Objectives:To observe the clinical characteristics of familial chylomicronemia syndrome (FCS) in Chinese population and explore the suitable diagnostic criteria of FCS in China.Methods:We screened 6856 patients with triglyceride ≥10 mmol/L from 9 hospitals in China between January 2010 and December 2023.The overall clinical information was collected and FCS-related gene testing was performed in 102 patients with high suspicion of FCS.Demographic characteristics were analyzed and FCS diagnosis was drafted.The FDA FCS diagnostic criteria was used to verify the FCS patients diagnosed in this study.At the same time,the prevalence of FCS patients with fasting triglyceride ≥10 mmol/L was explored in some of the above hospitals.Results:In this study,the diagnostic criteria for FCS in Chinese population were drafted based on European and American diagnostic criteria and Chinese clinical practice:(1) Fasting triglyceride ≥ 10 mmol/L after standard lipid-lowering treatment;(2) With one of the below-listed conditions:positive detection of FCS related genes;family history of hypertriglyceridemia pancreatitis (HTGP);history of pancreatitis in adolescence or adult HTGP;history of repeated hospitalization with unexplained abdominal pain.According to this criteria,60 were preliminarily diagnosed with FCS from the 102 patients enrolled.Their average age was (43.0±8.6) years old,male accounted for 70％,average triglyceride was (20.0±15.0) mmol/L.FCS related gene mutations were detected in 6 patients,all were lipoprotein lipase gene mutations.According to the American FCS diagnostic criteria,among the 60 FCS patients diagnosed in this study,the proportion of patients with "possible FCS" was 98.3％ (59/60),and the proportion of patients with "confirmed FCS" was 70.0％ (42/60).According to the diagnostic criteria of FCS patients in this study,the prevalence of FCS patients with TG ≥10 mmol/L is about 0.5％ (33/6722).Conclusions:The clinical and genetic characteristics of Chinese patients with FCS are consistent with those of European and American patients.The diagnostic criteria for FCS drafted in this study can be further verified and promoted in Chinese population.

Objective:To explore the effect of pre-hospital health education based on "Internet +" on self-management efficacy, anxiety and depression of patients undergoing pulmonary tumor surgery, and provide reference for pre-hospital nursing care of this group.Methods:It was a randomized controlled trial. A total of 100 outpatients in Department of Pulmonary Oncology, Tumor Hospital of Tianjin Medical University to be hospitalized for surgery during November 2021 to March 2022, were divided into experimental group ( n=50) and control group ( n=50) by random number method according to the order of outpatient visits. Patients in control group received routine thoracic surgery nursing and health education after admission. Those in experimental group received "Internet +" pre-hospital health education during patients waiting for hospital beds for surgery, nursing care after admission was the same as that of the control group. The scores of Strategies Used by People to Promote Health (SUPPH) and Hospital Anxiety and Depression Score (HADS) were compared between the two groups 1 day before discharge. Results:At the time of outpatient visit, there was no statistically significant difference between the two groups in the comparison of all dimensions and total scores of SUPPH and the score of HADS ( P>0.05). Total SUPPH score, and self-decompression, self-decision and positive attitude score of experimental group were (110.62 ± 17.73) points, (38.74 ± 6.56) points, (11.98 ± 2.20) points, (59.90 ± 10.18) points, respectively, while in the control group, the scores were (92.86 ± 18.91) points, (32.46 ± 7.39) points, (9.76 ± 2.00) points and (50.64 ± 11.50) points, respectively, with statistically significant differences ( t values were -5.29--4.27, all P<0.01). Besides, the anxiety and depression scores of HADS in experimental group were (3.92 ± 2.25) points and (3.36 ± 2.38) points, respectively, while those in control group were (7.12 ± 3.49) points and (7.00 ± 3.53) points, respectively, and the differences were statistically significant ( t=5.45, 6.04, both P<0.01). Conclusions:The "Internet +" pre-hospital health education model can promote the self-management efficacy of patients undergoing pulmonary tumor surgery, and improve their psychological state during treatment. The study provided reference for promoting diversified extended nursing services in clinical practice.

Objective: To analyze the correlation between behavior and lifestyle factors and anxiety symptoms of left-behind children, and to provide evidence for mental health intervention of left-behind children. Methods: 1 188 children aged from 13 to 18 (617 non-left-behind children and 571 left-behind children) in B County of Shangrao City, Jiangxi Province were evaluated with Generalized Anxiety Disorder Scale (GAD-7). Meanwhile, physical activity, TV watching time, computer usage time, dietary behavior, sleep and other behavioral lifestyle factors of left-behind children were investigated by questionnaire. Results: The detection rate of anxiety among left-behind children (66.0%) was higher than that of non-left-behind children (60.5%). The detection rates of mild, moderate, and severe anxiety were 26.3%, 15.4% and 24.3%, respectively. Under the condition of controlling age and gender, Logistic regression analysis showed that left-behind childrens anxiety symptoms and computer usage time>3 h/d (mild anxiety, OR=3.00, 95%CI=1.27-4.16; moderate anxiety, OR=4.09, 95%CI=1.55-10.78; severe anxiety, OR=3.44, 95%CI=1.46-8.11), mobile phone usage time >3 h/d (mild anxiety, OR=4.93, 95%CI=2.71-8.94; moderate anxiety, OR=5.93, 95%CI=2.98-11.79; severe anxiety, OR=7.11, 95%CI=3.85-13.15), skipping breakfast behavior (moderate anxiety, often skipping, OR=6.09, 95%CI=1.59-23.36; severe anxiety, often skipping, OR=5.49, 95%CI =1.68-7.97, sometimes eat breakfast, OR=2.68, 95%CI=1.10-6.53) was positively correlated; with appropriate sleeping time (moderate anxiety, OR=0.28, 95%CI=0.13-0.60) was negatively correlated. Conclusion The unhealthy behavior and lifestyle of left-behind children may be a potential risk factor for anxiety symptoms.

Objective:To explore the application of enhanced recovery around surgery (CMERAS) by integrated Traditional Chinese Medicine & western medicine in perioperative period of laparoscopic anterior resection for rectal cancer patients.Methods:100 patients with rectal cancer who were treated by laparoscopic anterior resection in Shuguang Hospital from July 2017 to July 2019 were divided into two groups with random number table method, 50 patients in each group. The control group received enhanced recovery after surgery (ERAS) perioperative treatment and the observation group received CMERAS perioperative treatment. Both groups were treated for 7 days. The degree of intestinal cleansing during the operation and postoperative rehabilitation quality were observed of the two groups, including the time of first exhaust, hospitalization time and the incidence of complications. Serum CRP level was detected by immunoturbidimetry, serum IL-6 level was detected by chemiluminescence immunoassay and peripheral blood CD4, CD8 and CD4/CD8 were detected by flow cytometry. Adverse reactions were recorded for the two groups.Results:There was no significant difference in the degree of intestinal cleansing between the two groups during operation ( Z=-1.140, P=0.254). The first postoperative exhaust time in the observation group (29.7 ± 4.6 h vs. 36.1 ± 3.8 h, t=7.590) was earlier than that of the control group, the hospitalization time (4.2 ± 0.5 d vs. 4.7 ± 0.6 d, t=4.379) was less than that of the control group, and the incidence of complications [8.0% (4/50) vs. 30.0% (15/50), χ2=6.498] was lower than that of the control group ( P<0.01). On the third day after the operation, serum CRP (11.84 ± 4.69 mg/L vs. 23.63 ± 5.04 mg/L, t=12.106) and IL-6 (34.31 ± 5.93 ng/L vs. 44.39 ± 8.81 ng/L, t=6.714) in the observation group were lower than those in the control group ( P<0.05). CD4 levels [(37.74 ± 7.28)% vs. (33.55 ± 5.07)%, t=-3.344], CD4/CD8 ratio (1.36 ± 0.27 vs. 1.13 ± 0.22, t=-4.920) were higher than those in the control group ( P<0.01), and CD8 levels [(28.04 ± 4.68)% vs. (30.22 ± 4.04)%, t=2.487] was lower than that of the control group ( P<0.05). There were no adverse reactions in two groups during the treatment. Conclusion:CMERAS could promote the perioperative recovery of patients with rectal cancer if treated with laparoscopic anterior resection and fewer complications would occur.

Objective:To study the role of ultraviolet fluorescence detection technology in personal protective equipment education (PPE) and training.Methods:A study was designed to inspect the risk of self-contamination during PPE doffing between 77 healthcare workers. Used a fluorescent tracer slurry which put on the hands, chest, abdomen, knees to simulate the contaminations. Self-contamination of scrubs and skin was measured using ultraviolet light visualization respectively.Results:According to the uv-fluorescer simulating study, 43 (55.8%) of the medical staff had contamination after the removal of PPE, and the main sites of contamination included: left side of the abdomen 11 (11.70%), left side of the chest 9 (9.57%), left forearm 6 (6.38%), left foot instep 6 (6.38%), neck 6 (6.38%), right shoulder 5 (5.32%), etc. Among them, the frequency of simulated fluorescence pollution in the group with working years less than 6 years was less than that in the other groups, and the difference was statistically significant compared with the group of 11-15 years ( t value was -3.685, P value was 0.001 ). Conclusion:Ultraviolet fluorescence labeling detection technology can directly, quickly and effectively evaluate and feedback the key contaminated parts in the process of using PPE, which can provide detailed evidence for redesigning PPE and improve the PPE training process to reduce the contamination.

Desmoplastic small round cell tumor( DSRCT) is a kind of highly uncommon malignant tumor which is reported in very recent years.Since it has been firstly reported by Gerald in 1991,DSRCT has only a-round one hundred reported cases globally, and very limited cases domestically.This report illustrates the first case of DSRCT in our hospital so as to share and to discuss with clinicians.

ObjectiveTo observe the effect of combined acupuncture-medication anesthesia on visceral function of patients undergone laparoscopic cholecystectomy, and to furtherexplore its clinical and theoretical significance.MethodSixty patients who were going to receive laparoscopic cholecystectomy were enrolled and randomized into an observation group (combined acupuncture-medication anesthesia) and a control group (conventional general anesthesia) by the random number table, 30 subjects in each group. The total bilirubin (TBIL), alanine aminotransferase (ALT), aspertate aminotransferase (AST), urea nitrogen, creatinine, and Mini-Mental State Examination (MMSE) were observed to evaluate the effect of combined acupuncture-medication anesthesia on visceral function of patients undergone laparoscopic cholecystectomy.ResultBefore operation, there were no significant differences in TBIL, ALT, urea nitrogen, creatinine, and MMSE between the observation group and the control group (P>0.05), while the AST level of the observation group was significantly higher than that of the control group before operation(P<0.05). On post-operation day 1, there were no significant differencesin comparing ALT and AST levels between the two groups (P>0.05); the TBIL level in the observation group was significantly lower than that in the control group (P<0.01); there were no significant differences in comparing the urea nitrogen and creatinine levels between the two groups (P>0.05). The MMSE score of the observation group was significantly higher than that of the control group 6 h after the operation (P<0.01). on post-operation day 1, there was no significant difference in comparing MMSE score between the two groups (P>0.05).ConclusionThe combined acupuncture-medication anesthesia and conventional general anesthesia play equal effects on liver and kidney functions in laparoscopic cholecystectomy, thus the two anesthesia methods are both safe andeffective. The combined acupuncture-medication anesthesia has certain protective effects on brain function in early stage, and possibly produces certain protective effectson liver function.

Objective To explore the expression of HO-1 in the dermal tissue of rats with type Ⅱ diabetic foot ulcer and the association between HO-1 level and ulcer healing.Methods Forty-five Wistar male healthy rats with the weight ranging from 250 to 350 g were randomly divided into 3 groups:the diabetic foot ulcer group(the DF group),the diabetic foot ulcer hyperbaric oxygen therapy group (or the DFHBO group or simply the HBO group) and the normal control group (or the NC group),each consisting of 15 animals.The mRNA transcription of HO-1 was detected by RT-PCR.The animals of the 3 groups were all raised with high lipid feed (88.8％ basic feed,1％ cholesterol,10％ lard and 0.2％ bile salt) for a period of one month.Then,the animal model of type Ⅱ diabetes was developed,and the model of type Ⅱ diabetes with diabetic food ulcer was further developed in the animals of the DF group and the DFHBO group.The animals in the DFHBO group were treated with HBO,at a pressure ofO.2 MPa,one session a day,for a duration of 15 days.Then,skin samples were collected from the foot of the animals in the 3 groups.The expression of HO-1 mRNA was detected by PT-PCR and the expression of HO-1 was determined by immunohistochemistry.Results The mRNA expression levels in the DF group,the HBO group and the NC group were respectively 1.063 ± 0.006,1.082±0.007 and 0.950 ± 0.013.Statistical significance could be noted,when comparisons were made between the 3 groups (P ＜0.01),and statistical difference could also be seen,when comparisons were made between the DF group and the HBO group (P ＜ 0.01).Immunohistochemistry detection indicated that HO-1 expression was more significant in epidermal tissue for the NC group.The expression levels of HO-1 both in the epidermal and dermal tissues for the animals of the DF group and the HBO group were more significant,when comparisons were made with that of the NC group,and the HO-1 expression level for the animals in the HBO group was higher than that of the DF group.Conclusions Diabetic foot ulcer was positively correlated with the expression of HO-1 in the dermal tissue.It was possible that antioxidation decreased in the dermal tissue of rats with diabetic foot ulcers.
