1.The impact of digital feedback on the effectiveness of dental crown preparation training

Objective:To develop a standardized training model incorporating feedback from a digital assessment tool and to evaluate whether the model provides effective training in tooth preparation.Methods:The study was based on the training data of 53 trainees enrolled between February and June 2018 from multiple institutions in China. The trainees were trained in a standardized training unit on the preparation of right maxillary mesial incisors (11 #) for metal ceramic crowns. Three sessions of practice-assessment-feedback before examination were performed in one day. A digital assessment system was used to obtain total and component scores for the preparation as indicators of observation. The scores of three practice sessions and examination were subjected to analysis of variance. Results:The mean total scores before training, after the first training session, after the second training session, after the third training session, and in the examination were (60.53±12.73), (60.12±12.98), (71.25±13.70), (70.70±11.84), and (69.67±12.85), respectively; the overall difference was statistically significant ( F=19.06, P<0.001). Compared to the total scores before training and after the first training session, the total scores after the second and third training sessions and in the examination were significantly increased ( P<0.001, P<0.001). The score deductions for cutting amount and shoulder were similar, with overall significant differences (score deductions for cutting amount, F=16.20, P<0.001; score deductions for shoulder, F=1.45, P=0.032). Compared to before training and after the first training session, the second and third training sessions and the examination showed significant decreases in score deductions for cutting amount and shoulder ( P<0.001, P=0.048). Conclusions:The feedback-based standardized training process established on a digital evaluation system can rapidly enhance the skills of dentists in tooth preparation through immediate and effective feedback and targeted improvements. The training process enables an independent practice mode and optimizes teaching outcomes.

Objective:To analyze the research hotspots of hyperbaric oxygen nursing in China.Methods:We retrieved the research literature on hyperbaric oxygen nursing in China from China National Knowledge Infrastructure (CNKI) and Wanfang Data, used the Statistical Analysis Toolkit for Informetrics (STAI 3.2) to make statistical and word frequency analysis on the key words of those literature, and constructed a co-word matrix. Then we carried out cluster analysis by using SPSS 23.0 and analyzed the research hotspots according to the tree diagram.Results:A total of 704 papers were retrieved, and 30 high-frequency key words were gained. Based on that, five hotspots were obtained by cluster analysis as follows: hyperbaric oxygen nursing solutions and measures, the role of psychiatric and mental health nursing in hyperbaric oxygen therapy, the effect and value of risk management in hyperbaric oxygen therapy, nursing measures in hyperbaric oxygen therapy for patients received tracheostomy, and the application of different nursing methods or models on patients in hyperbaric oxygen therapy.Conclusion:Based on word frequency analysis and co-word cluster analysis, this study objectively reveals the current research hotspots of hyperbaric oxygen nursing in China, which can be compared with overseas research hotspots in the same field to direct future research.

4.Determination of human papillomavirus in oral leukoplakia,oral lichen planus and oral squamous cell carcinoma

Objective:To investigate the possibility for human papillomavirus (HPV)infection to be a predictable signal for the carcinogenesis of oral mucosa by comparing the prevalences of HPV in each stage of oral mucosal carcinogenesis and to compare the sensitivity differences of the two methods in de-tecting HPV infection in oral cavity.Methods:The hybrid capture (HC-Ⅱ)was used to detect infection of HPV in 255 samples taken from 1 2 cases of healthy oral mucosa,21 1 cases of patients with pathologi-cal diagnosis and 32 cases of patients with clinical diagnosis.The diagnosed cases included 8 cases of be-nign lesions of the oral mucosa,precancerous lesions [74 cases of oral leukoplakia (OLK)with hyper-plasia and 42 cases of OLK with oral epithelial dysplasia (OED)],91 cases of precancerous condition
[oral lichen planus (OLP)]and 28 cases of oral squamous cell carcinoma (OSCC).And in situ hybri-dization (ISH)was used to detect infection of HPV in 33 cases of OSCC and 76 cases of OLK,including 30 cases of hyperplasia,1 5 cases of mild OED,1 5 cases of moderate OED and 1 6 cases of severe OED. Results:The prevalence of HPV in OLP samples was higher (1 2.1 2%,8/66 )than that of OLK (2.59%,3/1 1 6)(χ2 =4.666,P=0.031 )and OSCC(7.1 4%,2/28,χ2 =0.51 3,P=0.474).The prevalence of HPV in OSCC (7.1 4%,2/28)was higher than that of OLK (2.59%,3/1 1 6),and no significant difference was found.There was only one case of smoke spot and statistical analysis was not carried out.ISH was used to detect type 1 6/1 8 and type 31 /33 HPV DNA in 1 09 cases of oral mucosal lesions in paraffin sections and only one case of OSCC was HPV positive.Thirty-seven cases were detec-ted by HC-Ⅱ and ISH methods at the same time.The same negative results by the two methods were found in 94.6% samples (35/37).In the other two samples,one was OSCC with early infiltration and the other was OLK with hyperplasia,The HC-Ⅱ results were positive while the ISH results were nega-tive.The patients with OLP and HPV testing results were followed up and the average follow-up period was (36.2 ±1 0.5)months.It was found that three of them had a malignant transformation,and the ma-lignant transformation rate of HPV positive patients was 1 2.50% (1 /8),which was higher than that of HPV negative patients (3.45%,2/58),and the difference was not statistically significant,P=0.249. Conclusion:HC-Ⅱ assay was more sensitive in detecting HPV infection of oral mucosal lesions than ISH.The results of this study showed that there was insufficient evidence for taking HPV infection as a predictor of OLK carcinogenesis.Patients suffering from OLP were in a precancerous condition.The pre-valence of HPV in OLP patients of this study was higher than that in OLK and OSCC patients,suggesting that for some reason,OLP patients were susceptible to HPV.HPV testing can be considered as routine in patients with OLP,and HC-Ⅱassay was recommended.And patients with OLP and HPV positive should be followed up regularly.
5.Effect of bladder hydrodistension combined with hyperbaric oxygen on interstitial cystitis

Objective To investigate the mechanism of bladder hydrodistension and combined with hyperbaric oxygen (HBO) in the treatment of interstitial cystitis (IC) and evaluate its therapeutic efficacy.Methods Twenty-three confirmed IC patients were treated with bladder hydrodistension plus HBO therapy.Results Two IC patients abandoned the therapy,due to intolerance to HBO pressure,the remaining 21 patients all completed the prescheduled therapy.As compared with those before therapy,statistical significance could be noticed in the scores of interstitial cystitis symptom index (ICSI),maximum urine volume,visual analogue scale (VAS),24-hour urination and quality of life (QOL),following 3 and 6 months of treatment respectively.Interestingly,the therapeutic efficacy after 3 months of treatment was more prominent(P < 0.05) Conclusions Bladder hydrodistentioa combined with HBO therapy was a safe and effective treatment method for IC.However,its long term effect remained to be seen.

7.Effects of Qili Qiangxin capsules on NT-proBNP level and cardiac function in silicosis patients.

OBJECTIVETo investigate the effects of Qili Qiangxin capsules on the amino-terminal pro-brain natriuretic peptide (NT-proBNP) level and cardiac function in patients with silicosis.
METHODSHospitalized silicosis patients with heart failure were divided into treatment group (41 cases) and control group (30 cases) according to their own will. Both groups received comprehensive symptomatic treatment; in addition, the treatment group received Qili Qiangxin capsules. The treatment lasted 6 months. The observed items included NT-proBNP level, 6-minute walk test, ultrasonic cardiogram, and NYHA classification before and after treatment.
RESULTSAccording to NYHA classification, the response rate was 29.27%in the treatment group and 10.00%in the control group; there was a significant difference between the two groups (P < 0.05). The average walk distance in the treatment group was increased from 150.96±73.12 m before treatment to 169.32±77.04 m after treatment, and the improvement was statistically significant (P < 0.05). The average NT-proBNP level in the treatment group was reduced from 1154.44 ± 480.79 ng/L before treatment to 494.49 ± 342.61 ng/L after treatment, and the reduction was statistically significant (P < 0.01). Left ventricular ejection fraction was significantly improved in the treatment group (P < 0.05).
CONCLUSIONQili Qiangxin capsules in addition to comprehensive symptomatic treatment can significantly reduce NT-proBNP level and improve cardiac function in silicosis patients, and thereby improve patients' quality of life.

Obj ective To explore the value of renal artery (RA)hemodynamic changes in assessment of women with pregnancy-induced hypertension syndrome (PIH)and fetal growth re-striction (FGR).Methods 57 pregnant women with PIH and FGR and 57 normal pregnancy were examined by peak systolic flow velocity (Vmax),peak diastolic flow velocity (Vd),the average peak blood flow velocity cardiac cycle (Tamax),pulse rhythm(PI),resistance index(RI),the ratio (S/D)maximum blood flow speed during umbilical artery systole and diastole telophase.Results Vmax of fetal renal artery flow in the control group had a positive relationship with gestational age, and RI、PI、S/D showed a slow downward trend.Vmax and S/D of RA in pregnant women with PIH and FGR were significantly lower than those of the control group (P<0.001).Conclusion Normal fetal RA blood flow velocity is positively correlated with gestational age.The Vmax of RA<1 1 cm/s in pregnant women with PIH and FGR may predict a poor prognosis in fetal.

10.Clinical analysis of pregnancies after vaginal radical trachelectomy

Objective To explore the pregnancy outcome and obstetric management of pregnancy and delivery after vaginal radical trachelectomy (VRT).Methods Forty-two cases of VRT from December 2003 to May 2012 in Peking Union Medical College Hospital were analyzed retrospectively.Among them ten cases got pregnant successfully.Results The average age of patient at VRT surgery was (30.6 ± 3.7) years old and average follow-up time was 29.5 months.There were 31 patients attempted conception.Ten of them got fourteen conceptions successfully.Overall conception rate was 45% (14/31).There were four cases of first trimester abortion.Among them,two were miscarriage,two were elective abortion.There was one case of ectopic pregnancy operation and non of second trimester loss.Nine cases reached the third trimester.The total preterm delivery rate was 4/9.There were two cases delivered before 32 gestational weeks (2/9).Cesarean section was performed through a transverse incision in all of nine cases.No uterine rupture and postpartum hemorrhage occurred.All newborns had good outcomes.The average follow-up time after postpartum was 22.9 months.All cases were disease-free.Conclusions The conception rate of patients after VRT in our series is 45%.The preterm birth rate of pregnancy after VRT is higher.Routine cerclage of cervix during VRT procedure and pregnancy is not necessary.Cesarean section shortly after full term pregnancy through a transverse incision should be considered as a suitable and safe procedure.
