHg (1 mmHg=0.133 kPa) before TAVR. The overall mortality was 11.9% during 26 (15‒47) months of follow-up. Before TAVR, cumulative mortality was higher with an increase in the risk stratum level (log-rank, both P<0.001); each increase in the risk stratum level resulted in an increased risk of death (hazard ratio (HR) 2.53, 95% confidential interval (CI) 1.54‒4.18, P<0.001), which was independent of age, sex, estimated glomerular filtration rate (eGFR), hemoglobin, albumin, and valve type (HR 1.76, 95% CI 1.01‒3.07, P=0.047). Similar results were observed at 30 d after TAVR. COMPERA 2.0 can serve as a useful tool for risk stratification in patients with SAS undergoing TAVR, indicating its potential application in the management of PH-LHD. Further validation is needed in patients with confirmed post-capillary PH by right heart catheterization.

Objective:To explore the clinical application value of magnetic resonance imaging (MRI)-guided wire localization to the non-palpable breast lesions (NPBL) identified on MRI only. Methods:A retrospective case series study was conducted. A total of 171 patients with NPBL identified on MRI only who underwent MRI-guided wire localization from April 2017 to May 2024 in Shaanxi Provincial Cancer Hospital were collected. All patients had breast MRI Breast Imaging Report and Data System (BI-RADS) 4a and above lesions, and underwent MRI-guided wire localization within the same menstrual cycle within 2 days to 2 months after diagnostic MRI examination. Based on postoperative pathological results, the MRI characteristics of benign and malignant lesions were compared, and the clinical application value of MRI-guided wire localization was evaluated. Results:There were 179 lesions in 171 patients, including 54 malignant lesions (30.17%) and 125 benign lesions (69.83%). There was no statistically significant difference in the enhancement morphology between pathological benign and malignant NPBL lesions ( χ2 = 0.04, P = 0.982), while there were statistically significant differences in breast background parenchymal enhancement, lesion time-signal intensity curve and BI-RADS classification ( χ2 values were 32.03, 20.72 and 37.60, respectively, all P < 0.05). Conclusions:For NPBL that is identified on MRI only and evaluated as BI-RDADS 4a or above, MRI-guided wire localization can improve the accuracy of diagnosis and treatment of intraductal carcinoma, early invasive cancer and high-risk lesions.

Acta Medicinae Universitatis Scientiae et Technologiae Huazhong 2025;54(4):535-540
Objective The high global prevalence of adenovirus serotype 5(Ad5)infection,particularly in developing coun-tries,has led to widespread pre-existing neutralizing antibodies(NAbs)against this serotype. These NAbs may substantially im-pair the efficacy of Ad5-based therapeutics, necessitating the development of novel strategies to circumvent neutralizing antibody interference. Methods A novel Ad5-based vector was engineered via homologous recombination, wherein the hexon hypervari-able region(HVR)1,2,and 5 of Ad5 were strategically replaced with their counterparts from the less prevalent adenovirus sero-type 35(Ad35).This chimeric design aimed to reduce vector neutralization by Ad5-specific NAbs while preserving infectivi-ty.Results The hexon HVR-modified Ad5 vector exhibited reduced neutralization by pre-existing Ad5 antibodies while retai-ning efficient infectivity in tumor cells. In animal models, thesemodified vectors demonstrated excellent oncolyticability and still maintained strong tumor-killing activity in nude mice that received passive immunization with Ad5 neutralizing antibody ser-um.Conclusion The hexon HVR-modified Ad5 vectors demonstrate effective tumor infection rates and oncolytic efficacy in the presence of pre-existing Ad5 NAbs,suggesting promising clinical potential.
