1.The molecular subtyping of urothelial carcinoma using GATA-3 and CD44

Purpose To investigate the correlation between molecular typing of urothelial carcinoma(UC)and its clinicopathologic features and prognosis,in order to explore the prognostic markers and therapeutic targets for UC.Methods 115 patients with UC were retrospectively analyzed.Immunohistochemical markers GATA-3,CK20,CK5/6 and CD44 were used for molecular typing of UC(luminal-like type,basal-like type and null).Correlations between molecular typing and clinicopathological features were analyzed using the Chi-square test and Fisher precise test.Sur-vival analysis was performed using the Kaplan-Meier test and Log-rank test.Results The expression of GATA-3 and CK20 was negatively correlated with the clinical stage of UC,while the expression of CD44 was positively correlated with the clinical stage of UC(both P<0.05).CK20 was a marker of good prognosis(P=0.03).The proportion of clinically advanced UC with basal-like type was significantly higher than that of luminal-like type(78.4%vs 53.4%,P=0.033).Among the histologic variants,UC with neuroendocrine differentiation(100%),sarcomatoid carcinoma(80.0%)and squamous differentiation(77.8%)were basal-like type.All plasmacytoid and lymphoepithelioma-like types,as well as 81.8%of micropapillary UC.Among the null phenotypes,the differential variant predominated(66.7%).Compared with the luminal-like type,although the prognosis of basal-like UC was worse,there was no sta-tistically significant difference(P>0.05).Conclusion Patients with CK20-expressing UC had a significantly better prognosis.The main histologic variants types of basal-like type and coelomofacial type are different.Molecular typing of UC using immunohistochemical markers is suggestive of clinical staging and prognosis of patients.
2.Correlation of POSTN and NECTIN-3 expression with clinicopathological features and prognosis in urothelial carcinoma

Purpose To investigate the relationship between the expression of POSTN and NECTIN-3 in urothelial carcinoma(UC)and its clinicopathologic features and prognosis.Methods Clinical data of 115 UC patients were col-lected.EnVision two-step method was used for immunohistochemical analysis to determine the expression of POSTN and NECTIN-3 in UC and their correlation with clinicopathological features and prognosis.Transcriptomic data from TCGA database were used to analyze the correlation between the expression of POSTN and NECTIN-3 in bladder cancer and the pathological stage and prognosis of bladder cancer.qPCR and Western blot were used to detect the expression levels of POSTN and NECTIN-3 in tumor tissues and adjacent tissues of patients.Results TCGA database analysis showed that the expression level of NECTIN-3 in urinary bladder urothelial carcinoma(UBUC)was significantly lower than that in paracancer tissues,and POSTN expression was positively correlated with pathological stage.Prognostic a-nalysis showed that POSTN expression was negatively correlated with the overall survival of UBUC(P>0.05),and NECTIN-3 expression was negatively correlated with disease-free survival of UBUC(P>0.05).The experimental re-suits showed that patients with positive POSTN expression were more prone to perineural invasion[25 cases(86.2%)vs 4 cases(13.8%),P=0.019],vascular invasion[36 cases(83.7%)vs 7 cases(16.3%),P=0.007],and lymph node metastasis[24 cases(88.9%)vs 3 cases(11.1%),P=0.033].Additionally,the positive expression rate of POSTN in UBUC was significantly higher than that in upper tract urothelial carcinoma(UTUC)(75.0%vs 54.3%,P=0.028).Higher expression levels of POSTN and NECTIN-3 were associated with shorter overall survival,but the difference was not statistically significant(P>0.05).Conclusion There is no significant correlation between NECTIN-3 and the clinicopathological features and prognosis of urothelial carcinoma,while the expression of POSTN is correlated with the invasive clinicopathological features of UC,which has certain suggestive significance for clinical stage and prognosis.

6.Correlation of POSTN and NECTIN-3 expression with clinicopathological features and prognosis in urothelial carcinoma

7.Mechanism of oxymatrine promoting the apoptosis of osteosarcoma MG63 cell line through mitophagy mediated by COX-2/PINK1/Parkin signaling pathway

OBJECTIVE To study the mechanism of oxymatrine inducing apoptosis of osteosarcoma MG63 cell line based on mitophagy mediated by cyclooxygenase-2 (COX-2)/PTEN-induced putative kinase-1 (PINK1)/Parkinson disease protein-2 (Parkin) signaling pathway. METHODS MG63 cells were treated with 2.0, 4.0, 8.0 mg/mL oxymatrine and 6 μmol/L 5-fluorouracil, then the apoptotic rate, the expression of apoptosis-related proteins [B-cell lymphoma-2 (Bcl-2), Bcl-2 related X protein (Bax)], the proportion of decrease in mitochondrial membrane potential, the level of mitophagy as well as the protein expressions of PINK1, Parkin, and microtubule-associated protein 1 light chain-3Ⅱ (LC3-Ⅱ) were detected. PINK1 small interfering RNA (siRNA) was adopted to intervene in the expression of PINK1, the cells were divided into control group, PINK1 siRNA group, oxymatrine group, and PINK1 siRNA+oxymatrine group; the protein expressions of PINK1, Parkin, and LC3-Ⅱ, the proportion of decrease in mitochondrial membrane potential (MMP) as well as apoptotic rate were detected. The lentivirus infection technique was used to overexpress COX-2, the cells were divided into control group, oxymatrine group, COX-2 group, and COX-2+oxymatrine group. The protein expressions of COX-2, PINK1, and Parkin, as well as the proportion of decrease in MMP were detected. RESULTS After being treated with oxymatrine, the apoptotic rate, the protein expressions of Bax, PINK1, Parkin, and LC3-Ⅱ, the level of mitophagy as well as the proportion of decrease in MMP were significantly increased (P<0.05), while the protein expression of Bcl-2 was significantly decreased (P<0.05). Compared with the oxymatrine group, the protein expressions of PINK1, Parkin, and LC3-Ⅱ, apoptotic rate and the proportion of decrease in MMP were significantly decreased in PINK1 siRNA+oxymatrine group (P<0.05). Compared with the oxymatrine group, the protein expression of COX-2 in the COX-2+oxymatrine group was increased significantly (P<0.05), while the protein expressions of PINK1 and Parkin as well as the proportion of 526087266@qq.com decrease in MMP were decreased significantly (P<0.05). CONCLUSIONS Oxymatrine can mediate the overactivity of mitophagy based on the PINK1/Parkin signaling pathway by inhibiting COX-2 expression, thus promoting the apoptosis of the MG63 osteosarcoma cell line.
8.Effect and mechanism of berberine-induced ferroptosis in osteosarcoma cells

OBJECTIVE To investigate the effect of berberine on ferroptosis in MG63 osteosarcoma cells and its mechanism. METHODS Using cells without drug treatment as control, the cell viability, proliferation, the related indexes of ferroptosis [nuclear proliferation associated-antigen (Ki67), mitochondrial ultrastructure, ferric ion (Fe2+), reactive oxygen species (ROS), malondialdehyde (MDA), and glutathione (GSH)], the protein expression of signal transducer and activator of transcription 3 (STAT3), tumor protein 53 (p53), and solute carrier family 7 member 11 (SLC7A11) were detected after being treated with different concentrations of berberine. Cells were transfected with p53 siRNA and then assigned to the control group, p53 siRNA group, berberine group, and p53 siRNA+berberine group to explore the role of p53 in berberine-induced ferroptosis. After 24 h incubation with 10.0 μmol/L berberine, the protein expressions of p53 and SLC7A11, the levels of mitochondrial membrane potential, GSH, and MDA content were determined. Cells were transfected with STAT3 overexpressed plasmid and then assigned to the control group, berberine group, STAT3 group, and STAT3+berberine group to explore the effect of STAT3 on the regulation of the p53/SLC7A11 pathway. After 24 h incubation with 10 μmol/L berberine, the protein expressions of p-STAT3, STAT3, p53, and SLC7A11 were detected. RESULTS Compared with the control cell, the concentrations of 2.5, 5.0 and 10.0 μmol/L berberine could reduce the cell viability and expression of Ki67, and induce the morphological changes in ferroptosis-related mitochondria, increase the levels of Fe2+, ROS and MDA, and the protein expression of p53, reduce the level of GSH, the binding activity of STAT3 with DNA, and the protein expressions of p-STAT3 and SLC7A11; the above differences were statistically significant (P< 0.05 or P<0.01). Compared with the berberine group,significantly down-regulated p53 protein expression and MDA level, up-regulated SLC7A11 protein expression, and increased mitochondrial membrane potential and GSH level were observed in the p53 siRNA+berberine group (P<0.01). Compared with the berberine group, the protein expressions of p-STAT3, STAT3, and SLC7A11 in the STAT3+berberine group were significantly increased (P<0.01), while the protein expression of p53 was significantly decreased (P<0.01). CONCLUSIONS Berberine can induce the ferroptosis of MG63 cells by mediating STAT3/p53/SLC7A11 signaling pathway.

Depression is a common mood disorder in perioperative period.Both preoperative depres-sion and postoperative new depression can affect postoperative rehabilitation of patients,and there is a lack of effective intervention measures.Related studies have indicated that esketamine can alleviate perioperative depressive symptoms.In order to provide references for clinical work and reduce the occurrence of periopera-tive depression,this review summarizes perioperative depression,esketamine and its anti-depression mecha-nism,the application of esketamine in prevention and treatment of perioperative depression and its related adverse reactions.
10.Effect of intraoperative continuous pump infusion of remimazolam on postoperative delirium in eld-erly patients with hip fracture

Objective To observe the effect of intraoperative continuous pump infusion of remima-zolam on postoperative delirium(POD)in elderly patients with hip fracture under subarachnoid block.Methods A total of 236 elderly patients undergoing elective hip fracture surgery under subarachnoid block,82 males and 154 females,aged≥65 years,BMI 18.5-31.0 kg/m2,ASA physical status Ⅰ-Ⅲ,were divided into two groups by using the randomized number table method:remimazolam group(group R)and control group(group C),118 patients in each group.When the surgical site was disinfected and cov-ered,the patients in group R were administered the first dose of reminmazolam 0.1 mg/kg and followed by a continuous infusion 0.2 mg·kg-1·h-1 intravenously to maintain the modified observer's assessment of alert/sedation(MOAA/S)1 or 2 scores.Reminmazolam infusion was stopped at the end of the surgery.The same volume of normal saline was injected for patients in group C.POD was assessed by the confusion as-sessment method-Chinese revised version(CAM-CR)1-3 days after the surgery.The anxiety scores one day before and one day after the surgery,and plasma cortisol concentrations when the patients were admitted into the operating room and at the end of the surgery were recorded.The incidence of intraoperative hypertension,hypotension,bradycardia,hypoxemia,and postoperative nausea and vomiting were also recor-ded.Results Compared with group C,POD incidence in group R was significantly decreased(P<0.05),anxiety score one day after surgery and plasma cortisol concentration at the end of the surgery in group R were significantly decreased(P<0.05),and the incidence of intraoperative hypertension in group R was significantly decreased(P<0.05).There were no significant differences in the plasma cortisol con-centrations when the patients were admitted into the operating room and the incidences of intraoperative hy-potension,bradycardia,hypoxemia,and postoperative nausea and vomiting between the two groups.Conclusion Remimazolam can reduce the incidence of postoperative delirium in elderlypatients undergoing hip fracture surgery under subarachnoid block,which may be related to alleviating intraoperative anxiety and inhibiting intraoperative stress response.
