1.Clinical features and prognostic analysis of colorectal extranodal NK/T cell lymphoma

Objective:To investigate the clinical manifestations, endoscopic characteristics, and prognostic factors of patients with colorectal extranodal NK/T cell lymphoma.Methods:The clinical data of 52 patients with colorectal extranodal NK/T cell lymphoma admitted to the First Affiliated Hospital of Zhengzhou University from January 2013 to January 2023 were retrospectively analyzed. Their clinical manifestations and endoscopic characteristics were summarized, and the prognostic factors were analyzed by Cox regression model.Results:Among the 52 patients with colorectal extranodal NK/T cell lymphoma, there were 35 males and 17 females, with a male-to-female ratio of 2.06∶1. Among the general symptoms, abdominal pain was the most common (39 cases), and B symptoms occurred in 47 patients, among which fever was the most common lymphoma B symptom (42 cases), and gastrointestinal perforation was the most common complication (18 cases). Forty-three patients underwent colonoscopy, and the main manifestations under endoscopy were the ulceration type (24 cases). The ulcers were irregular at the edges and often covered with moss at the bottom. The median survival time was 4.3 months. Multivariate Cox regression analysis showed that hemocytic syndrome ( HR=8.50,95% CI: 1.679-8.328, P=0.001), serum albumin ( HR=3.59,95% CI: 1.017-6.551, P=0.048), and with or without chemotherapy ( HR=0.31, 95% CI: 0.246-1.061, P=0.025) were independent factors influencing the overall survival of patients with colorectal extranodal NK/T cell lymphoma. Conclusions:Colorectal extranodal NK/T cell lymphoma is a rare disease with a very poor prognosis. When patients present with abdominal pain and lymphoma B symptoms, and when ulcers with irregular edges and moss covering the bottom are found under endoscopy, the disease should be considered, and endoscopic biopsy should be taken in time for pathological diagnosis. The prognosis of patients with hemophagocytic syndrome and hypoproteinemia is poor. This disease should be treated with chemotherapy and surgery, and on this basis, hemophagocytic syndrome and hypoproteinemia should be treated to improve the prognosis of patients.
2.Construction and application of a mixed-reality distance teaching model in neurosurgery

Objective:To explore the application effects of a neurosurgical mixed-reality distance teaching (NMDT) model in standardized residency training in neurosurgery.Methods:We built an NMDT system using mixed-reality technology and remote interaction technology, and designed the implementation procedure according to the teaching objectives. After the teaching activities were completed, a teaching satisfaction questionnaire survey was conducted among 20 neurosurgery resident trainees, in which they provided satisfaction scores for the same teaching content with different teaching models (i.e., the NMDT model and traditional teaching model). SPSS 22.0 software was used to perform the t test for data analysis. Results:There were significant differences between the NMDT model and the traditional teaching model in key indicators including the score for "completion of teaching objectives" (9.20±0.68 vs. 8.25±0.70, P<0.001) and the score for "satisfaction with learning gains" score (8.95±0.67 vs. 8.05±0.92, P=0.001). The NMDT model also outperformed the traditional teaching model in the other individual scores and the total score. Conclusions:The NMDT model can improve teaching quality, increase training efficiency, and enrich teaching content, which is worthy of promotion.

4.Distribution and antimicrobial resistance pattern of bacteria isolated from alveolar lavage fluid in pediatric patients with lower respiratory tract infection

Objective:To analyze the distribution and antimicrobial resistance pattern of bacteria isolated from alveolar lavage fluid in pediatric patients with lower respiratory tract infection from 2016 to 2018.Methods:The alveolar lavage fluid of pediatric patients aged <18 years old with lower respiratory tract infection in ten hospitals from January 2016 to December 2018 were collected, and the pathogenic bacteria were cultured and isolated. The paper diffusion method or minimum inhibitory concentration method was used to conduct drug susceptibility tests for the isolated strains.The distribution of pathogenic bacteria and the source department of the strains were analyzed.Chi-sqare test was used for statistical analysis.Results:Of 1 271 isolates, 606 strains (47.7%) were gram-negative bacteria, 628 strains (49.4%) were gram-positive bacteria, and 37 strains (2.9%) were fungi. The common pathogens were Streptococcus pneumoniae (36.7%, 466/1 271), Acinetobacter baumannii (16.8%, 214/1 271), Staphylococcus aureus (12.7%, 162/1 271), Klebsiella pneumoniae (8.3%, 105/1 271), Haemophilus influenzae (7.4%, 94/1 271), Pseudomonas aeruginosa (6.2%, 79/1 271), Burkholderia cepacia (5.3%, 67/1 271) and Escherichia coli (3.7%, 47/1 271). The main pathogens in the intensive care unit (ICU) were gram-negative bacteria (80.1%, 428/534), among which Acinetobacter baumannii was the most common (3.7%, 199/534). The main pathogens in the non-ICU were gram-positive bacteria (70.8%, 522/737). The detection rates of methicillin-resistant Staphylococcus aureus (MRSA) were 45.1%(23/51) in the ICU and 43.2% (48/111) in the non-ICU, respectively. The detection rates of penicillin-resistant Streptococcus pneumoniae (PRSP) were 10.9%(6/55) in the ICU and 18.5% (76/411) in the non-ICU, respectively. The detection rates of carbapenem-resistant Klebsiella pneumoniae (CRKP) were 57.3%(43/75) in the ICU and 33.3%(10/30) in the non-ICU, respectively. The detection rate of carbapenem-resistant Pseudomonas aeruginosa (CRPA) in the ICU was higher than that in the non-ICU (49.1%(27/55) vs 25.0%(6/24)), and the difference was statistically significant ( χ2=3.98, P=0.046). Eight strains (17.0%) of carbapenem-resistant Escherichia coli were detected, and 164 strains(76.6%) of carbapenem-resistant Acinetobacter baumannii were detected. Seventy-eight Haemophilus influenzae isolates were isolated from non-ICU, and the resistance rate to ampicillin was 57.4%(54/94). Burkholderia cepacia isolates were all isolated from ICU, and the resistance rates to piperacillin-tazobactam, levofloxacin, ciprofloxacin and compound sulfamethoxazole were 13.4%(9/67), 3.0%(2/67), 0(0/67) and 9.0%(6/67), respectively. Conclusions:Streptococcus pneumoniae is still the most common pathogen in pediatric patients with lower respiratory tract infection. Gram-negative bacilli are the main pathogens in pediatric patients with lower respiratory tract infection in the ICU. The dection rates of MRSA, PRSP and carbopenem-resistant gram-negative bacilli are high. And the resistance rate of Haemophilus influenzae to ampicillin is also high. The clinical empirical treatment should be determined according to different clinical background.
5.Hereditary haemochromatosis presenting as diabetes and complicated with abnormal liver function, hypogonadism, osteoporosis: a case report

Hereditary hemochromatosis(HH) is relatively rare in the Chinese population, and the disease can involve multiple systems. It is easy to be missed and misdiagnosed due to nonspecific clinical manifestations. We report on a case with diabetes as the first diagnosis and being confirmed HH later. In addition to abnormal liver function, this patient also developed a variety of endocrine and metabolic diseases such as hypogonadism and osteoporosis. Included with this case report is a literature based discussion of clinical features, management of HH along with its relationship with endocrine dysfunction to improve disease understanding.
6.Advances in Treatment of Brain Metastasis from Lung Cancer

Lung cancer is the malignant tumor with the highest incidence and mortality in China, and is prone to brain metastasis in the process of disease development, which seriously affects the quality of life and survival of patients. The treatment methods for brain metastasis of lung cancer include surgery, chemotherapy, whole brain radiotherapy, stereotactic radiosurgery, molecular targeted therapy, immunotherapy, anti-angiogenesis therapy, etc. It's one of the research hotspots to choose reasonable and effective treatment schemes for different patients. This paper reviews the research progress in the treatment of brain metastasis from lung cancer, to provide reference for selecting more reasonable clinical treatment for the patients.
7.Investigation on freshwater crab populations and Paragonimus infections in the Minjiang River basin along the middle section of Wuyi Mountain

Objective To investigate the populations of freshwater crabs, the intermediate host of Paragonimus and Paragonimus infections in freshwater crabs in the Minjiang River basin along the middle section of Wuyi Mountain, so as to provide baseline data for parasitic disease control and research and expansion of the parasite resource bank. Methods From November 2020 to April 2021, freshwater crabs were sampled from streams and ditches neighboring residential areas in Jianning County and its neighboring Ninghua, Shaowu, Jiangle and Shunchang counties. The crab species was identified based on the morphological features of the terminal segment of the first abdominal appendage of male crabs, and Paragonimus infections were detected in freshwater crabs. The Paragonimus metacercariae were isolated, and the types of metacercariae were identified based on the metacercaria size, cystic wall thickness, and the excretory bladder and intestinal tract morphology. In addition, the prevalence, intensity and index of metacercaria infections were calculated in freshwater crabs. Results There were seven crab species found in Jianning County and six neighboring water systems along the Minjiang River basin, including Sinopotamon jianglense, S. fukinense, Huananpotamon lichuanense, H. lini, H. shenni, H. planopodum, Bottapotamon engelhardti, and there were metacercariae of three Paragonimus species detected in these crabs, including P. westermani, P. skrjabini and P. sanpingensis, with a prevalence rate of 43.6% (125/287). The infection rates of P. westermani, P. sanpingensis and P. skrjabini were 57.1% (48/84), 26.2% (22/84) and 61.8% (21/34) in S. jianglense, and the infection rates of P. westermani and P. sanpingensis were 52.6% (51/97) and 30.9% (30/97) in S. fukinense, while the rate of P. westermani infection was 6.9% (5/72) in H. lichuanense, which is the first record of P. westermani infections in H. lichuanense. Mixed P. westermani and P. sanpingensis infections were predominantly found in freshwater crabs sampled from Jianning County, where the rate of Paragonimus infections was 70.4% (76/108), with 15.3 metacercariae identified in each crab with Paragonimus infections and 1.9 metacercariae found in each gram of crabs with Paragonimus infections, and the index of metacercariae infections was 20.5. In addition, P. westermani, P. skrjabini and P. sanpingensi metacercariae were found in freshwater crabs sampled from Jianning-neighboring counties, where the rate of Paragonimus infections was 52.3% (56/107), with 9.8 metacercariae identified in each crab with Paragonimus infections and 0.9 metacercariae found in each gram of crabs with Paragonimus infections, and the index of metacercariae infections was 4.6. Conclusion There are multiple freshwater crab species and Paragonimus infection is high in freshwater crabs in Jianning County and its neighboring Minjiang River basin, which is a high-risk natural focus for Paragonimus infections.
8.Changes of serum biomarkers in rats after blast-induced traumatic brain injury in cabin and the related clinical significance

Objective:To investigate the early changes of serum biomarkers in rats with mild blast-induced traumatic brain injury (bTBI) in cabin.Methods:Explosion source in the simulated cabin was detonated by initiator. The bTBI rat models caused by explosion shock wave in cabin were established. A total of 24 adult male Sprague-Dawley rats were divided into normal control group ( n=6) and bTBI group ( n=18), according to the random number table. Rats in bTBI group were subdivided at 3, 24, and 72 h post-blast, with 6 rats at each time point. Shock wave pressure at the rat head was measured during the explosion. At 3, 24, and 72 h post-blast, the general condition of rats was observed. Rat blood was collected by cardiac puncture. Then brains were taken completely and quickly for pathological observation. HE staining was used to observe the changes of neurons in hippocampal CA1 area. The collected serum was tested for levels of biomarkers, including interleukin- 6 (IL-6), neuron specific enolase(NSE), S100-β, alpha Ⅱ-spectrin breakdown product-145 (SBDP-145) and Tau. Results:The maximum peak value of the shock wave pressure curve at the rat head was (818.2±33.3)kPa, and the duration was about 1 000 μs. After the explosion, the activity of the rats decreased significantly, the hair was dull, and the appetite decreased. General observation showed that the brain tissue was obviously swollen, the blood vessels on the brain surface were thickened, and there was a little patchy bleeding, but no obvious brain contusion was seen. HE staining showed that some neurons in the hippocampus CA1 area had apoptosis or necrosis. At 3, 24, and 72 hours post-blast, the levels of IL-6 were (155.3±10.7)pg/ml, (171.3±25.3)pg/ml and (155.6±18.2)pg/ml, all of which were significantly higher than that in normal control group [(116.3±7.3)pg/ml]( P<0.05); the levels of NSE were (12.0±1.0)ng/ml, (11.0±1.0)ng/ml and (11.0±1.2)ng/ml, all of which were significantly higher than that in normal control group [(8.1±0.5)ng/ml]( P<0.05); the levels of S100-β were (71.9±10.7)pg/ml, (58.0±11.5)pg/ml and (56.5±12.2)pg/ml, all of which were significantly higher than that in normal control group [(35.2±2.5)pg/ml] ( P<0.05); the levels of SBDP-145 were (29.4±2.8)ng/ml, (24.5±4.8)ng/ml and (20.7±2.1)ng/ml, and only the level at 3 h post-blast was significantly higher than that in normal control group [(20.9±1.2)ng/ml]( P<0.05); the levels of Tau were (141.4±11.7)pg/ml, (189.5±28.2)pg/ml and (179.1±32.5)pg/ml, all of which were significantly higher than that in normal control group [(97.8±5.9)pg/ml]( P<0.05). Conclusion:The serum levels of IL-6, NSE, S100-β, SBDP-145 and Tau in mild bTBI rats increase in various degrees at early time, which provides a theoretical basis for use of serum markers in the early diagnosis of mild bTBI.
9.Transradial interventional treatment of coronary artery lesions by using sheathless common 7-F guiding catheter: its clinical efficacy

Objective To compare the efficacy and safety of the use of sheathless common 7-F guiding catheter with those of conventional 6-F sheath guiding catheter in performing transradial percutaneous coronary intervention (PCI) for coronary artery lesions.Methods The clinical data of 160 consecutive patients with coronary artery lesions,who were treated with transradial PCI during the period from January 2014 to June 2015,were retrospectively analyzed.The patients were randomly divided into sheathless group (n=80,using sheathless common 7-F guiding catheter) and sheath group (n=80,using conventional 6-F sheath guiding catheter).The accuracy of the guiding catheter placement,the compression time for the punctured radial artery,the success rate of hemostasis,the incidence of radial artery spasm,the postoperative diameter of radial artery,the fading time of swelling at distal arm,the forearm pain or discomfort,the formation of forearm hematoma,the incidence of pseudoaneurysm and radial artery occlusion,the skin ischemia necrosis,etc.were recorded,and the results were compared between the two groups.Results In the sheathless group and the sheath group,the radial artery spasm was observed in 9 patients and 2 patients respectively,the forearm pain or discomfort was seen in 7 patients and one patient respectively,the differences between the two groups were statistically significant (P<0.05).No statistically significant differences in the accuracy of the guiding catheter placement,the compression time for the punctured radial artery,the formation of forearm hematoma,the success rate of hemostasis,the postoperative diameter of radial artery,the incidence of radial artery occlusion and the skin ischemia necrosis existed between the two groups (P>0.05).Conclusion Compared to the use of conventional 6-F sheath guiding catheter,the use of sheathless common 7-F guiding catheter is safe and effective in treating coronary artery lesions with transradial PCI.
10.Experimental study on brain injury in Beagle dogs caused by adjacent cabin explosion in warship

Objective Through the establishment of adjacent cabin blast injury model of Beagle dog,to investigate the pathophysiological changes in the experimental animals in this scenario,then speculate on the mechanisms of injury.Methods Several adjacent cabins were built in the same size with the real warship.Seven Beagle dogs were subjected to injuries from the explosion,from whom one was selected randomly to implant intracranial pressure transducers before blast,the others were tested on the pathophysiological changes after blast.The dogs were mounted on the platform of a cabinet in the adjacent cabin,subjected to injury from 650g bare TNT explosive blast.The transducers recorded the value of space and intracranial shock wave pressure.Following blast treatment,the serum levels of IL-6,IL-8,neuron specific enolase (NSE),brain and chest CT and pathological changes of the brain tissue were observed.Results Serum levels of IL-6,IL-8 and NSE were elevated to varying degrees after blast.All of them increased significantly at different time points after blast (P<0.05).Brain and chest CT examinations did not show any significant positive results.Pathological results showed that there was a little necrosis in the brain,some neurons had karyopycnosis,karyolysis or disappearance of the nucleoli,and the cell boundaries were blurred.The blast wave was blocked greatly by the scalp and skull (about 90%),but could still penetrate them and cause brain injuries.Conclusions Explosion in the adjacent cabin causes mainly mild traumatic brain injuries.Blast wave can be blocked by the scalp and skull greatly.
