Objective:To explore the necessity and basic connotation of building an integrated eye health service system and propose a systematic const ruction pathway.Methods:A multi-dimensional analytical framework for an integrated eye health service system was constructed using literature review,interviews,and the rainbow model theory.Results:Considering the current state of eye disease prevention and control in China,along with the practical need to address the imbalance in the supply and demand structure of ophthalmic resources,it is necessary to build an integrated eye health service system that is fair,accessible,systematic,continuous,high-quality and efficient.Conclusion:It is needed to further improve the top-level design framework,enhance system integration,consolidate the service network at the grassroots level,strengthen organizational coordination,establish specialized eye health prevention and treatment centers,promote professional collaboration,deliver integrated and proactive service models,optimize service integration,and reinforce enabling factors through functional unification and standardized protocols.

Objective To investigate the relationship between neutrophil/high-density lipoprotein cholesterol ratio(NHR)and the severity of coronary artery lesions in young adults with acute myocardial infarction.Methods A total of 109 young a-dults(aged 18～45 years)with acute myocardial infarction admitted to the First Affiliated Hospital of Anhui Medical University from June 2019 to June 2023 were selected and divided into Syntax low-risk group(n=85)and Syntax medium-high-risk group(n=24)according to Syntax score.Patients with negative coronary angiography during the same period were negative control group(n=52).General clinical data and related serum markers were collected,and NHR value was calculated.Pearson correla-tion analysis and logistics regression analysis were performed to examine the correlation between NHR and the severity of coro-nary artery lesions.Subgroup analysis and interaction testing were further conducted.The receiver operating curve(ROC)was performed to evaluate the value of NHR in predicting the severity of coronary artery lesions in young adults with acute myocar-dial infarction.Results Gender,smoking rate,alcohol consumption rate,diabetes history,absolute number of neutrophils,tri-glyceride,HDL-C,NHR,VLDL-C,apolipoprotein B(APOB),fasting blood glucose and glycosylated hemoglobin were signifi-cantly different among the three groups(P＜0.05).In pairwise comparison,the absolute number of neutrophil and NHR were significantly different among the three groups.BMI was significant different between non-CHD group and Syntax medium-high-risk group.HDL-C,fasting blood glucose and glycosylated hemoglobin were significant different between Syntax low-risk group and non-CHD group,as well as between Syntax medium-high-risk group and non-CHD group(P＜0.05).Creatinine and APOB were significant different between Syntax low-risk group and non-CHD group.Triglyceride,VLDL-C and LP(a)were significant different between Syntax low-risk group and Syntax medium-high-risk group(P＜0.05).Pearson correlation analysis showed a positively correlation between NHR and Syntax score(r=0.322,P＜0.01).Univariate and multivariate binary logistic regres-sion analysis showed that NHR(OR=1.265,95％CI1.097～1.458,P＜0.05)was an independent risk factor for the severity of coronary artery lesions in young adults with acute myocardial infarction.Subgroup analysis showed that NHR was significantly associated with the severity of coronary artery lesions in patients with BMI≥24 kg/m2,smokers,drinkers and non-drinkers(P＜0.05).No significant difference was observed in interaction analysis.The ROC curve showed that the area under the curve(AUC)predicted by NHR for acute myocardial infarction in young adults was 0.758,with a cut-off value of 7.0319,sensitivity of 83.3％,and specificity of 63.53％.Conclusion NHR has a certain value in predicting the severity of coronary artery lesions in young adults with acute myocardial infarction.

Objective:To investigate the association between occupational hazard exposures and small airway function among middle-aged and elderly people.Methods:From July to December in 2015, a multistage cluster random sampling method was used to select 3 600 residents aged 40 years old and above from 6 chronic obstructive pulmonary disease surveillance points in Jiangsu province. A cross-sectional survey was conducted to collect relevant information. Multivariable linear regression model was performed to determine the relationship between occupational hazard exposures and small airway function.Results:A total of 3 347 participants were included in the final analysis, and 44.6% of participants had been exposed to occupational hazard exposures. Compared with participants without the exposure history of occupational hazards, the significantly lower post-bronchodilator FEF 50%, FEF 75% and MMEF levels were observed in those with the exposure history of occupational hazards (β=-82.74, -55.43 and -91.57, respectively). Post-bronchodilator FEF 75% and MMEF (β=-51.78 and -79.47, respectively) in the participants with the exposure history of occupational dust and post-bronchodilator FEF 50%, FEF 75% and MMEF (β=-96.84, -32.87 and -75.72, respectively) in the participants with the exposure history of occupational harmful gas all showed a lower level. Post-bronchodilator FEF 75% was negatively associated with occupational hazard exposures in males (β male=-91.65 vs. β female=-27.21, P for interaction=0.022). Conclusions:The small airway function is worse in the middle-aged and elderly population with the exposure history of occupational hazards, and it is more significant in the male population.

PURPOSE: The long non-coding RNA H19, a conservatively imprinted gene, acts as a molecular sponge for the let-7 family, which has been identified as a set of tumor suppressors. However, the combined prognostic value of H19 and let-7a signature in breast cancer patients remains unclear. METHODS: In this research we assessed the prognostic value of the combined H19 and let-7a signature in breast cancer patients by retrospectively reviewing that data of 79 patients who underwent neoadjuvant chemotherapy; we also investigated the expression and function of H19 in breast cancer cell lines in vitro. Survival data were calculated using the Kaplan-Meier method and compared using the log-rank test. Univariate and multivariate survival analyses were conducted using the Cox proportional hazards regression method. As determined using X-tile, the optimal cutoff value for the risk score to assess progression-free survival (PFS) based on the combined signature was –0.1. RESULTS: Patients with an overall positive treatment response had higher let-7a and lower H19 levels. In addition, let-7a expression was negatively correlated with H19 expression. Patients with a risk score of >–0.1 had shorter overall survival and PFS. In vitro data showed that chemoresistant cell lines exhibit higher H19 and lower let-7a levels and knockdown H19 restores paclitaxel sensitivity. CONCLUSION: Our results suggest that the combined let-7a and H19 signature is a novel prognostic factor for breast cancer patients treated with neoadjuvant chemotherapy.

The physicochemical environment and action are similar between the traditional decoction and the extract technics with water or alcohol in the production of Chinese patent drug. Different heating time inevitably differs Chinese patent drug from its decoction; and the alteration of extracting dissolvent make great changes in the chemical constitution. All these lead to the change in the nature of a Chinese patent drug. The authors hold that it is difficult to embody exactly the aim of the prescription of Chinese drug in the existing production technology of Chinese patent drug. It is necessary to advance innovative thoughts of adopting modern technology to extract effective ingredients from single Chinese drug and in the reference of traditional decoction, recombining the composition and dosage of Chinese patent drug.

On the basis of thermal stability of andrographolide and dehydroandrographolide,the influences of medicinal material factors(habitats,collecting time,storage time),thermal stability,production technology(solvent,temperature,heating time and storage condition)on the content of andrographolide and dehydroandrographolide in Andrographis Tablet(AT)were explored so as to increase the quality of AT.The results showed that the excellent medicinal material of Herba Andrographis,optimized production technology,shorter heating time and lower temperature are the effective measures for increasing the quality of AT.

Objective To develop a RP-HPLC method for the determination of piperine in the root of Piper nigrum L.Methods RP-HPLC was carried out on Luna C18 column(250 mm? 4.60 mm,5 ? m) with the column temperature of 35 ℃.The mobile phase consisted of a mixture of methanol-water(77 :23) at a flow rate of 1.0 mL? min-1.The determination wavelength was at 343 nm.Results The calibration curve was linear within the concentration range of 0.164 ? g～ 0.984 ? g,r=0.9996,and the average recovery was 98.09 %,RSD=2.67 %(n=9).The average content of piperine in three batches of pepper roots was in the range of 6.67～6.77mg?g-1.Conclusion Pepper root contains piperine,and this method is suitable for the quality control of the root of Piper nigrum L.

Objective The rational medication route of puerarin was explore d by studying the concentration- time curve and by comparing the histological and organic distribution difference of puerarin administered by intravenous injecti on or gastric gavage in mice, so as to supply a referential data for its rationa l application. Methods The NIH mice were used as experimental subject. The pu erarin concentrations in the plasma, tissue and organs at different time points were determined by HPLC. The PK solutions 2.0 program, a noncompartmental model software, was applied to calculate the pharmacokinetic parameters of puerarin an d to construct its plasma concentration- time curve. Results (1)The pharmacok inetic parameters of puerarin in mice were shown that the T1/2E of puerarin susp ension (0.2 mg? g- 1) by oral administration is 38.061min, CL =991.534 mL? mi n- 1, Cmax =3.6? g? mL- 1, Tmax =30 min, and AUC(0- ∞ ) =201.7? g? min? mL- 1, the bioavailability of puerarin suspension is 3.77 % compared to i.v puerarin injection. (2) Administered by intravenous injection (i.v), the puerari n distributed in the liver, kidney, plasma, spleen, muscle, lung, uterus and tes ticle rapidly, and the concentration of puerarin was the highest in the liver an d kidney and lower in the heart and brain. Distribution of puerarin suspension b y oral administration is similar to puerarin injection by i.v. However, the conc entration of puerarin in the tissues and organs by oral administration was lower than by i.v; the liver/heart, liver/brain, kidney/heart and kidney/brain concen tration ratios of puerarin by gavage administration were lower than those by i.v . Conclusion The bioavailability of puerarin by oral administration was poor, but the histological distribution characteristics of puerarin shows that the tox ic and side effects of puerarin are lesser by oral use than by intravenous injec tion.
