1.Altered expression of adenosine A1 receptor in the paraventricular thalamus and its role in the regulation of sleep

Objective To explore the expression profile of adenosine A1 receptor in the paraventricular thalamic nucleus(PVT)in wakefulness/sleep state and its role in regulating sleep.Methods Male C57BL/6J mice(6~12 weeks old,weighing 22~28 g)were randomly divided into ZT0,ZT6,ZT10,ZT12,and sleep deprivation and recover0y sleep groups(n=16 or n=4).RT-qPCR and fluorescence in situ hybridization were used to observe the changes of adenosine A1 receptor in PVT.Whole-cell patch-clamp recording was conducted to determine the effects of adenosine on the membrane potential and discharge frequency of PVT neurons.The experimental animals were divided into adenosine A1 receptor interference group(n=8)and interference control group(n=7).After RNA interference,electroencephalography(EEG)and electromyography(EMG)were applied simultaneously to observe the changes in wake and sleep time between the 2 groups during sleep deprivation for 6 h and sleep recovery for 4 h.Results RT-qPCR and fluorescence in situ hybridization confirmed that the expression of adenosine A1 receptor in PVT was affected by wakefulness/sleep state,with the level in the ZT0 group(active stage)significantly higher than that in the ZT12 group(non-active stage,P<0.05).Whole-cell patch clamp recording indicated that adenosine exerted inhibitory effects on PVT neurons through 2 distinct response types(P<0.05),and the inhibition was in a diminishing trend with a decrease in the expression level of adenosine A1 receptor.After sleep deprivation,the expression level of adenosine A1 receptor showed significant intergroup differences:the level was significantly higher in the sleep deprivation group than the recovery sleep group(P<0.05),while that of the recovery sleep group was higher than that of the ZT6 group and that of the ZT10 group(P<0.05).After knocking down adenosine A1 receptor(shRNA-A1R)in the PVT,the wakefulness timing of the shRNA-A1R group was significantly increased,while the sleep timing was significantly decreased within 1 h after sleep recovery when compared with the interference control group(P<0.05).Conclusion The expression of adenosine A1 receptor in the PVT is dynamically regulated by sleep pressure,which was increasing as sleep pressure rises.
2.Mid-and long-term effect of Kegel training combined with Pilates training on urinary control recovery in pa-tients with post-prostatectomy incontinence with different body mass index

Objective To observe the mid-and long-term effects of Kegel training combined with Pilates training on urinary conti-nence recovery in different body mass index(BMI)male patients with urinary incontinence after prostatectomy.Methods From May,2023 to June,2024,48 patients in Beijing Bo'ai Hospital were recruited and divided into group A(<25 kg/m2,n=15),group B(25 to 30 kg/m2,n=18)and group C(>30 kg/m2,n=15)according to their BMI.All the groups performed Kegel training combined with Pilates training for two months,and followed up at six months from baseline.They were evaluated with one hour pad test,the number of daily urinary incontinence,In-ternational Consultation on Incontinence Questionnaire-Short Form(ICIQ-SF)and modified Oxford Rating Scale before treatment,and four weeks,eight weeks and six months after treatment.Results The intra-group effect,the inter-group effect and interaction effect were significant in the results of one hour pad test and the daily number of urinary incontinence(F>2.955,P<0.05).Post Hoc test showed that they were worse in group C than in groups A and B(P<0.05),and the number of daily urinary incontinence was more in group B than in group A(P<0.05).There was significant difference in the scores of ICIQ-SF and modified Ox-ford Rating Scale among groups in different time points after treatment(Z>10.476,P<0.05)except the score of ICIQ-SF four weeks after treatment(P>0.05),and they were the worst in group C.BMI(group A=1,group B=2,group C=3)was correlated with the results of one hour pad test(r=0.79,P<0.001),the number of daily uri-nary incontinence(r=0.68,P<0.001),and the scores of ICIQ-SF(r=0.68,P<0.001)and modified Oxford Rating Scale(r=-0.47,P=0.001)six months after treatment.Conclusion Kegel training combined with Pilates training could improve the urinary control in patients with urinary in-continence after prostatectomy.The decrease of BMI can promote the recovery of urinary control,and improve the symptoms of later urinary incontinence in mid-and long-term.
3.The efficacy of plasma gasdermin D C-terminal fragment in the early diagnosis of sepsis

Objective To assess the effectiveness of the Gasdermin D C-terminal fragment(GSDMD-CT)as a novel plasma biomarker for the clinical diagnosis of sepsis.Methods Between July 2021 and November 2024,245 patients from Tangshan Gongren Hospital were enrolled in this study.In accordance with the diagnostic criteria for sepsis and the systemic inflammatory response syndrome(SIRS),patient samples were classified into the sepsis group and the SIRS group.Meanwhile,healthy individuals were selected as the healthy control(HC)group.Sepsis patients were further categorized into the Gram-positive bacterial group,the Gram-negative bacterial group,and the fungal group based on the type of pathogen infection.The levels of GSDMD-CT,C-reactive protein(CRP),and procalcitonin(PCT)were measured in all subjects.Nonparametric tests were employed to compare the differences in various indices among different groups.The diagnostic value of GSDMD-CT in sepsis was evalu-ated by constructing the receiver operating characteristic(ROC)curve.Spearman's correlation analysis was used to examine the relationships among GSDMD-CT,CRP,and PCT.Results The plasma GSDMD-CT levels in the sepsis group 23.02(16.71,33.01)pg/mL and in the SIRS group 16.52(11.26,22.22)pg/mL were significantly higher than those in the healthy control group 7.02(4.42,11.43)pg/mL(U=-10.175,-7.890,P<0.001).Moreover,the plasma GSDMD-CT levels in the sepsis group were significantly higher than those in the SIRS group(U=-2.941,P<0.05).In the Gram-positive bacterial group,the Gram-negative bacterial group,and the fungal group,the GSDMD-CT levels were 23.01(17.16,27.51)pg/mL,23.41(16.78,35.50)pg/mL,and 16.29(14.53,56.27)pg/mL,respectively.When compared with the healthy control group,the GSDMD-CT levels in these three groups were all significantly higher(P<0.05).However,there were no significant differences in GSDMD-CT levels among these three groups(P>0.05).The area under the curve(AUC)of plasma GSDMD-CT for diagnosing sepsis was 0.881(95%confidence interval:0.833~0.929),with a Youden index(YI)of 0.695,a sensitivity of 85.0%,and a specificity of 84.5%.Spearman correlation analysis indicated a weak correlation between GSDMD-CT and C-reactive protein(CRP)(r=0.32,P<0.001)and a positive correlation between GSDMD-CT and procalci-tonin(PCT)(r=0.65,P<0.001).Conclusion GSDMD-CT exhibits significant clinical value in the diagnosis of sepsis and holds great potential as a biomarker in the diagnostic process of sepsis.
4.Research on the protective effect and mechanism of kaempferol on oxidative damage and apoptosis of human lens epithelial cells induced by H2O2

Objective To investigate the effects of kaempferol on the oxidative damage and apoptosis of human lens epithelial cells induced by hydrogen peroxide(H2O2),as well as its regulatory role in the Janus kinase 2/signal transducer and activator of transcription 3(JAK2/STAT3)signaling pathway.Methods Human lens epithelial cells HLE-B3 were cultured in vitro and divided into the blank group(no intervention),the model group(400 pmol?L-1 H2O2),the kaempferol group(400 μmol?L-1 H2O2+40 pmol?L-1 kaempferol),the AG490 group(400 pmol?L-1 H2O2+50 μmol?L-1 JAK2/STAT3 signaling pathway inhibitor AG490),the inhibitor group(400 pmol?L-1 H2O2+40 pmol?L-1 kaempferol+50 μmol?L-1 AG490),and the activator group(400 pmol?L-1 H2O2+40 pmol?L-1 kaempferol+0.5 μmol?L-1 JAK2/STAT3 signaling pathway activator C-A1).After 24 h of intervention,Hoechst 33258 staining was used to determine the ap-optosis rate of these cells.Malondialdehyde(MDA)and superoxide dismutase(SOD)kits were used to detect the levels of MDA and SOD.The protein expression levels of Cysteine aspartic protease 3(Caspase-3),B-cell lymphoma-2(Bcl-2),JAK2,STAT3,p-JAK2,and p-STAT3 were detected by Western blot.Results Compared with the blank group,the apop-tosis rate,the MDA level,and the protein expression levels of Caspase-3,p-JAK2,and p-STAT3 increased(all P<0.05)in the model group,while the protein expression levels of SOD and Bcl-2 decreased(all P<0.05).Compared with the model group,the apoptosis rate,the MDA level,and the protein expression levels of Caspase-3,p-JAK2,and p-STAT3 decreased in the kaempferol and AG490 groups(all P<0.05),while the protein expression levels of SOD and Bcl-2 increased(all P<0.05).Compared with the kaempferol group,the apoptosis rate,the MDA level,and the protein expression levels of Caspase-3,p-JAK2,and p-STAT3 decreased in the inhibitor group,while the protein expression levels of SOD and Bcl-2 in-creased(all P<0.05);the changes in the above indicators in the activator group were opposite to those in the inhibitor group(all P<0.05).Conclusion Kaempferol can protect HLE-B3 cells from H2O2-induced oxidative damage and inhibit their apoptosis by inhibiting JAK2/STAT3 signaling pathway.

Myxedema coma is a rare condition, typically arising from long-standing, untreated hypothyroidism and triggered by factors such as infection, hypothermia, or severe illness. This report details a successfully treated case of myxedema coma with cardiac attest, accompanied by a literature review, to enhance clinical awareness and improve the diagnosis and management of this critical condition.
6.Survival analysis of newly diagnosed non-transplant multiple myeloma patients

Objective:To analyze overall survival (OS), influential factors, and causes of death in patients with newly diagnosed non-transplant multiple myeloma (NDMM) at grassroots hospitals.Methods:The clinical data of 56 patients with NDMM admitted to the Department of Hematology, Wuwei People's Hospital from January 2018 to October 2022 were retrospectively analyzed. Using the Kaplan-Meier and Cox regression survival analysis models, univariate and multivariate analyses were performed to evaluate the factors affecting the OS of patients with NDMM.Results:The median OS of patients with NDMM was 27 months. Univariate analysis indicated that whether or not to receive chemotherapy was a risk factor affecting OS. Multivariate analysis revealed that age ( HR: 2.215, 95% CI 1.056-4.648, P < 0.05), renal dysfunction ( HR: 3.482, 95% CI 1.359-8.923, P < 0.05), and whether or not to receive chemotherapy ( HR: 0.83, 95% CI 0.021-0.331, P < 0.001) were all significant risk factors affecting OS (all P < 0.05). Conclusions:The median survival time of patients with NDMM is short. Age, renal dysfunction, and not receiving chemotherapy are unfavorable factors affecting OS. Pulmonary infection, renal failure, and disease progression are the main causes of death in patients with NDMM.
