1.A case report and literature review of developmental epileptic encephalopathy caused by CACNA1E gene mutation

Objective:To report a case of developmental epileptic encephalopathy caused by CACNA1E gene mutation and explore C Clinical phenotype and prognosis of 2069G>A (p.Gly690Asp) locus.Methods:The clinical data and follow-up of a case of developmental epilepsy related to CACNA1E gene mutation admitted to the Maternity And Children Health Hospital of Xiangtan City in September 2020 were summarized and analyzed.Results:The male patient, with delayed intellectual and motor development, sought medical attention and gradually developed seizures. The genetic results showed that the CACNA1E gene c. 2069G>A (p.Gly690Asp) was a newly discovered variant, and the parental site was a wild-type. Based on clinical manifestations, the diagnosis was a pathogenic variant.Conclusions:Developmental epileptic encephalopathy caused by mutations in the CACNA1E gene can have intellectual and motor impairments as the initial manifestation; Site C 2069G>A (p.Gly690Asp) presents with severe epilepsy and poor prognosis, with the possibility of brain atrophy. The combination of valproic acid and clonazepam may be effective in treating epilepsy.
2.Transconjunctival premaxillary space fat repositioning in lower blepharoplasty

Objective:To investigate the efficacy of transconjunctival orbital fat repositioning into the premaxillary space for the treatment of eyelid bag and tear trough deformity.Methods:A retrospective review of patients undergoing orbital fat repositioning from April 2018 to June 2019 was performed at the Department of Laser and Plastic Surgery, General Hospital of Southern Theater Command, PLA. 68 patients were included in this series with their age ranging from 19 to 53 (5 males and 63 females). Through a transconjunctival incision, blunt dissection of orbital septum and orbicularis oculi was performed along the preseptal plane to just above the arcus marginalis. Both the orbicularis oculi retaining ligament and the tear trough ligament were sharply released with cutting cautery at its bony origin. Premaxillary space was then entered and separated by blunt dissection between orbicularis oculi and sub-orbicularis oculi fat. Pedicled orbital fat was transposed into the space and secured with percutaneous sutures so as to blend the tear tough. Postoperative result were assessed by self-satisfactory scale and Hirmand classification of tear trough deformity. Scores of self-satisfactory scale were determined as 1 to 5 points according to very satisfied, satisfied, acceptable, not satisfied and very unsatisfied degree respectively.Results:The follow-up was (145.8±82.9) d. The self-satisfactory scale scored 4.47±0.82. Number of the class Ⅰ, Ⅱ, Ⅲ patients reduced from 24, 37, 7 to 7, 0, 0 respectively after the surgery. 20 patients with cheek depression had also been greatly improved. 12 patients had undercorrection of the eyelid bags. 2 patients underwent dry eye that was treated with sodium hyaluronate eye drops and healed within one month. One patient had eyelid numbness which resolved spontaneously. Hematoma developed in one patient who underwent hematoma evacuation surgery. No other complications were seen in any patients.Conclusions:Transconjunctival premaxillary space fat repositioning blepharoplasty is a safe and effective approach for treating eye bags, especially for patients with tear trough deformity and midface depression.

This article collects related literatures which is about the Chinese medicine adjuvant treatment of bladder urothelial carcinoma, and sums up the etiology, pathogenesis and TCM auxiliary treatment methods of this disease. Through the analysis, it is believed that the pathogeny of the disease is mainly concentrated in the aspects of damp, heat, blood stasis and poison. The literature on the adjuvant treatment of bladder urothelial carcinoma mainly focuses on the treatment of syndrome differentiation, postoperative recovery, postoperative perfusion, adjuvant chemotherapy and palliative therapy. The progress of its research is summarized as follows.

Stress Urinary Incontinence (SUI) has a higher incidence in middle-aged and elderly women. At present, for mild and moderate SUI patients, the treatment of kegel practice or combined with TCM ,and acupuncture, acupoint application or pelvic floor muscle exercise show benefit effect. This article summarized the etiology and pathogenesis of SUI, TCM therapy, integrated traditional Chinese and Western Medicine, in recent years, in order to provide reference for the treatment of the disease and the further study.
6.A clinical research of Lingshao-Zaoren granule for the female with overactive bladder symptom: a randomized, double-blind, placebo-controlled trial

Objective To evaluate the effectiveness of Lingshao-Zaoren granule in the treatment of female overactive bladder. Methods A total of 60 female OAB patients who met the inclusion criteria were randomly divided into 2 groups, 30 cases in each group. The control group recieved the Tolterodine Tartrate Sustained Release Tablets and the Lingshao-Zaoren granule placebo, and the treatment group used the Tolterodine Tartrate Sustained Release Tablets and the true Lingshao-Zaoren granule treatment. Both groups were treated for 4 weeks. Overactive bladder symptom scale (OABSS) was used to determine the severity of OAB.Results After treatment 14,28 d,OABSS scores of the treatment group(5.3 ± 2.3,1.4 ± 1.2 vs.8.4 ± 2.4,F=137.209),and control group(7.8 ± 1.9,6.8 ± 1.4 vs.8.6 ± 2.6,F=8.927),were significantly lower than the baseline of each group respectively (P<0.01). Besides, OABSS scores of the treatment group after 14 and 28 d were significantly lower than the control group (t=4.668, 15.678, P<0.01). The pain scores (5.9 ± 1.9, 2.7 ± 1.1 vs.9.5 ±2.3,F=108.819)of treatment group at 14 and 28 d were significantly lower than the baseline(P<0.01);and the pain scores of treatment group at 14 and 28 d were significantly lower than the control group (t=6.342, 14.812,P<0.01).The lower abdomen discomfort scores at 14,28 d in treatment group(1.9 ± 1.4,1.1 ± 1.0 vs. 3.3 ±1.1,F=28.762),and control group(2.7 ±1.0,2.4 ±0.8 vs.3.4 ±1.2,F=12.103)were significantly lower than the baseline of each group (P<0.01); and the abdominal discomfort scores of treatment group at 14, 28 d were significantly lower than the control group (t=2.521, 5.041, P<0.05 or P<0.01). Conclusions The Lingshao-Zaoren granule could decrease OABSS score,pain score,abdominal discomfort symptoms,improve clinical symptoms of the female patients with OAB.
7.Effect ofHuayu-Niaoshidecoction on renal injury in the patients with kidney stones after extra corporeal lithotripsy

ObjectiveTo investigate the effect ofHuayu-Niaoshidecoction on renal injury in the patients with kidney stones after extra corporeal lithotripsy (ESWL). MethodsAccording to the inclusion criteria, 120 patients diagnosed with kidney stones were randomly divided into 2 groups with the random number table method, 60 patients in each group. Both groups were treated with double pulse extracorporeal shock wave lithotripsy (ESWL). Based on the ESWL, the control group was added with levofloxacin mesylate tablets for 3 days with the start of the day of the gravel; and the treatment group was treated withHuayu-Niaoshi decoction for 14 days based on the treatment of control group. The levels of serum endothelial endothelin 1 (ET-1), malondialdehyde (MDA), urinary N-acetyl-β-Dglucosaminidase (NAG) N-acetyl-beta-d Glucosaminidase and urine beta 2 microspheres protein (beta eukaryotic 2-mg) were detected at time of gravel, 2 hours after gravel and 2 weeks after gravel. ResultsAfter 2 weeks treatment, the levels of urinary NAG (7.33 ± 4.39 U/L vs. 9.93 ± 5.10 U/L;Z=2.587,P=0.002), serum MDA (4.63 ± 0.90 nmol/mlvs. 7.29 ± 2.16 nmol/ml;Z=5.520, P=0.001), serum ET-1 (35.06 ± 10.51 pg/mlvs. 46.90 ± 18.74 pg/ml;Z=3.315,P=0.001) and urineβ2-MG (0.14 ± 0.19) mg/Lvs. 0.19 ± 0.19 mg/L;Z=1.759,P=0.079) in the treatment group were significantly lower than those in the control group. Conclusions TheHuayu-Niaoshidecoction can improve renal injury in the patients with
kidney stones after extra corporeal lithotripsy.

Male late-onset hypogonadism (LOH) has seriously affected the quality of life in elderly men.The pathogenesis of this disease is related with growing old,endocrine function degradation and hormone levels decreased.The core principle of the western medicine treatment is to supple the androgen,but recent discovery found that long-term androgen supplementation have certain side effects,which will increase the benign prostatic hyperplasia and prostate cancer risk.Traditional Chinese medicine treatment has a unique advantage in treating this disease,it can regulate the body's endocrine and hormone levels,and delay aging.So it can improve the conditions of palpitations,insomnia,irritability,sweating,sexual loss,fatigue,weakness and other symptoms.Now we reviewed the theory of governance,classical prescriptions,self prescription treatment,acupuncture therapy,and exercise therapy from the aspect of traditional Chinese medicine.
9.Separation and Preparation of 5 Phenylpropanoids from Cinnamomum Cassia by Semi-preparative HPLC and Their Immunosuppressive Activity Investigation

Objective:To simultaneously separate 5 phenylpropanoids from Cinnamomum cassia by semi-preparative HPLC, and explore their immunosuppressive activity. Methods:After extracted by ethanol, the ethyl acetate part of Cinnamomum cassia was isola-ted by semi-preparative HPLC. The separation was conducted on an Ultimate XB-C18 (250 mm × 10 mm, 5μm) semi-preparative chro-matographic column and the mobile phase was acetonitrile-water with gradient elution. The detection wavelength was 330 nm, and the flow rate was 2. 5 ml·min-1 . The sample volume was 0. 3 ml. MS and NMR spectroscopic analysis were used to determine the config-urations. A CCK-8 method was used to detect the immunosuppressive activity of phenylpropanoids. Results: Totally 5 phenylpro-panoids were separated from Cinnamomum cassia by the semi-preparative HPLC, and identified as erythro-guaiacylglycerol, (7R,8S)-syringoylglycerol, (7S,8S)-syringoylglycerol, 3-methoxyphenyl-acrylaldehyde and O-methoxy cinnamaldehyde. The inhibitory rates of T cells and B cells of the compound 3 was more than 20% at the concentration of 800μmol·L-1 . Conclusion:The method is conven-ient with good separation effect, which can simultaneously separate 5 phenylpropanoids from Cinnamomum cassia, and among them, the compound 3 shows immunosuppressive effect to some extent.
10.Efficacy observation on compound Tufuling tablets in the treatment of chronic prostatitis due to damp heat stagnation

Objective To evaluate the clinical curative effect of compoundTufuling tablets in the treatment of chronic prostatitis due to damp heat stagnation.Methods A total of 60 patients with chronic prostatitis due to damp heat stagnation were selected from July 2012 to July 2013 in Urology Outpatient of Urology, Xiyuan Hospital and recruited randomly divided into 2 groups, 30 patients in each group. The treatment group was treated with compound SmilaxTufuling tablets, while the control group was treated with Qianliexin capsule. The National Institutes of Health Chronic Prostatitis Symptom Index (NHI-CPSI), and the TCM symptoms score for chronic prostatitis were used to evaluate the treatment outcome.Results The total efficiency in the treatment group was significant higher than that in the control group (89.3%vs. 64.3%;χ2=4.909,P=0.027). After treatment , the NIH-CPSI total score (6.00 ± 2.36vs. 13.61 ± 2.91;t=10.748, P=0.000), and the subscales of pain or discomfort (2.54 ± 1.14vs. 4.82 ± 1.47;t=6.486,P=0.000), urination (1.61 ± 1.45vs. 4.79 ± 2.75;t=5.413,P=0.000), the quality of life (1.86 ± 1.38vs. 7.00 ± 1.36;t=14.038, P=0.000), TCM symptoms score (8.36 ± 3.03vs. 5.32 ± 2.74;t= 9.015,P=0.000) in the treatment group were decreased than those in the control group.Conclusions CompoundTufuling tablets can significantly decrease the NIH-CPSI scale and the TCM symptoms score in patients with chronic prostatitis due to damp heat stagnation, boosting satisfactory clinical effects.
