A novel fumigation moxibustion device has been designed to enable adjustable and controllable moxa smoke temperature, maintaining a relatively stable fumigation temperature while improving the utilization efficiency of moxa smoke. The device consists of five main components: a temperature control chamber, fumigation outlet, temperature measurement module, moxa smoke filtration chamber, and elastic band. It is compact, refined, and easy to operate. The device allows users to set the desired fumigation temperature according to therapeutic needs and simultaneously filters and eliminates residual moxa smoke after treatment. This design addresses the challenges of traditional fumigation moxibustion therapy, including unstable moxa smoke temperature, difficulty in regulation, low utilization efficiency, and high dependence on manual operation. It contributes to the promotion and application of fumigation moxibustion therapy and supports the establishment of a standardized moxibustion system.

Objective:To compare the efficacy of robot system-assisted versus freehand screw revision for ankylosing spondylitis (AS) with lower cervical fractures. Methods:A multicenter retrospective cohort study was conducted to analyze the clinical data of 57 patients with AS combined with lower cervical fractures admitted to Honghui Hospital Affiliated to Xi'an Jiaotong University School of Medicine, Henan Provincial People's Hospital, Zhengzhou Orthopedic Hospital, and Qilu Hospital of Shandong University, including 46 males and 11 females, aged 38-77 years [(65.4±9.5)years]. Injury segments involved C 3 in 7 patients, C 4 in 13, C 5 in 25, C 6 in 10, and C 7 in 2. All the patients underwent revision surgery, among whom, 22 patients were treated with robot system-assisted cervical pedicle screw placement (robot nailing group, with 190 screws), and 35 with freehand cervical pedicle screw placement (freehand nailing group, with 300 screws). The operative duration, intraoperative bleeding volume, frequency of intraoperative fluoroscopy, incision length, and length of hospital stay of the two groups were compared; the time of single nscrew insertion, the number of single nail revisions, the distance between screws and the anterior cortex, the accuracy of screw placement of grade 0 and grade 0+1 were recorded in the two groups. The visual analogue scale (VAS), Japanese Orthopedic Society (JOA) score, neck dysfunction index (NDI), American Spine Injury Association (ASIA) classification before operation, at 3 days, 3 months after operation and at the last follow-up were compared between the two groups. The complication rate was also noted. Results:All the patients were followed up for 12-16 months [(14.3±2.1)months]. The operative duration, intraoperative bleeding volume, and frequency of intraoperative fluoroscopy were (186.4±12.9)minutes, (486.1±68.6)ml, and (3.4±1.3)times in the robot nailing group, which were shorter or less than (206.7±14.4)minutes, (660.3±45.2)ml, and (13.5±3.6)times in the freehand nailing group ( P<0.01). The incision length was (9.4±2.4)cm in the robot nailing group, longer than (5.6±1.2)cm in the freehand nailing group ( P<0.01), and the length of hospital stay was (3.7±0.4)days, shorter than (4.4±1.4)days in the freehand nailing group ( P<0.01). The length of single nail insertion, the number of single nail revision, and the distance between the screws and the front cortex were (6.5±0.4)minutes, (1.1±0.1)times, and (3.5±1.3)mm in the robot nailing group, which were shorter or less than (11.6±0.2)minutes, (1.5±0.2)times, and (12.4±4.7)mm in the freehand nailing group ( P<0.01). The accuracy of the screw placement in the robot nailing group was 90.0% (171/190) and 95.8% (182/190) with level 0 and 0+1 screws, better than 80.0% (240/300) and 89.0% (267/300) in the freehand nailing group ( P<0.05). There was no significant difference in VAS, JOA score, NDI, or ASIA grading between the two groups before operation ( P>0.05). The VAS, JOA, and NDI scores at 3 days after operation were (3.1±0.6)points, (12.1±1.2)points, and (15.6±2.9)points, respectively in the robot nailing group, which were better than (5.0±1.4)points, (11.3±1.1)points and (22.5±3.7)points, respectively in the freehand nailing group ( P<0.05). No statistically significant difference was observed in the ASIA grade between the two groups at 3 days after operation ( P>0.05). There were no significant differences in VAS, JOA, NDI scores, or ASIA grading between the two groups at 3 months after operation and at the last follow-up ( P>0.05). Compared with those before operation, the VAS, JOA, NDI scores, and ASIA grading were significantly improved at 3 days, 3 months after operation and at the last follow-up in the two groups, which were further improved with the passage of time. Two patients in the robot nailing group had pneumonia, with a complication rate of 9% (2/22), while 2 patients in the freehand nailing group had dural sac rupture and cerebrospinal fluid leakage and 3 had lung infection after operation, with a complication rate of 14% (5/35) ( P<0.05). Conclusion:Compared with freehand nailing, the robot system-assisted nailing revision for AS with lower cervical fracture has more advantages in terms of the operative duration, length of hospital stay, intraoperative bleeding volume, frequency of intraoperative fluoroscopy nailing speed and accuracy, screw holding force, early pain relief, function restoration, and complication rate, despite longer surgical incision.

ObjectiveTo explore the mechanism by which Buyang Huanwutang regulates the glutathione peroxidase 4 (GPX4)-acyl-CoA synthetase long-chain family member 4 (ACSL4) axis to inhibit ferroptosis and promote neurological functional recovery after spinal cord injury (SCI). MethodsNinety rats were randomly divided into five groups: sham operation group, model group, low-dose Buyang Huanwutang group (12.5 g·kg-1), high-dose Buyang Huanwutang group (25 g·kg-1), and Buyang Huanwutang + inhibitor group (25 g·kg-1 + 5 g·kg-1 RSL3). The SCI model was established by using the allen method. Tissue was collected on the 7th and 28th days after operation. Motor function was assessed by using the Basso-Beattie-Bresnahan (BBB) scale. Hematoxylin-eosin (HE), Nissl, and Luxol fast blue (LFB) staining were performed to observe spinal cord histopathology. Transmission electron microscopy was used to examine mitochondrial ultrastructure. Immunofluorescence staining was used to detect the number of NeuN-positive cells and the fluorescence intensity of myelin basic protein (MBP), GPX4, and ACSL4. Real-time fluorescent quantitative polymerase chain reaction (Real-time PCR) was used to analyze the mRNA expression of GPX4 and ACSL4. Enzyme linked immunosorbent assay (ELISA) was performed to measure the levels of reactive oxygen species (ROS), malondialdehyde (MDA), glutathione (GSH), and superoxide dismutase (SOD). Colorimetric assays were used to determine the iron content in spinal cord tissue. ResultsCompared to the sham operation group, the model group exhibited significantly reduced BBB scores (P<0.01), severe pathological damage in spinal cord tissue, and marked mitochondrial ultrastructural disruption. In addition, the model group showed a decrease in the number of NeuN-positive cells (P<0.01), reduced fluorescence intensity of MBP and GPX4 (P<0.01), lower levels of GSH and SOD (P<0.01), and downregulated mRNA expression of GPX4 (P<0.01). Moreover, compared to the sham operation group, the model group had elevated levels of ROS, MDA, and tissue iron content (P<0.01), along with increased fluorescence intensity and mRNA expression of ACSL4 (P<0.01). Compared with the model group and Buyang Huanwutang + inhibitor group, the Buyang Huanwutang group showed significantly improved BBB scores (P<0.05, P<0.01) and exhibited less severe spinal cord tissue damage, reduced edema and inflammatory cell infiltration, increased neuronal survival, and more intact myelin structures. Additionally, mitochondrial ultrastructure was significantly improved in the Buyang Huanwutang group. Compared to the model group and Buyang Huanwutang + inhibitor group, the Buyang Huanwutang group significantly increased the number of NeuN-positive cells and the fluorescence intensity of MBP (P<0.05, P<0.01). Furthermore, Buyang Huanwutang significantly increased the fluorescence intensity and mRNA expression of GPX4 (P<0.01) and decreased the fluorescence intensity and mRNA expression of ACSL4 (P<0.01) compared to the model group and Buyang Huanwutang + inhibitor group. Finally, the Buyang Huanwutang group significantly decreased ROS, MDA, and tissue iron content (P<0.01) and significantly increased GSH and SOD levels (P<0.01) compared to the model group and Buyang Huanwutang + inhibitor group. ConclusionBuyang Huanwutang inhibits ferroptosis through the GPX4/ACSL4 axis, reduces secondary neuronal and myelin injury and oxidative stress, and ultimately promotes the recovery of neurological function.

Objective:To explore the correlation among sensory information processing, self-regulation abilities, and clinical symptoms in children with autism spectrum disorder (ASD).Methods:From December 1, 2023 to May 1, 2024, a total of 73 ASD children aged 3-7 years old meeting the inclusion criteria were recruited, and they were divided into mild group ( n=20), moderate group ( n=26), and severe group ( n=27) based on clinical symptoms. The sensory processing and self-regulation checklist (SPSRC) was used to assess sensory processing and self-regulation abilities. Clinical symptoms of ASD were assessed by the social responsiveness scale (SRS), the autism behavior checklist (ABC), and the childhood autism rating scale (CARS). Statistical analysis was conducted by SPSS 25.0 software, including Chi-square test, one-way ANOVA, and Pearson correlation analysis. Results:Abnormal rates of sensory processing across the six domains in ASD children were as follows: auditory (31.51%, 23/73), visual (32.88%, 24/73), tactile (32.88%, 24/73), gustatory/olfactory (32.88%, 24/73), vestibular (24.66%, 18/73), and proprioception (27.40%, 20/73).Abnormal rates of self-regulation abilities across the three domains were as follows: physiological state (38.36%, 28/73), emotional regulation (49.32%, 36/73), and adaptability (31.51%, 23/73).The severe group showed higher abnormal rates in SPSRC domains compared to the moderate and mild groups (all P<0.05). Total scores of each SPSRC domain were negatively correlated with total scores of SRS, ABC, and CARS ( r=-0.648--0.236, all P<0.05). Conclusion:The more severe the impairment in sensory processing and self-regulation abilities in ASD children, the more severe their clinical symptoms. Improving sensory processing and self-regulation abilities may contribute to the rehabilitation of children with ASD.

ObjectiveTo evaluate the clinical efficacy of different acupuncture-related therapies in treating postoperative pain in patients with osteoporotic vertebral compression fractures (OVCFs) after percutaneous kyphoplasty (PKP) or percutaneous vertebroplasty (PVP) using a network meta-analysis.MethodsA systematic search was conducted in PubMed, Cochrane Library, Embase, Web of Science, China National Knowledge Infrastructure, Wanfang Database, Chinese Scientific Journal Database, and Chinese Biomedical Literature Database (SinoMed) from their inception to January 15, 2025. Outcome measures included the Visual Analog Scale (VAS) score, Oswestry Disability Index (ODI) score, and overall efficacy rate. Literature screening, data extraction, and risk-of-bias assessment were independently performed by two researchers. Data analysis was conducted using Stata 17.0 software.ResultsA total of 35 randomized controlled trials involving 2860 patients were included. The data analysis revealed that, in terms of improving VAS and ODI scores, the top three effective therapies were Fu's subcutaneous needling, wrist-ankle acupuncture, and acupotomy. For the overall efficacy rates in pain treatment, the top three therapies were wrist-ankle acupuncture, warm acupuncture and moxibustion, and Fu's subcutaneous needling. Based on the combined results across the three outcome measures, Fu's subcutaneous needling was found to be the most effective in relieving pain and improving lumbar function.ConclusionFu's subcutaneous needling, wrist-ankle acupuncture, warm acupuncture and moxibustion, and acupotomy were all effective in treating postoperative pain post-PKP/PVP and improving lumbar function. However, further high-quality, large-sample studies are required to confirm these findings.

Objective:To investigate the association between triglyceride glucose index (TyG) and early vascular aging measured by brachial ankle pulse wave velocity (baPWV) in young and middle-aged population. Methods:It was a cross-sectional study. A total of 5 680 subjects aged 20 to 59 years who underwent health check-ups at the Health Management Center of General Hospital of Tianjin Medical University from January to December in 2020 were selected as the research subjects. All the research subjects completed the health risk assessment questionnaire, physical examination, laboratory test, and multi-functional vascular lesion detection. The TyG was calculated and the research subjects were divided into four groups with the quartiles of TyG (Q 1 to Q 4, with a cut-off value of 8.22, 8.60 and 9.01, respectively). The baPWV value was converted into a Z-score, and those with a Z-score above the 95th percentile were defined as having early vascular aging. The Spearman correlation method, multiple linear regression model, binary logistic regression model and the area under the receiver operating characteristic curve (AUC) were used to analyze the association between TyG and early vascular aging. Results:Among the 5 680 middle-aged and young people included in the analysis, there were 3 117 males and 2 563 females, with an age of 46 (39, 52) years, a TyG of 8.60 (8.22, 9.01), and a baPWV of 1 279.25 (1 147.50, 1 434.25) cm/s. The prevalence rate of early vascular aging was 5.02% (285/5 680). Taking group Q 1 as the reference, in the multiple linear regression model adjusted for multiple factors, group Q 4 was significantly associated with a 47.64 (95% CI: 28.18-67.11) cm/s increase in baPWV ( P for trend<0.001). In the multivariate adjusted binary logistic regression model, compared with that in the Q 1 group, the OR of early vascular aging occurrence in the Q 2, Q 3, and Q 4 groups was 1.52 (95% CI: 0.75-3.07), 1.78 (95% CI: 0.89-3.58), and 3.04 (95% CI: 1.47-6.31), respectively. Elevated TyG level was positively correlated with the occurrence of early vascular aging ( P for trend<0.001). The AUC of TyG in predicting early vascular aging was 0.732 (95% CI: 0.704-0.759), with the optimal cut-off value being 8.86. The AUC of TyG in predicting early vascular aging in males was lower than that in females [0.665 (95% CI: 0.628-0.702) vs 0.796 (95% CI: 0.748-0.843)] ( P<0.001). Conclusions:There is a correlation between TyG and early vascular aging measured by baPWV in the middle-aged and young population. When TyG≥8.86, clinical intervention measures should be taken in a timely manner.

Sigma-1 receptor (σ 1R) has become a focus point of drug discovery for central nervous system (CNS) diseases. A series of novel 1-phenylethan-1-one O-(2-aminoethyl) oxime derivatives were synthesized. In vitro biological evaluation led to the identification of 1a, 14a, 15d and 16d as the most high-affinity (K i < 4 nmol/L) and selective σ 1R agonists. Among these, 15d, the most metabolically stable derivative exhibited high selectivity for σ 1R in relation to σ 2R and 52 other human targets. In addition to low CYP450 inhibition and induction, 15d also exhibited high brain permeability and excellent oral bioavailability. Importantly, 15d demonstrated effective antipsychotic potency, particularly for alleviating negative symptoms and improving cognitive impairment in experimental animal models, both of which are major challenges for schizophrenia treatment. Moreover, 15d produced no significant extrapyramidal symptoms, exhibiting superior pharmacological profiles in relation to current antipsychotic drugs. Mechanistically, 15d inhibited GSK3β and enhanced prefrontal BDNF expression and excitatory synaptic transmission in pyramidal neurons. Collectively, these in vivo proof-of-concept findings provide substantial experimental evidence to demonstrate that modulating σ 1R represents a potential new therapeutic approach for schizophrenia. The novel chemical entity along with its favorable drug-like and pharmacological profile of 15d renders it a promising candidate for treating schizophrenia.
