3.Study on the value of LINC00665 combined with CA153 in the diagnosis and prognostic evaluation of breast cancer

Objective To explore the value of the combined detection of long intergenic non-coding RNA(RNA LINC)00665 and carbohydrate antigen 153(CA153)in the diagnosis and prognosis assessment of breast cancer.Methods 50 breast cancer patients admitted to Jiujiang First People's Hospital were selected to be included in breast cancer group from January to December 2023,50 breast fibroadenoma patients treated in the hospital during the same period of time were selected to be included in breast fibroadenoma group,and another 50 female healthy individuals undergoing physical examination in the hospital during the same period of time were selected to be control group.The clinical data of the enrolled patients were collected,and the levels of LINC00665 and CA153 were detected by reverse transcription quantitative polymerase chain reaction and electrochemiluminescence respectively.The differences in the expression levels of LINC00665 and CA153 among three groups were compared,and the correlation between the expression levels of serum LINC00665 and CA153 was analyzed by Spearman correlation analysis;The diagnostic value of LINC00665,CA153 and the combined detection of the two in the diagnosis of breast cancer was analyzed by receiver operating characteristic(ROC)curve,and the factors affecting the prognosis of breast cancer were analyzed by multifactorial regression.Results Compared with control group,serum LINC00665 and CA153 expression levels were higher in breast cancer group,and serum LINC00665 expression levels were higher in breast fibroadenoma group.Serum LINC00665 and CA153 expression was correlated with TNM stage,pathological type,and lymph node metastasis of breast cancer(P<0.05).Spearman correlation analysis showed that there was a linear positive correlation between serum LINC00665 and CA153 expression in patients with breast cancer(r=0.651,P<0.05).ROC curve analysis showed that the value of combined diagnosis of LINC00665 and CA153 was higher than that of individual diagnosis(P<0.05).The results of multifactorial regression analysis showed that LINC00665,CA153,TNM stage,and lymph node metastasis were independent risk factors affecting the prognosis of breast cancer patients(P<0.05).Conclusion The higher expression levels of serum LINC00665 and CA153 in breast cancer patients were correlated with TNM stage,pathologic type,and lymph node metastasis,which can be used in the diagnosis and prognosis evaluation of breast cancer.
4.Characteristics of drug resistance and molecular transmission networks among preoperative HIV/AIDS patients in Ningxia from 2018 to 2023

Objective:This study aimed to analyze the genetic subtypes and drug resistance transmission characteristics of HIV-1 among the preoperative population in Ningxia from 2018 to 2023, to provide a scientific basis for the prevention and control of the AIDS epidemic.Methods:Plasma samples and demographic information of HIV/AIDS patients receiving antiviral treatment in Ningxia from 2018 to 2023 were collected. Blood samples with a viral loads >200 copies/ml from preoperative testing were amplified, sequenced, and subjected to genotypic resistance testing to analyze their genetic subtypes and drug resistance characteristics. The TN93 model in MEGA11 software was used to calculate the genetic distance between each pair of all sequences, and a molecular transmission network was constructed in Cytoscape 3.10.0 with 1.9% as the genetic threshold.Results:Among 101 preoperative HIV/AIDS patients, CRF07_BC and CRF01_AE were the predominant subtypes. The majority were male (85.15%, 86/101), aged 41-60 years (45.54%, 46/101), residing in Yinchuan city (61.39%, 62/101), and infected via heterosexual transmission (71.29%, 72/101), with most cases being late-detected. Of 39 drug-resistant sequences, resistance to non-nucleoside reverse transcriptase inhibitors (NNRTIs) alone (18.81%, 19/101) and dual resistance to nucleoside reverse transcriptase inhibitors (NRTIs)-NNRTIs (13.86%, 14/101) were most common. Among 44 sequences forming 13 transmission clusters, nine clusters harbored drug-resistant mutations. Four subtypes entered the molecular network, primarily involving heterosexual transmission, individuals with junior high school education or below, and men aged≥50 years.Conclusions:From 2018 to 2023, the preoperative HIV/AIDS patients had diversified genetic subtypes, with higher rates of overall drug resistance and late detection, stronger drug resistance and higher mortality rate. Strengthening molecular epidemiological research and developing targeted screening strategies are critical to improve early detection and reduce transmission risks.

Hashimoto thyroiditis(HT)is one of the most common autoimmune diseases."Wood depression and earth stagnation"is its important pathogenesis.Liver depression and qi stagnation,wood depression multiplies the earth,spleen deficiency and earth stagnation,phlegm and blood stasis intermingle,resulting in wood depression and earth stagnation.Or middle-earth stagnation insults wood in turn,liver yang deficiency,wood depression,and failure to disperse and discharge,resulting in earth stagnation and wood depression.It may be related to the intestinal immune mechanism in modern medical study.Based on the theory of"wood depression and earth stagnation",this article discussed the pathogenesis of HT from the perspective of wood depression and imbalance of microbe-gut-brain axis,earth stagnation and dysbiosis of intestinal flora,damage to the intestinal barrier,and proposed the treatment principles,i.e.,"disperse stagnated liver qi for relieving qi stagnation,ascend yang and regulate qi""eliminate stagnation and remove turbidity,invigorate spleen and restore normal movement",which could provide the ideas for mechanism research and clinical treatment of HT.
6.Study on the effects of different tidal volume ventilation on respiratory mechanics and hemodynamics in children with severe pneumonia

Objective To investigate the effect of ventilation with different tidal volumes on respiratory mechanics and haemodynamics in children with severe pneumonia.Methods A total of 104 children with severe pneumonia were divided into the control group and the observation group using a random number table method,with 52 cases in each group.Both groups underwent mechanical ventilation treatment.The control group was treated with conventional tidal volume of 10-12 mL/kg,while the observation group was given a low tidal volume of 6-8 mL/kg.The respiratory mechanics indicators,hemodynamic indicators and inflammatory factor indicators before and after treatment were compared between the two groups.Results After treatment,the airway resistance(AR)and airway closing pressure(AOP)were lower in the observation group than those of the control group(P<0.05),and pulmonary dynamic compliance(Cdyn)was higher in the observation group than that of the control group(P<0.05).There were no significant differences in heart rate(HR),mean arterial pressure(MAP)and central venous pressure(CVP)after treatment between the two groups(P<0.05).After treatment,the levels of interleukin(IL)-6,IL-8 and tumor necrosis factor-α(TNF-α)were all lower in the observation group than those in the control group(P<0.05).Conclusion Small tidal volume mechanical ventilation is effective in improving the respiratory mechanics of children with severe pneumonia without causing large fluctuations in their hemodynamics.At the same time,it can reduce the level of inflammatory factors in the organism.
7.Neuropathological characteristics of autopsy brain tissues in schizophrenia patients

Objective:To explore the neuropathological characteristics of brain tissues from autopsy of patients with schizophrenia.Methods:Forty-two autopsy cases from National Human Brain Bank for Health and Disease, School of Medicine, Zhejiang University from January 2013 to December 2024 were selected as research subjects, among which, 21 were schizophrenia patients(schizophrenia group) and 21 were non-schizophrenia patients (non-schizophrenia group). Clinical data of patients from the two groups were compared. HE staining was used to detect the pathological changes such as infarction, hemorrhage and arteriosclerosis in the brain tissues, silver-nitrate staining was used to detect the amyloid plaques in the brain tissues, Congo red staining was used to detect the pathological changes related to cerebral amyloid angiopathy (CAA) in the brain tissues, modified Gallyas silver staining was used to detect the neurofibrillary tangles in the brain tissues, and immunohistochemical staining was used to detect the expressions of phosphorylated tau protein, β-amyloid protein (Aβ), TAR DNA binding protein 43 (TDP-43), and α-synuclein in the brain tissues. Alzheimer's disease neuropathologic change (ADNC), primary age-related tauopathy (PART), limbic-predominant age-related TDP-43 encephalopathy (LATE), aging-related tau astrogliopathy (ARTAG), Lewy body disease (LBD), and cerebrovascular disease (CVD)-related pathological changes in the brain tissues were evaluated, and differences in positive rates of the above pathological changes were compared.Results:No significant difference in gender, age of death, brain weight, or apolipoprotein E genotype was noted between the schizophrenia group and non-schizophrenia group ( P>0.05). Six schizophrenia patients exhibited low-to-intermediate ADNC, including 4 with low ADNC and 2 with intermediate ADNC. Compared with the non-schizophrenia group, the positive rates of ADNC- and CVD-related pathological changes in the schizophrenia group were significantly higher (0 vs. 28.6%; 9.5% vs. 47.6%, P<0.05). No significant differences in positive rates of PART-, LATE-, ARTAG-, and LBD-related pathological changes were noted between the schizophrenia group and non-schizophrenia group ( P>0.05). Conclusion:Schizophrenia patients show high proportions of ADNC- and CVD-related pathological changes, but relatively low ADNC severity.
8.Auditory outcomes and influencing factors by different bilateral intervention modes in children with cochlear implantation

Objective To investigate the auditory effects of cochlear implantation in quiet and noisy environ-ments in children with different bilateral intervention modes,as well as the factors influencing these effects.Methods A total of 185 children with bilateral severe to profound sensorineural hearing loss were divided into three groups:bimodal hearing mode group(BIM,n=55),simultaneous bilateral cochlear implantation group(SCI,n=70),and sequential bilateral cochlear implantation group(SBCI,n=60).The Parents' Evaluation of Aural/Oral Performance of Children(PEACH)was used to assess the PEACH scores of the three groups in quiet and noisy environments one year after binaural hearing aid intervention.Additionally,the effects of cochlear implantation age,preoperative residual hearing,hearing aid usage,rehabilitation training mode,family system,and other factors on auditory per-formance in quiet and noisy environments were analyzed.Results The PEACH scores in quiet environments were higher than those in noisy environments for all three groups(all P<0.05).The SCI group had higher PEACH scores in both quiet and noisy environments compared to the BIM group(P<0.05).Multifactorial analysis revealed differences in factors influencing auditory performance in quiet and noisy environments among the three groups.First cochlear implantation before 3 years of age,preoperative hearing aid usage,and home-based rehabilitation training mode were common favourable influencing factors for auditory performance in both environments.Preopera-tive residual hearing below 95 dB HL was an favourable influencing factor for auditory performance in quiet environ-ments in the BIM group.The higher the level of parental education,the better auditory performance in both quiet and noisy environments for the SCI and SBCI groups.Implantation interval of 24 months or less and hearing aid usage during the inter-implantation period were favourable influencing factors for auditory performance in both envi-ronments for the SBCI group.Conclusion Children with severe to profound prelingual deafness after simultaneous bilateral CI implantation had better hearing performance than bimodal listening in quiet and noise environments.Ear-ly implantation,preoperative or inter-implantation hearing aid usage are recommended to improve auditory perform-ance in noisy environments,regardless of the bilateral intervention mode.The interval between bilateral cochlear im-plantations should be less than 12 months.
9.Implementation of integrated collaborative emergency management among multiple hospital districts from the perspective of disaster vulnerability

Objective This study focuses on the analysis of disaster vulnerability among multiple hospital districts to ex-plore the core of integrated emergency management among these districts.Methods Taking the First Affiliated Hospital of Guan-gzhou Medical University as an example,the Kaiser model analysis tool was used to identify risk events in the three hospital dis-tricts.A 42-item questionnaire was developed based on four dimensions,and surveys were conducted among administrative de-partment heads and some frontline staff in the three districts.After selecting valid data,the arithmetic mean of single-item scores from multidimensional scorers was calculated as the final single-item score for risk evaluation.Results The analysis revealed significant differences in the ranking of the top 10 risk events among the three hospital districts.The top risk event in the riverside district was typhoon,with a risk coefficient of 60.2%,which was higher than the top risk events in the Tan Sha district(55.2%)and Hai Yin district(52.8%).This indicates that the overall risk coefficient in the riverside district is higher,with a higher probability of different types of risk events occurring.Conclusion The study found differences in risk events among multiple hospital districts and identified conflicts in the implementation of integrated collaborative emergency management.To ad-dress these differences and conflicts,it is necessary to establish a unified command system,enhance information sharing,in-tegrate resources,strengthen risk assessment,and optimize management culture,thereby achieving efficient operation of compre-hensive emergency management in multiple hospital districts.
10.Application and evaluation of case-based learning + Seminar teaching method based on Toulmin model in education and teaching of clinical medicine

Objective:To analyze the application effects of case-based learning (CBL) + Seminar teaching method based on Toulmin model in the education and teaching of clinical medicine innovation class.Methods:A total of 60 students from the clinical medicine innovation class at The Second Affiliated Hospital of Guangzhou Medical University from September 2021 to September 2023 were selected as research objects. These students were divided into control group ( n=30) and research group ( n=30) according to different teaching programs. The traditional teaching method was adopted in the control group, and the CBL + Seminar teaching method based on Toulmin model was adopted in the research group. The two groups were compared in terms of clinical knowledge, clinical skill, and clinical case analysis scores; changes in clinical thinking ability after teaching; and satisfaction with the teaching process. SPSS 22.0 was used for t-test and chi-square test. Results:The research group significantly outperformed the control group in clinical knowledge [(89.59±3.46) points vs. (83.23±3.02) points], clinical skill [(88.87±3.23) points vs. (83.62±3.13) points], and clinical case analysis [(89.73±3.51) points vs. (82.62±3.19) points] ( t=7.58, 6.39, 8.21, P<0.001). The clinical thinking ability after teaching was significantly higher in the research group compared to the control group [(258.49±13.36) points vs. (242.56±13.02) points] ( t=4.67, P<0.001). The overall satisfaction rate of teaching was significantly higher in the research group compared to the control group (100.00% vs. 80.00%, χ2=4.63, P=0.031). Conclusions:The CBL + Seminar teaching method based on Toulmin model can effectively improve student learning performance and clinical thinking ability, demonstrating a promising application value in the education and teaching of clinical medicine innovation class.
