1.Multicenter retrospective analysis of the efficacy of neoadjuvant combined with adjuvant therapy in intrahepatic cholangiocarcinoma

Background and Aims:Intrahepatic cholangiocarcinoma(ICC)is a highly malignant liver tumor,with an increasing incidence worldwide,particularly in Asia.Although radical surgical resection is currently the only potentially curative treatment,the high recurrence rate and low postoperative overall survival(OS)rate of ICC remain major clinical challenges.Adjuvant therapy(AT)and neoadjuvant therapy(NAT)are important strategies to reduce postoperative recurrence and prolong OS.Several studies have shown certain efficacy of these treatments.However,the specific efficacy and safety of combined NAT and AT in ICC treatment require further validation.This study was conducted to evaluate the value of combining NAT and AT in improving the therapeutic outcomes of ICC patients through a multicenter retrospective analysis,so as to provide scientific evidence for optimizing treatment strategies.Methods:The clinicopathologic data of 576 patients with ICC who underwent radical resection and were pathologically confirmed from 13 hospitals in China between December 2011 and December 2017 were retrospectively collected.Patients were grouped based on their treatment modality:NAT+AT group,AT group,and non-NAT/AT group.The three patient groups were matched pairwise in a 1∶1 ratio using propensity score matching(PSM)to balance baseline data.The Kaplan-Meier method was used to analyze OS and disease-free survival(DFS),and subgroup analyses were conducted according to the 8th edition of the AJCC TNM staging system.Results:A total of 395 ICC patients were included in the final analysis,with 42 patients(10.6%)in the NAT+AT group,62 patients(15.7%)in the AT group,and 291 patients(73.7%)in the non-NAT/AT group.Before PSM,significant differences were observed between groups in terms of CA19-9,liver function Child-Pugh classification,intraoperative blood loss,surgical margin,differentiation grade,vascular invasion,ECOG score,and lymph node dissection ratio(all P<0.05).After PSM,there were no significant differences in baseline characteristics between the groups(all P>0.05).After matching,the median OS and DFS in the NAT+AT group were significantly better than in the AT and non-NAT/AT groups(both P<0.05),while there were no significant differences in OS and DFS between the AT and non-NAT/AT groups(both P>0.05).Subgroup analysis showed that in TNM stage I patients,DFS in the NAT+AT group was significantly better than in the non-NAT/AT group(P<0.05),but OS was not significantly different(P>0.05).In TNM stage Ⅱ and Ⅲ patients,both OS and DFS in the NAT+AT and AT groups were significantly better than in the non-NAT/AT group(both P<0.05),and DFS in the NAT+AT group was significantly better than in the AT group in TNM stage Ⅲ patients(P<0.05).Conclusion:NAT combined with AT provides better survival benefits for patients with locally advanced ICC,but its benefit for early-stage ICC patients is limited.However,the retrospective design and sample size limitations of this study may affect the stability of the results,and future large-sample,multicenter,prospective studies are needed for further validation.
2.Clinical effect of shoulder arthroscopic rotator cuff repair combined with shoulder three needles for rotator cuff injury

Objective To investigate the clinical effect of shoulder arthroscopic rotator cuff repair combined with shoulder three needles in treatment of rotator cuff injury.Methods 86 patients with rotator cuff injury from June 2021 to May 2023 were divided into two groups using a random number table method.The research group received 43 cases of rotator cuff repair under arthroscopy combined with shoulder three needles rotator cuff repair,while the control group received 43 cases of rotator cuff repair under arthroscopy combined with placebo needling.Then compare the pain level,shoulder joint function,and shoulder joint range of motion between the two groups before and 6 months after surgery,and calculate the excellent and good shoulder joint function rates between the two groups at 6 months after surgery,and compare the changes in inflammatory factors between the two groups at before and 4 weeks after surgery.Results 6 months after surgery,the visual analogue scale(VAS)score of both groups was lower than that before surgery,and the VAS score in research group(including the moderate tearing subgroup and the major tearing subgroup)was lower than that of the control group(including the moderate tearing subgroup and the major tearing subgroup),while the American Shoulder and Elbow Surgeons(ASES)score and shoulder joint range of motion were higher than those before surgery,and the ASES score and shoulder joint range of motion in research group(including the moderate tearing subgroup and the major tearing subgroup)were higher than those of the control group(including the moderate tearing subgroup and the major tearing subgroup),the differences were statistically significant(P<0.05).6 months after surgery,the excellent and good rate of shoulder joint function in the research group was higher than that in the control group,the difference was statistically significant(P<0.05).At 4 weeks after surgery,the levels of tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),interleukin-6(IL-6),and interleukin-8(IL-8)in both groups were lower than those before surgery,and the research group(including the moderate tearing subgroup and the major tearing subgroup)were lower than those of control group(including the moderate tearing subgroup and the major tearing subgroup),the differences were statistically significant(P<0.05).Conclusion Arthroscopic rotator cuff repair combined with shoulder three needles can alleviate the pain level of patients with rotator cuff injury,improve shoulder joint function and range of motion,increase the excellent and good rate of shoulder joint function,and reduce inflammatory reaction.It is worthy of clinical application.

5.Efficacy of all-arthroscopic suture bridge fixation versus posteromedial approach open reduction and internal fixation with screws for avulsion fractures of the posterior cruciate ligament attachment in the knee joint

Objective To compare the clinical outcomes of all-arthroscopic suture bridge fixation and posteromedial approach open reduction and internal fixation with screws for avulsion fractures of the posterior cruciate ligament (PCL) attachment in the knee joint. Methods A retrospective analysis was conducted on the clinical data of 45 patients with avulsion fractures of the PCL attachment in the knee who were admitted to the Orthopedics and Traumatology Joint Ward of Zhangjiagang Traditional Chinese Medicine Hospital of Jiangsu Province between April 2021 and July 2023. The patients were divided into arthroscopic group (undergoing all-arthroscopic suture bridge fixation) and open internal fixation group (undergoing posteromedial approach open reduction and internal fixation with screws) based on varied surgical approach they received. The operative time and length of hospital stay for both groups were recorded, and the Visual Analogue Scale (VAS) scores before surgery, at 1 month and 6 months after surgery were assessed. The International Knee Documentation Committee (IKDC) subjective score, Lysholm knee function score, and Hospital for Special Surgery (HSS) knee function score were used to evaluate knee function before surgery and at 6 months after surgery. At 6 months after surgery, the range of motion of the affected knee joint was recorded, and the posterior drawer test was performed to assess the degree of PCL injury. Results None of 45 patients experienced vascular or nerve injury intraoperatively, and there were no postoperative wound infections. X-ray examinations conducted 3 days after surgery showed good reduction of the fractures. All patients were followed up for 6 months. 6.Study on the determination of compound levonorgestrel sustained release microspheres injection by HPLC

Objective:To establish a high-performance liquid chromatography (HPLC) for determining the content of levonorgestrel (LNG) and ethinylestradiol (EE) in compound levonorgestrel sustained-release microsphere injection.Methods:C18 column Inertsil ODS-3 (4.7 mm×250 mm, 5 μm) was used. The mobile phase was acetonitrile-water (50∶50, V/V). The flow rate was 1.0 mL/min. The column temperature was 35 ℃. The detection wavelength was 265 nm, and the injection amount was 20 μL. The content of microspheres was determined after validation of HPLC detection methodology.Results:The linear relationship was good in the range of 5.03-201.20 μg/mL LNG, 1.55-61.92 μg/mL EE ( r2=0.999 8). The specificity, precision, recovery, repeatability, stability, etc, met the requirements. The content of LNG in the compound LNG sustained-release microspheres injection was 16.26%, and the content of EE was 2.58%. Conclusion:This HPLC can determine the content of compound levonorgestrel sustained-release injection microspheres, and the method is simple, stable, and has good reproducibility, providing a reference for the determination of levonorgestrel and ethinylestradiol in compound sustained-release preparations.

Literature synthesis It is generally considered that Schmorl′s nodes of lumbar spine are cartilaginous nodes formed by the herniation of the intervertebral discs tissue into the vertebral body through the broken cartilaginous endplates. There are different opinions on the epidemiology and pathogenesis of Schmorl′s nodes of lumbar spine. Because most of them have no obvious clinical symptoms, they were once considered to be of no research value. However, the investigation showed that the further study on Schmorl′s nodes of lumbar spine in flying personnel is realistic. Schmorl′s nodes of lumbar spine may not only cause lumbago, but also are closely related to lumbar intervertebral discs degeneration. The formation of Schmorl′s nodes of lumbar spine in flying personnel will affect the viscoelasticity of lumbar intervertebral discs, and this will affect their anti-G and anti-vibration abilities, accelerate the degeneration of lumbar spine and intervertebral discs, and induce clinical symptoms or reduce flight endurance. Conclusions:Schmorl′s nodes of lumbar spine are closely related to lumbar disc degeneration and lumbar instability. Schmorl′s nodes are commonly seen in the lumbar spine and have an important effect on the overall stability of lumbar spine of flying personnel.

10.A study on the application of organ dose modulation technique to reduce breast radiation dose in chest CT imaging

Objective:To investigate the effect of organ dose modulation (ODM) technique on reducing the breast radiation dose in chest CT scanning.Methods:In the phantom test, the PBU-2 adult chest module was used. The clinical chest scan protocol was used and three sets of scans performed on the chest module: (1) ODM off group, ODM was not used; (2) ODM part group, ODM was applied only in the breast region; (3) ODM all group, ODM was applied in the whole scan scope. Other scan parameters were same for the three groups, with smart mA applied. The volume CT dose index (CTDI vol) was recorded for all three groups. A long rod ionization chamber was placed in a fixed position in front of the right breast area to measure the breast skin dose (D). The contrast noise ratio (CNR) and the figure of merit (FOM) were measured respectively. In clinical research, 72 female patients who underwent chest CT scanning in Beijing Tongren Hospital Capital Medical University from August to November 2018 were retrospectively recruited. According to the application of ODM, the patients were divided into ODM off group (without ODM, 36 cases) and ODM part group (ODM applied in the breast region, 36 cases). The CTDI vol and the dose length product (DLP) were recorded. CNR, noise of images were measured and calculated, respectively. The image quality was evaluated by subjective evaluation scores. The one way ANOVA analysis was used in comparing the difference of CNR among the 3 groups in module test. As for clinical cases, the independent samples t test was used to compare the difference in CTDI vol, DLP, CNR and the noise between two groups; and the rank-sum test was used for comparison in image quality subjective evaluation. Results:In module test, the radiation dose was highest in ODM off group, and lowest in ODM all group. The CTDI vol were (6.90±0.02), (6.26±0.02) and (5.99±0.02) mGy, and the D values were (9.17±1.01), (8.01±0.92) and (7.58±0.87) mGy for ODM off group, ODM part group and ODM all group respectively. The CNR values of images with soft tissue algorithm reconstruction were highest in ODM off group and lowest in ODM all group, while no statistically significant difference was displayed ( P>0.05). The CNR values of the images with lung algorithm reconstruction showed the same trend, with statistically significant difference among the three groups ( F=154.732, P=0.006). The FOM of the lung and soft tissue algorithm images was maximized when the ODM was partially applied. As for clinical cases, compared with ODM off group, the dose of ODM part group showed significantly decreased, with CTDI vol decreased by 16.12% ( t=2.604, P=0.011), and the DLP decreased by 16.85% ( t=3.293, P=0.002). No significant difference was found in CNR, noise and subjective score by two doctors between two groups ( P>0.05). Conclusion:The application of ODM in chest CT imaging can reduce the radiation dose of breast with simultaneously maintaining the image quality.
