1.Application of Insect and Vine Medicinal Pairs in the Treatment of Postoperative Recurrence and Metastasis of Bladder Cancer:from the Perspective of Blood Collaterals Theory

Based on the theory of blood collateral, postoperative recurrence and metastasis of bladder cancer are considered to arise primarily from the binding of stasis and toxin, which accumulate and hide within the blood collaterals. Accordingly, treatment should focus on clearing and resolving the deeply concealed stasis toxin retained in the blood collaterals. The paired use of insect and vine medicinals may exert synergistic effects by simultaneously searching out and eliminating pathogenic factors, guiding the action of herbs to the channels, and unblocking the collaterals. Drawing on clinical practice, the stasis-toxin pathogenesis of postoperative recurrence and metastasis of bladder cancer can be divided into four stages including stagnation and astringent of collateral qi, formation of fixed stasis nests, transformation of persistent stasis into toxin, and deficiency of healthy qi with lingering toxin. Accordingly, four herb pairs are proposed for each stage based on conventional treatment, which are Dilong (Pheretima)-Daxueteng (Caulis Sargentodoxae), Shuizhi (Hirudo)-Jixueteng (Caulis Spatholobi), Wugong (Scolopendra)-Luoshiteng (Caulis Trachelospermi), and Quanxie (Scorpio)-Qianjinteng (stephania). Their potential modern pharmacological mechanisms are further discussed.
2.Accuracy of modified implant template of assisted implantation in missing second molars

BACKGROUND:Computer-assisted implant surgery can improve implantation accuracy,but the use of implant template in the posterior tooth area is limited for patients with small opening and small interocclusal distance.Therefore,the digital guide has been improved. OBJECTIVE:To study the effect of modified implant template on the accuracy of assisted implantation in missing second molars. METHODS:From July 2020 to July 2023,40 patients who received digital guide plate implantation or free hand implantation to repair missing second molars were selected from First Affiliated Hospital of Guangzhou Medical University.According to the coin toss method,patients were randomly divided into a trial group(n=22;modified digital guide assisted implantation)and a control group(n=18;free hand implantation).The data of neck deviation,tip deviation,depth deviation,and angle deviation were compared between groups for preoperative and postoperative cone beam CT overlap analysis.One week after the operation,the patients'satisfaction with the operation was assessed by visual analog scale score. RESULTS AND CONCLUSION:(1)The trial group included 25 implants(12 in the upper jaw and 13 in the lower jaw);the control group included 23 implants(8 in the upper jaw and 15 in the lower jaw).The neck deviation,tip deviation,depth deviation,and angle deviation of the trial group were all smaller than those of the control group(P<0.05,P<0.001).There was no significant difference in accuracy between the maxillary and mandibular implant site in the trial group(P>0.05).(2)There was no significant difference in satisfaction with the operation between the two groups(P>0.05).(3)The results showed that improving the digital guide plate for assisted implantation for missing second molar can improve surgical accuracy and is suitable for patients with small opening and small interocclusal distance in the posterior tooth area.
3.Discovery and preliminary validation of five early gastric cancer biomarkers,including TAGLN2 and CTSD,based on serum proteomics

Background and purpose:Gastric cancer,as one of the most common malignant tumors,requires early diagnosis and treatment to improve patient prognosis.This study,based on serum quantitative proteomics research of early-stage gastric cancer patients and non-gastric cancer patients,aims to identify potential diagnostic biomarkers for early gastric cancer.Methods:Serum samples from primary gastric cancer patients and healthy control individuals were collected from Lanzhou University Second Hospital between June and December 2023,following inclusion and exclusion criteria.A protein spectral library was established using Data-Dependent Acquisition(DDA)mode,and each sample was analyzed using Data-Independent Acquisition(DIA)mode.The STRING database was used to analyze protein-protein interactions of upregulated proteins in gastric cancer serum.Kyoto Encyclopedia of Genes and Genomes(KEGG)and Gene Ontology(GO)were used to analyze the pathways and functional annotations of the corresponding genes.Gene expression levels in gastric cancer and non-gastric cancer tissues were analyzed using GEPIA 2,and overall survival of each gene in gastric cancer was analyzed using Kaplan-Meier Plotter.Differential gene expression in clinical gastric cancer and adjacent tissues was validated by quantitative reverse transcription polymerase chain reaction(qRT-PCR)This study was approved by the Ethics Committee of Lanzhou University Second Hospital(Ethical No.:2023A-459)and was exempt from the informed consent.Results:Finally,serum samples from 30 primary gastric cancer patients,29 healthy control individuals,along with the para-cancerous tissues from 8 patients were collected.A total of 666 intersecting proteins were identified through serum quantitative proteomics.Among them,16 proteins showed upregulated expression and 22 proteins showed downregulated expression in the gastric cancer group(P<0.05,|FC|≥1.5).STRING database analysis showed that 10 upregulated proteins were involved in interaction networks.KEGG and GO analysis indicated that these genes were closely related to the biological processes of cancer occurrence and development.GEPIA 2 and Kaplan-Meier Plotter analysis showed that 6 genes,B2M,TAGLN2,CTSD,HSP90AB1,SH3BGRL3,and CFL1,which were highly expressed in the gastric cancer group(P<0.05)and associated with poor prognosis.Clinical verification by qRT-PCR confirmed that TAGLN2,CTSD,SH3BGRL3,CFL1 and HSP90AB1 were highly expressed in gastric cancer tissues(P<0.05).Conclusion:TAGLN2,CTSD,SH3BGRL3,CFL1,and HSP90AB1 have the potential to serve as clinical early gastric cancer diagnostic serum biomarkers,which may facilitate early diagnosis and treatment of gastric cancer.
4.A case of occipitotemporal cavernous hemangioma

This report describes a 72-year-old male patient with occipitotemporal cavernous hemangioma(CCH).The patient presented with persistent pain in the right occipital and retro auricular areas for over one year.Physical examination detected a tender mass in the right occipital region.Imaging studies showed destruction of the right occipitotemporal bone with a heterogeneous signal mass and irregular enhancement on magnetic resonance imaging(MRI).Positron emission tomography/computed tomography(PET/CT)examination revealed increased fluorodeoxyglucose(FDG)uptake(SUVmax=5.9),suggesting a benign lesion.Complete surgical excision of the tumor and involved skull was performed,with pathological diagnosis confirming cavernous hemangioma.The patient's symptoms completely resolved with no recurrence during three months of follow-up.This case represents the first report of PET/CT application in diagnosing occipitotemporal CCH,providing valuable reference for improving diagnostic accuracy and reducing misdiagnosis rates for this rare condition.
5.The value of MRI radiomics model for predicting pathologic response to neoadjuvant therapy in human epidermal growth factor receptor 2-positive breast cancer

Objective:To investigate the value of MRI radiomics model in evaluating the pathological complete response (pCR) status of human epidermal growth factor receptor 2(HER-2) positive breast cancer after neoadjuvant therapy.Methods:The study was a cross-sectional study. The clinical, pathological, and MRI data of 243 HER-2 positive breast cancer patients who received neoadjuvant therapy in Shanxi Province Cancer Hospital from January 2021 to June 2023 were retrospectively analyzed. All patients were female, aged 26?75 years. All patients were randomly divided into training set (146 cases) and validation set (97 cases) at a ratio of 6∶4 according to the simple random sampling method. Univariate and multivariate logistic regression were used to screen independent predictors of pCR. Radiomics features were extracted from the early-phase (the 2nd phase) images of breast dynamic contrast-enhanced-MRI after neoadjuvant therapy.The four-step procedure was adopted for feature screening. The radiomics model was constructed by logistic regression. A combined model was constructed by integrating radiomics features and independent predictors. Two radiologists (Reader 1 with 10 years experience and Reader 2 with 13 years experience) who major in breast MRI visually evaluated the pCR status of breast cancer after neoadjuvant therapy. The receiver operating characteristic curve and area under the curve (AUC) were used to evaluate the efficacy of Reader 1, Reader 2, the radiomics model, and the combined model in predicting pCR status. The Hosmer-Lemeshow goodness-of-fit test was used to evaluate the calibration of the model.Results:Among 243 HER-2 positive breast cancer patients, totally 118 achieved pCR. In clinical and pathological features, HER-2 3+ was an independent predictor of pCR ( OR=2.71, 95% CI 1.03?7.12, P=0.043). In the training set and validation set, the AUCs of the radiomics model in predicting pCR status were 0.899 and 0.853, respectively.The AUCs of the combined model were 0.917 and 0.890, respectively. In the validation set, the AUC value of the radiomics model in predicting pCR status was higher than that of Reader 1 and Reader 2. Hosmer-Lemeshow goodness-of-fit test showed that there was no significant difference between the prediction of pCR status by the combined model and radiomics model and the actual results in the training set and validation set, and the fitting was good ( P>0.05). Conclusion:The MRI-based radiomics model can be used to predict pCR status in HER-2 positive breast cancer and outperforms the visual qualitative assessments of radiologists.

7.Analysis of Factors Influencing the Determination of Medical Causal Force for Medical Injury Incidents in Tianjin

Objective To review the damage outcomes of patients in medical damage events and to analyze the influencing factors of medical causality determination in medical damage events in Tianjin city,with a view to reducing the occurrence of medical damage events by avoiding risk factors and standardizing the diagnostic and therapeutic activities of medical personnel.Methods 316 cases of medical damage events handled by the Medical Damage Identification Office of Tianjin Medical Association from 2017 to 2023 were selected as the research object,and descriptive analysis was used to summarize the basic situation of medical damage events,and the medical fault factors affecting the magnitude of the cause force of the medical side of medical damage events were explored through single-factor analysis and the construction of multiomal logistic regression models.Results Medical malpractice(OR=3.140)and incomplete preoperative assessment(OR=6.008)are factors that influence the determination of the liability of the medical institution in medical malpractice events(P<0.05),and all three regression coefficients were greater than 0,all three were positively correlated with the probability that the medical cause power was judged to be greater than or equal to the equivalent cause.Conclusion The number of medical damage incidents in Tianjin shows a fluctuating downward trend.In Tianjin,the number of medical injury incidents showed a fluctuating downward trend,and the number of medical injury incidents in tertiary public general hospitals was the largest.Therefore,medical institutions should establish and improve multidisciplinary diagnosis and treatment mode to improve the level of diagnosis and treatment.Strengthen preoperative evaluation to avoid surgical risk.
8.Regulatory role and mechanism of lobetyolin in the proliferation and apoptosis of brain glioma cells

Objective To investigate the regulatory role and mechanism of lobetyolin(LBT,a poly-acetylene glycoside isolated from the roots of Codonopsis pilosula)in the proliferation and apop-tosis of brain glioma cells based on the Akt/GSK-3β/Snail signaling pathway.Methods Human brain glioma cell line U-373MG was randomly divided into normal,SC79(Akt activator),LBT,and LBT+SC79 groups.After corresponding interventions,CCK-8 assay,colony formation assay,and flow cytometry were used to detect the proliferation and apoptosis of the cells.Western blot-ting was employed to measure the protein expression levels of the molecules related to prolifera-tion,apoptosis,and Akt/GSK-3β/Snail signaling pathway.After tumor xenograft nude mouse model of U-373MG cells was established,followed by grouping and interventions as above cell experiments,the tumor weight and volume were measured.Immunohistochemical assay and TUNEL assay were performed to detect the proliferation and apoptosis of tumor cells.Western blotting was applied to detect Akt/GSK-3β/Snail signaling pathway related proteins in the nude mouse groups.Results In the LBT+SC79 group,cell viability,number of formed colonies,pro-tein levels of cyclin D1,Bcl-2 and Snail,p-Akt/Akt and p-GSK-3β/GSK-3β,tumor weight and vol-ume,and positive ratios of Ki67,cyclin D1 and Bcl-2 in transplanted tumors were increased(P<0.05),and cell apoptotic rate[(3.20±1.14)%vs(46.15±1.52)%,P<0.05],Bax protein level(0.51±0.07 vs 0.89±0.06,P<0.05),and positive ratios of TUNEL[(51.56±7.13)%vs(74.95±8.61)%,P<0.05]and Bax[(32.71±5.43)%vs(41.86±4.90),P<0.05]in transplanted tumors were declined when compared with the LBT group.Conclusion LBT can induce apoptosis and inhibit proliferation of brain glioma cells in vitro and in vivo by blocking activation of the Akt/GSK-3β/Snail signaling pathway.
9.A 2-week online metaphorical acceptance-based intervention for negative emotions in college students

Objective:To investigate the effect of metaphorical acceptance-based intervention on alleviating negative emotions in college students.Methods:Totally 112 college students were assigned to the metaphorical in-tervention group(n=40),the non-metaphorical intervention group(n=36),and the control group(n=36).Both intervention groups received online group intervention once a week for 2 weeks,while the control group received no intervention.The Generalized Anxiety Disorder-7(GAD-7),Patient Health Questionnaire-9(PHQ-9),Acceptance and Action Questionnaire-2nd Edition(AAQ-Ⅱ),and a self-report acceptance item were used to assess participants'anxiety,depression,experiential avoidance,and acceptance at baseline,2-week post-intervention,1-week follow-up,and 3-week follow-up.Results:After 2-week interventions,the scores of GAD-7,PHQ-9,and AAQ-Ⅱ in the meta-phorical group were lower than that at baseline,and the scores of self-report acceptance were higher than that at baseline(Ps<0.01).In the non-metaphorical group,the scores of GAD-7 at 1-week follow-up,the scores of PHQ-9 at 2-week post-intervention and 1-week follow-up,and the scores of AAQ-Ⅱ after 2-week post-intervention were all lower than that at baseline(Ps<0.05).The scores of self-report acceptance after 2 weeks post-intervention were all higher than baseline(Ps<0.001).The decrease of scores of AAQ-Ⅱ only mediated the reduction of scores of GAD-7(95%CI=-2.39--0.38)and PHQ-9(95%CI=-2.65--0.44)caused by metaphorical acceptance-based intervention.Conclusion:Online metaphorical acceptance-based intervention may have larger efficacy than non-metaphorical intervention in continuously alleviating anxiety,and the reduction of experiential avoidance plays an important role in alleviating anxiety and depression in metaphorical acceptance-based intervention.
10.Full genome analysis of G4P23porcine rotavirus and its pathogenicity in suckling mice and piglets

To perform the phylogenetic characterization of an isolated porcine rotavirus(PoRV)and investigate its pathogenicity in suckling mice and piglets.A G4P[23]genotype PoRV strain JSJR2023 was successfully isolated from the diarrheic piglet feces through propagation in MA104 cells.The viral proliferation kinetics were analyzed using TCID50 assays,followed by complete genome sequencing through Sanger sequencing platforms.Comprehensive genotyping and phylogenetic reconstruction were conducted using MEGA7.0 with maximum likelihood algorithms.Pathogenicity was assessed in the following animal models:5-day-old C57BL/6 mice and 3-day-old piglets.Multidimensional evaluation included clinical monitoring(diarrhea scoring,growth parameters),virological detection,and histopathological analysis of intestinal tissues.The virus strain JSJR2023 could replicate efficiently in MA104 cells,achieving peak titers of 107.5 TCID50/mL.Whole genome genotype analysis showed that the strain belonged to G4-P[23]-I5-R1-C1-M1-A8-N1-T1-E1-H1.Phylogenetic analysis indicated that the VP3 and NSP4 genes of JSJR2023 strain were most closedrelated to human species rotaviruses,suggesting genetic reassortment between human and porcine RV strains.The animal experiments in suckling mice showed that the JSJR2023 strain infection caused diarrhea symptoms,intestinal edema and congestion,and shedding of intestinal villus epithelial cells.The pathogenicity experiments in piglets showed that compared with the control group,the challenged group of pig-lets had severe diarrhea symptoms,accompanied by reduced appetite and listlessness.Post-mortem examination revealed that the intes-tines were significantly thinner,congested,and filled with yellow watery contents.The challenged piglets showed typical pathological changes such as thinning of the intestinal wall and shortening and shedding of intestinal villi.In conclusion,this study successfully iso-lated a human-porcine recombinant G4P[23]PoRV strain and established the infection models in suckling mice and piglets,providing important tools for investigating the pathogenic mechanism of PoRV,evaluating vaccines and developing antiviral drug.
