1.Astrocytic dopamine D1 receptor modulates glutamatergic transmission and synaptic plasticity in the prefrontal cortex through d-serine.

The prefrontal cortex (PFC) plays a pivotal role in orchestrating higher-order emotional and cognitive processes, a function that depends on the precise modulation of synaptic activity. Although pharmacological studies have demonstrated that dopamine signaling through dopamine D1 receptor (DRD1) in the PFC is essential for these functions, the cell-type-specific and molecular mechanisms underlying the neuromodulatory effects remain elusive. Using cell-type-specific knockout mice and patch-clamp recordings, we investigated the regulatory role of DRD1 on neurons and astrocytes in synaptic transmission and plasticity. Furthermore, we explored the mechanisms by which DRD1 on astrocytes regulate synaptic transmission and plasticity at the cellular level, as well as emotional and cognitive functions at the behavioral level, through two-photon imaging, microdialysis, high-performance liquid chromatography, transcriptome sequencing, and behavioral testing. We found that conditional knockout of the Drd1 in astrocytes (CKOAST) increased glutamatergic synaptic transmission and long-term potentiation (LTP) in the medial prefrontal cortex (mPFC), whereas Drd1 deletion in pyramidal neurons did not affect synaptic transmission. The elevated level of d-serine in the mPFC of CKOAST mice increased glutamatergic transmission and LTP through NMDA receptors. In addition, CKOAST mice exhibited abnormal emotional and cognitive function. Notably, these behavioral changes in CKOAST mice could be reversed through the administration of d-serine degrease to the mPFC. These results highlight the critical role of the astrocytic DRD1 in modulating mPFC synaptic transmission and plasticity, as well as higher brain functions through d-serine, and may shed light on the treatment of mental disorders.
2.Research progress on PANoptosis in ischemia-reperfusion injury of liver transplantation

Liver transplantation is the preferred treatment for cirrhosis, end-stage liver failure, and hepatocellular carcinoma, and is also the only effective curative method. Liver ischemia-reperfusion injury (IRI) is one of the main adverse reactions of liver transplantation. During the operation, ischemia mediates the occurrence of liver IRI, promoting the cascade activation of reactive oxygen species and pro-inflammatory signals in Kupffer cells. With continued hepatocellular death during ischemia, damage-associated molecular patterns (DAMPs) accumulate and are released into systemic circulation, triggering a cytokine and chemokine storm, resulting in poor prognosis, postoperative liver failure, systemic inflammatory response syndrome (SIRS), and multiple organ dysfunction syndrome (MODS). In liver transplantation-related IRI, PANoptosis—including apoptosis, pyroptosis, necroptosis, ferroptosis, and autophagy—participates in the process, but a comprehensive review is lacking. This article systematically elaborates on the roles of different types of cell death in liver IRI and the crosstalk among these pathways. It also discusses the protective effects of inhibiting different forms of cell death, aiming to provide direction for future basic research and offer new ideas and strategies for the clinical treatment of liver IRI.
3.The efficacy of femoral triangle block versus adductor canal block on postoperative analgesia in total knee arthroplasty: a meta-analysis

Objective:To compare the analgesic effect of femoral triangle block and adductor canal block after total knee arthroplasty.Methods:Clinical studies comparing the analgesic efficacy of femoral triangle block and adductor canal block after total knee arthroplasty were retrieved from CNKI, Wanfang Data Knowledge Service Platform, VIP, Chinese Medical Journal Full-text Database, PubMed, Embase, and the Cochrane Library, with the retrieval time ranging from the establishment of the databases to October 2024. Data on knee pain scores, morphine consumption, quadriceps muscle strength, postoperative timed up-and-go test (TUGT), and postoperative complications were extracted. The modified Jadad scale was used to evaluate the methodological quality of the included literature. Meta-analysis was performed using Stata 18.0 statistical software.Results:A total of 670 patients from 9 randomized controlled trials were included in the meta-analysis. There were 333 patients in the femoral triangle block group, with an average age of 60.6-72.3 years, and there were 337 patients in the adductor canal block group, with an average age of 61.7-73.5 years. All the 7 English articles were of high quality, and the modified Jadad score was 6-7 points. Two Chinese articles were of low quality and the modified Jadad score was 3. The results of meta-analysis showed that there were no statistically significant differences between the two groups in postoperative morphine consumption [ SMD=0.13, 95% CI(-0.45, 0.71), P=0.658], postoperative 24h knee resting pain score [ SMD=-0.35, 95% CI(-1.48, 0.78), P=0.545], postoperative 48h knee resting pain score [ SMD=-0.56, 95% CI(-1.71, 0.60), P=0.347], postoperative 24h TUGT [ SMD=0.01, 95% CI(-0.27, 0.30), P=0.933], postoperative 48h TUGT [ SMD=0.04, 95% CI(-0.61, 0.69), P=0.905], and incidence of postoperative complications [ RR=1.17, 95% CI(0.56, 2.45), P=0.669]. The quadriceps strength of the adductor canal block group was greater than that of the femoral triangle block group at 24 h after operation, and the difference was statistically significant [ WMD=-0.73, 95% CI(-0.93, -0.53), P<0.001]. Conclusions:Both femoral triangle block and adductor canal block can effectively relieve pain after total knee arthroplasty. Patients with adductor canal block have better muscle strength recovery in the early postoperative period.

8.Application of the “TST-IGRA” two-step method in screening close contacts of active tuberculosis patients among three high schools

Objective To explore the application of the TST-IGRA two-step method in the screening of close contacts of active tuberculosis patients among three high schools in Jinshan District in 2020-2021,and to provide evidence for improving the prevention and control of tuberculosis in schools. Methods Three school tuberculosis outbreaks in Jingshan District from 2020 to 2021 were included in the present study. After excluding active tuberculosis, tuberculin skin test and γ-interferon release assay (IGRA) were conducted to screen latent infection of Mycobacterium tuberculosis in close contacts, and the necessity of the two-step method was analyzed. Results A total of 362 close contacts were screened in the three outbreaks, and 350 people were tested by TST. Comparing the results of TST and IGRA among different types of close contacts, it was found that the positive rate of TST in teachers was higher than that in students(54.84% vs 20.38%,P<0.05). The positive rate of TST among students from high-burden areas of pulmonary tuberculosis in Shanghai was higher than that of local students(24.71% vs 18.80%,P<0.05). IGRA detection was performed on those with moderate and strong positive TST results, and the results showed that the positive rate of IGRA in those with strong positive TST results was only 17.7%, with Kappa value of 0.3. Conclusion The prevention and control of tuberculosis in school should strengthen the annual health examination of teachers and staff, promote timely medical treatment for suspected tuberculosis symptoms, reasonably increase the screening frequency of students in high-burden areas of tuberculosis in Shanghai, and collect students’ previous history of tuberculosis exposure. When screening close contacts, after excluding active tuberculosis, it is recommended that TST and IGRA should be combined to determine latent Mycobacterium tuberculosis infection and achieve accurate intervention.

Blast injury of the chest injury is the most common wound in modern war trauma and terrorist attacks, and is also the most fatal type of whole body explosion injury. Most patients with severe blast injury of the chest die in the early stage before hospitalization or during transportation, so first aid is critically important. At present, there exist widespread problems such as non-standard treatment and large difference in curative effect, while there lacks clinical treatment standards for blast injury of the chest. According to the principles of scientificity, practicality and advancement, the Trauma Society of Chinese Medical Association has formulated the guidance of classification, pre-hospital first aid, in-hospital treatment and major injury management strategies for blast injury of the chest, aiming to provide reference for clinical diagnosis and treatment.
10.Relationship between 20 m shuttle run test performance and lifestyle behaviors of junior high school students

Objective:
To explore the relationship between junior high school students lifestyles and 20 m shuttle run test (20 m SRT) performance, so as to provide a theoretical basis for promoting a healthy lifestyle among junior high school students.
Methods:
From April to June 2021, a total of 2 397 junior high school students aged 12-17 years in Yushan, Wuyuan, Hengfeng and Yugan counties of Shangrao City, Jiangxi Province were selected by random cluster stratified sampling. A questionnaire survey and 20 m SRT test were conducted. Logistic regression analysis was used to explore the association between lifestyles and 20 m SRT scores.
Results:
The passing rate of 20 m SRT for middle school students was 83.3%. Chi square test results showed that there were statistically significant differences between gender, grade, household registration, type of schooling, father s education and the 20 m SRT scores of junior high school students ( χ 2=25.26, 25.04, 6.99, 7.96, 16.22, P <0.05). Significant differences were found in 20 m SRT scores between vigorous and moderate physical activity and breakfast behavior in the last seven days ( χ 2=6.78, 6.29, 9.13, P < 0.05). The results of Logistic regression analysis showed that after controlling for confounding factors, "no vigorous physical activity in the last seven days" was positively correlated with the lower performance of 20 m SRT ( OR =1.31, 95% CI =1.03- 1.67 , P < 0.05). "Breakfast skipping" was positively correlated with 20 m SRT lower performance (frequent eating, OR =1.37, 95% CI = 1.09 -1.73, P <0.01).
Conclusion
Vigorous physical activity and regular breakfast consumption behavior are associated with higher performance 20 m SRT of junior high school students. Schools should encourage students to develop a habit of actively exercising and consuming breakfast regularly in order to ensure the healthy development of cardiopulmonary endurance levels.
