1.Effect of 3% diquafosol sodium eye drops on the tear film homeostasis of patients wearing orthokeratology lenses

AIM:To investigate the effect of 3% diquafosol sodium eye drops on tear film homeostasis in patients wearing overnight orthokeratology lenses.METHODS:Retrospective study. A total of 340 patients(564 eyes)fitted with night-worn orthokeratology lenses from June to December 2022 were respectively divided into a diquafosol sodium group(200 cases, 323 eyes, treated with 3% diquafosol sodium eye drops)and a sodium hyaluronate group(140 cases, 241 eyes, treated with sodium hyaluronate eye drops). The uncorrected visual acuity(LogMAR), ocular surface disease index(OSDI), non-invasive tear film break-up time(NIBUT), meibomian gland infrared imaging score, and corneal epithelial fluorescein staining were analyzed and compared between the two groups.RESULTS:Compared to baseline, both groups showed significant improvements in uncorrected visual acuity(LogMAR)at 1 wk, 1, and 3 mo after lens wear(all P<0.01). However, no statistically significant difference in uncorrected visual acuity(LogMAR)was observed between the two groups at any time point(all P>0.05). No significant differences in NIBUT or OSDI scores were found between the groups before and at 1 wk after lens wear(all P>0.05). At the 1- and 3 mo follow-ups, the 3% diquafosol sodium group demonstrated significantly longer NIBUT(all P<0.001)and lower OSDI scores(all P<0.001)compared to the sodium hyaluronate group. After wearing lens for 3 mo, the meibomian gland infrared imaging scores were significantly better in the diquafosol sodium group(P<0.001), whereas no significant intergroup difference was observed in corneal fluorescein staining(P>0.05). Furthermore, the incidence of adverse events during the study period did not differ significantly between the two groups(P>0.05).CONCLUSION:Compared with sodium hyaluronate, 3% diquafosol sodium eye drops were more effective in maintaining tear film homeostasis in patients wearing overnight orthokeratology lenses.
2.Analysis of factors influencing the accuracy of flash glucose monitoring techniques in critically ill patients

Objective:To explore the relevant factors affecting the accuracy of scanning glucose monitoring (FGM) technology in critically ill patients.Methods:A total of 53 patients who were admitted to the Intensive Care Unit (ICU) of the Second Affiliated Hospital of Soochow University and used FGM glucose monitoring from September 2022 to December 2023 were selected by means of a convenience sampling method. The paired data of arterial blood glucose and FGM glucose were analysed. The accuracy of FGM blood glucose measurement in critically ill patients was evaluated using Bland-Altman analysis. The blood glucose data were divided into two groups: the FGM monitoring accuracy group and the FGM monitoring non-accuracy group.. The t-test and χ2 test were used for the comparison of one-way analysis of arterial versus FGM accuracy among critically ill patients with different demographic characteristics. Logistic regression analysis was used to analyze the factors influencing the accuracy of FGM. Results:The results of Bland-Altman analysis of the 53 patients indicated that the mean bias value of FGM blood glucose was elevated at 1.215 mmol/L in comparison with arterial-blood gas analysis blood glucose values. An incidence of exceeding the upper and lower limits of the range was observed, amounting to 6.349%. The discrepancy between the studies was found to be statistically significant (95% CI:1.0394~1.3908, P<0.001). The effects of the changes in height ( OR=0.877, 95% CI:0.780~0.987, P=0.029), changes in leukocyte counts( OR=0.917, 95% CI:0.868~0.969, P=0.002), changes in C-reactive protein( OR=1.009, 95% CI:1.002~1.017, P=0.016), changes in albumin counts( OR=0.986, 95% CI:0.974~0.999, P=0.031), and whether or not sepsis ( OR=3.937, 95% CI:1.192~13.008, P=0.025) on the accuracy of FGM had a statistically significant. Conclusion:The mean bias value of the accuracy of FGM was relatively higher compared with that of arterial blood gas analysis blood glucose values, and the influencing factors involved height, white blood cells, C-reactive protein, albumin, and whether or not sepsis was present. With the development of science and technology, applying the FGM system to critically ill patients has an absolute advantage in determining the overall glycemic trend, and the application value of FGM technology deserves further study.
3.The relationship between serum myosin-like BCL2 interacting protein, tissue non-specific alkaline phosphatase and arrhythmia in patients with hypertension and chronic heart failure

Objective:To investigate the relationship between serum myosin-like BCL2 interacting protein (Beclin-1), tissue non-specific alkaline phosphatase (TNAP) and arrhythmia in patients with hypertension and chronic heart failure (CHF).Methods:A total of 121 patients with hypertension and CHF who experienced arrhythmia and 121 patients with hypertension and CHF who did not experience arrhythmia and were admitted to Xi'an Traditional Chinese Medicine Brain Disease Hospital from April 2022 to April 2024 were included as the study subjects, and were assigned into the occurrence group and the non occurrence group, respectively. Enzyme-linked immunosorbent assay (ELISA) method was applied to detect the expression levels of Beclin-1 and TNAP in the serum of patients with hypertension and CHF. Pearson method was applied to analyze the correlation between Beclin-1, TNAP levels and mean heart rate, New York Heart Association functional class (NYHA), left ventricular ejection fraction (LVEF), dynamic stiffness index (AASI), and N-terminal pro-brain natriuretic peptide (NT-proBNP) in patients with hypertension complicated with CHF arrhythmia. Multivariate logistic regression was applied to analyze the influencing factors of arrhythmia in patients with hypertension and CHF. Receiver operating characteristic (ROC) curve was applied to evaluate the diagnostic value of serum Beclin-1 and TNAP levels for arrhythmia in patients with hypertension and CHF.Results:The expression level of Beclin-1 in the serum of the occurrence group was lower than that of the non-occurrence group: (4.18 ± 1.12) μg/L vs. (5.26 ± 0.69) μg/L, while the TNAP level was higher than that of the non-occurrence group: (6.31 ± 1.32) μg/L vs. (5.27 ± 1.24) μg/L, and the differences were statistically significant ( P<0.05). In patients with hypertension combined with CHF and arrhythmia, Beclin-1 was negatively correlated with average heart rate, NYHA classification, AASI and NT-proBNP, and positively correlated with LVEF ( P<0.05), while TNAP was positively correlated with average heart rate, NYHA classification, AASI and NT-proBNP, and negatively correlated with LVEF ( P<0.05). Mean heart rate, NYHA classification, AASI, NT-proBNP and TNAP were risk factors for arrhythmia in patients with hypertension and CHF, while LVEF and Beclin-1 were protective factors for arrhythmia in patients with hypertension and CHF ( P<0.05). The AUC of Beclin-1 and TNAP in diagnosing arrhythmia in patients with hypertension and CHF was 0.813 and 0.818, respectively. When the AUC of their combined diagnosis increased to 0.904, the AUC of their combined diagnosis for arrhythmia in patients with hypertension and CHF was better than that of Beclin-1 ( Z = 2.583, P<0.05) and TNAP ( Z = 2.559, P<0.05). Conclusions:Beclin-1 is downregulated and TNAP is upregulated in the serum of patients with hypertension complicated with CHF and arrhythmia. The combination of the two has a high value in diagnosing arrhythmia in patients with hypertension and CHF.
4.The molecular subtyping of urothelial carcinoma using GATA-3 and CD44

Purpose To investigate the correlation between molecular typing of urothelial carcinoma(UC)and its clinicopathologic features and prognosis,in order to explore the prognostic markers and therapeutic targets for UC.Methods 115 patients with UC were retrospectively analyzed.Immunohistochemical markers GATA-3,CK20,CK5/6 and CD44 were used for molecular typing of UC(luminal-like type,basal-like type and null).Correlations between molecular typing and clinicopathological features were analyzed using the Chi-square test and Fisher precise test.Sur-vival analysis was performed using the Kaplan-Meier test and Log-rank test.Results The expression of GATA-3 and CK20 was negatively correlated with the clinical stage of UC,while the expression of CD44 was positively correlated with the clinical stage of UC(both P<0.05).CK20 was a marker of good prognosis(P=0.03).The proportion of clinically advanced UC with basal-like type was significantly higher than that of luminal-like type(78.4%vs 53.4%,P=0.033).Among the histologic variants,UC with neuroendocrine differentiation(100%),sarcomatoid carcinoma(80.0%)and squamous differentiation(77.8%)were basal-like type.All plasmacytoid and lymphoepithelioma-like types,as well as 81.8%of micropapillary UC.Among the null phenotypes,the differential variant predominated(66.7%).Compared with the luminal-like type,although the prognosis of basal-like UC was worse,there was no sta-tistically significant difference(P>0.05).Conclusion Patients with CK20-expressing UC had a significantly better prognosis.The main histologic variants types of basal-like type and coelomofacial type are different.Molecular typing of UC using immunohistochemical markers is suggestive of clinical staging and prognosis of patients.
5.Correlation of POSTN and NECTIN-3 expression with clinicopathological features and prognosis in urothelial carcinoma

Purpose To investigate the relationship between the expression of POSTN and NECTIN-3 in urothelial carcinoma(UC)and its clinicopathologic features and prognosis.Methods Clinical data of 115 UC patients were col-lected.EnVision two-step method was used for immunohistochemical analysis to determine the expression of POSTN and NECTIN-3 in UC and their correlation with clinicopathological features and prognosis.Transcriptomic data from TCGA database were used to analyze the correlation between the expression of POSTN and NECTIN-3 in bladder cancer and the pathological stage and prognosis of bladder cancer.qPCR and Western blot were used to detect the expression levels of POSTN and NECTIN-3 in tumor tissues and adjacent tissues of patients.Results TCGA database analysis showed that the expression level of NECTIN-3 in urinary bladder urothelial carcinoma(UBUC)was significantly lower than that in paracancer tissues,and POSTN expression was positively correlated with pathological stage.Prognostic a-nalysis showed that POSTN expression was negatively correlated with the overall survival of UBUC(P>0.05),and NECTIN-3 expression was negatively correlated with disease-free survival of UBUC(P>0.05).The experimental re-suits showed that patients with positive POSTN expression were more prone to perineural invasion[25 cases(86.2%)vs 4 cases(13.8%),P=0.019],vascular invasion[36 cases(83.7%)vs 7 cases(16.3%),P=0.007],and lymph node metastasis[24 cases(88.9%)vs 3 cases(11.1%),P=0.033].Additionally,the positive expression rate of POSTN in UBUC was significantly higher than that in upper tract urothelial carcinoma(UTUC)(75.0%vs 54.3%,P=0.028).Higher expression levels of POSTN and NECTIN-3 were associated with shorter overall survival,but the difference was not statistically significant(P>0.05).Conclusion There is no significant correlation between NECTIN-3 and the clinicopathological features and prognosis of urothelial carcinoma,while the expression of POSTN is correlated with the invasive clinicopathological features of UC,which has certain suggestive significance for clinical stage and prognosis.

9.Effects of preoperative ureteroscopy on the prognosis of patients with upper tract urothelial carcinoma after radical nephroureterectomy

[Objective] To evaluate the effects of preoperative ureteroscopy (URS) on the prognosis of patients with upper tract urothelial carcinoma (UTUC) after radical nephroureterectomy (RNU). [Methods] Data of 712 UTUC patients who received RNU in West China Hospital during May 2003 and Jun.2019 were retrospectively analyzed. Patients were divided into URS group (n=187) and non-URS group (n=525) according to whether URS was performed before RNU. Kaplan-Meier curves were used to analyze the overall survival (OS), cancer-specific survival (CSS), and intravesical recurrence-free survival (IVRFS). Cox proportional risk model was used for risk assessment. Subgroup analysis and interaction test were used to further verify the results. [Results] Significant differences were observed between the two groups in terms of body mass index (BMI), diabetes mellitus, surgical method, hydronephrosis, tumor location, tumor grade, lymph node metastasis, lymphovascular invasion and tumor diameter (P<0.05). Kaplan-Meier survival analysis showed that IVRFS was significantly lower in the URS group than in the non-URS group (P<0.001), and the difference was more pronounced in renal pelvis carcinoma (P<0.001); there were no differences in OS and CSS between the two groups (P>0.05). Cox multivariate regression showed that URS was an independent risk factor for intravesical recurrence (HR=2.12, 95%CI: 1.34-3.36, P<0.001). [Conclusion] Preoperative URS can increase the recurrence rate of UTUC, but it has no effect on the OS and CSS.

The condition of patients with severe traumatic brain injury (sTBI) complicated by corona virus 2019 disease (COVID-19) is complex. sTBI can significantly increase the probability of COVID-19 developing into severe or critical stage, while COVID-19 can also increase the surgical risk of sTBI and the severity of postoperative lung lesions. There are many contradictions in the treatment process, which brings difficulties to the clinical treatment of such patients. Up to now, there are few clinical studies and therapeutic norms relevant to sTBI complicated by COVID-19. In order to standardize the clinical treatment of such patients, Critical Care Medicine Branch of China International Exchange and Promotive Association for Medical and Healthcare and Editorial Board of Chinese Journal of Trauma organized relevant experts to formulate the Chinese expert consensus on clinical treatment of adult patients with severe traumatic brain injury complicated by corona virus infection 2019 ( version 2023) based on the joint prevention and control mechanism scheme of the State Council and domestic and foreign literatures on sTBI and COVID-19 in the past 3 years of the international epidemic. Fifteen recommendations focused on emergency treatment, emergency surgery and comprehensive management were put forward to provide a guidance for the diagnosis and treatment of sTBI complicated by COVID-19.
