1.Advances in the basic research on traditional Chinese medicine for prevention and treatment of hepatic fibrosis based on omics technology

Hepatic fibrosis is the common key pathological link of various chronic liver diseases and can progress to malignant diseases such as liver cirrhosis and hepatocellular carcinoma; however, there is still a lack of effective targeted therapeutic drugs at present. Traditional Chinese medicine (TCM) has a marked clinical effect in the prevention and treatment of hepatic fibrosis, yet its precise clinical application and global promotion are greatly limited by the complex components of compound prescriptions and unclear mechanism of action. In recent years, multimodal high-throughput omics technology has achieved rapid development, providing strong technical support for elaborating on the scientific connotation of TCM in the treatment of complex diseases due to its advantages of systematic profiling, big-data analytics, and precise target prediction. In particular, integrated transcriptomic, proteomic, and metabolomic strategies comprehensively elucidate key signaling networks, cellular phenotypic transitions, and extracellular matrix metabolic homeostasis modulated by TCM compounds and monomers and assist in the screening and assessment of effective component groups and novel biomarkers. This article systematically reviews the advances in basic research on TCM prevention and treatment of hepatic fibrosis based on multi-omics technologies in the past five years, summarizes the “drug-target-pathway-phenotype” regulatory network, and elaborates on the core mechanisms of TCM in regulating hepatic stellate cell activation and reversing hepatic fibrosis. Future studies should further delve into the interdisciplinary integration and dynamic analytical methodologies of multi-omics technologies, precisely identify the core regulatory target networks modulated by TCM, and systematically unravel the scientific connotation of compatibility rule in compound prescriptions, in order to provide a theoretical basis for developing efficient targeted drugs for hepatic fibrosis and individualized diagnosis and treatment strategies.
2.Development and preliminary application of the Treatment Burden Scale for patients with chronic heart failure

Objective To develop a Treatment Burden Scale for patients with chronic heart failure(CHF)and evaluate its reliability and validity,aiming to provide an effective tool for assessing treatment burden levels in this population.Methods According to Cumulative Complexity Model,the study was conducted in 2 phases.A preliminary item pool was established through literature review and semi-structured interviews,from August 2022 to March 2023,followed by expert consultation to finalize the initial scale.A convenience sample of CHF patients from 4 tertiary A hospitals in Hunan and Hubei provinces was conducted to refine scale items and assess psychometric properties,from June 2023 to April 2024.Results The final scale contained 26 items.Exploratory factor analysis revealed 6 domains,including family financial support burden,healthcare utilization burden,the challenges patients face in acquiring knowledge about proper medication use and water/sodium dietary management,the behavioral burden associated with tracking daily water/sodium intake and symptom patterns,psychological burden,and medication management burden,cumulatively explaining 68.661%of variance.Confirmatory factor analysis demonstrated satisfactory model fit;the x2/df was 2.076;the root mean square error of approximation was 0.070;the normed fit index was 0.912;the content validity score was 0.974.The total Cronbach's α coefficient of the scale was 0.903;the split-half reliability was 0.785;the test-retest reliability was 0.936.Conclusion The Treatment Burden Scale for CHF patients developed in this study has good reliability and validity psychometric properties,and it can be used to evaluate treatment burden levels and influencing factors in clinical practice.
3.One case report of jaw bone metastasis of bladder cancer

The most common site of bone metastasis in advanced bladder cancer is spine,and jaw metastasis is rare. This article reports a 77-year-old male patient who had a solitary metastatic lesion in the left jaw 1 year after radical cystectomy for bladder cancer. The pathological features of the metastasis were consistent with bladder urothelial carcinoma metastasis. The patient received a GC regimen combined with toripalimab immunotherapy for 4 cycles,followed by maintenance of immunotherapy and anti-bone metastasis protection therapy. No new metastasis was found during the 6-month follow-up.
4.Acteoside ameliorates hepatocyte ferroptosis and hepatic ischemia-reperfusion injury via targeting PCBP2.

Hepatic ischemia-reperfusion injury (HIRI) has been considered as an inevitable process of liver transplantation. Hepatocyte ferroptosis is a key factor in HIRI development, yet precise mechanism and potential therapies are still unclear. Here, we demonstrated a strong correlation between hepatocyte ferroptosis and the downregulation of poly(rC)-binding protein (PCBP2), which compromised the stability of antiporter system Xc- (consisted of SL3A2/SLC7A11). Besides, inhibiting PCBP2 contributed to facilitating cofactor p300 to enhance the transcriptional activity of HIF1α, leading to the expression and secretion of HMGB1. Then, released HMGB1 from ferroptotic hepatocytes worsened M1 macrophage recruitment and immune response during HIRI. Additionally, acteoside (ACT) was shown to assist PCBP2 in stabilizing the mRNA stability of Slc3a2 and Slc7a11, as well as enhance the binding affinity of PCBP2-system Xc-. Beyond that, ACT also supported PCBP2 to limit HMGB1-induced M1 macrophage recruitment through imposing restrictions on p300 and HIF1α. Furthermore, specific knockdown of PCBP2 in hepatocytes directly interposed the therapeutic efficacy of ACT on HIRI mice. In conclusion, ACT alleviated hepatocyte ferroptosis and HIRI via promoting PCBP2 to maintain the stability of system Xc- and limit HIF1α/p300-HMGB1 signaling. These findings highlight the therapeutic benefits of ACT in treating HIRI and offer insights into innovative therapeutic strategies.
5.Measurement of psychological stress in nursing staff based on BiLSTM+Attention analysis of EEG signals

As a non-invasive physiological indicator,electroencephalography signal provides an objective assessment of psychological stress levels among nursing staff in major public health emergencies,offering a scientific basis for targeted psychological interventions while overcoming the limitations of subjective bias inherent in traditional questionnaire-based methods.A psychological stress classification model based on bidirectional long short-term memory and attention mechanism is proposed to classify the psychological stress of clinical nurses more effectively by analyzing their electroencephalography signals.Experimental results show that the proposed model exhibits better classification performance than the traditional long short-term memory model on the DREAMER dataset,the Feeling Emotions dataset and the self-built dataset.This study provides a novel approach for assessing psychological stress,which is helpful to improve the pertinence and effectiveness of clinical nursing work.
6.Status and influencing factors of cognitive frailty in elderly patients with heart failure based on random forest algorithm

Objective:To investigate the status quo of cognitive frailty in elderly patients with heart failure and analyze its influencing factors, so as to provide evidence for healthcare professionals to formulate effective intervention strategies.Methods:A total of 330 elderly patients with heart failure admitted to the First Affiliated Hospital of the University of South China and the Second Affiliated Hospital of the University of South China from October 2023 to January 2024 were selected as the study objects by convenience sampling method. General data questionnaire, Frailty Phenotype, Montreal Cognitive Assessment, Clinical Dementia Rating Scale, Geriatric Depression Scale-15 and Short Form Mini Nutritional Assessment were used for a sectional investigation. Random forest algorithm was used to rank the importance of variables and binary logistic regression was combined to explore the influencing factors of elderly patients with heart failure.Results:According to the evaluation criteria of cognitive frailty, 330 elderly patients with heart failure were divided into cognitive frailty group (124 cases) and non-cognitive frailty group (206 cases). The incidence of cognitive frailty was 37.6% (124/330). Among which, the median age of the cognitive frailty group was 73 years old, with 63 males and 61 females. The median age of the non-cognitive frailty group was 71 years old, with 117 males and 89 females. The random forest results showed that the top 7 variables in importance ranking were weekly intellectual activity, frequency of physical exercise, age, educational levels, depression status, cardiac function grade and risk of malnutrition. Binary logistic regression analysis showed that weekly intellectual activity ( OR=0.076, 95% CI 0.027-0.216), requency of physical exercise ( OR=0.184, 95% CI 0.079-0.430), age ( OR=1.173, 95% CI 1.077-1.277), educational levels ( OR=0.283, 95% CI 0.143-0.559), depression status ( OR=4.440, 95% CI 1.451-13.585), cardiac function grade ( OR=3.030, 95% CI 1.673-5.489) and risk of malnutrition ( OR=3.833, 95% CI 1.530-9.602) were the main influencing factors (all P<0.05). Conclusions:The incidence of cognitive frailty in elderly patients with heart failure is high. Healthcare professionals ought to focus on the screening and assessing of cognitive frailty in elderly patients with heart failure, and formulate effective intervention strategies by considering the above influencing factors to mitigate the occurrence of cognitive frailty.
