1.Clustering analysis of chronic diseases risk factors among adult residents in Rui'an City

Objective:
To analyze the prevalence status and clustering patterns of risk factors for chronic diseases among adult residents in Rui' an City, Zhejiang Province, so as to provide a basis for formulating strategies for the prevention and control of chronic diseases and implementing risk factor interventions.
Methods:
A multi-stage random cluster sampling method was used to select residents aged ≥18 years in 5 townships (sub-districts) of Rui' an City as survey subjects from December 2023 to March 2024. Data on basic information, history of major chronic diseases, lifestyle behaviors, height, weight, and blood biochemical indicators were collected through questionnaire surveys, physical examinations, and laboratory tests. Descriptive analysis was used to analyze the prevalence status of 5 risk factors for chronic diseases. K-means clustering analysis was used to analyze clustering patterns.
Results:
A total of 3 060 people were surveyed, including 1 476 males, accounting for 48.24%, and 1 584 females, accounting for 51.76%. The median age was 49.00 (interquartile range, 25.00) years. There were 1 275 cases (41.67%) of hypertension, 462 cases (15.10%) of diabetes, and 1 460 cases (47.71%) of dyslipidemia. Insufficient fruit and vegetable intake, excessive salt intake, overweight and obesity, excessive red meat intake, and smoking involved 2 201, 1 878, 1 541, 1 337, and 571 people, with prevalences of 71.93%, 61.37%, 50.36%, 43.69%, and 18.66%, respectively. K-means clustering analysis identified 4 clustering patterns: smoking-insufficient fruit and vegetable intake type (249 people, accounting for 8.20%), low intake type (1 421 people, accounting for 46.75%). high red meat type (245 people, accounting for 8.07%), and high BMI-high salt type (1 118 people, accounting for 36.96%). The prevalences of hypertension and diabetes were higher in adult residents of the high BMI-high salt type, at 56.53% and 20.30%, respectively. The prevalence of dyslipidemia was higher in adult residents of the smoking-insufficient fruit and vegetable intake type, at 59.44%.
Conclusions
The prevalence of insufficient fruit and vegetable intake among adult residents in Rui' an City is relatively high. The clustering pattern of chronic disease risk factors is dominated by the low intake type. The prevalence of chronic diseases is higher in adult residents of the high BMI-high salt type and smoking-insufficient fruit and vegetable intake type. It is suggested to carry out health education and collaborative intervention of risk factors for different risk groups.
2.Application of left ventricular assist device in the treatment of end-stage heart failure:a multi-center report of 48 cases

Objective:To evaluate the clinical efficacy of left ventricular assist device(LVAD) in the treatment of end-stage heart failure.Methods:This is a retrospective case series study. Data were retrospectively analyzed from 48 patients treated at eight domestic cardiac surgery centers in China between March 2022 and September 2024, who underwent LVAD implantation due to end-stage heart failure. All surgeries were led by the cardiovascular surgery team from Nanjing First Hospital. The study included 42 male and 6 female patients, aged (59.1±12.3) years (range:28 to 73 years). All patients had a preoperative New York Heart Association(NYHA) functional class of Ⅳ. General preoperative data, intraoperative extracorporeal circulation time, cross-clamp time, postoperative ICU stay, hospital stay, pump parameters at discharge, intraoperative and postoperative left ventricular end-diastolic diameter, left ventricular ejection fraction, and follow-up results post-discharge were collected. The generalized estimation equation was used to compare the difference of cardiac function indexes in patients with different follow-up time points. The Tukey′s Honestly Significant Difference test was used for pairwise comparison within the group.Results:The surgery was successfully completed for 47 patients, and 1 perioperative death. The extracorporeal circulation time was (122.4±21.6) minutes (range: 101 to 200 minutes), the cross-clamp time was (70.1±18.3) minutes (range: 101 to 200 minutes), the postoperative ICU length of stay was (6.9±3.2) days (range: 2 to 23 days), and the hospital length of stay was (28.1±4.2) days (range: 23 to 42 days). At discharge, the blood bump flow rate was (3.4±0.6) L/min(range: 3.1 to 3.6 L/min), and speed was (2 791.3±142.0) r/min(range: 2 680 to 3 250 r/min). The follow-up period was (12.2±0.78) months (range:2 to 24 months) of 47 patients. One case underwent heart transplantation during the follow-up period, and 3 cases died, with no major device malfunction events. Three months post-surgery, the patients showed a significant reduction in left ventricular end-diastolic diameter compared to pre-surgery ((69.1±8.3) mm vs. (77.3±8.3) mm, q=6.73, P<0.01); the left ventricular ejection fraction significantly increased ((29.6±8.0)% vs. (23.2±5.8)%, q=6.49, P<0.01); and the 6-minute walk distance significantly improved ((382.0±12.8)metres vs. (114.8±18.4)metres, q=116.56, P<0.01). Six months postoperatively, all patients′ cardiac function had recovered to NYHA class Ⅰor Ⅱ. Conclusions:Preliminary research findings indicate that left ventricular assist device therapy yields favorable outcomes in the treatment of end-stage heart failure. serving as a bridge to transplantation, recovery, or a destination therapy.
3.Pneumonia caused by Rhizopus microsporus:a case report and literature review

Objective To review the characteristics of Rhizopus microsporus infection for better awareness of the disease.Methods One case of pneumonia caused by R.microsporus was reported in a 66-year-old male patient.Similar reports on infections caused by R.microsporus were retrieved in PubMed and CNKI databases since 2013.The characteristics of patients with R.microsporus infection were reviewed.Results This case involves a 66-year-old male patient presenting with a 10-day history of cough,sputum production,and hemoptysis.The patient had a previous history of diabetes mellitus.Pulmonary CT scan revealed an irregular soft tissue density mass in the right lower lobe and pneumonia.The clinical presentation and laboratory findings were consistent with pulmonary mucormycosis caused by R.microsporus.R.microsporus was isolated from bronchoalveolar lavage fluid culture.The patient was treated with intravenous liposomal amphotericin B followed by oral posaconazole for antifungal therapy,and achieved a favorable prognosis.A total of 24 cases(18 males,5 females,1 unknown)of R.microsporus infection were reviewed(including this one).The specific site of infection included pulmonary infection(n=10),ocular infection(n=1),skin tuberculosis(n=1),splenic abscess(n=1),oral mucositis(n=1),gouty arthritis(n=1),esophageal ulceration(n=1),abdominal infection(n=1),and others(n=7).The clinical symptoms varied with the organs involved.Majority of the patients(n=11)were cured by surgery combined with antimicrobial therapy.Overall,13 patients died.Conclusions Clinicians should be aware of the possibility of R.microsporus infection in case of pulmonary infection,especially those with diabetes mellitus.

6.Efficiency analysis of hyperbaric oxygen therapy for paroxysmal sympathetic hyperactivity after brain injury: a multicenter retrospective cohort study.

OBJECTIVE:
To investigate the effect of hyperbaric oxygen (HBO) on paroxysmal sympathetic hyperexcitation (PSH) after brain injury.
METHODS:
A multicenter retrospective study was conducted. Fifty-six patients with PSH who received HBO treatment from four hospitals in Henan Province from January 2021 to September 2023 were selected as the HBO group, and 36 patients with PSH who did not receive HBO treatment from Zhengzhou People's Hospital from May 2018 to December 2020 were selected as the control group. PSH assessment measure (PSH-AM) score [clinical feature scale (CFS) score+diagnostic likelihood tool (DLT) score] and Glasgow coma scale (GCS) were compared before and after HBO treatment, and between HBO group and control group to evaluate the effect of HBO treatment on prognosis of PSH patients.
RESULTS:
There were no statistically significant differences in age, gender, PSH etiology, GCS score, time from onset to occurrence of PSH, CFS score, CFS+DLT score and frequency of PSH episodes between the two groups, indicating comparability. The duration of HBO treatment ranged from 3 to 11 days for 56 patients receiving HBO treatment, and the duration of HBO treatment ranged from 3 to 5 courses. Compared with before treatment, after HBO treatment, PSH symptoms in HBO patients were significantly relieved (body temperature increase: 14.29% vs. 64.29%, heart rate increase: 25.00% vs. 98.21%, shortness of breath: 14.29% vs. 76.79%, blood pressure increase: 8.93% vs. 85.71%, sweating: 10.71% vs. 85.71%, muscle tone increased: 19.64% vs. 75.00%, all P < 0.05), CFS+DLT score decreased significantly (16.90±4.81 vs. 22.12±3.12, P < 0.01), GCS score improved (12.31±5.34 vs. 5.95±2.18, P < 0.01). After 30 days of hospitalization, compared with the control group, PSH symptoms in the HBO group were improved (body temperature increase: 14.29% vs. 19.44%, heart rate increase: 19.64% vs. 25.00%, shortness of breath: 10.71% vs. 27.78%, blood pressure increase: 7.14% vs. 22.22%, sweating: 8.93% vs. 25.00%, muscle tone increased: 19.64% vs. 38.89%, all P < 0.05 except body temperature increase), CFS+DLT score decreased (16.90±3.81 vs. 19.98±4.89, P < 0.05), GCS score increased (14.12±4.12 vs. 12.31±4.14, P < 0.01), the length of intensive care unit (ICU) stay was shortened (days: 18.01±5.67 vs. 24.93±8.33, P < 0.01).
CONCLUSIONS
HBO treatment can significantly relieve the symptoms of patients with PSH after brain injury and provide a new idea for the treatment of PSH patients.

7.Emergency single drainage tube and dual target thalamic hematoma ventricular drainage surgery based on body surface marker localization for the treatment of thalamic hemorrhage breaking into the ventricle with hydrocephalus

Objective To explore the application value of emergency temporal body surface positioning for single drain dual-target thalamic hematoma ventricular drainage in the treatment of thalamic hemorrhage breaking into ventricle with hydrocephalus.Methods A retrospective analysis was conducted on 223 patients with thalamic hemorrhage breaking into the ventricles with hydrocephalus,including a study group of 115 cases who underwent emergency single drain dual-target thalamic hematoma ventricular drainage surgery with temporal body surface positioning,and a control group of 108 cases who underwent emergency ventricular drainage first and then underwent stereotactic thalamic hematoma drainage surgery after the condition stabilized.Compare the differences in postoperative complications and treatment outcomes between two groups of patients,and evaluate the application value of temporal surface positioning for single drain dual-target thalamic hematoma ventricular drainage surgery in the treatment of thalamic hemorrhage breaking into the ventricle with hydrocephalus.Results The postoperative rebleeding rates,hematoma clearance and death were 5.2%,87.5%±7.3%and 13.9%in the study group and 4.7%,90.2%±8.5%and 15.7%in control group,respectively.There was no significant difference between the two groups(P>0.05).The tube time,postoperative intracranial infection,Shunt dependent hydrocephalus,effective treatment and favorable prognosis of and the control group were(75.5±18.4)h,3.5%,19.1%,53.9%and 51.3%in the study group and(130.8±22.9)h,13.9%,35.1%,38.7%and 38.0%,respectively.The difference between the two groups was statistically significant(P<0.05).Conclusion Body surface landmark-guided emergency single drain dual-target thalamic hematoma ventricular drainage surgery for the treatment of thalamic hemorrhage breaking into the ventricle with hydrocephalus is safe and reliable,and can improve the patient's prognosis.
8.Study of the effect of PU-H71 on increasing radiosensitivity of cervical cancer cells

Objective:To investigate the effect of heat shock protein 90 (Hsp90) inhibitor PU-H71 combined with X-ray on radioresistant human cervical cancer cells.Methods:The expression levels of Hsp90 gene between cervical cancer tissues and adjacent tissues were analyzed by bioinformatics. Radioresistant cervical cancer cell lines HeLa RR and SiHa RR were obtained by fractional irradiations (2 Gy per fraction, 30 fractions). The cell lines were divided into the control group (treated with dimethyl sulfoxide), irradiation alone group, PU-H71 group (treated with 0.5 μmol/L PU-H71), and PU-H71+irradiation group (irradiation at 24 h after treatment with 0.5 μmol/L PU-H71). Cell survival was detected by clonal formation assay. Immunofluorescence assay was used to detect γH2AX foci at 1, 6, and 24 h after cell treatment. The expression level of Rad51 protein at 1, 2, 6, 12, and 24 h after cell treatment was detected using Western blot. The expression level of phosphorylated DNA-dependent protein kinase catalytic subunit (p-DNA-PKcs) was measured at 2 h after cell treatment. Cell apoptosis at 48 h after cell treatment was assessed by flow cytometry. Results:PU-H71 enhanced the sensitivity of radioresistant cervical cancer cells to X-ray. Compared with the irradiation alone group, the radiation sensitization ratios (SER) of HeLa RR and SiHa RR cells at 10% survival were 1.36 and 1.27, and the apoptosis rates were increased by approximately 72.1% and 63.1% in the PU-H71+irradiation group, respectively. PU-H71 delayed the duration of γH2AX foci induced by X-ray, inhibited the phosphorylation of DNA-dependent protein kinase catalytic subunit (DNA-PKcs), thus preventing non-homologous end joining (NHEJ) repair and delaying homologous recombination repair.Conclusion:PU-H71 increases the radiosensitivity of radioresistant cervical cancer cells by inhibiting the repair pathway of DNA double-strand break, which is expected to be a radiosensitizer to enhance the efficacy of radiotherapy for cervical cancer.
9.Embedded 3D printing of porous silicon orbital implants and its surface modification.

OBJECTIVE:
To prepare customized porous silicone orbital implants using embedded 3D printing and assess the effect of surface modification on the properties of the implants.
METHODS:
The transparency, fluidity and rheological properties of the supporting media were tested to determine the optimal printing parameters of silicone. The morphological changes of silicone after modification were analyzed by scanning electron microscopy, and the hydrophilicity and hydrophobicity of silicone surface were evaluated by measuring the water contact angle. The compression modulus of porous silicone was measured using compression test. Porcine aortic endothelial cells (PAOECs) were co-cultured with porous silicone scaffolds for 1, 3 and 5 days to test the biocompatibility of silicone. The local inflammatory response to subcutaneous porous silicone implants was evaluated in rats.
RESULTS:
The optimal printing parameters of silicone orbital implants were determined as the following: supporting medium 4% (mass ratio), printing pressure 1.0 bar and printing speed 6 mm/s. Scanning electron microscopy showed that the silicone surface was successfully modified with polydopamine and collagen, which significantly improved hydrophilicity of the silicone surface (P < 0.05) without causing significant changes in the compression modulus (P > 0.05). The modified porous silicone scaffold had no obvious cytotoxicity and obviously promoted adhesion and proliferation of PAOECs (P < 0.05). In rats bearing the subcutaneous implants, no obvious inflammation was observed in the local tissue.
CONCLUSION
Poprous silicone orbital implants with uniform pores can be prepared using embedded 3D printing technology, and surface modification obviously improves hydrophilicity and biocompatibility of the silicone implants for potential clinical application.

Ma-huang Lian-yao Chi-xiao-dou Tang is one of the three formulas for typhoid jaundice.But it has not received much attention,and the source of Lian-yao in the formula is unknown and the status and role are difficult to identify.In this paper,we first analyzed the functions of Ephedra and the symptoms of the whole formula.The formula was mainly used to"clear damp-heat"in order to remove jaundice.On this basis,it is proved that Lian-yao is the root of Forsythia,not"Qiao-gen".Today,the use of Forsythia instead of Lian-yao has the same effect.Then,the indispensable and important position of Forsythia in the formula was analyzed.It also pointed out some valuable directions for the medicinal use and research and development of Forsythia based on its wide range of effects but the lack of main symptoms.This paper provides insights for in-depth theoretical and practical research on Forsythiae Fructus.
