Objective This study aimed to investigate the association between long-term radon exposure and changes in peripheral blood DNA methylation levels, as well as alterations in the expression of methylation-related regulatory genes (DNMT1, DNMT3A, and MBD2) among underground non-uranium metal miners. Methods Five non-uranium metal mines with varying levels of radon exposure were enrolled in this study. From each mine, 30 underground miners and 30 aboveground workers were randomly recruited. Basic demographic information was collected via a standardized questionnaire, and peripheral venous blood samples were obtained from all participants. An enzyme-linked immunosorbent assay (ELISA) colorimetric method was used to detect peripheral blood DNA methylation levels. Real-time quantitative PCR was used to measure mRNA expression levels of DNMT1, DNMT3A, and MBD2. These indicators were compared between underground and aboveground groups and among underground miners with different radon exposure levels. Multiple linear regression analysis was conducted to examine the associations between cumulative radon dose and two outcomes: peripheral blood DNA methylation levels, and the expression levels of methylation-related regulatory genes in underground miners. Results The average radon concentration in the underground workplaces of the five mines was 4,088 Bq/m³ (range: 116-18,188 Bq/m³), corresponding to an estimated annual effective dose of approximately 25.67 mSv for miners. The average radon concentration in aboveground workplaces was 58 Bq/m³ (range: 28-1796 Bq/m³), equivalent to an annual effective dose of approximately 0.51 mSv for aboveground workers. The average cumulative radon dose of underground miners was approximately 26.56 times that of aboveground workers (Z=−12.469, P<0.001). Compared with the aboveground workers, the peripheral blood DNA methylation level of underground miners was significantly decreased, approximately 0.77 times that of the aboveground group (Z=−2.313，P<0.05), accompanied by decreased expression of DNA methyltransferase genes DNMT1 and DNMT3A, which were 0.83 and 0.68 times those of the aboveground group, respectively (Z=−3.002、−4.190，P<0.01). After adjusting for age, working years, body mass index, smoking status, and alcohol consumption, multiple linear regression analysis revealed two key findings: cumulative radon dose was significantly and inversely correlated with peripheral blood DNA methylation levels (β=−0.752, P<0.001); and cumulative radon dose was significantly negatively associated with the expression of DNMT1 and DNMT3A (β=−0.319, −0.369, P<0.01). Conclusion The reduced DNA methylation levels and downregulated expression of DNA methyltransferase genes in underground miners are closely associated with long-term radon exposure.

Objective To observe the value of transvaginal two-dimensional ultrasonography(2D-TVS)combined with three-dimensional ultrasonography OmniView for diagnosing intrauterine adhesion(IUA).Methods Totally 200 women with suspected IUA were retrospectively enrolled,including 172 cases who were ultimately diagnosed IUA and the rest 28 without IUA.The efficacy of 2D-TVS combined with OmniView for diagnosing IUA was analyzed.Results Among 200 cases,150 cases were diagnosed IUA with 2D-TVS(135 cases were diagnosed correctly),167 cases were diagnosed IUA with OmniView(159 cases were diagnosed correctly),and 170 cases were diagnosed with combination of the above two(165 cases were diagnosed correctly).The area under the curve(AUC)of 2D-TVS,OmniView and 2D-TVS combined with OmniView for diagnosing IUA was 0.625,0.819 and 0.890,respectively,and 2D-TVS combined with OmniView had the highest AUC(both P＜0.05),with sensitivity of 95.93％,specificity of 82.14％,positive predictive value of 97.06％and negative predictive value of 76.67％.Conclusion 2D-TVS combined with OmniView could be helpful for diagnosing IUA.

Objective To compare the therapeutic effects of modified Shenqi Maiwei Dihuang Decoction combined with Buzhong Yiqi Pill and Buzhong Yiqi pill alone in the patients with cold and heat mixed type di-abetic foot.Methods A total of 123 patients with cold and heat mixed type diabetic foot receiving the treat-ment in this hospital from April 2022 to December 2023 were selected and divided into the modified Shenqi Mai-Dihuang Decoction and Buzhong Yiqi Pill group(combined group,60 cases)and Buzhong Yiqi Pill group(monotherapy group,63 cases).The wound healing,mean blood flow amount of foot dorsal artery,blood glu-cose and changes in vascular diameter,inflammatory factors and vascular endothelial growth factor after 2 courses of treatment were compared betweeb the 2 groups.Results The ulcer reduction rate in the combina-tion group was significantly higher than that in the monotherapy group(P=0.001).The fasting blood glu-cose and 2 h postprandial blood glucose after treatment in the combination group all were lower than those in the monotherapy group(P=0.001).The average blood flow of foot dorsal artery after treatment in combina-tion group was higher than that in the monotherapy group,and the difference was statistically significant(P=0.013).There were 34 cases of Wagner grade 3 in the combined group and 33 cases of Wagner grade 3 in the monotherapy subgroup respectively.There was statistically significant difference in the CRP level between the two subgroups(P=0.045).Conclusion Shenqi Maiwei Dihuang Decoction combined with Buzhong Yiqi Pill in treating diabetic foot ulcer is more effective than Buzhong Yiqi Pill alone,moreover the safety is high.

Objective To observe the value of transvaginal two-dimensional ultrasonography(2D-TVS)combined with three-dimensional ultrasonography OmniView for diagnosing intrauterine adhesion(IUA).Methods Totally 200 women with suspected IUA were retrospectively enrolled,including 172 cases who were ultimately diagnosed IUA and the rest 28 without IUA.The efficacy of 2D-TVS combined with OmniView for diagnosing IUA was analyzed.Results Among 200 cases,150 cases were diagnosed IUA with 2D-TVS(135 cases were diagnosed correctly),167 cases were diagnosed IUA with OmniView(159 cases were diagnosed correctly),and 170 cases were diagnosed with combination of the above two(165 cases were diagnosed correctly).The area under the curve(AUC)of 2D-TVS,OmniView and 2D-TVS combined with OmniView for diagnosing IUA was 0.625,0.819 and 0.890,respectively,and 2D-TVS combined with OmniView had the highest AUC(both P＜0.05),with sensitivity of 95.93％,specificity of 82.14％,positive predictive value of 97.06％and negative predictive value of 76.67％.Conclusion 2D-TVS combined with OmniView could be helpful for diagnosing IUA.

Objective To investigate the effect of"zero channel"emergency mode on the treatment of patients with severe traumatic brain injury.Methods A total of 147 patients with severe traumatic brain injury admitted to the Second Hospital of Jiaxing from January 2020 to December 2021 were selected as study objects.Sixty-two patients hospitalized in traditional emergency mode from January to December 2020 were included in control group,and 85 patients hospitalized in"zero channel"emergency mode from January to December 2021 were included in observation group.The initiation time of rescue,completion time of CT examination,completion time of blood transfusion,duration of operation,clinical prognosis and complication rate were compared between two groups.Results The initiation time of rescue,completion time of CT examination,completion time of blood transfusion,and duration of operation in observation group were significantly shorter than those in control group(P<0.05).The proportion of good recovery in observation group was significantly higher than that in control group(68.20%vs.38.70%,χ2=12.671,P<0.001).The complication rate of observation group was significantly lower than that of control group(21.18%vs.80.65%,χ2=51.000,P<0.001).Conclusion"Zero channel"emergency mode can effectively shorten the treatment time of patients with severe traumatic brain injury,improve the success rate of rescue,reduce the incidence of complications,worthy of clinical use and promotion.

Objective:To investigate the effect of panax notoginseng saponins(PNS)on the expression of tumor necrosis factor-α(TNF-α)in lipopolysaccharide(LPS)-induced activated BV2 microglia through p38 mitogen-activa-ted protein kinase(p38 MAPK)pathway.Methods:BV2 microglia were divided into control group,LPS activated group and LPS+panax notoginseng saponins intervention group(LPS+PNS).The CCK-8 method was used to detect the viability of BV2 microglia and determine the optimal drug intervention concentration.Western Blot and immunofluo-rescence were used to detect the expression of p38 MAPK and TNF-α and the phosphorylation level of p38 MAPK(p-p38 MAPK)in BV2 microglia.Results:Compared with the blank control group,there was no significant difference in the cell viability of BV2 microglia,and finally 100 mg/L was selected as the drug intervention concentration.Western Blot and immunofluorescence results indicated that after LPS activation,the expression of TNF-α and the phosphoryla-tion level of p38 MAPK in BV2 microglia were significantly increased(P＜0.05).After PNS intervention,compared with LPS-activated group,the expression of TNF-α and the phosphorylation level of p38 MAPK were significantly decreased(P＜0.05).After treatment with p38 MAPK pathway inhibitor(SB203580),there was no significant differ-ence in the expression levels of p-p38 MAPK and TNF-α in PNS combined with SB203580 group(LPS+PNS+I)com-pared with LPS+PNS group(P＞0.05).In addition,the changes of p38 MAPK in each group were not statistically sig-nificant(P＞0.05).Conclusion:PNS may inhibit the expression of inflammatory factor TNF-α secreted by activated BV2 microglia through p38 MAPK pathway.

Objective:To compare the efficacy of 3D-printed guide plate assisted versus freehand placement of cannulated screws for the treatment of Sanders type II and III calcaneal fractures.Methods:A retrospective cohort study was conducted to analyze the clinical data of 29 patients with Sanders type II and III calcaneal fractures admitted to Chonggang General Hospital from June 2020 to October 2022. Among them, there were 18 males and 11 females, with an age range of 22-69 years [(40.1±11.5)years]. Nineteen patients were treated with individualized 3D-printed guide plate assisted placement of cannulated screws (3D-printed group) and 10 were treated with freehand placement of cannulated screws (freehand group). The surgical time, fluoroscopy times, postoperative 6-month calcaneal morphology (length, width, height, B?hler angle and Gissane angle), and American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot score and Maryland functional score assessed at 3, 6 months after operation and at the final follow-up were compared between the two groups. The incision healing and complications were observed.Results:The patients were followed up for 6-24 months [(11.3±2.5)months]. The surgical time and fluoroscopy times in the 3D-printed group were (53.4±9.1)minutes and (7.3±1.1)times, respectively, which were shorter than (90.2±16.0)minutes and (16.0±3.2)times in the freehand group (all P<0.01). At 6 months after operation, there was no significant difference in calcaneal length between the two groups ( P>0.05); the calcaneal width, height, B?hler angle and Gissane angle in the 3D-printed group [(34.0±1.8)mm, (47.2±1.6)mm, (27.8±1.0)°, (129.2±2.8)°] were superior than those in the freehand group [(37.5±2.0)mm, (43.0±2.7)mm, (25.8±1.5)°, (125.9±2.5)°] (all P<0.01). At 3, 6 months after operation and at the final follow-up, the values of AOFAS ankle-hindfoot score in the 3D-printed group [(72.2±2.3)points, (79.7±2.3)points, (86.5±4.4)points] were higher than those in the freehand group [(64.2±6.9)points, (73.4±4.2)points, (81.8±3.1)points] (all P<0.05); the values of Maryland score in the 3D-printed group [(71.4±7.7)points, (84.7±2.6)points, (91.5±2.5)points] were higher than those in the freehand group [(65.2±5.6)points, (79.1±3.8)points, (87.1±2.9)points] (all P<0.05). All surgical incisions were healed in stage I. In the 3D-printed group, there were no complications regarding infection, iatrogenic vascular or nerve injury, or fixation failure after surgery. In the freehand group, one patient with lateral sural cutaneous nerve injury was resolved spontaneously without specific treatment. Conclusion:Compared with freehand placement of cannulated screws, 3D-printed guide plate assisted placement of percutaneous placement has the advantages of shorter surgical time, fewer fluoroscopy times, lower reduction loss, better ankle joint function recovery, and less complications in treating Sanders type II and III calcaneal fractures.

Objective:To explore the potential molecular mechanism of Erigeron breviscapus in the prevention and treatment of ischemic stroke through network pharmaco-molecular docking.Methods:The Chinese Herbal Medicine Systematic Pharmacology Platform(TCMSP)database provided active ingredients and potential targets of Erigeron breviscapus.Ischemic stroke-related targets were searched through the Online Mendelian Inheritance in Man(OMIM)database,the bioinformatics and chemoinformatics(DrugBank)database and human gene comprehensive database(GeneCards).The targets criteria were de-weighted by the Protein Data Bank(Uniprot)and imported into the Venny online platform to obtain the intersecting targets of both.The intersecting targets were visualized by STRING database and Cytoscape 3.7.1 software for protein-protein interaction(PPI),followed by the enrichment analysis of intersection targets for gene ontology(GO)function and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway.AutoDock Vina1.5.6 software was used to verify the molecular docking of key active ingredients and core targets and realize the vi-sualization of docking results.Results:Eleven active ingredients and 176 targets were obtained.There were 690 targets ischemic stroke-related targets and 69 intersection targets.Through PPI network,10 core genes were screened according to the degree value,including tumor necrosis factor(TNF),interleukin-6(IL-6),serine/threonine protein kinase 1(AKT1),interleukin-1β(IL-1β)and vascular endothelial growth factor A(VEGFA).KEGG enrichment included the advanced glycation end products-receptor(AGE-RAGE)signaling pathway,interleukin-17(IL-17)signaling pathway,tumor necrosis factor(TNF)signaling pathway,etc.The top 3 active ingredients and the top 5 target proteins were selected according to the degree value,and the molecular docking results demonstrated a considerable binding ability.Conclusion:Erigeron breviscapus in the treatment of ischemic stroke may work through multiple active ingredients,such as quercetin,kakaferol,and luteolin,which act on TNF,IL-6,AKT1,IL-1β,and VEGFA,and through a varie-ty of signaling pathways such as IL-17,TNF,etc.showing the characteristics of multi-components,multi-targets,and multi-pathways.
