1.Predictive value of changes in serum VIP and 5-HT levels for the outcome of spinal cord electrical stimulation in patients with postherpetic neuralgia

Objective To investigate the predictive value of serum vasoactive intestinal peptide(VIP)and 5-hydroxytryptamine(5-HT)levels on the outcome of spinal cord electrical stimulation(SCS)in patients with postherpetic neuralgia(PHN).Methods A total of 96 PHN patients who received SCS treatment in Ziy-ang Central Hospital from January 2022 to December 2023 were selected.According to the disease outcomes of all PHN patients after 6 months of treatment,a good group(n=71)and a poor group(n=25)were set up.The clinical data of the two groups were collected and the serum VIP and 5-HT levels were detected in all pa-tients before treatment.The predictive value of serum VIP and 5-HT on disease outcome after SCS treatment in PHN patients was evaluated by receiver operating characteristic(ROC)curve,and the influencing factors of disease outcome after SCS treatment in PHN patients was explored by multivariate Logistic steppe gression a-nalysis.Results The levels of serum VIP and 5-HT in poor group were higher than those in good group(P<0.05).The area under the curve(AUC)of serum VIP and 5-HT for predicting the disease outcome of PHN patients after SCS treatment were 0.829(95%CI:0.779-0.874)and 0.743(95%CI:0.693-0.793),respec-tively,and the AUC of combined prediction was 0.941(0.891-0.986).There were no significant differences in age,gender,body moss index,education,location of onset,hypertension and drinking history between the two groups(P>0.05).The time of initial hospital admission in the poor group was longer than that in the good group,skin rash area in the poor group was larger than that in the good group,and diabetes mellitus and smoking history in the poor group were higher than those in the good group(P<0.05).The time of admis-sion for initial treatment>3 d(OR=2.188,95%CI:1.383-3.461),skin rash area>10 cm2(OR=2.018,95%CI:1.283-3.173),diabetes mellitus(OR=2.264,95%CI:1.379-3.717),serum VIP level ≥41.78 ng/L(OR=3.022,95%CI:1.685-5.420),serum 5-HT level ≥99.27 ng/mL(OR=3.579,95%CI:1.885-6.793)were the influencing factors of disease outcome after SCS treatment in PHN patients(P<0.05).Con-clusion The elevated levels of serum VIP and 5-HT before treatment are associated with poor outcomes after SCS in patients with PHN,and could be used as potential markers to predict the outcomes of SCS in patients with PHN.
2.Clinicopathological features and surgery-related outcomes of duodenal adenocarcinoma: a multicenter retrospective study

Objective:This multicenter retrospective study aimed to analyze the clinicopathological features of duodenal adenocarcinoma (DA) and identify prognostic factors for postoperative survival.Methods:Demographic characteristics, clinicopathological features, treatment outcomes and survival of DA patients undergoing surgical treatment at 18 Chinese medical centers from January 2012 to December 2023 were retrospectively analyzed.Results:Among the 2 056 DA patients included, 46.8% (963) had extra-ampullary DA (EA-DA), and 53.2% (1 093) had peri-ampullary DA (PA-DA). The 1-, 3-, and 5-year overall survival (OS) rates for patients who underwent radical surgery were 93.2%, 71.0%, and 57.2%, respectively. The median overall survival was 76 months, and the median progression-free survival (PFS) was 65 months. No differences in survival were observed between the laparotomy group and minimally invasive surgery (MIS) group either before or after propensity score matching (OS: 76 vs. 75 months before PSM, P=0.986; OS: 75 vs. 75 months after PSM, P=0.602). Furthermore, there were no significant differences between-group in operation time and postoperative complications ( P>0.05). The MIS group experienced less intraoperative blood loss and shorter hospital stays. The multivariate Cox regression analysis revealed that advanced age ( HR=1.43,95% CI:1.18-1.73), elevated carbohydrate antigen 19-9 levels ( HR=1.24,95% CI:1.02-1.51), perineural invasion ( HR=1.44,95% CI:1.14-1.81), vascular invasion ( HR=1.35,95% CI:1.07-1.71), advanced T stage (T3-4 vs. T1-2: HR=1.86,95% CI:1.49-2.31), regional lymph node metastasis ( HR=1.93,95% CI:1.58-2.36), preoperative biliary drainage ( HR=1.26,95% CI:1.04-1.53), intraoperative blood loss ( HR=1.34,95% CI:1.11-1.62), clinically significant postoperative pancreatic fistulas ( HR=1.53,95% CI:1.12-2.09), and postoperative hemorrhage ( HR=1.62,95% CI:1.14-2.29) were independent risk factors for poor prognosis after surgery (all P<0.05). Conclusions:Radical surgery is associated with favorable overall survival among DA patients, and no difference in survival is observed between EA-DA and PA-DA patients. MIS is a reliable alternative for DA treatment.
3.Meta-analysis of the efficacy of dydrogesterone combined with estradiol valerate for the prevention of intrauterine adhesion and prognosis improvement after induced abortion

OBJECTIVE To systematically evaluate the efficacy of dydrogesterone combined with estradiol valerate for the prevention of intrauterine adhesion (IUA) and prognosis improvement after induced abortion. METHODS Retrieved from CNKI, Wanfang Data, VIP, CBM, PubMed, Embase and the Cochrane Library, randomized controlled trial (RCT) about conventional treatment combined with dydrogesterone and estradiol valerate (trial group) versus conventional treatment (control group) for the prevention of IUA in patients after induced abortion were collected from the inception to Dec. 2024. After screening the literature, extracting data and evaluating the quality of literature, meta-analysis was performed using RevMan 5.4 software. RESULTS A total of 12 RCTs were included, involving 1 109 patients. Meta-analysis showed that the postoperative incidence of IUA [RR=0.30, 95%CI (0.22, 0.41), P<0.000 01], postoperative vaginal bleeding time [MD=-1.69, 95%CI (-2.05, -1.32), P<0.000 01], postoperative vaginal bleeding volume [MD=-10.78, 95%CI (-12.19, -9.37), P<0.000 01], postoperative menstrual resumption time [MD=-6.99, 95%CI (-8.27, -5.71), P<0.000 01], and the incidence of postoperative reduced menstrual flow [RR=0.25, 95%CI (0.12, 0.56), P=0.000 7] were significantly lower, less or shorter than control group; postoperative endometrial thickness [MD= 1.90, 95%CI (1.68, 2.13), P<0.000 01] and the rate of postoperative re-pregnancy [RR=6.26, 95%CI (1.88, 20.83), P=0.003] were significantly higher than control group. CONCLUSIONS Dydrogesterone combined with estradiol valerate may reduce the incidence of IUA after induced abortion patients, decrease postoperative vaginal bleeding volume, shorten postoperative vaginal bleeding time and postoperative menstrual resumption time, and increase postoperative endometrial thickness.
4.Antiviral effect of Lopinavir against porcine hemagglutinated encephalomyelitis virus

This study explores the antiviral effects of Lopinavir on porcine hemagglutinating en-cephalomyelitis virus(PHEV)in vitro and in vivo.Using PHEV-infected N2a cells as an in vitro experimental model,the impact of varying concentrations of Lopinavir on PHEV replication was analyzed through Western blot and qRT-PCR techniques.The results demonstrated that Lopinavir was beneficial to PHEV replication at low-concentration,but as the concentration increased,Lopi-navir began to exert an inhibitory effect,with the most pronounced effect observed at a concentra-tion of 8 μmol/L.PHEV-infected 3-week-old male BALB/c mice were utilized in vivo experi-ments,with Lopinavir(10 mg/kg)administered intragastrically three days post-infection.Follow-ing the onset of illness in the control group,all mice were euthanized,and brain tissues were col-lected for histopathological examination.The findings indicated that Lopinavir significantly reduced the distribution of PHEV and ameliorated the pathological damage in brain tissue,and prolonged the survival time of the mice.In conclusion,Lopinavir exhibits an antiviral effect against PHEV both in vitro and in vivo,offering a theoretical basis for the prevention and treatment of PHEV in-fections in clinical practice.

7.Bioinformatics, expression, purification, and inflammation-inducing effect of Mycoplasma genitalium GroEL protein.

To preliminarily understand the pathogenic mechanism of Mycoplasma genitalium (Mg) GroEL protein, we used bioinformatics tools to predict the structure and function of Mg GroEL protein and then constructed the recombinant plasmid pET-28a-GroEL. The protein expression was induced by 0.2 mmol/L IPTG, and the expressed protein was purified by Ni-iminodicitic acid (IDA) column affinity. Tohoku Hospital Pediatrics-1 (THP-1) cells were exposed to 2 μg/mL Mg rGroEL. The levels of interleukin (IL)-1β and tumor necrosis factor (TNF)-α in the cell supernatant were measured by ELISA, and that of IL-6 was measured by an automatic chemiluminescence instrument. The activation of the nuclear factor-kappa B (NF-κB) signaling pathway was visualized by immunofluorescence and Western blotting. The results showed that Mg GroEL was a stable hydrophilic protein composed of 543 amino acid residues, with the relative molecular mass of 58.44 kDa, an isoelectric point of 5.68, and a molecular formula of C2568H4300N700O825S8. The secondary structure was mainly composed of α-helices and random coils. Mg GroEL contained 12 B-cell dominant epitopes and 10 T-cell dominant epitopes. It exhibited high homology with the GroEL proteins from Mycoplasma pneumoniae, M. agalactiae, M. arthritidis, M. hyopneumoniae, and M. bovis. Mg rGroEL activated the NF-κB signaling pathway and promoted the secretion of IL-1β, IL-6, and TNF-α in THP-1 cells. These results suggest that Mg GroEL exhibits substantial antigenicity and possesses the capability of triggering inflammation in host cells. This study establishes a theoretical basis for future investigations pertaining to the role and pathogenic mechanisms of Mg GroEL.

Recently, malaria incidence has sharply resurgence in the border area of northern Myanmar, with the parasite incidence rate in 2023 being 21.47 times (95% CI: 18.84-24.48) that of 2019 in Kachin State's Razan and nearby areas. This resurgence caused the number of imported malaria cases to increase from 188 in 2019 to 398 in 2023 in Yunnan Province. In addition to the impact of military conflict, the border malaria joint prevention and control cooperation mechanism and malaria control measures established between China and Myanmar have failed to be implemented effectively due to the impact of the international COVID-19 epidemic. Hence, it is recommended that relevant departments evaluate the quality and effectiveness of the current cross-border transmission measures for malaria in the China-Myanmar border area from a technical perspective, and provide a large demand for primaquine, which can block the spread of malaria and cure vivax malaria, in response to the current prevalent characteristics of vivax malaria predominating in northern Myanmar. Moreover, to effectively reduce the mortality of imported malaria patients and prevent re-importation and transmission, it is necessary to enhance clinical physicians' knowledge, awareness, and vigilance regarding malaria diagnosis and treatment in the Yunnan border region, as well as China's ability and quality of appropriate response to imported malaria.
9.Construction and evaluation of a nomogram for adverse cardiovascular events within 1 year after PCI in elderly patients with ACS

Objective To construct a nomogram prediction model for major adverse cardiovascular events(MACE)within 1 year after percutaneous coronary intervention(PCI)in elderly patients with acute coronary syndrome(ACS).Methods A retrospective analysis was conducted on 551 patients with diagnosed ACS and undergoing PCI in Department of Cardiovascular Medicine of Air Force Medical Center from 1 January 2020 to 1 April 2022.According to the occurrence of MACE during 1 year of follow-up,they were classified into MACE group(n=176)and non-MACE group(n=375).Risk factors for the occurrence of MACE in elderly ACS patients within 1 year after PCI were analysed using univariate and multivariate logistic regression,a nomogram prediction model was constructed,and the predictive power of the model was assessed using the area under the ROC curve(AUC).Results The MACE group had significantly higher Gensini score,systemic immune-inflammation index,and GRACE score,but obviously lower prognostic nutritional index than the non-MACE group(P<0.01).Multivariate logistic regression analysis showed that recent smoking(OR=2.222,95%CI:1.361-3.628,P=0.010),hyperlipidaemia(OR=1.881,95%CI:1.145-3.089,P=0.013),prognostic nutritional index(OR=4.645,95%CI:2.788-7.739,P=0.001),LVEF(OR=5.177,95%CI:3.160-8.483,P=0.001),systemic immune-inflammation index(OR=5.396,95%CI:3.179-9.159,P=0.001),and preoperative di-agnosis of non-STEMI(OR=2.829,95%CI:1.356-5.901,P=0.006)or STEMI(OR=3.451,95%CI:1.596-7.463,P=0.002)were independent influencing factors for occurrence of MACE after PCI in elderly ACS patients.ROC curve analysis showed that the AUC value of the nomo-gram model for predicting MACE within 1 year after PCI in elderly ACS patients was 0.888.Con-clusion Our developed nomogram model is simple and practical,and can effectively predict the occurrence of MACE within 1 year after PCI in elderly ACS patients.And external validation should be carried out to ensure its generality.

Malocclusion,identified by the World Health Organization(WHO)as one of three major oral diseases,profoundly impacts the dental-maxillofacial functions,facial esthetics,and long-term development of~260 million children in China.Beyond its physical manifestations,malocclusion also significantly influences the psycho-social well-being of these children.Timely intervention in malocclusion can foster an environment conducive to dental-maxillofacial development and substantially decrease the incidence of malocclusion or reduce the severity and complexity of malocclusion in the permanent dentition,by mitigating the negative impact of abnormal environmental influences on the growth.Early orthodontic treatment encompasses accurate identification and treatment of dental and maxillofacial morphological and functional abnormalities during various stages of dental-maxillofacial development,ranging from fetal stages to the early permanent dentition phase.From an economic and societal standpoint,the urgency for effective early orthodontic treatments for malocclusions in childhood cannot be overstated,underlining its profound practical and social importance.This consensus paper discusses the characteristics and the detrimental effects of malocclusion in children,emphasizing critical need for early treatment.It elaborates on corresponding core principles and fundamental approaches in early orthodontics,proposing comprehensive guidance for preventive and interceptive orthodontic treatment,serving as a reference for clinicians engaged in early orthodontic treatment.
