Objective To explore the expression profile of adenosine A1 receptor in the paraventricular thalamic nucleus(PVT)in wakefulness/sleep state and its role in regulating sleep.Methods Male C57BL/6J mice(6～12 weeks old,weighing 22～28 g)were randomly divided into ZT0,ZT6,ZT10,ZT12,and sleep deprivation and recover0y sleep groups(n=16 or n=4).RT-qPCR and fluorescence in situ hybridization were used to observe the changes of adenosine A1 receptor in PVT.Whole-cell patch-clamp recording was conducted to determine the effects of adenosine on the membrane potential and discharge frequency of PVT neurons.The experimental animals were divided into adenosine A1 receptor interference group(n=8)and interference control group(n=7).After RNA interference,electroencephalography(EEG)and electromyography(EMG)were applied simultaneously to observe the changes in wake and sleep time between the 2 groups during sleep deprivation for 6 h and sleep recovery for 4 h.Results RT-qPCR and fluorescence in situ hybridization confirmed that the expression of adenosine A1 receptor in PVT was affected by wakefulness/sleep state,with the level in the ZT0 group(active stage)significantly higher than that in the ZT12 group(non-active stage,P<0.05).Whole-cell patch clamp recording indicated that adenosine exerted inhibitory effects on PVT neurons through 2 distinct response types(P<0.05),and the inhibition was in a diminishing trend with a decrease in the expression level of adenosine A1 receptor.After sleep deprivation,the expression level of adenosine A1 receptor showed significant intergroup differences:the level was significantly higher in the sleep deprivation group than the recovery sleep group(P<0.05),while that of the recovery sleep group was higher than that of the ZT6 group and that of the ZT10 group(P<0.05).After knocking down adenosine A1 receptor(shRNA-A1R)in the PVT,the wakefulness timing of the shRNA-A1R group was significantly increased,while the sleep timing was significantly decreased within 1 h after sleep recovery when compared with the interference control group(P<0.05).Conclusion The expression of adenosine A1 receptor in the PVT is dynamically regulated by sleep pressure,which was increasing as sleep pressure rises.

Objective To retrieve and analyze the evidences on insulin injection position selection and rotation in patients with diabetes, and to summary the best evidence. MethodsWe took "diabetes mellitus""insulin" "injection" "position" "rotation" as subject terms to retrieve the evidences on insulin injection position selection and rotation in the evidence-based database such as the British Medical Journal (BMJ) best practice, Uptodata, Cochrane Library (2014), Joanna Briggs Institute Library, Registered Nurses' Association of Ontario (RNAO), Scottish Intercollegiate Guidelines Network (SIGN) and National Guideline Clearinghouse (NGC) as well as the comprehensive database such as the Embase, Elsevier science Direct, PubMed, Web of Science, OVID, China Biological Medicine (CBM) and WanFang data. ResultsA total of 13 evidences were included, 2 evidence summaries, 7 guidelines and 4 consensus. Four kinds of evidence were summarized including training for insulin standardized injection and management, evaluation, injection position and rotation. ConclusionsMedical staff should standardize the clinical practice behavior in insulin injection position selection and rotation based on relevant levels of evidence-based medicine to guarantee the safety of patient and improve quality of nursing.

Objective To systematically evaluate the association between frailty and risk of postoperative delirium.Methods Systematic review of literature was conducted using eight electronic databases:PubMed,Embase,CENTRAL,Web of Science,CNKI,CBM,VIP and Wanfang Data,and prospective cohort studies about association between frailty and postoperative delirium published before March 2017 were included.Two authors independently screened the literature,extracted the data,and assessed the quality using NOS Scale,and meta-analysis was conducted by RevMan 5.3 software.Results A total of eight studies involving 846 patients were included in this review.Meta-analysis showed that:frailty was associated with higher risk of postoperative delirium [OR=3.63,95％CI (2.06,6.40),P＜0.001].Subgroup analysis showed that:①Frailty assessment tool:Fried frailty criteria and other frailty assessment were associated with increased risk of postoperative delirium[OR=5.81,95％CI(3.54,9.77),P＜0.001],[OR=1.76,95％CI(1.06,2.92),P=0.03].②Age:frailty patients aged 60～74 had increased risk of postoperative delirium [OR=5.05,95％CI (3.14,8.12),P＜0.001],but for patients aged ≥ 75,frailty wasn't associated with postoperative delirium [0R=1.73,95％CI (0.99,3.00),P=0.05].③Type of surgery:for cardiovascular and non-cardiovascular surgery patients,frailty was associated with increased risk of postoperative delirium [OR=3.40,95％CI (1.64,7.05),P＜0.001],[OR=4.95,95％CI (2.41,10.16),P＜0.001].Conclusion Frailty can increase the risk of postoperative delirium.In consideration of quantity and quality of included studies,the conclusion needs to be validated by multi-centered prospective cohort studies with large sample size.

OBJECTIVETo compare the clinical efficacy differences between acupoint plaster therapy with midnight-noon ebb-flow hour-prescription method and traditional acupoint plaster therapy for senile osteoporosis (SOP).

METHODSWith randomized controlled blind design, 76 SOP patients with deficiency of liver and kidney syndrome were randomly divided into an observation group and a control group, 38 cases in each one. Based on oral administration of caltrate D, the patients in the observation group were treated with acupoint plaster therapy with midnight-noon ebb-flow hour-prescription method at Yingu (KI 10), Taixi (KI 3), Dazhong (KI 4), Fuliu (KI 7) and Zhiyin (BL 67), while the patients in the control group were treated with traditional acupoint plaster therapy. Each plaster therapy lasted for 6 h, once a day; there was an interval of 2 d after consecutive 5-day treatment; 4 weeks were taken as one course, and totally 2 courses were given. Visual analogue scale (VAS) and Oswestry Disability Index (ODI) were used to evaluate the pain and dysfunction before intervention, after 4 weeks and 8 weeks intervention. Osteoporosis symptom rating sale and quality of life questionnaire of the European foundation for Osteoporosis (QUALEFFO-41) were adopted to evaluate the TCM syndrome and quality of life before and after 8-week intervention.

RESULTSAll the outcomes were significantly improved after treatment in the two groups (<0.01,<0.05); after 4 weeks and 8 weeks of treatment, the VAS and ODI in the observation group were lower than those in the control group (all<0.05). Repeated ANOVA indicated the VAS and ODI were significant in group effect, time effect and interaction effect (all<0.01). Further comparison showed that VAS and ODI at later time points were lower than those in the early time points (all<0.01). After the treatment, the scores of TCM syndrome and QUALEFFO-41 in the observation group were lower than those in the control group (all<0.05). The effective rate was 85.7% (30/35) in the observation group, which was superior to the effective rate in the control group[74.3%(26/35),<0.05].

CONCLUSIONSThe acupoint plaster therapy with midnight-noon ebb-flow hour-prescription method is superior to traditional acupoint plaster therapy in improving pain, dysfunction, TCM syndrome and quality of life in SOP patients; in addition, its clinical efficacy is significant.

Eight-year medical education is to train medical personnel with high research abi lity and innovative ability, while the experimental teaching, as an important part of physiology teaching, plays an important role in training students in observation, problem-solving skills and practical ability. This article mainly discusses our exploration and practice of teaching in physiology experiment for eight-year program medical students from teaching contents and organization of class teaching, in order to fully increase the studying enthusiasms and meet the scientific mind of these students.

Objective To investigate the modulatory effect of orexin A on glutamate receptor-mediated current in the freshly isolated pyramidal neurons from the rat prefrontal cortex (PFC). Methods Deep layer (Ⅴ-Ⅵ) prefrontal cortical pyramidal neurons from postnatal 10 to 14 day-old Wistar rats were acutely dissociated by a combination of mechanical and enzymatic method. Subsequently, the effect of orexin A on the current induced by glutamate was studied by the technique of whole cell patch clamp. Results Both orexin A and glutamate dose-dependently evoked the inward transmembrane current. The current was evoked by 1 mmol/L glutamate as a control group(100%). Objective To investigate the effect of extragenous adenosine(AD) by microinjection in the orexinergic area of rat lateral hypothalamus(LH) on the sleep-wake cycle.Methods A total of 14 male SD rats were given 1,10 or 20 nmol AD and artificial cerebrospinal fluid(aCSF) as own control through three different models of microinjections into the LH.Their electroencephalogram(EEG) and electromyogram(EMG) were recorded by a polygraph recording instrument to observe the changes of sleep-wake after microinjections.Results AD administration to LH at doses of 1,10 and 20 nmol decreased the total amount of wakefulness by 84%,62% and 60% fold during that first 3-hour recording period and concomitantly increased non rapid eye movement(NREM) and REM sleep,when compared with aCSF treatment.The sleep-inducing effect was maintained even in 3 hour after the 20 nmol AD microinjection.The maximal sleep-inducing effect was obtained at the 20 nmol dose of AD administration into the orexinergic LH(P
