1.Single-incision bariatric surgery: advantages and challenges

Single-incision bariatric surgery, as a significant advancement in minimally invasive surgery, achieves weight loss goals through a single small incision, offering advantages such as minimal trauma, concealed scarring, and rapid postoperative recovery. The single-incision laparoscopic sleeve gastrectomy (SILSG) currently represents the most widely performed single-incision bariatric procedure globally, demonstrating weight loss efficacy and complication rates comparable to traditional multi-incision laparoscopy alongside significantly higher patient satisfaction. However, the clinical adoption of single-incision bariatric surgery still faces technical challenges with relative scarcity in robust clinical research evidence and underdeveloped standardized surgical protocols and training systems. To further establish a specialized training system and expert consensus for single-incision bariatric surgery that aligns with China's national conditions is of great significance for promoting the standardized implementation of SILSG-represented procedures.
2.Discussion and Analysis of the Special Syndromes of the Five Zang Organs in Zhang Yuansu's Medication Patterns

The Song,Jin,and Yuan dynasties marked a period of transformation in traditional Chinese medicine for visceral syn-drome differentiation,with the emergence of numerous new ideas and theories.Zhang Yuansu was a key figure in the transition from Han and Tang to Jin and Yuan medicine.His Medication Patterns for Treating Deficiency-Excess and Cold-Heat of Five Viscera embod-ies his thinking on visceral syndrome differentiation.This work breaks away from the pre-Song classification of visceral syndrome differ-entiation based on deficiency and excess,focusing on the specific physiological and pathological patterns of the five zang organs and proposing corresponding treatments for their specific syndromes.Medication Patterns propose the diagnosis and treatment of reproductive system diseases based on the Mingmen,assuming that the Mingmen undertakes some of the functions of the kidneys.This enriched the connotations of visceral syndrome differentiation and provided an important theoretical basis for identifying certain complex pathogenesis.The visceral syndrome differentiation concept in the Medication Patterns has promoted subsequent research on visceral syndrome differentiation,exemplified by works such as Li Dongyuan's"Treatise on the Spleen and Stomach",Zhu Danxi's"Treatise on the Xianghuo"and the"Mingmen Theory".In-depth exploration of the inherent differentiation logic and theoretical framework of the Medication Patterns is of great significance to current research on differentiation standards and norms.
3.Effects of changes in disease and injury spectrum on the health-adjusted life expectancy of permanent residents aged 55 and above in Shenzhen City from 2016 to 2030

Objective:To analyze the effects of the disease and injury spectrum on health-adjusted life expectancy (HALE) among permanent residents aged 55 and above in Shenzhen from 2016 to 2030.Methods:Based on the mortality surveillance data and the permanent resident population data in Shenzhen from 2016 to 2022, the Sullivan method was used to calculate the HALE during 2016—2022. The Bayesian age-period-cohort model and the grey system model were used to predict the HALE during 2023—2030. The HALE changes in the two periods were decomposed into the contributions of 20 categories of diseases and injuries, respectively.Results:From 2016 to 2022, the HALE increased from 31.41 years (95% CI: 30.50-32.32) to 33.57 years (95% CI: 32.47-34.67). During this period, the mortality effect of neurological disorders slowed the increase of HALE, with a reduction of 0.27 years. By 2030, it is anticipated that the HALE will reach 36.40 years (95% CI: 34.78-38.01). This is expected to be influenced by the mortality effects of nutritional deficiencies (-0.40 years) and mental disorders (-0.29 years), as well as the disability effects of musculoskeletal disorders (-0.66 years), skin and subcutaneous diseases (-0.21 years) and nutritional deficiencies (-0.13 years). Conclusion:The HALE of permanent residents aged 55 years and above in Shenzhen demonstrated an increasing trend over time. Greater attention should be paid to the adverse effects of neurological disorders, nutritional deficiencies, mental disorders, musculoskeletal disorders, and skin and subcutaneous diseases on the continuous increase of HALE in this population.
4.Treatment Based on Syndrome Differentiation in Internal Injury Fever Based on ZHANG Jingyue's Theory of"Ministerial Fire Has Tangible Position"

[Objective]Based on ZHANG Jingyue's theory of"ministerial fire has a place and is visible",to explore the syndrome differentiation thinking and treatment plan about injury fever syndrome differentiation,in order to provide new ideas for the clinical treatment of internal injury fever such as deficiency fever,spermatorrhea,blood syndrome,diabetes syndrome and so on.[Methods]By studying the relevant works of ZHANG Jingyue's Jingyue Encyclopedia and Classified Classic,combined with the works of other doctors such as LIU Wansu,ZHU Danxi,LI Dongyuan,etc.,it discussed the application of ministerial fire thought in the syndrome differentiation and treatment of internal injury fever.Combined with medical records,three ideas of syndrome differentiation and treatment of internal injury fever were summarized:spleen-Qi deficiency,Yin essence deficiency and original Yang deficiency,corresponding to three treatment methods of regulating middle-Jiao,benefiting essence and blood,and warming body origin.Combined with clinical practice,it summarized the diagnostic and treatment thought of internal injury fever,and discussed the role of ZHANG Jingyue's ministerial fire theory in the treatment of specific febrile diseases such as deficiency fever,spermatorrhea,blood syndrome and diabetes syndrome.[Results]ZHANG Jingyue believed that ministerial fire"has a place",which was born in Mingmen and all over the viscera.All endogenous fevers in the human body were related to it.And ZHANG proposed that the incidence of internal injury fever was mainly deficiency,and ministerial fire was"tangible",which could be differentiated and treated according to the fact that ministerial fire must be damaged by"blood Qi,viscera and other evidence".Its pathogenesis mainly included three aspects:spleen-Qi deficiency,Yin essence deficiency and original Yang deficiency.The diagnosis and treatment should pay attention to regulating the middle palace and heat,benefiting essence and blood to make fire,warming kidney and life to absorb fire.According to the syndrome differentiation situation,in terms of medication,it should pay attention to the use of sweet and warm,sweet and cold drugs also warming and tonifying Yang prescriptions flexibly.[Conclusion]ZHANG Jingyue had a very profound understanding of the theory of ministerial fire.On the basis of absorbing the essence of predecessor's thoughts,he often had his own new insights.His theory of ministerial fire had important value in discussing the pathogenesis and treatment of internal injury fever,which was worthy of reference for the later generations.
5.Predictive value of combined application of serum inflammation-related indicators for stroke-associated pneumonia

Objective To explore the predictive value of combined serum soluble hemoglobin scavenger receptor 163(sCD163),hypersensitive C-reactive protein(hs-CRP),and procalcitonin(PCT)for stroke-associated pneumonia(SAP).Methods A total of 100 patients with acute ische-mic stroke admitted to the First Hospital of Zhangjiakou from October 2021 to January 2023 were se-lected as the study subjects.According to whether they developed SPA within 7 days of admission,they were divided into SAP group(n=64)and non-SAP group(n=36).Based on pneumonia se-verity index(PSI),patients in the SAP group were further divided into mild SAP group and severe SAP group.Enzyme-linked immunosorbent assay(ELISA)was used to detect the serum levels of sCD163,hs-CRP,and PCT.The clinical data of the patients were collected and compared.Pearson's method was used to analyze the correlation between the PSI score and the serum levels of sCD163,hs-CRP,and PCT in SAP patients.Multivariate Logistic regression analysis was employed to screen for factors influencing the occurrence of SAP.Receiver operating characteristic(ROC)curve analy-sis was used to evaluate the predictive efficacy of serum sCD163,hs-CRP,and PCT for the occur-rence of SAP.Results The proportion of patients with dysphagia and the National Institutes of Health Stroke Scale(NIHSS)score in the SAP group were significantly higher than those in the non-SAP group(P<0.05).The serum levels of sCD163,hs-CRP,and PCT in the SAP group were significantly higher than those in the non-SAP group(P<0.05).The serum levels of sCD163,hs-CRP,and PCT in the severe SAP group were significantly higher than those in the mild SAP group(P<0.05).Pearson's correlation analysis showed that the PSI score was positively correlated with the serum levels of sCD163,hs-CRP and PCT in SAP patients(r=0.356,0.413,0.391,P<0.001).Logistic regression analysis revealed that serum sCD163,hs-CRP,PCT,NIHSS score,and dysphagia were all influencing factors for the occurrence of SAP(P<0.05).The areas under the curve(AUC)for predicting SAP using serum sCD163,hs-CRP,PCT and their combina-tion were 0.842,0.924,0.866 and 0.973,respectively,with sensitivities of 73.44%,84.37%,67.19%and 90.62%,and specificities of 88.89%,83.33%,97.22%and 94.44%,respectively.The predictive value of the combined detection was superior to that of the individual detection of ser-um sCD163,hs-CRP and PCT(P<0.05).Conclusion The serum levels of sCD163,hs-CRP,and PCT are elevated in SAP patients,and their changes are closely related to the severity of the disease.The combined detection of these three indicators has a high value in predicting the occur-rence of SAP and may serve as auxiliary markers for predicting early SAP.

7.Supplementing rehabilitation training with low-frequency transcranial magnetic stimulation improves the abnormal spine posture of persons with Parkinson′s disease

Objective:To observe any effect of supplementing rehabilitation training with low-frequency repetitive transcranial magnetic stimulation (rTMS) on the abnormal spine posture of Parkinson′s disease (PD) patients.Methods:A total 40 PD patients with Pisa syndrome (PS) were randomly divided into an observation group and a control group, each of 20. Both groups received conventional drug therapy and rehabilitation training (including myodynamia training and balance function training), while the observation group was additionally provided with low-frequency rTMS of the primary motor cortical area (M1) on the convex side of the scoliosis. Before the treatment and after 2 and 3 weeks, the scoliosis angle was measured, and motor functioning and balance were evaluated using the timed up and go test (TUGT) and the Berg balance scale (BBS). The subjects′ mental state was quantified using the exercise self-efficacy (ESE) scale, and the modified Barthel Index (MBI) was used to quantify their ability in the activities of daily life.Results:After the treatment, significant improvement was observed in the average scoliosis angles, TUGT, BBS, ESE and MBI scores of both groups compared to the pre-treatment levels. In the control group, all of the indicators had returned to their pre-treatment levels 3 weeks after treatment, but in the observation group they remained significantly improved.Conclusions:Low-frequency rTMS combined with rehabilitation training can significantly reduce the scoliosis angle of PD patients, improve their motor functioning and balance, increase their exercise confidence and improve their ability in the activities of daily living over the long term. The combination is worth applying and promoting in clinical practice.
8.Supplementing aerobic exercise with transcranial magnetic stimulation better improves the cognitive functioning of early stage Parkinson′s disease patients

Objective:To observe any effect of combining transcranial magnetic stimulation (rTMS) with aerobic exercise on the cognitive functioning of early stage Parkinson′s disease (PD) patients.Methods:A total of 120 PD patients in the early stage were randomly divided into an observation group and a control group, each of 60. Both groups received conventional drug treatment and moderate-intensity aerobic exercise training, while the observation group was additionally provided with high-frequency rTMS treatment. Before and after 3 months of the treatments, everyone′s cognitive and motor functioning was evaluated using the Montreal Cognitive Assessment Scale (MoCA) and the third part of the Unified Parkinson′s Disease Rating Scale (UPDRS-Ⅲ), respectively. Negative emotions were evaluated using the Hamilton Anxiety Scale (HAMA) and the Hamilton Depression Scale (HAMD). Auditory event-related potentials were also detected, and the latency and amplitude of P300 were analyzed.Results:The average MoCA, UPDRS-Ⅲ, HAMA and HAMD scores, as well as the amplitude and latency of P300 had improved significantly in both groups after the treatment. At that point the observation group′s performance was significantly better than that of the control group in terms of the MoCA′s visuospatial and executive function, attention and delayed recall indicators, and also total score. The observation group′s average HAMA score (10.55±3.11), HAMD score (9.78±4.10), the P300 amplitude [(11.29±2.21)μV] and latency [(384.75±48.26)ms] were also significantly better. The UPDRS-Ⅲ scores were negatively correlated with the visuospatial and executive function scores of the MoCA scale in the observation group before and after treatment, while the average HAMA score was negatively correlated with the attention and delayed recall scores.Conclusions:Supplementing aerobic exercise with rTMS can significantly improve the cognition and motor functioning of early stage PD patients. The combination is more effective than aerobic exercise alone. Such combined therapy is worthy of popularization and clinical application.
