1.Comparative study on accuracy of three imaging methods in diagnosis of subacromial impingement syndrome.

OBJECTIVE:
To compare the diagnostic accuracy of supraspinatus muscle outlet X-ray film, oblique sagittal multislice helical CT (MSCT), and oblique sagittal MRI in the diagnosis of subacromial impingement syndrome (SIS).
METHODS:
A retrospective analysis was conducted on the imaging data of 106 patients diagnosed with SIS between January 2023 and December 2024. The cohort consisted of 32 males and 74 females, with ages ranging from 43 to 70 years (mean, 60.19 years). All patients underwent supraspinatus muscle outlet X-ray film, MSCT, and MRI scans, with MSCT further subjected to three-dimensional reconstruction. Two experienced radiologists independently evaluated the acromion morphology in each imaging modality using the Bigliani classification system. Inter-observer reliability was assessed via Kappa statistics. The CT three-dimensional reconstructions were used as the "gold standard". The overall consistency, Kappa values, sensitivity, and specificity of the three imaging modalities were calculated. Receiver operating characteristic (ROC) curves were plotted, and the area under the curve (AUC) was computed.
RESULTS:
The inter-observer reliability for supraspinatus muscle outlet X-ray film, oblique sagittal MSCT, and oblique sagittal MRI was moderate, with Kappa values of 0.62, 0.63, and 0.55, respectively. When compared to the CT three-dimensional reconstructions as the "gold standard", the overall consistency was 88.7% (94/106), 62.3% (66/106), and 58.5% (62/106), respectively. The supraspinatus muscle outlet X-ray film showed excellent consistency (Kappa=0.77), whereas the consistency of MSCT and MRI was lower (Kappa=0.34 and 0.29, respectively). In terms of diagnostic sensitivity and specificity, the supraspinatus muscle outlet X-ray film outperformed oblique sagittal MSCT and oblique sagittal MRI in distinguishing various acromion types. ROC analysis demonstrated that the AUC for the supraspinatus muscle outlet X-ray film was consistently higher than for oblique sagittal MSCT and oblique sagittal MRI, with the highest diagnostic performance observed for type Ⅲ hooked acromion (AUC=0.939).
CONCLUSION
Supraspinatus muscle outlet X-ray film provides the highest diagnostic accuracy for acromion classification in SIS patients, particularly in identifying type Ⅲ hooked acromion, which is strongly associated with SIS. Given its superior sensitivity and consistency, it should be considered the primary screening tool. MSCT and MRI serve as valuable supplementary modalities for complex cases and preoperative evaluation.

2.Research on the safety management for risk identification model based on DBN in precision medical research equipment of central laboratory

Objective:To construct a risk identification model based on the Dynamic Bayesian Network(DBN)and explore its safety management effect in the precision medical research equipment of the central laboratory.Methods:Based on the DBN model,a risk identification model was constructed after various risk factors in the management for precision medical research equipment of central laboratory were identified,and a risk index evaluation set was constructed to provide early warning and effective prevention of potential risks during the operation stage of the equipment.A total of using 51 precision medical research equipment in central laboratory of Shuguang Hospital of Shanghai University of Traditional Chinese Medicine from February 2023 to February 2024 were selected.According to different management modes,26 of them were managed by the conventional management mode,and 25 were managed by the DBN risk identification model management mode(DBN management mode).A self-made questionnaire on equipment management recognition was used to conduct a survey among 15 engineers,15 equipment operators and 10 doctors involved in equipment use.The equipment target achievement degree,clinical service quality,equipment risk incidence rate and the recognition scores of personnel related to equipment use for equipment management under the two management modes were compared.Results:The scores of equipment operation standardization,disinfection and cleaning completion and emergency management timeliness under the DBN management mode were(91.58±3.36),(90.58±3.69)and(93.69±4.25)points respectively,all of which were higher than those under the conventional management mode,and the differences were statistically significant(t=19.466,15.704,15.549,P<0.05).The average values of equipment execution rate,quality inspection qualification rate and fault handling rate were(92.69±3.69)%,(93.27±3.01)%and(94.57±3.65)%respectively,all of which were higher than those under the conventional management mode,and the differences were statistically significant(t=13.811,18.401,20.374,P<0.05).The equipment failure rate,component damage rate and unqualified rate of cleaning and disinfection were 4%(1/25),8%(2/25)and 4%(1/25)respectively,all of which were lower than those under the conventional management mode,and the differences were statistically significant(x2=10.395,12.829 and 13.542,P<0.05).The average recognition scores of engineers,operators and doctors involved in equipment use for the DBN management mode were(90.26±4.65),(91.54±4.62)and(93.25±3.65)points respectively,all of which were higher than those under the conventional management mode,and the differences were statistically significant(t=12.910,12.379,18.328,P<0.05).Conclusion:The application of the DBN-based risk identification model in the management for precision medical research equipment of central laboratory of hospitals can enhance the operation quality and efficiency of equipment,reduce the risk of equipment use,and improve the equipment service level.
3.Empirical study of the effects of a general-specialty hierarchical management mode for chronic heart failure: a randomised controlled trial

Objective:To explore and demonstrate the effect of general-specialty hierarchical management mode for chronic heart failure (CHF) in community.Methods:This was a single-blind, randomized, controlled study. A total of 530 CHF inpatients who attended Weifang Community Health Service Center (WCHSC) in Pudong New Area from February 2018 to September 2019 were consecutively enrolled. A random number table method was used to divide the patients into the management group ( n=265) and control group ( n=265). The demographic data and past medical history were collected 1 day before enrolment (baseline), and patients were assessed for New York Heart Association (NYHA) cardiac function classification and tested for blood N-terminal B-type natriuretic peptide proteins (NT-proBNP) levels, while Doppler echocardiography was performed to obtain the relevant indexes. The management group used a comprehensive management mode, co-delivered by both WCHSC (offering primary care) and RHSJUSM (offering specialty care) at Renji-Weifang CHF Studio in WCHSC , using the jointly developed CHF hierarchical CHF diagnosis and treatment criteria and referral procedure under the condition of sharing drugs and laboratory test results for CHF. The control group received routine heart failure care. Intergroup comparisons were made on baseline data obtained before follow-up and on NT-proBNP , left ventricular ejection fraction (LVEF), NYHA functional class , re-hospitalization rate and mortality rate at the end of the 6-month follow-up. Results:A total of 506 cases completed the follow-up. There were 253 patients in the management group, aged (68.26±9.41) years, 117 males (46.2%); 253 were in the control group, aged (66.98±9.63) years, 115 males (45.5%). There were no statistically significant differences in age, sex, marital status, education level, and comorbidities between the two groups (all P>0.05). At baseline, the differences in LVEF and NT-proBNP between the two groups were not statistically significant (all P>0.05), and at 6 months of intervention, LVEF, and NT-proBNP had significantly improved in both groups (all P<0.05) . Moreover, LVEF was higher in the management group than in the control group, and NT-proBNP was lower than in the control group (both P<0.01). At baseline, there were 166 cases (65.6%) in the control group with NYHA class Ⅲ/Ⅳ, and 145 cases (57.3%) in the comprehensive management group. There was no statistically significant difference between the two groups ( P>0.05). At 6 months of intervention, the percentage of NYHA class Ⅲ/Ⅳ patients in the comprehensive management group was lower than at baseline ( P<0.01),while that in the control group was higher than at baseline ( P<0.01), and the comprehensive management group was lower than that in the control group ( P<0.01). During the follow-up period, the rehospitalization rate for CHF in the management group was 13.83%(35/253), which was lower than that in the control group, which was 26.88%(68/253) ( P<0.001). Conclusion:The comprehensive management mode of CHF in the community through collaboration between general and specialized departments can significantly improve the management effect, suggesting that this mode is effective and can be promoted.
4.Research progress of decellularized extracellular matrix in the field of tissue engineering in thoracic and cardiac surgery

The extracellular matrix provides a unique tissue-specific microenvironment for resident cells, supporting the essential functions required for tissue architecture and biochemical signaling. Decellularized extracellular matrix (dECM) is designed to eliminate cells that mediate immunological rejection while preserving the native tissue structure and matrix functionality. dECM has attracted significant attention in tissue engineering applications and has evolved into a novel and increasingly sophisticated biomaterial. This article summarizes representative protocols for decellularization methods, explores the latest applications of decellularized tissue-derived materials and bioinks in the field of cardiothoracic surgery, analyzes the current challenges and issues confronting dECM, and discusses future perspectives for its development.

In cases where a tracheal injury exceeds half the length of the adult trachea or one-third of the length of the child trachea, it becomes difficult to perform end-to-end anastomosis after tracheal resection due to excessive tension at the anastomosis site. In such cases, tracheal replacement therapy is required. Advances in tissue engineering technology have led to the development of tissue engineering tracheal substitutes, which have promising applications. Hydrogels, which are highly hydrated and possess a good three-dimensional network structure, biocompatibility, low immunogenicity, biodegradability, and modifiability, have had wide applications in the field of tissue engineering. This article provides a review of the characteristics, advantages, disadvantages, and effects of various hydrogels commonly used in tissue engineering trachea in recent years. Additionally, the article discusses and offers prospects for the future application of hydrogels in the field of tissue engineering trachea.
6.Clinical efficacy of antagonistic needling therapy on post-stroke lower limb spasticity and its effect on muscle morphology.

OBJECTIVE:
To observe the effects of antagonistic needling therapy on lower limb spasticity and the muscle morphology of the tibialis anterior and gastrocnemius in patients with stroke.
METHODS:
A total of 100 patients with post-stroke lower limb spasticity were randomly divided into an antagonistic needling group (50 cases, 1 case dropped out) and a routine acupuncture group (50 cases, 1 case dropped out). Both groups received basic treatment and rehabilitation training. The routine acupuncture group was treated with scalp acupuncture at anterior oblique line of vertex-temporal and vertex lateral line 1, combined with body acupuncture at Jianyu (LI15), Hegu (LI4), Zusanli (ST36), Taichong (LR3), etc. on the affected side, with Quchi (LI11) and Hegu (LI4), Zusanli (ST36) and Fenglong (ST40), Yanglingquan (GB34) and Taichong (LR3) connected to an electroacupuncture device, using disperse wave at 2 Hz of frequency. The antagonistic needling group used the same scalp and upper limb acupoints as the routine acupuncture group, with additional antagonistic needling on the lower limb at Yanglingquan (GB34), Qiuxu (GB40), Jiexi (ST41), and Xuanzhong (GB39) on the affected side, with Quchi (LI11) and Hegu (LI4), Yanglingquan (GB34) and Qiuxu (GB40), Jiexi (ST41), and Xuanzhong (GB39) connected to an electroacupuncture device, using disperse wave at 2 Hz of frequency. Both groups received treatment once daily for 6 consecutive days per course, with a total of 4 courses. The modified Ashworth scale (MAS), Holden functional ambulation classification (FAC), lower limb Fugl-Meyer assessment (FMA), composite spasticity scale (CSS), and musculoskeletal ultrasound parameters (thickness and fiber length of the tibialis anterior and gastrocnemius, and pennation angle of the gastrocnemius on both sides) were evaluated before and after treatment. Clinical efficacy was compared between the two groups.
RESULTS:
Compared before treatment, the MAS grades and CSS scores were decreased in both groups after treatment (P<0.01), with greater reductions in the antagonistic needling group (P<0.05, P<0.01). FAC grades and FMA scores were increased in both groups after treatment (P<0.01, P<0.05), with greater improvements in the antagonistic needling group (P<0.05). The muscle thickness, fiber length of the tibialis anterior, the muscle thickness, fiber length and pennation angle of the gastrocnemius on the affected side were improved in both groups after treatment (P<0.01), with greater improvements in the antagonistic needling group (P<0.01, P<0.05). On the unaffected side, these parameters were also increased after treatment in both groups (P<0.01, P<0.05), but the antagonistic needling group showed smaller increases than the routine acupuncture group (P<0.01, P<0.05). The total effective rate in the antagonistic needling group was 91.8% (45/49), higher than 81.6% (40/49) in the routine acupuncture group (P<0.05).
CONCLUSION
Antagonistic needling could effectively reduce spasticity, improve motor function, and enhance muscle structure in patients with post-stroke lower limb spasticity.

8.Impact of miR-4674 expression changes on the biological characteristics of BGC-823 gastric cancer cell line

Objective To explore the impact of miR-4674 expression changes on the biological characteristics of BGC-823 gastric cancer cell line based on bioinformatics research. Methods Through bioinformatics screening, miR-4674 was prioritized as a gastric cancer-associated miRNA. We constructed miR-4674 mimic, inhibitor, and corresponding negative control (NC) transfected into the BGC-823 cell line. Reverse transcription-polymerase chain reaction (RT-PCR) method was used to detect the expression changes of miR-4674 in BGC-823 cells. Functional assays included: MTT assay for cell proliferation ability; Transwell assay for migration capacity; TUNEL staining for cell apoptosis detection.Results Compared with the control group and cells transfected with NC, the level of miR-4674 was significantly increased in cells transfected with miR-4674 mimic, and the proliferation and migration abilities of the cells were significantly improved (P< 0.05). In cells transfected with miR-4674 inhibitor, the level of miR-4674 was significantly decreased, and the proliferation and migration abilities of the cells were significantly reduced (P<0.05). The result of TUNEL showed no significant differences in apoptotic rates were observed across all groups. Conclusions In the BGC-823 gastric cancer cell line, inhibition of miR-4674 expression can reduce its malignancy, while overexpression of miR-4674 can enhance its malignancy, suggesting its potential as a therapeutic target for gastric cancer intervention, providing a new strategy for the treatment of gastric cancer.
9.Application of the combined tumor burden score and platelet-albumin-bilirubin score model for predicting postoperative tumor recurrence in liver transplant recipients with hepatocellular carcinoma

Objective To investigate the predictive value of the combined tumor burden score (TBS) and platelet-albumin-bilirubin (PALBI) score model for postoperative tumor recurrence in liver transplant recipients with hepatocellular carcinoma (HCC). Methods The general information of 158 recipients diagnosed with HCC and underwent liver transplantation at the 900th Hospital of the Joint Logistics Support Force of the Chinese People's Liberation Army from 2008 to 2021 was collected. Lasso regression analysis combined with multivariate Cox regression analysis were used to identify independent risk factors for postoperative tumor recurrence after liver transplantation with HCC. A nomogram prediction model was constructed based on variables selected by Lasso regression analysis, and the predictive performance of the model was verified by calibration curve and clinical decision curve. The optimal cut-off values for postoperative tumor recurrence in liver transplant recipients with HCC were determined by receiver operating characteristic (ROC) curve, and Kaplan-Meier analysis was used to compare survival differences among different groups. Results Among the 158 liver transplant recipients with HCC, 82 experienced tumor recurrence, with a recurrence rate of 51.9% and a median tumor-free survival time of 10 (4, 25) months. Results of Lasso regression analysis and multivariate Cox regression analysis showed that alpha-fetoprotein (AFP) ≥400 ng/mL, TBS and PALBI score were all independent risk factors for postoperative tumor recurrence in liver transplant recipients with HCC (all P<0.05). The combined high TBS-high PALBI score showed the highest predictive value (hazard ratio 6.909, 95% confidence interval 3.067-15.563, P<0.001). A nomogram prediction model was constructed based on six variables selected by Lasso regression analysis. Calibration curve showed good consistency between the model's predicted results and the ideal curve. Decision curve analysis indicated that the nomogram prediction model provided the highest clinical benefit for predicting 1-year tumor-free survival after liver transplantation with HCC. Time-dependent ROC curves at 1, 3 and 5 years after surgery showed that TBS-PALBI model had good predictive performance, with no significant difference in area under the curve (AUC) compared with TBS-PALBI-AFP model. The optimal cut-off values for predicting postoperative tumor recurrence were determined by ROC curve, with a PALBI score cut-off of −2.334 and a TBS cut-off of 5.305. Recipients were divided into a low TBS-low PALBI score group (n=47) and a low/high TBS-low/high PALBI score group (at least one score was high) (n=111). Kaplan-Meier survival analysis showed that the low TBS-low PALBI score group had a higher tumor-free survival rate than the low/high TBS-low/high PALBI score group, with a significant difference (P<0.05). Conclusions TBS-PALBI model provides a novel, simple and effective tool for assessing the prognosis of liver transplant recipients with HCC. The nomogram model constructed based on this has significant advantages in predictive performance and may serve as a reference for guiding individualized treatment plans and improving clinical outcomes.
10.Impact of Bias Plan dose superposition on postoperative radiotherapy planning for left breast cancer

Objective To investigate the dosimetry effect of Sum Plan and Bias Plan in the Monaco planning system for the addition of compensatory agents after left breast cancer surgery.Methods Twenty-nine patients with radical left mastectomy who received radiotherapy in this hospital from March 2023 to Feb-ruary 2024 were selected as the study objects.Based on the Monaco planning system and under the same opti-mal conditions,Sum Plan and Bias Plan were used to design the second-course plan based on the first-course plan.Sum Plan C1 and Bias Plan C2 were generated to compare the dosimetry differences of intensity modula-ted radiotherapy(IMRT)plans under the two dosimetric superposition methods.Results Compared with Sum Plan C1,the conformability index(CI)in the planned target area(PTV)of Bias Plan C2 was worse,the homogeneity index(HI)was better,and the mean cardiac dose(Dmean),V5,V10,V30,V40 in the organs at risk were better than that of Sum Plan C1.The Dmean,V5,V20 and contralateral V5 in the affected lung were lower,while the V5,V10,V15,V20,V25,V30 in the normal tissue were lower,while V35 was higher,and the number of subfields and machine hops were more,the difference was statistically significant(P<0.05).Conclusion Bias Plan dose overlay method was proposed in the design of segmental radiotherapy with compensator after left breast cancer surgery.
