On June 27, 2026, the Asian Pacific Association for the Study of the Liver (APASL) released updated management guidelines for chronic HBV infection, which systematically elaborates on screening, vaccination, antiviral treatment, and hepatocellular carcinoma surveillance and other aspects in 13 key areas, and provides comprehensive recommendations. This article summarizes and makes an excerpt of the key recommendations from the updated guidelines.
2.Construction of a new isovalerylspiramycin I producing strain by CRISPR-Cas9 system.

Isovalerylspiramycin (ISP)Ⅰ, as a major component of bitespiramycin (BT), exhibits similar antimicrobial activities with BT and has advantages in quality control and dosage forms. It has been under preclinical studies. The existing ISPⅠ producing strain, undergoing three genetic modifications, carries two resistant gene markers. Thus, it is hard for further genetic manipulation. It is a time-consuming and unsuccessful work to construct a new ISPⅠ strain without resistant gene marker by means of the classical homologous recombination in our preliminary experiments. Fortunately, construction of the markerless ISPⅠ strain, in which the bsm4 (responsible for acylation at 3 of spiramycin) gene was replaced by the Isovaleryltansferase gene (ist) under control of the constitutive promoter ermEp*, was efficiently achieved by using the CRISPR-Cas9 gene editing system. The mutant of bsm4 deletion can only produce SPⅠ. Isovaleryltransferase coded by ist catalyzes the isovalerylation of the SPⅠat C-4" hydroxyl group to produce ISPⅠ. As anticipated, ISPⅠ was the sole ISP component of the resultant strain (ΔEI) when detected by HPLC and mass spectrometry. The ΔEI mutant is suitable for further genetic engineering to obtain improved strains by reusing CRISPR-Cas9 system.

3.Effects of mitochondrial aldehyde dehydrogenase 2 on autophagy-associated proteins in neonatal rat myocardial fibroblasts cultured in high glucose.

OBJECTIVE:
To investigate whether autophagy mediates the effects of aldehyde dehydrogenase 2 (ALDH2) on the proliferation of neonatal rat cardiac fibroblasts cultured in high glucose.
METHODS:
Cardiac fibroblasts were isolated from neonatal (within 3 days) SD rats and subcultured. The fibroblasts of the third passage, after identification with immunofluorescence staining for vimentin, were treated with 5.5 mmol/L glucose (control group), 30 mmol/L glucose (high glucose group), or 30 mmol/L glucose in the presence of Alda-1 (an ALDH2 agonist), daidzin (an ALDH2 2 inhibitor), or both. Western blotting was employed to detect ALDH2, microtubule-associated protein 1 light chain 3B subunit (LC3B) and Beclin-1 in the cells, and a hydroxyproline detection kit was used for determining hydroxyproline content in cell culture medium; CCK- 8 kit was used for assessing the proliferation ability of the cardiac fibroblasts after the treatments.
RESULTS:
Compared with the control cells, the cells exposed to high glucose exhibited obviously decreased expressions of ALDH2, Beclin-1 and LC3B and increased cell number and hydroxyproline content in the culture medium. Treatment of the high glucose-exposed cells with Alda-1 significantly increased Beclin-1, LC3B, and ALDH2 protein expressions and lowered the cell number and intracellular hydroxyproline content, whereas the application of daidzin resulted in reverse changes in the expressions of ALDH2, Beclin-1 and LC3B, viable cell number and intracellular hydroxyproline content in high glucose-exposed cells.
CONCLUSIONS
Mitochondrial ALDH2 inhibits the proliferation of neonatal rat cardiac fibroblasts induced by high glucose, and the effect is possibly mediated by the up-regulation of autophagy-related proteins Beclin-1 and LC3B.

4.Activation of aldehyde dehydrogenase 2 attenuates myocardial injury in diabetic rats by regulating two-pore potassium channel TASK-1.

To investigate the effect of activating aldehyde dehydrogenase 2 (ALDH2) on TASK-1 two-pore potassium channel in myocardial injury of diabetic rats.
Methods: Diabetic rats were induced by intraperitoneal injection of streptozotocin (55 mg/kg). The diabetic rats were divided into 4 groups: normal group, diabetes at 4th week (DM4W) group, diabetes at 8th week (DM8W) group, and diabetes at 8th week+low concentration of ethanol intervention (DM8W+EtOH) group. The cardiac function of rats was determined by cardiac ultrasonography. The content of hydroxyproline was detected by ELISA. The appearance of myocardial morphous and positive material were observed by HE and PAS staining. The protein expression of TASK-1 was detected by Western blot. Whole-cell patch clamp technique was used to record the action potential duration at 30% and 90% repolarization (APD30, APD90) and two-pore potassium channel TASK-1 current in rat ventricular myocytes. Meanwhile, according to the sensitive electrophysiological characteristics of the potassium channel to acid and base, whether it is two-port potassium channel TASK-1current can be determined.
Results: Compared with the N group, end-diastole left ventricular diameter (LVIDd), end-systolic left ventricular diameter (LVIDs), hydroxyproline content, TASK-1 protein expression increased, APD30 and APD90 extend, left ventricular fractional shortening (LVFS) and left ventricular ejection fraction (LVEF), and TASK-1 current decreased (all P<0.01) in the DM4W group and the DM8W group. HE staining showed that myocardial cell and fiber arrangement disorder, myocyte hypertrophy, myocardial widened and PAS staining reveals that positive material increased in the DM4W group and the DM8W group. Compared with the DM4W group, these changs are more obvious in DM8W rats (P<0.01 or P<0.05). Compared with the DM8W group, in the DM8W+EtOH group, the left ventricular function was restored, the hydroxyproline content and expression of TASK-1 protein were decreased, the TASK-1 current was increased, and APD30 and APD90 were shortened (all P<0.01). HE staining showed that myocardial cell injury was ameliorate and PAS staining showed decreased deposition of positive substances in the DM8W+EtOH group.
Conclusion: Activation of aldehyde dehydrogenase 2 by low concentration of ethanol can reduce myocardial injury and fibrosis caused by diabetes, and its mechanism may be related to the changes of the two-por potassium channel TASK-1.

5.Changes of two-pore K+ channel TASK-1 in diabetic myocardial injury in rats.

OBJECTIVETo investigate the changes of the two- pore K channel TASK-1 in diabetic rats with myocardial injury.
METHODSThirty-six SD rats were divided into normal group (N), diabetes at 4 weeks (DM 4W) group, and diabetes at 8 weeks (DM 8W) group. The cardiac functions of the rats were determined using cardiac ultrasonography, and the body weight and heart weight of the rats at different time points were measured to calculate the heart/body weight ratio (HW/BW). Myocardial fibrosis in the rats was assessed using Masson's staining. The protein expression of TASK-1 in the myocardium was detected using Western blotting. Whole- cell patch clamp technique was used to record the action potential duration (APD) and twopore domain potassium channel TASK- 1 current in acutely isolated rat ventricular myocytes. meanwhile, The inhibition of TASK-1 current was observed by the TASK-1 specific inhibitor ML-365.
RESULTSCompared with the normal group, the diabetic rats showed significantly increased HW/BW ( < 0.05), end- diastole left ventricular diameter (LVIDd), end- systolic left ventricular diameter (LVIDs), and TASK-1 protein expression, with obviously decreased left ventricular diameter shortening rate (FS) and ejection fraction (EF) ( < 0.01). Masson staining showed that in diabetic rats, the collagen fibers were thickened, interwoven into a network with uneven arrangement and increased deposition. Compared with DM 4W group, the rats in DM 8W group exhibited progressive increases in LVIDd, LVIDs, HW/BW, and TASK-1 expression ( < 0.01 or 0.05); FS and EF were further decreased ( < 0.01). Masson staining showed worsened morphological changes of the myocardium with increased deposition. Compared with that in the normal group, the current of TASK- 1 in diabetic rats at 8 weeks was significantly reduced ( < 0.01) and the duration of action potential was extended ( < 0.05). The TASK-1 current was successfully inhibited by ML-365.
CONCLUSIONSDiabetes can induce myocardial fibrosis and aggravate myocardial injury possibly in relation to changes in the protein expression and current of the two-port potassium channel TASK-1.
6.Three-dimensional analysis of upper airway in the adolescents with different vertical skeletal patterns by Cone-Beam CT image

Objective: To study the difference of airway volume among the adolescent subjects with various vertical skeletal patterns. Methods: CBCT records of 88 adolescents with normal sagittal facial pattern were collected and divided into 3 groups according to their FH-MP angle. The subjects of the 3 groups were matched in age and sex. Airway volume and cross-sectional areas were compared among the 3 groups with Dolphin software. Correlation analyses of the airway dimensions with the maxillofacial variables was carried out. Results: There were statistical differences among the 3 groups in volumes of velopharynx,glossopharynx and oropharynx, minimal cross-section area of oropharynx and cross-section area on EP plane,and all the measurements decreased from low angle to normal angle to high angle groups(P < 0. 05). Cross-section areas of HP plane and SP plane in low angle group were significantly larger than that of the normal angle and high angle groups(P < 0. 05). The oropharyngeal airway dimensions showed negative correlation with FMA,and positive correlation with Co-Po,Ar-Gn (except cross-section area of HP). Conclusion: The pharyngeal airway volumes among different vertical skeletal patterns in adolescents are different.
7.Expression of 4"-O-isovaleryltransferase gene from Streptomyces thermotolerans in Streptomyces lividans TK24.

4"-O-isovaleryltransferase gene (ist) was regulated by positive regulatory genes of midecamycin 4"-O-propionyltransferase gene (mpt) in Streptomyces lividans TK24. A BamH I ~8.0 kb fragment from Streptomyces mycarofaciens 1748 was proved that it contained mpt gene and linked with two positive regulatory genes, orf27 and orf28. Orf of mpt was replaced by orf of ist and linked with two regulatory genes or orf27 single, and individually cloned into the vectors pKC1139 or pWHM3 (high copy number), and then transformed into S. lividans TK24. The levels of mpt and ist expression were evaluated by the bio-tramsformation efficacy of spiramycin into 4"-O-acylspiramycins in these transformants. The results showed that 4"-O-isovalerylspiramycins could be detected only in the transformants containing the plasmids constructed with pWHM3. The efficacy of bio-transformation of the transformants containing two regulatory genes was higher than that of orf27 single. So, the positive regulatory genes system of mpt gene could enhance ist gene expression.

8.Effects of blood pressure by intravenous infusion of different doses of oxytocin in cesarean section

Objective To observe the effects of blood pressure by intravenous infusion of different doses ofoxytocin in cesarean section.Methods Sixty full-term pregnant women undergoing cesarean section with continuous epidural anesthesia were divided into three groups by random digits table method with 20 cases each:group A,B and C.Three groups were injected 10 U oxytocin in uterine muscle after infant delivery.Group A,B and C received 5,10 and 20 U oxytocin (sodium lactate ringer,500 ml) continuous intravenous infusion at the speed of 10 ml/min.If happened uterine contractions bad,they were sublingual administering 0.2 mg misoprostol.If happened severe hypotension,they were intravenous injected 5 mg ephedrine.The change of mean arterial pressure (MAP) and heat rate before anesthesia (T0),after fetal childbirth (T1),5 m in (T2),10 min (T3),30 min (T4) after infusion of oxytocin and the dosage of ephedrine and misoprostol were recorded.Results There were no significant differences in MAP and heart rate at every time point between group A and B (P> 0.05).MAP decreased and heart rate increased in group C at T2,T3 compared with those in group A and B,and there were significant differences (P< 0.05).The number of cases of sublingual misoprostol were increased in group A (7 cases) compared with that in group B (2 cases) and group C(1 case).The 8 patients injected ephedrine in group C were more than group A(1 case) and group B (3 cases).Conclusion Cesarean section after the delivery of the fetus in the uterus muscle injection of oxytocin 10 U,after 10 U of oxytocin added 500 ml sodium lactate ringer injection at the speed of 10 ml/min intravenous infusion has little effect on the blood pressure and heart rate,and has good uterine contractions.
9.Relationship between health-promoting lifestyle and sub-health status in the employees of an enterprise.

OBJECTIVETo study the relationship between sub-health status and the health-promoting lifestyle of employees.
METHODSA total of 5316 employees in a company in Guangdong were surveyed using sub-health measurement scale version 1.0 (SHMS V1.0) and the health-promoting lifestyle profile (HPLP-II). The former scale included 3 subscales of somatic sub-health, psychological sub-health and social sub-health, and the latter included 6 subscales of self-actualization, healthy responsibility, physical exercise, nutrition, interpersonal sensitivity and stress treatment.
RESULTSThe total healthy rate was 12.86% among the employees, with 76.76% and 10.5% in sub-health and disease states. The mean scores of HPLP-II was 115.95∓21.468 in the total population surveyed, 134.23∓24.72 in healthy employees and 114.69∓19.25 in the patients. There was a significant difference in the grades of health-promoting lifestyle between sub-healthy and healthy employees (P<0.05) as well as in the scores of HPLP-II and the scores of the 6 subscales (P<0.05). An appreciable correlation was found between sub-health status and the 6 subscales, and self-realization, physical exercise and stress management showed significant inverse correlation with sub-health status.
CONCLUSIONSub-health status is related to health-promoting lifestyle, and self-realization, physical exercise and management are the protective factors that influence sub-health status.

10.The clinical investigation of retention respiration intubation general anesthesia in craniocerebral operation

Objective To study the effect and safety on the retention respiratory inhalation general anesthesia in craniocerebral operation.Methods Eighty-six patients undergoing craniocerebral operation were divided into conventional anesthesia group (group A) and retention respiratory anesthesia group (group B) by random digits table method with 43 cases each.Group A was given conventional induction and intubation,administration order:midazolam 0.05 mg/kg,fentanyl 2-4 μ g/kg,cisatracurium 0.2-0.3 mg/kg,propofol 1.0-1.5 mg/kg;group B was given rapid induction and intubation,administration order:midazolam 0.05 mg/kg,fentanyl 2-4 μg/kg,propofol 1.0-1.5 mg/kg,succinylcholine 1.5 mg/kg.The 2 groups after intubation done by Ohmeda 7100 anesthesia machine control respiration,trace pump continuous infusion of propofol and remifentanil,and maintained inhaling isoflurane anesthesia.Group A was discontinuity intravenous injection atracurium.Group B was changed autonomous breathing after the succinylcholine effect disappeared,respiratory frequency according to the regulation of remifentanil infusion rate,the respiration rate remained at 8-12 times/min.The consumption of propofol and remifentanil,additional times of fentanyl,intraoperative airway pressure,partial pressure of carbon dioxide in end expiratory gas (PE TCO2),intraoperative body movement,airway pressure,extubation time,OAA/S score after extubation 15 min and untoward reaction were recorded.Results There were no statistical differences in operation time,intraoperative body movement,intraoperative airway pressure and PETCO2 between the 2 groups (P> 0.05).The consumption of propofol and remifentanil,additional times of fentanyl in group B were significantly higher than those in group A [(69.2 ± 13.7) μg/(kg·min) vs.(61.0 ± 8.2)μg/(kg·min),(0.19 ± 0.06) μ g/ (kg· min) vs.(0.15 ± 0.05) μ g/ (kg· min),1.5 times vs.0 time],there were statistical differences (P< 0.05).The extubation time in group B was significantly shorter than that in group A [(6.4 ±3.2) min vs.(11.3 ± 4.5) min],the OAA/S score after extubation 15 min in group B was significantly lower than that in group A [1 (1-3) scores vs.2(1-3) scores],there were statistical differences between the 2 groups (P < 0.05).Conclusion Retention respiration intubation general anesthesia patients of craniocerebral operation can early recover,extubation time is significantly shorter,it is a safe and effective anesthesia method.
