1.Effect of omalizumab on patients with allergic rhinitis

Objective To investigate the effect of omalizumab on serum helper T lymphocytes,the balance of T-helper cell type 1(Th1)/T-helper cell type 2(Th2),cellular inflammatory factors and disease control in patients with allergic rhinitis.Methods A total of 86 patients with allergic rhinitis admitted to Hangzhou Xiaoshan Hospital of Traditional Chinese Medicine from March 2021 to January 2022 were selected and randomly divided into observation group and control group by single-blind random number table method,with 43 cases in each group.Patients in control group received conventional drug treatment,while those in observation group were treated with omalizumab in addition to the treatment of control group.The nasal symptom scores,clinical efficacy,disease control effect,related symptom scores,Th1/Th2,serological indicators and adverse reactions of two groups were compared.The patients were followed up for 2 years to record the recurrence situation.Results After treatment,the symptom scores of sneezing,nasal congestion,runny nose,and nasal itching,visual analogue scale(VAS),rhinoconjunctivitis quality of life questionnaire(RQLQ),serum immunoglobulin E(IgE),interleukin-4,Th2 levels and recurrence rates of both groups were all lower than those before treatment,and those of observation group were lower than those of control group(P<0.05).After treatment,the asthma control test(ACT)scores,Th1,Th1/Th2,and gamma interferon(IFN-γ)levels of both groups were all higher than those before treatment,and those of observation group were higher than those of control group(P<0.05).The total effective rate of observation group was higher than that of control group after treatment(P<0.05).Conclusion Omalizumab can improve the nasal symptoms and quality of life of patients with allergic rhinitis,regulate the Th1/Th2 balance,reduce the inflammatory response,lower the serum IgE level,and has a definite disease control effect,is safe and reliable,and has a good long-term prognosis.

3.Hypothalamic-Pituitary-Adrenal Axis and Epilepsy

Epilepsy is a recurrent, transient seizure disorder of the nervous system that affects the intellectual development, life and work, and psychological health of patients. People with epilepsy worldwide experience great suffering. Stressful stimuli such as infection, mental stress, and sleep deprivation are important triggers of epilepsy, and chronic stressful stimuli can lead to frequent seizures and comorbidities. The hypothalamic-pituitary-adrenal (HPA) axis is the most important system involved in the body’s stress response, and dysfunction thereof is thought to be associated with core epilepsy symptoms and related psychopathology. This article explores the intrinsic relationships of corticotropin-releasing hormone, adrenocorticotropic hormone, and glucocorticoids with epilepsy in order to reveal the role of the HPA axis in the pathogenesis of epilepsy. We hope that this information will yield future possible directions and ideas for fully understanding the pathogenesis of epilepsy and developing antiepileptic drugs.
4.Combined Let-7a and H19 Signature: A Prognostic Index of Progression-Free Survival in Primary Breast Cancer Patients.

PURPOSE: The long non-coding RNA H19, a conservatively imprinted gene, acts as a molecular sponge for the let-7 family, which has been identified as a set of tumor suppressors. However, the combined prognostic value of H19 and let-7a signature in breast cancer patients remains unclear. METHODS: In this research we assessed the prognostic value of the combined H19 and let-7a signature in breast cancer patients by retrospectively reviewing that data of 79 patients who underwent neoadjuvant chemotherapy; we also investigated the expression and function of H19 in breast cancer cell lines in vitro. Survival data were calculated using the Kaplan-Meier method and compared using the log-rank test. Univariate and multivariate survival analyses were conducted using the Cox proportional hazards regression method. As determined using X-tile, the optimal cutoff value for the risk score to assess progression-free survival (PFS) based on the combined signature was –0.1. RESULTS: Patients with an overall positive treatment response had higher let-7a and lower H19 levels. In addition, let-7a expression was negatively correlated with H19 expression. Patients with a risk score of >–0.1 had shorter overall survival and PFS. In vitro data showed that chemoresistant cell lines exhibit higher H19 and lower let-7a levels and knockdown H19 restores paclitaxel sensitivity. CONCLUSION: Our results suggest that the combined let-7a and H19 signature is a novel prognostic factor for breast cancer patients treated with neoadjuvant chemotherapy.

5.Surgical and orthodontic management of maxillary palatal impacted canines guided by cone-beam compu-ted tomography

Objective:To assess the guidance of 3D reconstruction in the surgical and orthodontic management of maxillary palatal impacted canines with cone-beam computed tomography (CBCT).Methods:22 adolescent patients with 26 impacted maxillary conines were examined by CBCT.CBCT 3D reconstruction was used to observe the impaction of the canines.Measurements were taken to evaluate spatial relationship between the canine and adjacent structures and resorption of neighboring incisor roots.Results:CBCT was effective to localize the impacted canines.All of the maxillary palatal impacted canines inclinated mesially,angles of impacted ca-nines to occlusal plane were 61.1°±18.1°,there were 8,5,3 and 10 impacted canines found in classⅠ,Ⅱ,Ⅲ and Ⅳ mesial mal-position respectively.3 adjacent lateral incisors and 2 central incisors were extracted because of severely root resorption.Normal occlu-sion was achieved for 26 impacted canines by the traction of the canines into dental arch and reaching the occlusal plane.Conclusion:CBCT can spatially show the palatal impaction of maxillary canines and the relationship to adjacent tooth,can guide the treatment plan and traction direction,therefore increase therapeutic effect.
