1.Intervention of Tongfenglian capsule on inflammation and intestinal microecology in gout arthritis model rats

Objective:To observe the regulatory effects of Tongfenglian capsule on intestinal flora,inflammatory factors and other indicators of gout in rats with gout arthritis(GA),and to explore possible mechanism of Tongfengli capsule on GA.Methods:A total of 36 male SD rats were randomly divided into blank group,Tongfenglian low,medium and high doses(0.216 g/kg,0.432 g/kg,0.864 g/kg)groups,colchicine group(0.18 mg/kg)and model group.Blank group and model group were given the same amount of distilled water,other groups were given the corresponding drug once a day for 14 days.On the 13th day after gavage,GA rat models were prepared according to Coderre modeling method,0.2 ml normal saline was injected into the right ankle joint for blank group,and general conditions of rats were dynamically observed to determine whether the model was established,and the time was recorded.Rate of foot swelling in each group was calculated.Abdominal aorta blood,colon tissue and intestinal contents of rats were collected,and colon length,pathological changes of colon tissue,serum levels of inflammatory factors(IL-1β,TLR4),mRNA expressions of NF-κB and TLR4 inflammatory pathway in intestinal tissue,and intestinal flora of rats were detected.Results:Compared with blank group,swelling rate of ankle joint,serum IL-1β and TLR4 levels,and mRNA expressions of TLR4 and NF-κB in intestinal tis-sue were significantly increased in model group,and a large number of inflammatory cells were infiltrated in the colon by HE staining,showing typical inflammatory pathological changes,colon length decreased significantly,intestinal flora richness and diversity de-creased,ratio of Firmicutes to Bacteroidetes increased,the abundance of Lactobacillus and Bifidobacterium decreased,and the abun-dance of Prevotella and Escherichia coli-Shigella increased.Compared with model group,all the indexes in all Tongfenglian groups were improved,the richness and diversity of intestinal flora were increased,and the reduction of intestinal microbial diversity and mi-crobial community structure caused by monosodiumurate were changed.Conclusion:Mechanism of Tongfenglian capsule alleviating inflammation may be related to the regulation of TLRs/NF-κB signaling pathway.Through the mediation of intestinal flora,Tongfengli-an capsule plays a role in regulating intestinal homeostasis,improving the abundance and diversity of intestinal microorganisms,and playing an intervention role in GA.This microecological intervention mechanism may be a potential means to prevent and treat GA.

3.Study on anti-inflammatory effects and mechanism of Zhuang medicine Tongfeng li’an capsules on gouty arthritis

OBJECTIVE To investigate the anti-inflammatory effects and mechanism of Zhuang medicine Tongfeng li’an capsules on gouty arthritis in combination with in vivo and in vitro experiments. METHODS Sixty rats were randomly divided into normal group, model group, positive control group (27 mg/kg allopurinol+0.27 mg/kg colchicine), Tongfeng li’an capsules low- dose, medium-dose and high-dose groups (2.2, 4.5, 9.0 g/kg), with 10 rats in each group. Except for normal group, gouty arthritis model of rats was induced in other groups. Rats in each administration group were given corresponding drugs intragastrically, and rats in the normal group and model group were given equal volume of water intragastrically for 14 consecutive days. The degree of ankle joint swelling, serum level of interleukin-1β (IL-1β) and protein expressions of nuclear factor kappa-B (NF-κB) in synovial tissue were detected, and the histopathological changes of synovium tissue in the ankle joint of rats were observed. The inflammation model was established by stimulating RAW264.7 cells with lipopolysaccharide. After Tongfeng li’an capsules (62.5, 125, 250 μg/mL) were given, the levels of nitric oxide (NO), reactive oxygen species (ROS) and IL-1β in the cells and protein expression of NF-κB were detected, and NF-κB localization in the cells was also determined. RESULTS Results of in vivo experiment showed that compared with normal group, the swelling degree of the ankle joint, serum IL-1β level and protein expression of NF-κB in synovium tissue were all increased significantly in model group (P<0.05); pathological changes such as synovial hyperplasia, edema, vascular congestion, capillary hyperplasia, and increased inflammatory cells were observed. Compared with model group, the levels of above indexes were all decreased significantly in Tongfeng li’an capsules high-dose group (P<0.05), and most of the above indexes were significantly reduced in Tongfeng li’an capsules medium-dose and low-dose groups (P<0.05); synovial hyperplasia of the ankle joint improved, and the infiltration of inflammatory cells 2019BS044) decreased. Results of in vitro experiment showed that Tongfeng li’an capsule could significantly reduce the levels of NO, ROS and IL-1β and protein expression of NF-κB(P<0.01), and inhibit NF- κB nucleation. CONCLUSIONS Tongfeng li’ancapsules have good anti-inflammatory effect on gouty arthritis, and its mechanism may be related to the inhibition of NF-κB signaling pathway activity.
4.Comparative analysis of effect of two kinds of biological agents in volume-reduced bullae

Objective To retrospectively analyze the effect of two kinds of biological agents in volume -re-duced bullae .Methods 11 patients who suffered from bullae were operated under large C-arm locating ,and infused two kinds of biological agents through micro catheter of fibreoptic bronchoscopy .All of them were randomly divided into the two groups .The biological agents in group A were fibrinogen and diluent thrombin , and that of group B was Porcine Fibrin Sealant Kit .In group A,the micro catheter with diameter of micro thread less than 1.2mm was placed in bullae through fibreoptic bronchoscope ,and then the 2mL lidocaine,5 ml fibrinogen,and double of 500u diluent thrombin were inproperorder injected through micro catheter .In group B,the Porcine Fibrin Sealant Kit was injected at the same method,and then the suspension fluid was exacted .The operation time was recorded ,and then the clinical efficacy and incidence rate of complications were compared .Results The operation time of group A was 5-15 minutes, and that of group B was 6-20 minutes.For all the patients ,4 cases were totally effective ,2 cases were significantly effective,and 2 cases were totally non-effective.The total effective rate was 81.82%(9/11).The incidence rates of common complications in group A and B were 52.38%(22/42),58.33%(14/24),respectively,the difference was not significant (χ2 =0.22,P>0.05).Moreover,there were no serious complications in all cases .Conclusion The security and effect of two kinds of biological agents might be well enough ,but in view of less cases ,they were worth to further popularized and applied in clinical practice .
