As the core vehicle for preserving and transmitting traditional Chinese medicine(TCM) academic thought and clinical experience, the establishment of a robust quality evaluation system for TCM clinical case reports is a crucial component in the current standardization and modernization of TCM. Based on the practical experience of constructing the China Clinical Cases Library of Traditional Chinese Medicine by the China Association of Chinese Medicine, this study conducted a comprehensive analysis of critical challenges, including insufficient authenticity and unfocused evaluation criteria. It proposed a three-dimensional evaluation framework grounded in the structure-process-outcome logic, encompassing three dimensions of authenticity and standardization, characteristics and advantages, application and translational impact. This framework integrated 12 key evaluation indicators in a systematic manner. The model preserved the academic characteristics of TCM syndrome differentiation and treatment, while aligning with modern scientific research standards, achieving a balance between individualized TCM experience and standardized evaluation. Concurrently, this study provided theoretical foundations and methodological guidance for evaluating the quality of TCM clinical cases, contributing significantly to the inheritance of TCM knowledge, evidence-based practice, and the reform of talent evaluation mechanisms.
2.Effect of ultrasound-guided intercostal nerve block on stress response and postoperative pain in patients with transaxillay gasless endoscopic thyroidectomy

Objective To observe the effect of ultrasound-guided intercostal nerve block(ICNB)on stress response and postoperative pain in patients with transaxillay gasless endoscopic thyroidectomy(TGET).Methods A total of 60 patients who underwent TGET at Ganzhou People's Hospital from January 2023 to December 2024 were enrolled as subjects,they were divided into ICNB group(n=30)and control group(n=30),by using a random number table method.Intraoperative anesthetic dosage,pain scores within 24 hours postoperatively,levels of prostaglandin E2,norepinephrine,and cortisol,as well as the incidence rates of postoperative adverse reactions including nausea and drowsiness were compared between two groups.Results The dosage of propofol and remifentanil in ICNB group during the operation was lower than that in control group.Prince-Henry score 24h after the operation in ICNB group was lower than that in control group.Levels of prostaglandin E2,norepinephrine and cortisol in ICNB group were all lower than those in control group,and the difference was statistically significant(P<0.05).The incidences of postoperative nausea and drowsiness in ICNB group were lower than those in control group.Conclusion Ultrasound-guided ICNB for TGET can reduce the use of anesthetic drugs during the operation,alleviate patient's stress response and postoperative pain,and lower the incidence of postoperative adverse reactions.
3.Preliminary application of MRI image fusion technology in guiding percutaneous balloon compression for trigeminal neuralgia

Objective To discuss the technical advantages of MRI image fusion technology in guiding percutaneous balloon compression(PBC)for trigeminal neuralgia(TN).Methods The clinical data of 13 patients with TN,who received MRI image fusion technology-guided PBC from November 2022 to July 2023,were retrospectively analyzed.The MRI images of the trigeminal nerve obtained one week before surgery were fused with the intraoperative DynaCT images of the skull base so as to simultaneously display the Meckel s cave and foramen ovale,and under the 3-D view the optimal puncture path was determined.The needle was positioned at the entry point of the skin,than the skin was cut open with a sharp surgical blade and the needle was inserted to the foramen ovale area to a predetermined depth.Lateral skull base fluoroscopy and DynaCT scan were used to check that the puncture needle tip was placed into the foramen ovale.Than the puncture needle was replaced by a fine guiding-needle and it was pushed into the Meckel's cave.Under the dual guidance of lateral fluoroscopy and fusion image,the balloon was push forward and was filled with iodine contrast media to compress the trigeminal ganglion within the Meckel's cave.After completion of the treatment,the balloon and puncture needle were removed and manual oppression was applied on the face to achieve hemostasis.Results Immediately after PBC,the pain was relieved in all patients.No permanent or serious complications occurred.One patient had a relapse 3 months after PBC and a second PBC procedure had to be carried out.No obvious pain recurrence was observed in the remaining patients during follow-up period.During the surgery,the mean number of foramen ovale puncturing was(1.31±0.46)times,the mean X-ray exposure time was(8.64±5.66)min,and the mean cumulative dose of X-ray was(570.29±257.15)mGy.After PBC,12 patients(92.31%)developed facial numbness and one patient(7.69%)developed facial pain,all of which were healed after treatment.Conclusion MRI image fusion technology can improve the visualization and accuracy of PBC procedure.It can also reduce the number of puncturing,decrease the radiation exposure dose,and improve the surgical ability of young doctors.Therefore,MRI image fusion technology should be further developed and applied in clinical practice.

5.Comparison of reconstructive effects and satisfaction analysis between latissimus dorsi myocutaneous flap and prosthesis implantation for breast reconstruction in breast cancer

Objective:Comparative clinical outcome and satisfaction analysis of latissimus dorsi breast reconstruction versus prosthetic breast reconstruction in the surgical management of female breast cancer.Methods:We retrospectively analysed 102 female breast cancer patients diagnosed in the Department of Breast Surgery of the Affiliated Hospital of Guangdong Medical University between Jan. 2020 and Dec. 2022, and collected clinical data from the latissimus dorsi muscle reconstruction group and the prosthesis reconstruction group, comparing them in terms of operation time, postoperative complications, average length of hospital stay, total hospitalization cost, postoperative satisfaction, and cosmetic effect.Results:Patients in both the latissimus dorsi muscle group and the prosthesis group underwent successful surgery. There were significant statistical differences between the two groups in terms of nipple preservation, radiotherapy, surgery time, average postoperative hospitalization time, total hospitalization costs, overall satisfaction, and social satisfaction ( P<0.001) . Conclusions:In female breast cancer, latissimus dorsi reconstruction has fewer complications and higher satisfaction than prosthetic reconstruction, and it can be popularized in the clinic.
6.Construction of a global model of cardiac surface motion based on average intensity projection image

Objective To construct a global model of cardiac surface motion based on average intensity projection(AIP)image.Methods The cine magnetic resonance imaging data were divided into training set and test set for model construction and validation.The datum points were obtained on the AIP surface point cloud,and the corresponding points of each cardiac phase surface point cloud were found based on surface features.Principal component analysis was used to extract feature information,establish the mapping relationship between the datum points and the corresponding points,and construct a model for predicting each temporal phase surface point cloud from the AIP surface point cloud.Results The RMSE of the model on test set for corresponding point prediction ranged from(0.209±0.020)mm to(0.841±0.074)mm,while the Euclidean distance for each time phase surface point cloud prediction ranged from(1.399±0.029)mm to(1.658±0.100)mm.Conclusion The proposed global model exhibit high accuracy and can provide a reference for image segmentation and clinical treatments.

8.Efficacy and safety of clear lens extraction with extended depth of focus intraocular lens implantation in the treatment of primary angle-closure suspect with presbyopia

Objective To assess the efficacy of clear lens extraction combined with extended depth of focus(EDOF)intraocular lens(IOL)implantation in enhancing visual quality and concurrently opening peripheral angle structures in patients diagnosed with primary angle closure suspect(PACS)and presbyopia.Methods A total of 80 patients with PACS,presbyopia,and transparent lenses were enrolled and divided into two groups based on the type of implanted lens:a control group(n=46)and an experimental group(n=34).The control group underwent clear lens extraction followed by monofocal intraocular lens implantation,while the experimental group underwent clear lens extraction followed by extended depth of focus(EDOF)intraocular lens implantation.Preoperative and postoperative evaluations at 3 months included visual acuity,intraocular pressure,anterior segment structure,defocus curve,and Catquest 9SF-CN questionnaire scores.Adverse events were recorded to assess safety.Results Regarding the angular structure,after 3 months of treatment,results indicated significant increases in central ante-rior chamber depth,angle opening distance at 750 μ m,angle recess area at 750 μ m,trabecular-iris space area at 750 μm,and trabecular-iris angle at 750 μm in both groups compared to preoperative values(all P<0.01).However,no significant differences were observed between the two groups(all P>0.05).For the defocusing curve,visual acuity in the experimental group improved from+1.0 D to-4.0 D after 3 months of treatment(all P<0.05),while the control group showed improvements at+0.5 D,-0.5 D,-1.0 D,-2.0 D,-3.0 D,-3.5 D,and-4.0 D(all P<0.05).Postoperatively at 3 months,the experimental group exhibited superior visual acuity to the control group at-1.0 D to-4.0 D(all P<0.05).According to the Catquest 9SF-CN scale,scores in both groups significantly im-proved after 3 months of treatment(P<0.01),with the experimental group scoring higher than the control group(P<0.01).During the study,transient intraocular pressure elevation occurred in 7 patients(3 in the control group and 4 in the experimental group),but no serious adverse reactions were reported in either group.Conclusions In patients with pseudoexfoliation glaucoma combined with presbyopia,significant improvements in angle structures were observed following clear lens extraction.The implantation of an EDOF IOL notably enhanced postoperative visual quality compared to monofocal IOLs.
9.BMP-2 gene-modified bone marrow mesenchymal stem cells combined with nano-hydroxyapatite / polyamide 66 implants improve atrophic nonunion of bone in rats

Objective:
To investigate the therapeutic effect of bone marrow mesenchymal stem cells ( BMSCs) mod- ified by bone morphogenetic protein ( BMP) -2 gene combined with nano-hydroxyapatite ( nHA) / polyamide 66 ( PA66) on femoral nonunion in rats.
Methods :
Rat BMSCs were isolated and divided into the stent + BMSCs group,stent + BMSCs + rhBMP-2 group,and stent + BMSCs + Ad-BMP-2 group; human recombinant BMP-2 ( rh- BMP-2) or adenovirus-infected BMP-2 ( Ad-BMP-2) vector was transfected into BMSCs and loaded onto nHA / PA66 stent materials.Flow cytometry was used to detect that BMSCs expressed specific surface markers.Cell pro- liferation was detected by MTT assay,related bioactive factors[platelet-derived growth factor ( PDGF) ,transfor- ming growth factor-β ( TGF-β) ,vascular endothelial growth factor ( VEGF) ,fibroblast growth factor ( FGF) ,os- teocalcin ( OCN) ,osteonectin ( ON) ]were detected by ELISA,alkaline phosphatase ( ALP) activity was detec- ted,and cell growth and adhesion were observed by scanning electron microscopy ( SEM) .In the atrophic nonun- ion model of SD rats,the stent + BMSCs + rhBMP-2 or stent + BMSCs + Ad-BMP-2 complex was implanted into the defect area,which was divided into the rhBMP-2 group and the Ad-BMP-2 group,and the therapeutic effect was e- valuated by X-ray and MicroCT.
Results:
The surface of the nHA / PA66 stent was smooth and porous,and BMSCs adhered well.Flow cytometry showed high expression of CD29 and CD90 and low expression of CD45 and CD34 in BMSCs.MTT showed that the cells proliferated rapidly after 72 h.Compared with the stent + BMSCs group and the stent + BMSCs + rhBMP-2 group,the ALP activity,the expressions of PDGF,TGF-β , VEGF,FGF,OCN,and ON in the stent + BMSCs + Ad-BMP-2 group were significantly up-regulated ( P<0. 05) .12 weeks after surgery, the stent complex in the Ad-BMP-2 group of rats was completely wrapped by newly formed cortical bone,and the surface was smooth and intact.X-ray showed that the complex in the Ad-BMP-2 group was completely wrapped by newly formed cortical bone,and the recovery degree was better than that in the rhBMP-2 group.Micro-CT results showed that compared with the rhBMP-2 group,the bone volume fraction,trabecular thickness,trabecular num- ber,bone mineral density,and bone mineral content in the Ad-BMP-2 group all increased 12 weeks after surgery ( P<0. 05) ,and the trabecular separation level decreased ( P<0. 05) .
Conclusion
Ad-BMP-2-transfected BM- SCs combined with nHA / PA66 stent material can express bioactivity more effectively,provide a continuous and good microenvironment for the proliferation and differentiation of BMSCs,which is beneficial to promoting local os- teogenic activity and bone defect repair.
10.A case report on β-electrode combined with laparoscopy in vesicovaginal fistula repair and review of the literature

Objective:To explore the technique methods and clinical application value of β-electrode (a plasma needle shape electrode) assisted laparoscopic repair of complex vesicovaginal fistula (VVF).Methods:Clinical data of one patient with complex VVF admitted to Chinese PLA General Hospital in April 2025 was retrospectively analyzed. A 36-year-old female presented with urinary leakage 2 months after hysterectomy. Computed tomography urography excluded ureterovaginal fistula. Cystoscopy revealed a 2 cm fistula on the posterior bladder wall with both ureteral orifices adjacent to the fistula edge. The procedure involved two steps: first, transurethral β-electrode pretreatment was performed to protect the ureteral orifices and create a passage from the bladder to the abdominal cavity. Then, laparoscopic separation, suture closure of the fistula, and omental flap coverage were conducted.Results:Total operation time was 180 min (the time of β- electrode operation was 30 min) with intraoperative estimated blood loss of 50 mL. The catheter was removed 3 weeks postoperatively, and the patient voided well without leakage during 4-month follow-up.Conclusions:β- electrode assisted laparoscopic repair of complex VVF have the advantage of precise manipulation, minimal invasion and rapid recovery. No similar technique have been reported domestically or internationally. This technique provides a new approach for the treatment of complex VVF.
