1.Simvastatin alleviates kidney ischemia reperfusion injury by inhibiting ferroptosis

ObjectiveTo investigate the effect and mechanism of simvastatin pretreatment on kidney ischemia reperfusion injury (IRI) in mice. MethodsFifteen male C57BL/6 mice aged 6-8 weeks were divided into three groups: Sham operation group (Sham group), kidney IRI group (IR group), and simvastatin pretreatment+kidney IRI group (SIM group). Hematoxylin-eosin (HE) staining of kidney tissue and detection of serum creatinine (SCr) and lactate dehydrogenase (LDH) were used to evaluate kidney injury. The levels of superoxide dismutase (SOD), reduced glutathione (GSH), malondialdehyde (MDA) and reactive oxygen species (ROS) were detected to evaluate oxidative stress. The contents of ferrous iron (Fe2+) and ferric iron (Fe3+) in kidney tissue were detected, and the morphological changes of mitochondria were observed by transmission electron microscope. The relative expression levels of Kruppel-like factor 2 (KLF2), glutathione peroxidase 4 (GPX4), solute carrier family 7 member 11 (SLC7A11), and acyl-coa synthetase long chain family member 4 (ACSL4) protein in kidney tissue were detected. ResultsCompared with the IR group, the SIM group had significantly reduced renal tubular injury and decreased contents of Scr and LDH in serum (P < 0.001). It also showed increased expression of SOD and GSH and decreased expression of MDA and ROS (P < 0.01). Simvastatin pretreatment reduced the contents of Fe2+ and Fe3+ in the tissues (P < 0.01) and alleviated mitochondrial damage. It also promoted the expression of KLF2 (P < 0.01), up-regulated the expression of ferroptosis-related protective proteins GPX4 and SLC7A11, and down-regulated the expression of ferroptosis-related damage protein ACSL4 (P < 0.05). ConclusionSimvastatin pretreatment may inhibit kidney ferroptosis by promoting the expression of KLF2 to alleviate kidney IRI.

3.A practical study on information management of full lifecycle for medical equipment based on service-oriented architecture

Objective:To explore effect of information management mode of full lifecycle for medical equipment on the basis of Service-Oriented Architecture(SOA)on the information management for information management of full lifecycle in hospital.Methods:An information management mode of full lifecycle for medical equipment was adopted to conduct information management for full lifecycle of medical equipment,so as to solve a series of problems included perception for the situation of equipment,and early warning for fault,and so on.A total of 100 medical equipment in clinical use at The First Rongjun Youfu Hospital of Shandong Province from February 2023 to February 2024 were selected for the study.In them,50 equipment were managed under a continuous improvement mode,while the other 50 equipment were managed by using a SOA-based information management mode of full lifecycle(information management mode).The score of management quality for equipment,management standardization and operational quality of the two kinds of management modes were compared.A self-designed satisfaction questionnaire of using and managing equipment was used to survey the satisfactions of medical engineers,operators of equipment,and staffs of equipment department in using and managing equipment,who involved in usage and management for equipment.Results:Under the information management mode,the scores for usage of equipment,the return efficiency,safety performance,maintenance quality,cleaning quality,and overall evaluation were respectively(22.98±2.11),(23.69±1.65),(22.74±1.36),(23.65±1.54),and(93.01±4.65),all of which were significantly higher than those of the continuous improvement mode,with statistically significant differences(t=6.672,11.428,9.256,12.976,13.182,P<0.05).The standardizations of usage operation of equipment,cleaning and disinfection,safety monitoring,and quality control of adopting information management mode were higher than those of adopting continuous improvement mode(x2=13.709,11.679,11.284,15.120,P<0.05).The failure rates of surgical equipment,therapeutic equipment,diagnostic equipment,dialysis equipment,and emergency equipment of adopting information management mode were all lower than those of adopting continuous improvement mode,with statistically significant differences(x2=16.872,13.878,17.156,11.679,12.805,P<0.05).Satisfaction scores of medical engineers,operators for equipment,and staffs of equipment department of adopting information management mode were significantly higher than those of adopting the continuous improvement mode,with statistically significant differences(t=16.872,13.878,17.156,P<0.05).Conclusion:The application of a SOA-based information management mode of full lifecycle for medical equipment in management for medical equipment can enhance management level for equipment,and improve operational quality of equipment,and enhance safety and utilization efficient of using equipment.
4.Effect of acupuncture on postoperative delirium in diabetic patients undergoing surgery under general anesthesia

Objective:To evaluate the effect of acupuncture on postoperative delirium (POD) in diabetic patients undergoing surgery under general anesthesia.Methods:In this randomized controlled trial, 92 diabetic patients of either sex, aged 30-80 yr, with a body mass index of 18-28 kg/m 2, of American Society of Anesthesiologists Physical Status classification Ⅱ or Ⅲ, scheduled for elective surgery under general anesthesia, were divided into 2 groups ( n=46 each) using a table of random numbers: control group (group C) and acupuncture group (group A). Group A received acupuncture at the Baihui (GV20), Shenting (GV24) and Sishencong (EX-HN1) acupoints before anesthesia. The needles were retained for 30 min, with manual stimulation applied every 10 min for 10 s each time. After 4 stimulations, routine anesthesia was carried out. Group C received routine anesthesia only. Regional cerebral oxygen saturation was recorded on admission to the operating room (T 0), after anesthesia induction (T 1), at the start of surgery (T 2), at the end of surgery (T 3), and immediately after tracheal extubation (T 4). The POD developed within 3 days after surgery was assessed. The occurrence of needle-related adverse effects such as fainting, subcutaneous bleeding, and local paresthesia was recorded. Results:Compared with group C, the incidence of POD was significantly reduced, and the regional cerebral oxygen saturation was increased at T 1, 4 in group A ( P<0.05). Conclusions:Acupuncture can decrease the development of POD in diabetic patients undergoing surgery under general anesthesia, which is related to an increase in regional cerebral oxygen saturation.
5.Analysis of Traditional Chinese Medicine Syndrome Related Factors in Sepsis Based on Cross-sectional survey

Objective To explore the correlation between different syndromes of sepsis and laboratory parameters,SOFA score,and APACHE Ⅱ score,providing reference for improving the objectivity of traditional Chinese medicine syndrome differentiation.Methods Based on the cross-sectional survey,information of the four diagnostic methods of traditional Chinese medicine,laboratory parameters,and disease assessment scales were collected from sepsis patients admitted to ICUs in 8 hospitals across the country.ANOVA,non-parametric tests,and Logistic regression analysis were used to screen for sepsis syndrome related factors.Results A total of 905 sepsis patients were included,and the distributions of traditional Chinese medicine syndromes were as follows:163(18.01%)of phlegm heat obstructing the lungs syndrome,141(15.58%)of blood stasis and toxin obstruction(damage)syndrome,130(14.36%)of intense exuberant heat toxin syndrome,129(14.25%)of Yin exhaustion and Yang deficiency syndrome,124(13.70%)of Yangming fu-organ excess syndrome,96(10.61%)of lung Qi deficiency syndrome,68(7.51%)of Yin deficiency and internal heat syndrome,and 54(5.97%)of Qi and blood deficiency syndrome.The results of ANOVA and non-parametric tests showed that there were differences in 14 indicators,including body temperature,heart rate,WBC,NE%,PCT,CRP,PLT,D-D,APTT,Lac,HCT,Hb,SOFA score,APACHE Ⅱ score among 8 common syndromes(P<0.05);Using the 14 indicators mentioned above as independent variables,and syndrome as the dependent variable,Logistic regression analysis was conducted.The results showed that in the excess syndrome category:①Phlegm heat obstructing lung syndrome was positively correlated with heart rate and WBC,and negatively correlated with Lac and APACHE Ⅱ scores;②Intense exuberant heat toxin syndrome was positively correlated with body temperature,WBC,NE%,PCT,CRP,and negatively correlated with PLT and SOFA scores;③Yangming fu-organ excess syndrome was positively correlated with body temperature and negatively correlated with PCT;④Blood stasis and toxin obstruction(damage)syndrome was positively correlated with body temperature,NE%,PLT,CRP,and D-D,and negatively correlated with heart rate,WBC,PCT,and APTT.Deficiency syndrome category:①Lung Qi deficiency syndrome was positively correlated with Hb,and negatively correlated with body temperature,WBC,and HCT;②Yin deficiency and internal heat syndrome were negatively correlated with NE%and CRP;③Qi and blood deficiency syndrome was positively correlated with APACHE Ⅱ score,and negatively correlated with body temperature,Hb,PLT,D-D;④Yin exhaustion and Yang deficiency syndrome was positively correlated with heart rate,PCT,Lac,SOFA score,and APACHE Ⅱ score,and negatively correlated with body temperature.Conclusion Patients with sepsis with excess syndrome category have higher WBC,NE%,PCT,CRP scores,which suggests a drastic inflammatory response.Among them,Blood stasis and toxin obstruction(damage)syndrome have highest PLT,D-D,and lowest APTT,suggesting a more severe disorder of coagulation function.Deficiency syndrome category,especially patients with Yin exhaustion and Yang prostration syndrome has higher Lac and SOFA score and APACHE Ⅱ score than other patients with sepsis of other syndromes,suggesting a worse condition and poor prognosis.
6.Management and prognosis of pediatric acute liver failure in pediatric intensive care unit

Objective:To understand the management of children with pediatric acute liver failure (PALF) in pediatric intensive care unit (PICU).Methods:A retrospective case-control study was conducted. A total of 101 children with PALF hospitalized in PICU of Beijing Children′s Hospital from July 2017 to October 2022 were included. Demographic, clinical management and prognosis data were collected. According to whether PALF was the main diagnosis, the patients were divided into primary diagnosis group and complication group. The primary diagnosis group was subdivided into effective group and ineffective group with routine treatment (except liver transplantation). The intergroup comparisons were performed using independent samples t-test, Mann-Whitney U test, χ2 test or Fisher exact test. Multivariate Logistic regression analysis was employed to identify risk factors associated with prognosis. Results:Among the 101 children with PALF, 58 were male and 43 were female, with an age of 30 (10, 103) months, 60 cases in primary diagnosis group and 41 cases in complication group. There were no significant differences in prothrombin time (PT) and international normalized ratio (INR) between the two groups (both P>0.05), while the total bilirubin, direct bilirubin and blood ammonia were all significantly higher in the primary diagnosis group (all P<0.05). Unoriginal liver failure (25 cases (42%)) and poisoning (13 cases (22%)) were the most common causes of PALF in the primary diagnosis group, while shock (17 cases, 43%) and hemophagocytic syndrome (14 cases (34%)) in the complication group. The mortality rate of the main diagnosis group was significantly lower than that of the complication group (25% (15/60) vs. 61% (25/41), χ2=13.18, P<0.001), as well as the incidence of combined organ function injury, while the amount of plasma used and the ratio of plasma exchange times to PICU hospitalization days were significantly higher (all P<0.05). In the primary diagnosis group, there were 32 cases (53%) in the effective group and 28 cases (47%) in the ineffective group. In the ineffective group, 15 cases (54%) died and 13 cases (46%) were transferred to another site for liver transplantation assessment. The hospitalization time of PICU in the effective group was significantly longer than that in the ineffective group, while the ratio of plasma exchange times to PICU hospitalization days, the average daily hours of continuous renal replacement therapy (CRRT), the rate of CRRT and the average daily plasma dosage in the effective group were all significantly lower than those in the ineffective group (all P<0.05). The worst PT, INR and blood ammonia, and the stage 4 hepatic encephalopathy morbidity and significant bleeding rate in the effective group were all significantly lower than those in the ineffective group (all P<0.05). Multivariate Logistic regression analysis showed that after adjusting for age, sex, total bilirubin, INR and blood ammonia, stage 4 hepatic encephalopathy was the independent risk factor for the failure of routine treatment of PALF ( OR=84.16,95% CI 4.04-1752.37, P=0.004). Conclusions:PT and INR could not specifically represent liver synthetic function in some PICU patients, so current PALF diagnostic criteria for PICU children has limitations. Complicated with stage 4 hepatic encephalopathy was an independent risk factor of the failure of conventional treatment in patients with PALF.

OBJECTIVES:

Results:Compared with group C, the incidence of POD was significantly reduced, and the regional cerebral oxygen saturation was increased at T 1, 4 in group A ( P<0.05). Conclusions:Acupuncture can decrease the development of POD in diabetic patients undergoing surgery under general anesthesia, which is related to an increase in regional cerebral oxygen saturation.
